目的探讨持续正压通气辅助治疗高血压并发睡眠呼吸暂停综合征(SAS)对血压的影响。方法41例高血压并发睡眠呼吸暂停综合征病人,随机分为常规组(21例)和持续正压通气治疗组(20例)均给予常规降压药物治疗4周,治疗在常规治疗基础上加用气道持续正压通气治疗,观察两组治疗前后24 h血压变化。结果治疗组治疗后24 h平均收缩压、舒张压进一步降低,夜间收缩压及舒张压则显著降低(P〈0.01)。结论高血压并发睡眠呼吸暂停综合征病人,在常规应用降压药物同时应用持续正压通气辅助治疗血压下降更理想。  相似文献

目的 研究腹外侧视前区(VLPO)γ-氨基丁酸(GABA)能神经元对大鼠睡眠—觉醒周期的影响。方法 采用脑立体定位、核团微量注射和多导睡眠描记技术。结果 VLPO双侧分别微量注射GABA合成关键酶(谷氨酸脱羧酶,GAD)抑制剂3-巯基丙酸(3-MP,5μg,0.1μl),与对照组相比,注射后当日对大鼠睡眠—觉醒周期无影响;注射后第1天大鼠睡眠量减少,觉醒时间增加;第2、3天恢复正常。结论 GABA能神经元在VLPO参与大鼠睡眠—觉醒周期调节且有促睡眠作用。  相似文献

Nocturnal Sleep‐Related Eating Disorder (NSRED) is a well‐documented sleeping disorder where the person is reported to experience bizarre eating behavior during sleep. Although various causes are implicated in this disorder, role of drugs cannot be ruled out. Here we narrate an interesting rare case report of a drug‐induced new onset NSRED, where a 45‐year‐old man on zolipdem performed an unexpected and bizarre eating behavior during somnambulistic state, type of which has not been reported earlier in the literature. 目的:探讨多部位采血快速血糖仪检测大鱼际毛细血管葡萄糖用于糖尿病患者血糖监测的准确性及优缺点。方法:对我科47例住院糖尿病患者在知情同意后进行血糖检测。行正常餐实验,在空腹时和正常餐后2h分别采静脉血用葡萄糖氧化酶法测定血糖、采手指血用乐康全血糖仪测定血糖、采手掌大鱼际血用Free- style多部位采血血糖仪检测血糖;同时进行采血疼痛感和使用方便性的主观调查,分最好、好、一般和差4级,转换为数字1、2、3、4进行评价。结果:3组的空腹血糖值差异无显著性(P>0.05);餐后2h血糖差异有显著性(P<0.05)。3组的空腹血糖及餐后2h血糖均有良好的线性相关(r=0.759~0.795,P<0.001)。3个部位的测试结果间均呈良好的线性相关,决定系数(R~2)=0.576~0.651(P<0.001)。两种血糖仪的使用方便性无明显差异.指尖采血的疼痛性要强于手掌大鱼际采血(P<0.05)。结论:多部位快速血糖仪(Freestyle)手掌大鱼际采血检测血糖的结果可靠,是糖尿病患者长期监测血糖的一种较为准确、方便、痛苦较少的方法。  相似文献

目的:探讨慢性阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopneasyndrome,OSAHS)患者心肌损伤、肾功能不全患病率其及与OSAHS病情严重程度的关系。方法:回顾性收集98例OSAHS患者的临床资料,并设立正常健康对照组40例,根据患者呼吸暂停低通气指数(apnea-hypopnea index,AHI)、最低血氧饱和度(SaO2m in)的水平将OSAHS患者分成轻度、中度、重度组。计算各组心肌损伤、肾功能不全患病率,探讨血清脂蛋白Lp(a)、心肌肌钙蛋白I(CTnI)及肌酐(Cr)水平的变化及与OSAHS病情严重程度的相关性。结果:OSAHS与对照组相比,Lp(a)、CTnI和Cr水平明显增高,差异有统计学意义(P<0.01);随着OSAHS病情严重程度分级的升高,血清脂蛋白(a)、心肌肌钙蛋白I及Cr浓度在升高,且三组间有统计学差别(P<0.01);OSAHS心肌损伤、肾功能不全的患病率分别为51.02%、24.48%,并且随着OSAHS病情严重程度分级的升高,心肌损伤、肾功能不全患病率也在升高。结论:随着OSAHS病情严重程度分级的升高,Lp(a)、CTnI和Cr水平在升高,心肌损伤、肾功能不全患病率也在升高,提示OSAHS患者中心肌损伤、肾功能不全较常见,Lp(a)、CTnI和Cr可以作为OSAHS病情严重程度的评价指标之一。  相似文献

Abstract  Although respiration in trained canines is well investigated, the process of preparing dogs has not been described in any great detail. Moreover, their daytime patterns of sleep and wakefulness during 1 or 2 h of electroencephalogram (EEG) and electrocardiogram (ECG) recordings are not clear. Therefore, we describe the process of selecting and training dogs, in which we recorded EEG and ECG in the laboratory. First, 14 of 1242 dogs dealt with over a 1 year period were chosen. They were trained for 2 h to lie quietly and to sleep in the laboratory; this training procedure was repeated 152 times. Three dogs were then selected and a permanent tracheostomy was performed in one. Finally, EEG and ECG were recorded with the bipolar fine needle electrodes; respiration was recorded simultaneously through a tube inserted to a tracheostomy in one dog. Wakefulness, slow wave sleep (SWS) and rapid eye movement (REM) sleep (REMS) were identified according to the EEG pattern and on the basis of the behavioral criteria. Recordings were performed 12 or 13 times in each dog. Complete sleep cycles, including wakefulness, SWS and REMS in this sequence, were observed 3.9–4.1 times. The mean duration of SWS was 2.2–4.4 min and that of REMS was 3.5–4.6 min. The REMS latency was 33.9–41.8 min. Fluctuation of heart rate with respiration, termed respiratory sinus arrhythmia, was noted in the ECG. Heart beat increased with inspiration and decreased with expiration. The present study demonstrates how to select and train sleeping dogs and shows their undisturbed daytime sleep and wakefulness patterns.  相似文献

When administered systemically, glucose attenuates deficits in memory produced by several classes of drugs, including cholinergic antagonists and opiate agonists. Glucose also enhances memory in aged rats, mice, and humans. In addition, glucose ameliorates age-related reductions in paradoxical sleep. Because deficits in paradoxical sleep are most marked in those individual aged rats that also have deficits in memory, treatments which improve one of these functions may similarly improve the other. The present experiments show that glucose attenuates deficits in paradoxical sleep and memory after atropine administration, with similar dose-response curves for both actions. In the first experiment, rats received saline, atropine (1 mg/kg), glucose (100 mg/kg) or combinations of atropine + glucose (10, 100, 250, and 500 mg/kg) 30 min before assessment on a spontaneous alternation task. In the second experiment, 3-h EEGs were assessed for spontaneous daytime sleep in rats administered saline, atropine (1 mg/kg), glucose (100 mg/kg) or combinations of atropine + glucose (10, 100 and 250 mg/kg). In both experiments, glucose significantly attenuated deficits at an optimal dose of 100 mg/kg. A third experiment assessed blood glucose levels after injections of atropine + glucose (100 mg/kg) and determined that blood glucose levels were similar to those produced by other treatments which enhance memory. These results are consistent with the view that paradoxical sleep and at least one test of memory are similarly influenced by atropine and glucose.  相似文献

Abstract We tried melatonin treatment in two patients with non-24 h sleep-wake syndrome, who did not respond to treatments by vitamin B12, bright light therapy, or hypnotics. In one patient, melatonin 5–10 mg improved difficulty in falling asleep and in waking, although it failed to improve the sleep-wake rhythm. In another patient, melatonin 3 mg successfully changed the sleep-wake rhythm from free-running pattern to delayed sleep phase pattern. However, melatonin re-administration after a 4-month drug-free interval failed to improve his free-running sleep-wake rhythm. These results suggest that melatonin acted as a sleep inducer in one patient and as a phase setter in the other, although the effect on the latter patient was transient.  相似文献

There is no universally accepted method to determine effective therapy for central sleep apnea (CSA). Continuous positive airway pressure (CPAP) applied acutely most often does not eliminate apneas and hypopneas. We hypothesized that the application of two or more therapeutic modalities after the diagnostic phase of polysomnography, a multi-modality titration study (MMTS), would identify a successful CSA treatment more often than a standard split-night study (SNS) and obviate the need for additional polysomnograms to determine a successful therapy. We retrospectively analyzed polysomnograms of patients diagnosed with CSA at our Sleep Disorders Center. We defined a therapy trial that resulted in an apnea–hypopnea index < 10 with at least one treatment modality as a therapeutic success. One hundred fifteen patients with CSA were studied. Sixty-six patients (57.4%) underwent a SNS, and 49 patients (42.6%) underwent a MMTS. SNS yielded only 8/66 (12.1%) successes on the first night, whereas a MMTS yielded 19/49 (38.8%) successes (p = 0.001, two-tailed Fishers exact). Patients who underwent a SNS eventually had similar rate of success as patients studied with MMTS (60.6 vs 63.3%, NS), but required more testing. Adaptive servo-ventilation was the most successful modality tested, yielding 36/46 (78.3%) successes. Trials of additional modalities following a failed trial of CPAP often produce a successful option that may guide therapy in patients with CSA. This approach may lead to establishing the diagnosis and treatment plans faster, while reducing unnecessary testing.  相似文献
