目的评价床边纤维支气管镜检查在可疑院内获得性肺炎诊断中的意义。方法对25例可疑院内获得性肺炎患者进行床边纤维支气管镜检查,同时进行痰标本和支气管肺泡灌洗液标本涂片细菌检测和培养。结果1例为肺出血,2例肺不张,1例肺水肿;21例为院内获得性肺炎,其中14例(72.7%)细菌培养阳性,革兰阴性杆菌占57.1%(12/21),以铜绿假单胞菌、不动杆菌属、嗜麦芽假单胞菌、肺炎克雷伯菌最常见,革兰阳性球菌2例(9.5%),主要是耐甲氧西林金黄色葡萄球菌。痰涂片和支气管肺泡灌洗液涂片阳性者分别为3例和10例。支气管肺泡灌洗液细菌涂片阳性率和细菌培养阳性率均高于痰标本(P=0.02和P=0.005)。结论床边纤维支气管镜检查有助除外疑似院内获得性肺炎的非感染性疾病。支气管肺泡灌洗液标本病原学检测优于痰标本。 相似文献

重庆地区小儿肺炎细菌耐药性及抗生素应用分析 总被引:1,自引:1,他引:0
目的分析重庆地区小儿肺炎常见病原菌构成及其耐药性,探讨临床抗生素的合理应用.方法吸取1 165例小儿肺炎下呼吸道分泌物,利用microscan全自动微生物分析仪进行细菌鉴定、培养及药敏试验,并统计临床初始抗生素应用情况.结果细菌总分离数为392株(33.7%),其中革兰阴性菌257株(65.6%),前5位依次为肺炎克雷伯菌、流感嗜血杆菌、副流感嗜血杆菌、铜绿假单胞菌及大肠埃希菌;革兰阳性细菌为135株(34.4%),依次为肺炎链球菌、金黄色葡萄球菌、表皮葡萄球菌.药物敏感实验显示:铜绿假单胞菌与葡萄球菌呈多重耐药,其它细菌对抗生素敏感性差异较大,较敏感的有头孢呋辛、头孢曲松等;而泰能、阿米卡星、万古霉素则高度敏感.1 165例小儿肺炎使用抗生素种类达21种,使用率100%.单用一种者占38.5%,联合二种者占61.5%;无一例首选万古霉素或泰能.初始抗生素治愈率为76.7%(894/1 165例),好转率为18.5%(216/1 165例).结论重庆地区小儿肺炎病原菌以革兰阴性菌占优势,不同细菌对抗生素敏感性差异较大;临床多选用二、三代头孢菌素.积极监测细菌耐药性,有助于提供选择抗生素的依据,对提高疗效和避免耐药菌株的快速增长有重要意义. 相似文献

This article describes the pathological studies of fatal severe acute respiratory syndrome (SARS) in a 73-year-old man during an outbreak of SARS in Taiwan, 2003. Eight days before onset of symptoms, he visited a municipal hospital that was later identified as the epicenter of a large outbreak of SARS. On admission to National Taiwan University Hospital in Taipei, the patient experienced chest tightness, progressive dyspnea, and low-grade fever. His condition rapidly deteriorated with increasing respiratory difficulty, and he died 7 days after admission. The most prominent histopathologic finding was diffuse alveolar damage of the lung. Immunohistochemical and in situ hybridization assays demonstrated evidence of SARS-associated coronavirus (SARS-CoV) infection in various respiratory epithelial cells, predominantly type II pneumocytes, and in alveolar macrophages in the lung. Electron microscopic examination also revealed coronavirus particles in the pneumocytes, and their identity was confirmed as SARS-CoV by immunogold labeling electron microscopy. This report is the first to describe the cellular localization of SARS-CoV in human lung tissue by using a combination of immunohistochemistry, double-stain immunohistochemistry, in situ hybridization, electron microscopy, and immunogold labeling electron microscopy. These techniques represent valuable laboratory diagnostic modalities and provide insights into the pathogenesis of this emerging infection. 相似文献

Severe acute respiratory syndrome (SARS) is the first new epidemic of the twenty - first century. A novel coronavirus (SARS - CoV) has been identified as the causative agent of SAP, S. The genome of SARS - CoV has 29,727 nucleatides in length. The genome organization, with 11 open reading flames, is similar to that of conronaviruses.Phylogenetic analyses and sequence comparisons showed that SARS- CoV is not closely related to any of the known coronaviruses, indicating neither a mutant nor recombinant of well -characterized coronaviruses. It is a complete new coronavirus from nonhuman hostPathological studies show that severe immune response, associated to cytokine dysregulation, may be related to the lung damage of fatal SRAS. Recombination of genomes of wild - type strains with vaccine coronavirus is a potential risk associated with the application of living attenuated coronavirus vaccines. The proteinases, controlling the activities of the SARS- CoV replication, and spike protein, involved in viral entry and pathogenesis, represent attractive targets of anti- SARS drug development. Comparative full-length genome sequence analysis of 14 SARS coronavirus isolates suggests a remarkable genetic conservation of the virus. Anti - SARS vaccine and drug development will benefit from this genetic conservation. SARS-CoV is not likely to change rapidly and thus may not readily mutate to a benign infection. The progress in anti - SARS research has been impressive. Central amygdaloid nucleus lesions in rats had no effect on recovery of preoperative body weight and food consumption levels. The brain damaged rats also recovered preoperative levels of water consumption as rapidly as control rats but then developed a mild but persistent hypodipsia. The experimental rats also drank less than control rats when food deprived and showed marginally reliable decreases in 0.1% quinine solution consumption and latency to consume a novel food. There was no detectable lesion effect on 0.1% saccharin solution consumption, exploration of a novel environment or formation of a learned taste aversion. It is suggested that the central amygdaloid nucleus has a role in mediating the relationship between food and water intake and in some taste mediated consummatory behavior. 相似文献

Neopterin and C-reactive protein (CRP) concentrations were determined in serum samples from 129 severe acute respiratory syndrome (SARS) patients and 156 healthy blood donors. In the patients with confirmed SARS, an early neopterin elevation was detected already at the day of onset of symptoms and rose to a maximum level of 45.0 nmol/L 3 days after the onset. All SARS patients had elevated neopterin concentrations (>10 nmol/L) within 9 days after the onset. The mean neopterin concentrations were 34.2 nmol/L in acute sera of SARS patients, 5.1 nmol/L in convalescent sera, and 6.7 nmol/L in healthy controls. In contrast, the mean CRP concentrations in both acute and convalescent sera of SARS patients were in the normal range (<10 mg/L). Serum neopterin level in SARS patients was associated with fever period and thus the clinical progression of the disease, while there was no significant correlation between the CRP level and the fever period. Serum neopterin may allow early assessment of the severity of SARS. The decrease of neopterin level was found after steroid treatment, which indicates that blood samples should be collected before steroid treatment for the neopterin measurement. 相似文献
