BackgroudOutcomes of traditional treatment for osteonecrosis of the femoral head (ONFH) are not always satisfactory. Hence, cell-supplementation therapy has been attempted to facilitate necrotic-tissue regeneration. Adipose-derived mesenchymal stem cell (ADMSC) transplantation is potentially advantageous over bone marrow-derived MSC implantation, but its outcomes for ONFH remain unclear. The aim of this study was to determine 2-year radiological and clinical outcomes of culture-expanded autologous ADMSC implantation for ONFH.MethodsEighteen hips with necrotic lesions involving ≥ 30% of the femoral head were included. ADMSCs were harvested by liposuction and culture expanded for 3 passages over 3 weeks. With a 6-mm single drilling, ADMSCs were implanted into the necrotic zone. All patients underwent magnetic resonance imaging (MRI), single-photon emission computed tomography/computed tomography (SPECT/CT) at screening and 6 months, 12 months, and 24 months postoperatively. The primary outcome was the change in the size of necrotic area on MRI. Secondary outcomes were changes in clinical scores and radioisotope uptake on SPECT/CT. Conversion total hip arthroplasty (THA) was defined as the endpoint.ResultsPreoperatively, the necrotic lesion extent was 63.0% (38.4%–96.7%) of the femoral head. The mean Harris hip score was 89.2, the University of California at Los Angeles (UCLA) score was 5.6, and Western Ontario and McMaster Universities Arthritis index (WOMAC) was 79.4. Three patients underwent THA and 1 patient died in an accident. Finally, 11 patients (14 hips) were available for ≥ 2-year follow-up. At the last follow-up, no surgery-related complications occurred, and 14 of 17 hips (82%) were able to perform daily activities without THA requirement. There was no significant decrease in lesion size between any 2 intervals on MRI. However, widening of high signal intensity bands on T2-weighted images inside the necrotic lesion was observed in 9 of 14 hips (64%); 11 of 14 hips (79%) showed increased vascularity on SPECT/CT at 2 years postoperatively. No significant differences were observed between preoperative and 24-month mean Harris hip score (89.2 vs. 88.6), WOMAC (79.4 vs. 75.7), and UCLA score (5.6 vs. 6.2).ConclusionsOur outcomes suggest that culture-expanded ADMSC implantation is a viable option for ONFH treatment without adverse events.  相似文献

目的 描述中国成年人的共病流行情况及其长期变化,以及常见的共病模式。方法 本研究纳入参与中国慢性病前瞻性研究第二次重复调查的25 033人,利用研究对象参加2004-2008年基线调查和2013-2014年第二次重复调查时采集的信息进行分析。根据自报疾病史、现场体格检查及血液样本检测等信息定义了13种慢性病或健康问题,将共病定义为同时患有≥2种慢性病或健康问题。采用系统聚类分析法描述共病模式。结果 研究对象在基线调查时的年龄为(51.5±10.1)岁,第二次重复调查时为(59.5±10.2)岁。2次调查平均间隔时间(8.0±0.8)年,共病率由33.5%上升至58.1%,人均患病数由1.15个增加至1.82个,平均每5年增长0.42个患病数。年长者、城市人群、文化程度低者的共病率较高,且患病数量随年龄的增速更快。吸烟及过量饮酒者的患病数量随年龄增速也更快。该人群中最常见的共病组合为:超重肥胖、高血压、糖尿病、中风和冠心病。结论 我国成年人群共病率较高,且随年龄增长而增加。共病情况在不同地区、文化程度水平和不同生活方式人群中存在差异。  相似文献

目的 量化分析1990-2019年我国动脉粥样硬化心血管病(ASCVD)疾病负担及其主要危险因素影响的变化趋势。方法 利用2019年全球疾病负担(GBD2019)研究结果,以伤残调整寿命年(DALY)及其年龄标准化率为指标,分析1990-2019年我国ASCVD疾病负担和危险因素归因疾病负担的变化情况,并采用Gupta建立的分解法对人口增长、老龄化、年龄别患病率和疾病严重程度所致的DALY变化进行了量化分析。结果 2019年我国61.00%的心血管病疾病负担由ASCVD所致,缺血性心脏病的DALY较1990年增长了133.66%,其中29.57%可归因于人口增长、108.74%归因于人口老龄化、8.87%归因于年龄别患病率的增加、-13.53%归因于疾病严重程度;缺血性卒中的DALY较1990年增长了138.64%,归因于上述4部分的变化率依次为30.95%、123.38%、55.80%和-71.49%。2019年高血压依然是ASCVD首要危险因素,其次是高LDL-C,归因于饮酒的年龄标准化DALY率较1990年增幅最大(486.01%),年均增长10.93%。结论 1990-2019年,人口老龄化是我国ASCVD疾病负担大幅增长最主要的原因,而其他可改变危险因素的不利趋势,特别是代谢性危险因素的持续流行同样不容忽视。  相似文献

目的了解2011年与2017年南京25岁及以上人群归因于豆类摄入不足的缺血性心脏病(IHD)疾病负担和期望寿命损失情况,为IHD的饮食防控策略提供数据支撑。方法本研究资料来源于2011年和2017年南京市慢性病及其危险因素监测、南京市死因监测和2017全球疾病负担(GBD)资料,采用GBD团队提供的Excel插件程序计算人群归因分值(PAF),估算豆类摄入不足导致的IHD死亡和寿命损失。使用SPSS 20.0软件进行t检验。结果2017年,南京市因豆类摄入不足导致IHD的死亡例数占IHD总死亡例数的13.73%,较2011年(12.44%)增长了10.37%。2017年南京市归因于豆类摄入不足的IHD死亡例数和过早死亡损失寿命年(YLL)分别为645例、8116人年,较2011年(449例、6610人年)分别上升43.65%、22.78%;而标化归因死亡率和标化YLL率分别由2011年的11.05/10万、155.72/10万下降至2017年的8.86/10万、118.66/10万,下降率分别为19.82%、23.80%。2017年,豆类摄入不足导致的IHD死亡使期望寿命损失0.17岁,男性、女性分别损失0.13、0.21岁;与2011年相比,2017年男性、女性及总人群归因于豆类摄入不足的期望寿命损失均上升。结论豆类摄入不足已成为影响南京市居民死亡和期望寿命的饮食危险因素之一,应积极倡导合理膳食,增加居民豆类摄入水平,从而减少心血管疾病负担。  相似文献

Responses of 67 Purkinje cells (P-cells) and 44 unidentified neurons (U-cells) located in the cerebellar anterior vermis were recorded in decerebrate cats during off-vertical axis rotation (OVAR). This stimulus consisted of a slow constant velocity (9.4°/s) rotation in the clockwise (CW) and counterclockwise (CCW) directions around an axis inclined by 5° with respect to the vertical. OVAR imposes on the animal head a 5° tilt, whose direction changes continuously over the horizontal plane, thus eliciting a selective stimulation of macular receptors. A total of 27/67 P-cells (40%) and 24/44 U-cells (55%) responded to both CW and CCW rotations. For these bidirectional units, the direction of maximum sensitivity to tilt (Smax) could be identified. Smax directions were distributed over the whole horizontal plane of stimulation. Among bidirectional neurons, 48% of the P-cells and 33% of the U-cells displayed an equal amplitude of modulation during CW and CCW rotations, indicating a cosinetuned behaviour. In these instances, the temporal phase of the unit response to a given direction of tilt remained constant, while the sensitivity was maximal along the Smax direction and declined with the cosine of the angle between Smax and the tilt direction. The remaining bidirectional units displayed unequal amplitudes of modulation during CW and CCW rotations. For these neurons, a nonzero sensitivity along the null direction was expected and the response phase varied as a function of stimulus direction. Finally, 31% and 23% of P-cells and U-cells, respectively, responded during OVAR in one direction only (unidirectional units). This behaviour predicts equal sensitivities along any tilt direction in the horizontal plane and a response phase that changes linearly with the stimulus direction. The possibility that the tested neurons formed a population which coded the direction of head tilt in space was also investigated. The data from the whole population of cells were analysed using a modified version of vectorial analysis. This model assumes that for a particular tilt each cell makes vectorial contributions; the vectorial sum of these contributions represent the outcome of the population code and points in the direction of head tilt in space. Thus, a dynamic head tilt along four representative directions was simulated. For each of the four directions, 12 population vectors were calculated at regular time intervals so as to cover an entire cycle of head tilt. The results indicate that for each selected time in the cycle the direction of the population vector closely corresponded to that of the head tilt, while its amplitude was related to the amount of head tilt. These data were particularly obtained for the P-cells. In view of their efferent connections, the cerebellar anterior vermis may provide a framework for the spatial organization of vestibulospinal reflexes induced by stimulation of otolith receptors.  相似文献

Summary The genetic polymorphism of transferrin (Tf) was studied in a sample of 385 healthy unrelated subjects of both sexes resident in the province of Cádiz (southern Spain). Isoelectric focusing was carried out in polyacrylamide gels, followed by staining with Coomassie Blue R250. The membrane properties of the neurons located in the ventral part of the periaqueductal grey (PAG) of the guinea-pig were studied using an in vitro slice preparation. Cells had low values of resting membrane potential (-53.3 +/- 1.3 mV, mean +/- standard error), high input resistance (195. +/- 16.2 M ohm) and moderate values of membrane time constant (12.6 +/- 0.7 ms). The last two parameters changed as recordings were made along the sagittal axis, higher values corresponding to the more rostral cells. Three main neuronal types-fusiform, triangular and stellate-were found in the ventral PAG using intracellular injection of Lucifer yellow. A study of the cell number and cell density was carried out in coronal and sagittal sections of the ventral PAG. This analysis showed a clear gradient of size in this region arising from the gradual disappearance of large (17 to 40 microm) neurons in the caudorostral direction. The neuronal density also increased in this direction. Therefore, some electrotonic and morphological parameters differ along the sagittal axis. These findings suggest a larger neuronal heterogeneity of the caudal part of the PAG, and might contribute to a functional segregation of this region.  相似文献

Emulsions (o/w) were prepared from solid-state emulsions comprised of various matrix materials and oils and the resultant particle size properties determined. Results suggest that for those matrices that can form solid-state emulsions, the droplet size decreased as a function of time, as previously observed. The final droplet size was dependent on the oil utilized but was independent of the matrix material. The use of mineral oil resulted in the smallest droplet diameter (1.5 µm) while isopropyl myristate resulted in the largest droplet diameter (3 µm). With the exception of mineral oil, the oil/water interfacial tension was found to be directly proportional to the droplet diameter. The rate of emulsification appeared to be bi-phasic. The initial emulsification phase appeared to be independent of the matrix material while the terminal phase was a function of the matrix material. Most importantly, it was found that solid state emulsions could be prepared from a diverse, yet specific, list of matrices.  相似文献

The pattern electroretinogram and the visual evoked potential were recorded simultaneously with various stimulus fields and artificial scotomata of increasing sizes. In contrast to an earlier study, a smaller check size (20) and two stimulus field sizes (20° × 20° and 10° × 10°) for the scotomata were used. With a concentric decreasing stimulus field, a reduction of both the pattern electroretinogram and visual evoked potential was found. Both showed a simultaneous reduction of amplitudes, but, compared with the amplitude in the full field, the reduction was more extensive for the pattern electroretinogram at each test field size. This implies a greater contribution to the pattern electroretinogram from more eccentric retinal parts. An artificial central scotoma of increasing size in the 20° × 20° field had less influence on the pattern electroretinogram than on the visual evoked potential. The percentage amplitude loss of the visual evoked potential was more pronounced. The visual evoked potential was eventually abolished by a scotoma size from 10° × 10° upward, while the pattern electroretinogram was still registrable. When scotomata of similar size were introduced in a smaller (10° × 10°) field, percentage pattern electroretinogram and visual evoked potential amplitude losses were less separated than in a larger (20° × 20°) test field.  相似文献
