目的探讨异位妊娠早期诊断、早期治疗对患者预后的影响。方法76例确诊异位妊娠的患者,按停经天数≤40 d及>40 d分组(各38例)。两组均采用了保守及手术等个性化治疗。结果停经天数≤40d组保守治疗成功率75.0%,停经天数>40 d组保守治疗成功率45.5%,两组差异有统计学意义(P<0.05)。停经天数≤40 d组手术18例,术中腹腔出血>500 ml者7例,其中>1000 ml者3例;停经天数>40 d组手术27例,术中腹腔出血>500 ml者21例,其中>1000 ml者16例,两组差异有统计学意义(P<0.05)。结论提高异位妊娠的早期诊断并得以早期住院治疗,可降低患者的手术率和腹腔内大量出血的比率。 相似文献

目的比较草酸铂及羟基喜树碱联合亚叶酸钙和氟尿嘧啶(5-FU)辅助化疗方案治疗进展期结直肠癌的疗效和不良反应。方法选择某院自2009年12月~2010年5月经病理证实的进展期结直肠癌患者98例,其中49例(OH组)采用草酸铂130mg/m2联合羟基喜树碱6.0mg/m2,5-FU1000(mg/m2.d)持续静脉泵入1~2d,亚叶酸钙200mg/(m2.d),连续化疗92个周期;49例(HLF组)采用羟基喜树碱6mg/m2+生理盐水250ml,5-FU1000(mg/m2.d)持续静脉泵入1~2d,亚叶酸钙200mg/(m2.d),连续化疗43个周期,2~3周期后对两组的疗效进行评价,同时观察两组的不良反应。结果 OH组、HLF组患者的近期有效率分别为34.69%(17/49)、28.57%(14/49),两组比较疗效差异无统计学意义(P﹥0.05);两组KPS改善情况比较差异无统计学意义(P﹥0.05)。不良反应主要为感觉神经末梢毒性、骨髓抑制、呕吐、腹泻等,两组间Ⅲ、Ⅳ度不良反应以血液学毒性和胃肠道反应为主,其中腹泻发生率HLF组高于OH组,比较差异有统计学意义(P﹤0.05)。结论草酸铂及羟基喜树碱联合亚叶酸钙和氟尿嘧啶治疗进展期结直肠癌,两组的疗效相似,草酸铂外周神经末梢毒性较为常见,羟基喜树碱的腹泻不良反应较为常见。 相似文献

[目的]观察和比较吉西他滨(GEM)联合奥沙利铂(LOHP)与吉西他滨联合顺铂(DDP)治疗老年晚期非小细胞肺癌的疗效和不良反应.[方法]将经病理组织学或细胞学确诊为晚期非小细胞肺癌的老年患者(年龄≥70岁)作为研究对象,46例患者随机分配到吉西他滨联合奥沙利铂组(GL)和吉西他滨联合顺铂组(GC), GL组给予GEM 1000mg/m2,静脉滴注,d1.8; LOHP 130mg/m2,静脉滴注,d1;21d为1周期,用药2周期后评价疗效及不良反应.GC组给予GEM 1000mg/m2,静脉滴注;DDP 30mg/m2,静脉滴注,d1、2、3;同时给予水化,21d为1周期,用药2周期后评价疗效和不良反应.[结果]两组均有23例患者可评价疗效和不良反应,GL组有效率为52.2%,GP组有效率为47.8%,两组不良反应主要为骨髓抑制,胃肠道反应,肾脏毒性和神经毒性等.[结论]GL方案和GC方案治疗老年晚期非小细胞肺癌疗效相似.GL方案较GC方案的不良反应轻,恢复快,更容易耐受. 相似文献

目的研究在急性缺血性卒中患者中进行超早期康复治疗能否更好地改善其日常生活的独立性。方法将96名发病24h内入院的前循环急性缺血性卒中患者随机分为2组,超早期康复组在发病24h内开始接受包括下床站立活动的康复治疗;标准康复组在发病72h至7d开始康复治疗。康复治疗均由专业治疗师进行,每日1次,每次不少于30min,共14d。主要观察指标包括恢复至独立行走50m所需的天数,次要指标包括发病后30d和90d时的NIHSS评分和Bathel指数。结果超早期康复组共纳入41名患者,标准康复组55名患者。和标准康复组相比,超早期康复组恢复至独立行走50m的时间要明显缩短[(8.71±14.162)dvs(16.64±20.338)d,t=-2.138,P=0.035)]。90d时超早期康复组患者获得良好独立日常生活能力的比例(以Bathel指数达到95~100的比例表示,75.6%vs56.4%,χ2=3.891,P=0.049)更大。其余观察指标两组间无统计学差异。结论超早期康复治疗可以使缺血性卒中患者更早地恢复独立行走能力,并能改善功能结局。 相似文献

目的观察晚期非小细胞肺癌应用吉西他滨联合顺铂治疗的疗效及不良反应。方法选择经病理组织学或细胞形态学检查确诊为非小细胞肺癌的晚期患者76例,随机分组,治疗组38例,采用吉西他滨联合顺铂治疗,给予吉西他滨1000mg/m2,d1-8,顺铂25mg/m2,d1-3,水化;每21d重复1次,连用2~6周期。对照组38例,仅采用吉西他滨1000mg/m2,d1-8。两组均给予常规止吐治疗。每2周期评价疗效及不良反应。结果治疗组的治疗疗效,完全缓解(CR)2例,部分缓解(PR)15例,稳定(SD)19例,进展(PD)2例,总有效率44.74%,对照组完全缓解(CR)1例,部分缓解(PR)13例,稳定(SD)20例,进展(PD)4例,总有效率36.84%,差异有统计学意义,P<0.05,两组化疗不良反应轻微。结论吉西他滨联合顺铂治疗晚期肺癌有较好的疗效,并且毒性反应可以控制,值得临床推广使用。 相似文献

[目的]通过检测血清血管内皮生长因子(VEGF)、绒毛膜促性腺激素(HCG)在宫内早孕(IUP)、宫内胚胎停育(aIUP)及异位妊娠(EP)中的不同浓度,探讨VEGF在停经早期鉴别EP及aIUP的意义。[方法]选择孕40d左右妇女,采用酶联免疫聚合反应(ELISA)检测20例正常IUP妇女(正常对照组)、15例aIUP(自然流产组)及20例EP妇女血清VEGF和HCG的浓度,并进行对比分析。[结果]血清VEGF浓度EP组为(295.6±52.1)pg/m l,明显高于IUP组的(40.4±21.5)pg/m l和aIUP组的(110.3±34.5)pg/m l,差异均有显著性(P<0.01,P<0.05);HCG在EP组为(2092.8±1634.1)m Iu/m l、aIUP组为(2352.4±1712.8)m Iu/m l,都明显低于IUP组的(14780.6±8777.3)m Iu/m l,差异均有显著性(P<0.01),而EP组与aIUP相比差异无显著性(P>0.05)。[结论]VEGF在EP的高表达将有助于EP的早期诊断,VEGF可用于鉴别早期EP与aIUP,与HCG相比具有优越性。 相似文献

目的 分析视屏暴露和户外活动与幼儿睡眠的相关性,为预防和干预儿童睡眠问题提供科学依据。方法 基于已建立的马鞍山市母婴健康出生队列,对2015年6月—2016年6月出生的单胎活产儿,随访至2岁,收集社会人口统计学、视屏暴露、户外活动和睡眠情况的资料。共获得有效问卷1 575份,男童833人,女童742人。使用χ2检验和Logistic回归模型对数据进行分析。结果 幼儿睡眠质量差的发生率为26.16%;电视打开时间(不一定在看)≥2 h/d、看电视≥1 h/d、玩手机≥30 min/d分别占50.73%、45.21%和26.03%,户外活动≥2 h/d的比例为71.30%;去户外场所的频率≥2次/周和在户外场所≥1 h/次的比例分别为76.31%和37.71%。调整人口统计学等混杂因素后,打开电视时间(不一定在看电视)≥2 h/d(OR=1.40,95%CI:1.11~1.77)、看电视时间≥1 h/d(OR=1.39,95%CI:1.11~1.75)、玩手机时间≥30 min/d(OR=1.55,95%CI:1.21~1.99)均会增加幼儿睡眠质量差发生的风险;去户外场所的频率≥2次/周(OR=0.72,95%CI:0.55~0.93)和在户外场所时间≥1 h/次(OR=0.78,95%CI:0.62~0.99)则能降低发生幼儿睡眠质量差的风险。结论 幼儿睡眠问题存在一定比例,视屏暴露和户外活动均与幼儿睡眠相关,在儿童保健门诊中应对家长进行科学指导,减少视屏暴露时间,增加户外活动,以减少儿童睡眠问题的发生。 相似文献

We carried out a longitudinal study on the associations between residential greenness and depression risk in urban areas in Finland. Residential greenness indicators were estimated within various buffer sizes around individuals' home locations (selected n = 14424) using time-series of normalized differential vegetation index (NDVI) and CORINE land cover data (CLC). We estimated individuals’ cumulative exposure to residential greenness over a 5-years and 14-years follow-up. We used doctor-diagnosed depression and Beck Depression Inventory for depression assessment. Our multi-logistic model showed an inverse association between residential greenness and depression, implying lowered depression risk for individuals with higher residential greenness. The association was particularly evident when using NDVI-based residential greenness (within a buffer of 100 m radius) and doctor-diagnosis depression data, adjusted with individual-level covariates. The odds ratio was 0.56 (95% CI 0.33 to 0.96) for the 5-years follow-up, and 0.54 (95% CI 0.30 to 0.98) for the 14-years follow-up. The associations between CLC-based total residential green space and depression varied across the different buffer sizes. In general, all the associations depended on the type of depression assessment, quality of greenness indicators, and the spatial scale of analysis. 目的 研究高校新生的睡眠质量及体重指数现状,探讨二者的相关性。方法 采用多阶段分层整群随机抽样的方法,应用匹兹堡睡眠质量指数量表和自编调查问卷,对12323名新生进行问卷调查,并测量身高、体重。结果 体重过轻、正常体重、超重、肥胖的检出率分别为21.75%、65.70%、9.15%、3.42%,性别、家庭所在地、学校类型及午睡习惯不同,BMI得分差异均有统计学意义;平均睡眠时间为(7.03±1.04)小时,PSQI得分为 0~17 分,总均分为(5.31±2.63)分,睡眠障碍检出率为18.32%;不同BMI分组下睡眠障碍检出率差异有统计学意义,肥胖组的睡眠障碍检出率最高;多因素logistic 回归分析显示,性别(OR=0.707, 95%CI:0.650~0.768)、家庭所在地(OR=1.151, 95%CI:1.053~1.256)、肥胖(OR=1.458, 95%CI:1.187~1.790)、午睡习惯(OR=1.380, 95%CI:1.251~1.522)是新生睡眠障碍的影响因素(P值均<0.05)。结论 高校新生睡眠质量问题与肥胖关系密切,合理的体重及良好的睡眠习惯控制有助于改善睡眠质量。 相似文献

We assessed the possible association of gastrointestinal cancers with cruciferous vegetables and mushrooms in a multicenter, hospital-based case-control study in an agricultural area of Japan. One hundred forty-nine cases and 287 controls for stomach cancer and 115 cases and 230 controls for colorectal cancer were matched by age, sex, and residential area. In stomach cancer, the protective effect of vegetables (consumption of total vegetable) was obscure, but it became clearer when we examined specific kinds of vegetables. Marginal associations were observed in the group of the highest consumption of Chinese cabbage (odds ratio [OR] = 0.61; 95% confidence interval [CI] = 0.35-1.07), broccoli (OR = 0.60; 95% CI = 0.34-1.08), Hypsizigus marmoreus (Bunashimeji) (OR = 0.57; 95% CI = 0.31-1.04) and Pholita nameko (Nameko) (OR = 0.56; 95% CI = 0.30-1.06). In colorectal cancer, we observed decreased risks from the highest tertile of total vegetables (OR = 0.22; 95% CI = 0.08-0.66) and low-carotene-containing vegetables (OR = 0.28; 95% CI = 0.08-0.77), and inverse associations were observed in the group of the highest consumption of broccoli (OR = 0.18; 95% CI = 0.06-0.58). Although the sample size was limited, subgroup analyses showed that the associations differed with the histopathological subtype. These findings suggest that cruciferous vegetables decrease the risk of both stomach and colorectal cancer, and that mushrooms are associated with a decreased risk of stomach cancer. 相似文献

This study examined relationships between greenness exposure and free-living physical activity behavior of children in smart growth and conventionally designed communities. Normalized Difference Vegetation Index (NDVI) was used to quantify children's (n=208) greenness exposure at 30-s epoch accelerometer and GPS data points. A generalized linear mixed model with a kernel density smoothing term for addressing spatial autocorrelation was fit to analyze residential neighborhood activity data. Excluding activity at home and during school-hours, an epoch-level analysis found momentary greenness exposure was positively associated with the likelihood of contemporaneous moderate-to-vigorous physical activity (MVPA). This association was stronger for smart growth residents who experienced a 39% increase in odds of MVPA for a 10th to 90th percentile increase in exposure to greenness (OR=1.39, 95% CI 1.36-1.44). An individual-level analysis found children who experienced >20 min of daily exposure to greener spaces (>90th percentile) engaged in nearly 5 times the daily rate of MVPA of children with nearly zero daily exposure to greener spaces (95% CI 3.09-7.20). 相似文献

The present study investigated whether associations between greenness and birth outcomes can be detected in children belonging to a Swedish birth cohort (BAMSE). Normalized difference vegetation index (NDVI) within a 500 m buffer zone around maternal address was used as estimate of greenness. Ordinary least squares and quantile regression models were performed to investigate associations between neighbourhood NDVI and birthweight (n = 2619), birth length (n = 2490) and head circumference (n = 2243). Logistic regression analyses were used to detect the association between NDVI and odds of being born as “small-” or “large-for-gestational-age”. There were no clear associations between NDVI and birth weight in the total sample. However, in a suburban sub-sample, increased NDVI levels were significantly associated with elevated birthweight of small new-borns (β2nd percentile = 276 g, 95% CI 61 to 492, p = 0.012), and significantly reduced the odds ratio (OR) for children being born as small-for-gestational-age (OR = 0.31 95% CI 0.1 to 1, p = 0.049). No significant associations were found between NDVI and birth length or head circumference. In conclusion, neighbourhood greenness appears not to be associated with birthweight as such, but rather decrease the odds of being born underweight, in particular in suburban areas. 相似文献
