The changes of intraoperative body temperature in adult liver transplantation: A retrospective study

Background: Body temperature is poorly regulated in patients with end-stage liver disease. Due to the prolonged surgery time and anhepatic time as well as the complex surgical procedures performed in liver transplantation, the body temperature fluctuates greatly. This study investigated the effect of intraoperative body temperature fluctuations on the prognosis of liver recipients. Methods: The body temperatures of liver recipients recorded from the induction of anesthesia(T 0) until the end of surgery(T 14) were retrieved. The patients were divided into two groups: the hypothermia group( 35 °C and ≥ 5 min) and the normothermia group( ≥ 35 °C or 35 °C but 5 min). Intraoperative and postoperative variables were compared between the two groups, and the correlations between the duration of hypothermia and the medical variables were analyzed. Results: Of the 107 patients, 67 patients were in the normothermia group, and 40 in the hypothermia group. The lowest body temperature was at 5 min after reperfusion for the whole cohort. Compared with the normothermia group, patients in the hypothermia group were more prone to bleeding, had a longer intubation time and increased rates of bacterial infection and acute pulmonary edema after liver transplantation( P 0.05). Hypothermia time was positively correlated with bleeding volume, intubation time, units of blood transfusions and intensive care stay, but negatively correlated with urine output. Conclusions: The intraoperative body temperature exhibited a graphical "V" trend, and the lowest temperature was at 5 min after reperfusion. The longer the duration of hypothermia, the more unfavourable the prognosis. 相似文献

Plasma levels of 17,20β-dihydroxy-4-pregnen-3-one (17,20βP), and the timing of ovulation were investigated in female Arctic charr (Salvelinus alpinus) reared at 5 °C and at 10 °C during the pre-spawning period. The effects of switching from 5 to 10 °C, and from 10 to 5 °C were also investigated. 17,20βP plasma levels were higher at 5 °C than at 10 °C. A switch from 10 to 5 °C stimulated 17,20βP secretion, whereas a switch from 5 to 10 °C had the opposite effect. Ovulation occurred spontaneously in the females kept at 5 °C, and in those switched from 10 to 5 °C. In contrast, ovulation was inhibited in females reared at 10 °C, and in those switched from 5 to 10 °C. Oocyte maturation at 5 °C and at 10 °C in the presence of LH or of 17,20βP was also investigated in vitro using donor females reared at 5 or 10 °C. Both LH and 17,20βP stimulated oocyte maturation more effectively in oocytes incubated at 5 °C than at 10 °C. At both incubation temperatures, the rearing temperature of the donor females had a significant impact on their responsiveness to LH stimulation, but had no effect on their responsiveness to 17,20βP stimulation. In addition to the inhibition of LH secretion, which had already been reported, the results reported here show that in Arctic charr raising the temperature above the physiological range reduces both follicular responsiveness to LH stimulation and the sensitivity of oocytes to 17,20βP stimulation. 相似文献

AIM: To discuss the relationship between onset of peptic ulcer (PU) and meteorological factors (MFs). METHODS: A total of 24 252 patients were found with active PU in 104 121 samples of gastroscoic examination from 17 hospitals in Nanning from 1992 to 1997. The detectable rate of PU (DRPU) was calculated every month, every ten days and every five days. An analysis of DRPU and MFs was made in the same period of the year. A forecast model based on MFs of the previous month was established. The real and forecast values were tested and verified. RESULTS: During the 6 years, the DRPU from November to April was 24.4 -28.8%. The peak value (28.8%) was in January. The DRPU from May to October was 20.0-22.6%, with its low peak (20.0%) in June. The DRPU decreased from winter and spring to summer and autumn (P<0.005). The correlated coefficient between DRPU and average temperature value was -0.8704,-0.6624,-0.5384 for one month, ten days , five days respectively (P<0.01). The correlated coefficient between DRPU and average highest temperature value was -0.8000, -0.6470,-0.5167 respectively (P<0.01). The correlated coefficient between DRPU and average lowest temperature value was -0.8091, -0.6617, -0.5384 respectively (P <0.01). The correlated coefficient between DRPU and average dew point temperature was -0.7812, -0.6246, -0.4936 respectively (P <0.01). The correlated coefficient between DRPU and average air pressure value was 0.7320, 0.5777, 0.4579 respectively (P <0.01). The average temperature, average highest and lowest temperature, average air pressure and average dew point temperature value of the previous month, ten days and five days could forecast the onset of PU, with its real and forecast values corresponding to 71.8%, 67.9% and 66.6% respectively. CONCLUSION: DRPU is closely related with the average temperature, average highest and lowest temperature, average air pressure and average dew point temperature of each month, every ten days and every five days for the same period. When MFs are changed, the human body produces a series of stress actions. A long-term and median-term based medical meteorological forecast of the onset of PU can be made more accurately according to this. 相似文献

目的 进行利用剪切波弹性成像(Shear wave elastography,SWE)监控肝脏温度变化的实验研究,以探讨SEW是否可用于肝肿瘤光热消融治疗时正常肝脏组织温度值的精准评价。方法 以水浴法对新鲜离体猪肝进行加热。利用SWE结合肝组织病理切片分析肝组织自35℃升温至85℃时的组织硬度值;利用SWE结合肝组织病理切片分析肝组织维持一定温度10分钟后组织硬度值的变化。结果 在55℃~85℃时,肝组织硬度值随温度增高而增加,各温度组间比较有统计学差异(P<0.05),肝组织病理切片有明显变化。当肝组织温度为55-80时,肝组织维持温度值10分钟前后肝组织硬度值比较均无统计学差异(P>0.05),肝组织病理切片有明显变化。结论 肝组织温度值为55℃~80℃时,可以通过SWE测量肝组织硬度值这一参数精确评价光热治疗时正常肝组织的温度值变化。 相似文献

目的:通过室内质量控制提高血药浓度监测的质量。方法:采用酶放大免疫分析技术(EMIT)法测定他克莫司血药浓度低、中、高质控品各85个点,应用Excel自动作图功能绘制质控图,对监测过程进行回顾性质量分析;分别在21、22、23、24、25℃,测定低、中、高质控品各5份,考察温度对血药浓度测定结果的影响。结果:低、中、高质控品实测值各有9、1、5个点超出质控的允许误差范围;3水平质控的RSD分别为22.6%、14.5%和12.8%;环境温度对EMIT法的监测结果有显著影响。结论:室内质量控制对于应用EMIT法监测他克莫司的血药浓度很重要,可及时预防并纠正各类误差,提高监测结果的精确性,从而对监测结果作出更精确的判断。 相似文献

目的:探讨不同条件下保存的血标本对XS-800i血液分析仪测定结果的影响.方法:观察不同温度、不同时间保存的血标本白细胞(WBC)、红细胞(RBC)、血红蛋白(HGB)、红细胞压积(HCT),平均红细胞体积(MCV)、血小板(PLT)和平均血小板体积(MPV)的变化.结果:4℃条件下保存的血标本,WBC、RBC、HGB、HCT、MCV和PLT的测定值在24h内无明显变化; MPV 8h内无明显变化,8h后升高.22℃和32℃条件下保存的血标本,WBC、RBC、HGB、HCT、MCV和PLT的测定值在12h(22℃)和6 h(32℃)内无明显变化; MPV的测定值在6 h(22℃)和4 h(32℃)内无明显变化,6h后(22℃)和4h后(32℃)明显升高,尤以32℃显著.白细胞散点图和白细胞分类在12 h时(4℃)、8 h时(22℃)和4 h时(32℃)开始发生变化.结论:抗凝血标本保存的最佳温度为4℃,22℃(室温)保存的标本应在8 h内完成检测;如果室温大于32℃,血标本采集后应在4h内完成检测. 相似文献

背景:目前温度测量多为环境测量,无法准确测量胚胎发育温度和幼体发育温度。
目的:研究基于AD595芯片的医学动植物实验用热电偶测温装置的设计方案。
方法:实验设计包括信号的输入模块,信号的放大模块和信号的显示模块。信号由接插件端口送至AD595进行电阻电容网络去噪后送给AD620进行放大。放大后的信号经数模转换后送给单片机,最后通过3位8段码LED进行显示。
结果与结论:缝入动物体内的K型热电偶可以精确测量出胚胎发育温度和幼体发育温度。实验发现大明虾孵化温度为20~ 22 ℃有利于雌性化。采用AD595芯片设计的热电偶测温电路方案切实可行,效果稳定。同时可以有效降低电磁干扰和误差,便于软件的升级和改进。 相似文献
