三棱火针加拔罐治疗腘窝囊肿30例

ObjectiveTo explore the therapeutic effect of the fire needling with the three-edged needle and the cupping on popliteal cyst.MethodsA total of 60 patients of popliteal cyst were divided into two groups, 30 cases in each one according to the random number table. In the fire needling group, the burnt-red three-edged needle was inserted to the deep layer of the cyst. After the fluid was squeezed out, the cupping was exerted. The treatment was given 1 to 2 times a week, for 4 weeks totally. In the western medication group, prednisolone acetate 25 mg was injected in the cyst capsule, once a week, for 4 weeks totally. The effective rate, recurrence rate and Rauschning–Lindgren grading were evaluated before and after treatment.ResultsThe total effective rate was 80.0% in the fire needling group and was 83.3% in the western medication group, without statistical significance in comparison (P > 0.05). The recurrence rate was 6.7% in the fire needling group and was 30% in the western medication group. The recurrence rate in the fire needling group was lower than that in the western medication group, indicating the statistical significance (P < 0.05). After treatment, Rauschning–Lindgren grade was different statistically as compared with that before treatment in either group (both P < 0.05). Rauschning–Lindgren grade was distributed in 0 to I in the fire needling group after treatment, which was different significantly as compared with the western medication group (P < 0.05).ConclusionThe combined treatment with the fire needling combined with cupping therapy achieves the satisfactory effect on popliteal cyst and the very low recurrence rate and it is easy in operation and deserves to be promoted in primary care.     

范忠泽辨治小儿神经母细胞瘤经验

范忠泽教授认为小儿神经母细胞瘤的病因分为胎产因素及外感六淫、邪毒侵袭、饮食失节、情志所伤、接触有毒之品等导致气血运行不畅,痰凝气滞血瘀,聚而成瘤。临床难点是小儿五脏六腑形气皆不足,中医辨证为肺脾肾气虚,治则是调补肺脾肾,扶正为本不忘祛邪。临床谨守病机,辨证论治,颇有良效。     

CMV感染相关过敏性紫癜合并小肠出血性梗死及穿孔1例

<正>患儿女童，4岁，因间断腹痛13天，皮疹11天，关节肿痛3天，低热2天，于2016年12月26日入住当地医院。患儿偶有呕血，量多，拉黑水样便，量多，无脓液;给予抗炎、抗过敏、改善循环、止敏止血等处理，但体温反复，腹痛及皮疹无缓解。遂转诊至中国人民解放军联勤保障部队第九〇〇医院，门诊拟“过敏性紫癜、紫癜性肾炎”收住入院。实验室检查:尿素氮7.4 mmol/L，肌酐37.0μmol/L，白蛋白22.6 g/L;EBV DNA测定9.41E+03 IU/m L，巨细胞病毒(cytomegalovirus,CMV)的DNA测定1.44E+03 IU/m L; CMV的Ig G抗体(+)，CMV的Ig M抗体阴性;抗双链DNA抗体、抗核抗体、抗核抗体谱均阴性;抗心磷脂抗体检测阴性; p ANCA、c ANCA、MPO-ANCA及PR3-ANCA均阴性。     

新城镇居民自杀意念相关因素分析

目的 了解新城镇居民自杀意念的现状并分析其相关因素.方法 采用自编社会人口学问卷、PHQ-9健康问卷、广泛性焦虑量表(GAO-7)、自编自杀意念等,用便利抽样的方法对516名山东新城镇居民进行问卷调查.结果 新城镇居民过去一年内自杀意念检出率为7.33％,焦虑(OR=11.781,95％CI:4.531～30.633)是过去一年内自杀意念的危险因素,与父母关系一般(OR=0.179,95％CI:0.036～0.900)、与父母关系融洽(OR=0.084,95％CI:0.019～0.368)为保护因素.新城镇居民终生自杀意念检出率为13.88％,亲人有自杀意念(OR=8.653,95％CI:3.323～22.533)、偶尔饮酒(OR=2.579,95％CI:1,180～5.637)、睡眠异常(OR=2.602,95％CI:1.333～5.078)、焦虑(OR=3.680,95％CI:1.412～9.590)是终生自杀意念的危险因素,60岁及以上(OR=0.232,95％CI:0.100～0.537)、偶尔吸烟(OR=0.099,95％CI:0.014～0.687)为保护因素.结论 亲人有自杀意念、偶尔饮酒、睡眠异常、焦虑的新城镇居民易产生自杀意念,与父母关系融洽或一般、60岁及以上、偶尔吸烟的新城镇居民不易产生自杀意念.

从阳虚毒聚浅析王笑民教授辨治晚期肿瘤的经验。认为晚期肿瘤病机虚实夹杂,但整体为阳虚,局部以毒热为主;治疗以扶正补虚、温阳散结、清热解毒、祛瘀通络为原则;善用黄芪、党参扶正补虚,附子、干姜温阳散结,半枝莲、白花蛇舌草清热解毒,土鳖虫、穿山甲祛瘀通络。     

产后综合康复训练对产妇产后盆底肌力及性功能的影响

目的观察产后综合康复训练对产妇产后盆底肌力及性功能的影响。方法随机将2017年12月至2018年12月因盆底肌力降低于我院治疗的80例产妇作为本次实验的研究对象,随机将这80例患者分成实验组和对照组。对照组的患者采用常规的治疗方法,在产后对患者进行健康教育,指导患者的饮食和基本生活,并对患者进行恢复盆底肌力的指导;实验组的患者在进行对照组患者治疗的同时,接受电刺激治疗和穴位按摩治疗。结果实验前,实验组和对照组的盆底肌力和性功能差异没有统计学意义(P>0.05);在完成治疗之后,实验组的盆底肌力和性功能均明显优于对照组,且差异具有统计学意义(P<0.05);且实验组并发症的出现情况也明显的少于对照组,差异具有统计学意义(P<0.05)。结论产后综合康复训练对产妇产后盆底肌力及性功能有着较好的影响。
