目的 评估女性青少年特发性脊柱侧凸(adolescent idiopathic scoliosis,AIS)患者支具治疗效果及影响因素.方法 2003年7月至2009年7月,完成支具治疗或因侧凸进展而行手术治疗的女性AIS患者142例,初诊时平均年龄为(13.1±1.5)岁,平均主弯Cobb角29.6°±5.4°,平均Risser征为(2.0±1.5)级.定义侧凸畸形进展为末次随访Cobb角大于初诊6°以上或治疗期间建议行矫形手术(Cobb 角>45°),其余为非进展.根据侧凸进展或需手术治疗的比率来评估支具治疗效果.运用卡方检验和Logistic回归分析探讨影响支具治疗效果的因素.结果 平均支具治疗时间为(2.5±1.0)年.按照侧凸进展的定义,进展组为27例(19%),非进展组115例(81%);手术组病例18例(13%),因支具治疗而避免手术病例124例(87%).卡方检验发现侧凸进展组和手术组中以初诊年龄10.0~12.9岁、月经初潮未至、Risser征0~1级、初诊Cobb角>30°以及胸弯型居多.Logistic回归分析发现月经初潮未至(P=0.000)和胸弯型(P=0.012)是支具治疗后侧凸进展的独立预测因素,而初诊Cobb角>30°(P=0.022)是支具治疗期间因侧凸进展而需手术治疗的另一独立预测因素.结论 支具治疗可有效控制多数AIS患者的侧凸进展,而生长发育状态、侧凸严重程度和侧凸类型是影响支具疗效的重要因素.
Abstract:
Objective To analyze the outcomes of bracing treatment for girls with adolescent idiopathic scoliosis (AIS), and to investigate the predictive factors of the protocol. Methods This study included 142 girls with AIS who finished standardized bracing treatment from July 2003 to July 2009. These patients had a mean age of 13.1±1.5 years, a mean main curve of 29.6°±5.4°, and a mean Risser grade of 2.0±1.5 before bracing treatment. Curve progression was defined that Cobb angle was greater than 6° compared to bracing initiation or was aggravated to more than 45° (indicative for surgery). The outcomes of bracing treatment were assessed based on the ratio of curves of progression or indicative for surgery. Chi-square and Logistic regression Analyses were performed to investigate the predictive factors of bracing treatment. Results The duration of bracing treatment averaged 2.5±1.0 years. Twenty-seven girls with curve progression (19%)and 115 girls (81%) with non-progression were found. Final curve which was greater than 45° was found in 18 girls (13%) who need a correction surgery, the remaining 124 girls (87%) had completed bracing treatment and avoided surgery. Chi-square analyses revealed that curve progression were more common in younger girls with lower Risser grade, with initial larger Cobb angle and with a main thoracic curve pattern.Logistic regression analyses found that premenarchal status and a main thoracic curve pattern were the independent risk factors of curve progression despite bracing. While initial Cobb angle which was greater than 30° was the additional independent risk factor of progression requiring surgery. Conclusion Bracing treatment could effectively prevent curve progression in most girls with AIS. The degree of growth maturity, the pattern and grade of curve are the influencing factor for bracing treatment.  相似文献

Objective: To prospectively evaluate the feasibility, safety and efficacy of the percutaneous pedicle screw fixation through the pedicle of fractured vertebra in the treatment of type A thoracolumbar fractures using Sextant system in the retrospective non-randomized case-control study. Methods: Atotal of 38 consecutive non-randomized patients with type A thoracolumbar fractures, which had been stabilized posteriorly from December 2006 to March 2009, were examined retrospectively more than 9 months after surgery. Twenty-one patients had been treated conventionally with open pedicle screw fixation (OPSF) and 17 patients received minimally invasive treatment with Sextant percutaneous pedicle screw fixation (SPPSF). As a method of evaluation, the incision size, the intraoperation and post- operative volume of blood loss, operation time, postoperative hospital stay, blood transfusion, the radiological assessment of the sagittal Cobb's angle, vertebral body angle and vertebral body height were recorded and compared. Results: All patients were followed up for 8-24 months (average 11.6 months). There were significant differences in the incision size, surgical blood loss, surgical draining Joss, operation time, hospital stay after operation, blood transfusion, the proportion of antalgic supplement and postoperative incisional VAS between the two groups (P〈0.05). Mean preoperative kyphotic deformity was 16.0° and improved by 9.3° after surgery in OPSF group, but 15.2° and 10.3° respectively in SPPSF group. Mean preoperative angle of the fractured vertebral body was 15.9°and improved by 7.9° after surgery in OPSF group, but 14.9° and 6.6° respectively in SPPSF group. Mean anterior vertebral body height (% of normal) was 67.3% before surgery and 95.8% after surgery, but 69.1% and 90.1% respectively in SPPSF group. Mean posterior vertebral body height (% of normal) was 93.3% before surgery and 99.5% after surgery, but 88.9% and 93.3% respectively in SPPSF group. Among the patients whose 9-month follow-up films were available, 3.0° ofkyphosis correction was lost in OPSF group, but 3.2° in SPPSF group. And 1.0°of the angle of the fractured vertebral body correction was lost in OPSF group, but 1.5°in SPPSF group. Then 3.0% of the anterior vertebral body height correction was lost in OPSF group, but 2.2% in SPPSF group. And 3.0% of the posterior vertebral body height correction was lost in OPSF group, but 2.5% in SPPSF group. The sagittal Cobb's angle, vertebral body angle and anterior height of the fractured vertebra were all significantly different in each group before and after operation (P〈0.05). There were no significant differences in the postoperative sagittal Cobb's angle, vertebral body angle and the improvement of the vertebral body height and the kyphotic deformity correction between OPSF and SPPSF groups (P〉0.05), but there was significant difference in the postoperative anterior height of the fractured vertebra between the two groups (P〈0.05). Conclusion: The percutaneous pedicle screw fixation through the pedicle of fractured vertebra using Sextant system is a good minimally-invasive surgical therapeutic choice for patients with type A thoracolumbar fracture except for that the SPPSF has a little insufficiency in resuming the anterior height of the fractured vertebra compared with OPSF.  相似文献

Aim: To determine retrospectively the safety and efficacy of extracorporeal shock wave therapy (ESWT) in patients with Peyronie's disease. Methods: Fifty-three patients with stable Peyronie's disease underwent ESWT (group 1). Fifteen patients matched with the baseline characteristic of the patients in group 1, who received no treatment, were used as the control (group 2). The patients' erectile function (International Index of Erectile Function [IIEF-5] score), pain severity (visual analog scale), plaque size and degree of penile angulation were assessed before and after the treatment in group 1 and during the follow-up in group 2. Results: The mean follow-up time was 32 months (range: 6-64 months) in group 1 and 35 months (range: 9-48 months) in group 2. All the patients were available for the follow-up. Considering erectile function and plaque size, no significant changes (P 〉 0.05) were observed in group 1 before or after the ESWT. A total of 39 patients (74%) reported a significant effect in pain relief in group 1 after ESWT. However, regarding improvement in pain, IIEF-5 score and plaque size, no significant differences were observed between the two groups. In 21 patients (40%) of group 1, the deviation angle was decreased more than 10° with a mean reduction in all patients of 11° (range: 6-20°). No serious complications were noted considering ESWT procedure. Conclusion: ESWT is a minimally invasive and safe alternative procedure for the treatment of Peyronie's disease. However, the effect of ESWT on penile pain, sexual function and plaque size remains questionable.  相似文献

Methods Posterior tibial nerve SEPs were evaluated on 489 young operative treated AIS patients with a Cobb angle > 40°and 45 age-matched healthy control individuals. Absence of SEPs waveforms, prolongation of peek latency or asymmetrical peek latency were defined as pathological change. Base on the control reference, the incidence of pathological SEPs was determined in AIS group. The association of abnormal SEPs and curve severity for MS patients was also assessed. Results Peek latency corrected for body height was slightly longer in AIS patients than in controls, however, with no significant difference. Interside difference of latency was significantly larger in AIS patients. Abnormal SEPs were found in 166 of 489 AIS patients. Among these, 17 (10.2%) showed absent waveforms, 50 (30.1%) had unilateral latency prolongation, 38 (22.9%) had bilateral latency prolongation, and 120 (72.3%) showed significant interside difference. Statistical analysis failed to show a correlation between abnormal SEPs and the curve severity of spinal deformity. Conclusion Disorder of somatosensory pathways does exist in a subgroup of AIS patients, and it might be a primary factor other than secondary change, and could play an import role in the etiopathogenesis of AIS.  相似文献

目的 探讨matrilin-1基因多态性与青少年特发性脊柱侧凸(AIS)患者支具治疗效果的相关性.方法 自2005年1月至2008年12月,对门诊行规范化支具治疗的AIS患者进行前瞻性研究.入选对象要求:女性患者;骨骼发育为未成熟状态(Risser征0~3级);月经初潮未至或来潮1.5年以内;标准站立位全脊柱X线片上侧凸Cobb角为20°~40°;排除了先天性和神经肌源性以及其他原因引起的侧凸;无已知的可影响骨塑型和钙代谢的病史和药物使用史;初诊前无支具治疗史;接受支具治疗(Boston支具或Milwaukee支具)期间每3个月复诊1次,随访时间超过2年.符合下述条件之一者,则予以剔除:最终随访不足2年;支具治疗过程中依从性(每日实际佩戴时间与建议佩戴时间的比值)较差(<75%);支具治疗期间未按照医嘱自行更换支具.根据患者侧凸类型选择相应的支具进行治疗.记录初次就诊及末次随访的原发弯的Cobb角.支具治疗失败的标准为原发弯增加>5°.选取matrilin-1基因启动子区域位点rs1149048进行PCR-RFLP基因分型.按支具疗效分为治疗失败和治疗成功两组,比较两组之间的初诊年龄、Risser征、侧凸类型、初诊Cobb角及基因型分布的差异并进行统计学分析.结果 共有77例AIS女孩入选并完成本研究.初诊时平均年龄为(13.0±1.5)年,平均主弯Cobb角为29.5°±7.8°.在平均(2.6±0.3)年随访后,平均主弯Cobb角为30.3°±11.9°.支具治疗失败总共19例(24.7%),支具治疗成功58例(75.3%).支具治疗失败组的初诊Cobb角较大(P>0.05).在不同的侧凸类型中,胸腰双主弯的支具治疗失败率最低(19.4%).基因型GG个体支具治疗失败率(66.7%)明显高于基因型为从或AG的个体.结论 通过早期规范化支具治疗,大部分脊柱侧凸进展可得到控制.初诊Cobb角较大及基因型GG的AIS患者支具疗效较差.  相似文献

目的:探讨青少年特发性脊柱侧凸(adolescent idiopathic scoliosis,AIS)患者脊柱柔韧性的可能影响因素.方法:选取2006年12月~2008年4月在我院脊柱外科手术治疗的204例AIS患者,男性36例,女性168例,平均年龄15.0岁;平均Cobb角50.1°;平均Risser征3.4度;主弯跨度平均6.8个椎体;主弯顶椎旋转度平均2.0度.摄站立位全脊柱正侧位及仰卧左右侧屈位X线片,计算主弯柔韧性.采用相关分析研究各临床指标与主弯柔韧性的相关性.结果:女性AIS患者的脊柱柔韧性明显高于男性(P<0.05);胸腰弯组和腰弯组AIS患者的脊柱柔韧性显著大于胸弯组(P<0.05),胸腰弯和腰弯组之间无显著性差异(P>0.05).女性AIS患者中的年龄及主弯Cobb角(站立位与侧屈位)均与脊柱柔韧性显著负相关(P<0.05),且胸弯女性AIS患者的月经初潮至手术时间及顶椎旋转度也与脊柱柔韧性显著负相关(P<0.05).男性胸弯AIS患者中侧屈位主弯Cobb角、胸腰弯/腰弯组中主弯Cobb角(站立位与侧屈位)均与脊柱柔韧性显著负相关(P<0.05).主弯跨度及Risser征与脊柱柔韧性均无明显相关性(P>0.05).结论:女性AIS患者脊住柔韧性受年龄、月经初潮至手术时间、主弯Cobb角(站立位与侧屈位)、弯型及顶椎旋转度等因素影响;男性AIS患者的脊柱侧凸柔韧性主要受主弯Cobb角及弯型影响.  相似文献

目的 比较分析青少年特发性脊柱侧凸(adolescent idiopathic scoliosis,AIS)患者与对照组雌激素β受体(estrogen β receptor,ER β)基因型分布情况,探讨ER β基因多态性与AIS患者易感性、异常生长模式及低骨量之间的关系.方法 对2006年3月至2007年3月的288例(男17例,女271例)AIS患者进行人体形态学测量,记录其年龄、身高、体重、臂长、月经初潮情况、Cobb角、Risser征等指标,并收集其静脉血标本.经体检收集232名(男30名,女202名)健康青少年静脉血标本.应用聚合酶链反应-限制性片断长度多态性技术(PCR-RFLP)检测分析AIS组及正常青少年组ER β基因型,比较两组间及AIS组内各亚组ER β基因型分布情况.结果 AIS组与对照组ER β基因型分布差异无统计学意义(χ2=5.908,P>0.05).AIS组内比较,初潮年龄≥12岁组Rr型频率高于初潮年龄<12岁组(P<0.05),而根据身高、体重指数(BMI)、臂长、Cobb角、Risser征等分组,基因型分布比较差异无统计学意义(P>0.05).ER B基因两个多态性位点各基因型间所对应的骨密度比较差异无统计学意义(P>0.05).结论 ER β基因多态性与AIS患者易感性无关联.Rr型患者存在初潮延迟现象,可能在AIS的进展中起一定作用.ER β基因多态性与骨量无相关,可能在AIS的低骨量中不起作用.  相似文献

目的 探讨胰岛素样生长因子-1受体(IGF-1R)基因多态性与青少年特发性脊柱侧凸(AIS)易感性之间的关联性.方法 实验对象为200例MS患者(AIS组)和200名年龄、性别匹配的健康青少年(对照组),记录AIS组的身高、体蓖、月经初潮年龄(女性)、侧弯类型、最大Cobb角和Risser征等临床指标.采用聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)的方法对所有研究对象进行IGF-1R基因分型,比较不同基因型在AIS组和对照组之间的分布差异,并分析基因多态性与临床指标的相关性.结果 AIS组与对照组的IGF-1R基因等位基因及基因型分布无统计学差异(P>0.05).女性AIS患者不同基因型所对应的最大Cobb角、Risser征及月经初潮年龄之间差异也无统计学意义(P>0.05).结论 IGF-1R基因多态性与AIS的发生发展均无相关性.  相似文献

目的:探讨低骨密度对LenkeⅠ型青少年特发性脊柱侧凸(adolescent idiopathic scoliosis,AIS)患者后路矫形内固定术后矫形疗效的可能影响。方法:选取2007年6月~2008年8月在南京鼓楼医院脊柱外科行后路椎弓根螺钉矫形内固定融合术的LenkeⅠ型AIS女性患者37例,年龄11~17岁(14.3±1.3岁),术前Cobb角40°~66°(48.9°±6.7°)。术后随访12~36个月,平均20.8±7.4个月。术前均接受双能X线吸收骨密度仪扫描测定骨密度,根据股骨颈骨密度Z值将AIS患者分为骨密度正常组(Z值>-1.0)与骨密度减低组(Z值≤-1.0),分别测量两组患者术前、术后早期(术后3个月)及末次随访时的主弯Cobb角、胸椎后凸角(TK)、腰椎前凸角(LL)、冠状位顶椎偏移(AVT)、C7中垂线与骶骨中线的距离(C7-CSVL)和C7中垂线与骶骨后上缘的距离(SVA),比较两组间的差异。结果:37例患者中,15例骨密度正常(骨密度正常组),22例骨密度减低(骨密度减低组)。两组患者术前平均年龄、Risser征、内固定节段数、置入物密度及术后随访时间均无显著性差异(P>0.05);两组术前平均Cobb角、AVT和C7-CSVL无显著性差异(P>0.05)。术后3个月及末次随访时两组平均Cobb角和平均矫正率、末次随访时平均矫正丢失及矫正丢失率无显著性差异(P>0.05);术后3个月及末次随访时两组平均AVT、C7-CSVL无统计学差异(P>0.05);C7-CSVL改变亦无显著性差异(P>0.05)。两组术前、术后3个及末次随访时的平均TK、LL、SVA均无显著性差异(P>0.05),SVA改变亦无显著性差异(P>0.05)。结论:低骨密度状态对LenkeⅠ型AIS患者后路矫形内固定融合术的疗效无明显影响。  相似文献

目的 通过临床病例回顾,对后路全椎弓根螺钉系统和钉钩混合系统治疗青少年特发性脊柱侧凸的疗效进行对比分析研究.方法 从2000年至2004年进行手术治疗的青少年特发性脊柱侧凸患者中选取60例配对分为两组(A、B两组,各30例),A组采用后路钉钩混合系统固定,B组采用后路全椎弓根螺钉系统固定,配对患者年龄相似、融合节段相仿、Lenke分型相近.通过比较手术前后影像学改变、手术时间、术中出血等,对两种手术方法的疗效进行对比分析研究.结果 A组患者术前冠状面主弯Cobb角平均为61°,术后为25°,平均矫正率为59%;B组患者术前冠状面主弯Cobb角平均为60°,术后为18°,平均矫止率为70%.两组患者主弯矫正率差异有统计学意义(P=0.002),随访2年两组间主弯矫正率差异仍有统计学意义(A、B两组分别为49%、67%,P<0.001).矢状面上,与术前相比,术后2年A组的胸椎后凸角平均减少了1°,而B组则减少了6°,差异有统计学意义(P=0.026).随访结果 提示,两组在下端椎远端融合椎体数、平均失血量方面差异均无统计学意义.两组患者术后均未出现神经系统并发症.结论 全椎弓根螺钉系统较钉钩混合系统能提供更好的主弯矫止率,而在最下端固定椎体的选择、术中失血量方面,两种内固定系统未见明显差异.  相似文献

The aim of the study was to compare the results of brace treatment of adolescent idiopathic scoliosis (AIS) in male patients with matched female patients and to assess the effectiveness of bracing of boys in AIS and to discuss the results with published data. Between 1987 and 1995, 51 consecutive male patients with AIS were treated with the Boston brace. The patients were advised to wear the brace 23 h/day. The medical records of all patients were reviewed. Cobb angles and Risser signs were measured before bracing, in brace, at brace discontinuation and at final follow-up. Everyone of 51 male patients was compared with a female patient who was treated by the same method and matched by Risser sign, curve pattern, curve magnitude and duration of treatment and follow-up time. Compliance with brace was noted at every visit. Fourteen boys had worn the brace only during nighttime or occasionally and were considered non-compliant. Only compliant patients with treatment period > 1 year and follow-up > 1 year after treatment were accepted for the analyses of effectiveness of brace treatment and its prognostic factors. Thirty-three boys met these inclusion criteria. Bracing was considered to have a failure if > 5° progression occurred or if surgery was performed. At the final follow-up study progression > 5° was found in 16/51 (31.4%) of male patients. Corresponding figures of female patients were 11/51 (21.6%), respectively. In compliant boys progression > 5° occurred in 6/33 boys compared with 9/33 girls. The association between risk of progression and correction% in brace was statistically significant. The overall results of brace treatment of idiopathic scoliosis in male patients were inferior compared with matched females. One reason for inferior overall results in boys was poor compliance with brace wear. However, brace treatment in AIS may be recommended with the same principles in both genders.  相似文献

Summary of background data

The curve pattern of idiopathic scoliosis is important for making decisions concerning bracing. However, whether the curve pattern changes during brace treatment have not been fully documented. The aim of this study was to investigate the changes of curve pattern during brace treatment in skeletally immature patients with adolescent idiopathic scoliosis (AIS).

Methods

From January 2002 to January 2011, AIS patients treated with a Boston or Milwaukee brace were recruited after meeting the following inclusion criteria: older than 10 years of age at initiation of bracing; having a Cobb angle of 25°–40°; with a Risser sign 0–2; being regularly followed until the weaning of brace or the necessity of surgical treatment; and without history of previous treatment. A total of 130 female and 11 male AIS patients were included. The mean age was 12.9 years at initiation of bracing, and the female patients were, on average, 2.7 months past menarche. The mean follow-up period was 2.6 years (range 1.0–5.5 years). The definitions of changes in curve patterns were divided into four categories as follows: (1) shift of the apex of the main curve; (2) change in the curve span of more than two vertebrae; (3) change in the main curve type with regard to the apex location; and (4) change of curve direction. The patients were divided into two groups. Group A was comprised of patients who had experienced one or more categories of curve pattern changes, and Group B was comprised of those who had not.

Results

Of these 39 patients, 14 had apex shifting, 2 underwent curve span changes, 22 experienced changes in the main curve type, and one female had both changes in the apex and curve span. At the initiation of bracing, patients in Group A demonstrated significantly lower menarchal status (P = 0.018) and lower Risser grade (P = 0.025) than those in Group B. The difference in the percentage of patients who underwent Boston bracing between the two groups was statistically significant (41.5 % for Group A vs. 24.0 % for Group B, P = 0.023).

Conclusion

Changes in curve pattern can occur during brace treatment. Patients with less skeletal maturity and those treated with a Boston brace are more susceptible to this phenomenon.  相似文献

目的:分析术前支具治疗对女性青少年特发性脊柱侧凸(AIS)患者手术矫形效果的影响。方法 :筛选2001年7月~2009年12月在我院接受单一后路矫形内固定手术治疗的女性青少年特发性主胸弯脊柱侧凸患者70例,其中术前接受支具治疗组(A组)26例;未接受支具治疗组(B组)44例。两组发现畸形年龄、术时年龄、术前主胸弯冠状面Cobb角、凸侧Bending像Cobb角、侧凸柔韧性、手术融合椎体数比较均无统计学差异(P>0.05),A、B组随访时间超过1年者分别为23例和34例,随访时间分别为12~101个月(平均37.7个月)、12~87个月(平均28.7个月),两组比较无统计学差异(P>0.05)。比较两组患者的手术矫形效果。结果:A组与B组患者术前主胸弯冠状面Cobb角分别为52.8°±8.3°和54.0°±10.7°,术后分别矫正到12.3°±7.3°和11.5°±8.1°,术后较术前均明显改善(P<0.01),主胸弯矫形率分别为(77.0±12.6)%和(79.3±11.9)%,两组比较无统计学差异(P>0.05);末次随访时主胸弯冠状面Cobb角分别为16.7°±8.4°和15.4°±7.2°,两组比较无统计学差异(P>0.05),主胸弯矫形率分别为(68.8±14.5)%和(70.5±13.0)%,两组比较无统计学差异(P>0.05)。A、B组患者术前主胸弯顶椎偏距分别为41.4±14.3mm和36.8±13.7mm,两组比较无统计学差异(P>0.05),术后分别被矫正到10.4±5.4mm和7.2±5.6mm,B组优于A组(P<0.05);末次随访时分别为14.4±11.3mm和12.1±8.5mm,两组比较无统计学差异(P>0.05)。A、B组患者术前、术后、末次随访时冠状面失平衡的发生比例分别为15.4%(4/26)和9.1%(4/44),15.4%(4/26)和15.9%(7/44),4.3%(1/23)和8.8%(3/34),两组比较均无统计学差异(P>0.05)。A、B组患者术前主胸弯矢状面Cobb角分别为12.9°±11.1°和18.7°±11.3°,A组胸后凸更小(P<0.05),术后主胸弯矢状面Cobb角分别被矫正到18.0°±6.3°和22.3°±7.8°,矫正度分别为5.0°±9.8°和3.6°±12.6°,两组矫正度比较无统计学差异(P>0.05);末次随访时A、B组患者主胸弯矢状面Cobb角分别为20.0°±6.7°和22.4°±7.7°,两组比较无统计学差异(P>0.05)。结论:术前支具治疗对女性青少年特发性主胸弯脊柱侧凸患者手术矫形效果未产生明显影响。  相似文献
