Gamma钉固定治疗367例股骨转子间骨折的疗效分析   总被引:2,自引:0,他引:2  
目的 探讨Gamma钉崮定治疗股骨转子间骨折的临床疗效.方法 2000年10月至2006年12月应用Gamma钉治疗367例股骨转子间骨折患者,男135例,女232例;平均年龄73.1岁(20~103岁);低能量损伤309例,高能量损伤58例;骨折根据AO分型:31-A1型117例,A2型208例,A3型42例.受伤至手术时间平均为4.7 d(1~17 d).术后定期摄片随访,采用自报告髋关节Harris评分(HHS)量表对骨折前和末次随访时的髋关节功能进行评估;记录患者对手术效果的整体满意度.结果 331例患者术后获4~106个月(平均16.1个月)随访.330例患者骨折获愈合,愈合时间平均为14.2周(12~24周);1例发生不愈合,愈合率为99.7%.28例患者术后发生并发症.根据自报告HHS量表评分:骨折前评分为31~90分,平均(84.6±10.0)分;末次随访时评分为19~90分,平均(75.8±12.8)分,差异有统计学意义(t=6.510,P=0.000).212例随访时间超过1年的患者术后1年对手术结果的满意度:满意135例(63.7%),比较满意67例(31.6%),一般6例(2.8%),不满意4例(1.9%).51.4%(109/212)的患者术后存在不同程度的疼痛或不适.结论 Gamma钉固定是治疗股骨转子问骨折的有效方法,但治疗后局部疼痛和髋关节评分下降仍较明显,手术效果还有待于进一步提高.
Abstract:
Objective To investigate the clinical effects of Gamma nails in treatment of intertroehanteric fractures.Methods From October 2000 to December 2006,we used Gamma nails to treat 367 patients with intertrochanteric fractures.They were 135 males and 232 females,aged from 20 to 103years(average,73.1 years).Low-impact injury was found in 309 patients and high-velocity injury in 58patients.By AO classification,there were 117 cases of type A1,208 cases of type A2 and 42 cases of type A3.The period from injury to operation was 4.7 days on average,ranging from 1 to 17 days.The postoperative hip radiographs and the patient's satisfaction were evaluated.The Harris hip score(HHS)self-report was used to compare hip functions before fracture and at the last follow-up. Results Of all the patients,331were followed up for 4 to106 months(mean,16.1 months).On average,99.7%of the fractures(330 cases)healed in 14.2 weeks(range,12 to 24 weeks).Postoperative complications occurred in 28 cases.The average HHS before fracture was 84.6±10.0 points(range,31 to 90),which significantly decreased at the finial follow-up to 75.8±12.8 points(range,19 to 90)(t=6.510,P=0.000).63.7%(135/212)of the patients rated the treatment outcome as excellent,31.6%(67/212)as good,2.8%(6/212)as fair,and 1.9%(4/212)as poor.51.4%(109/212)of the patients felt more or less pain in the hip. Conclusions Gamma nails are effective in treatment of intertroehanteric fractures.However,as the postoperative local pain is evident and the postoperative hip function decreases obviously,this operation needs to be improved.  相似文献

Objective To investigate clinical features,operative treatments and outcomes of ipsilateral fractures of femoral shaft and neck. Methods Forty-two cases of ipsilateral fractures of femoral neck and shaft were treated from November,2004 to November,2009 in our institution.They were 37 males and 5 females.Their ages ranged from 21 to 84 years,with an average of 59.2 years.The fracture involved 18upper shafts,17 middle shafts and 7 lower shafts.By Garden classification,there were 5 cases of type Ⅰ,10cases of type Ⅱ,21 cases of type Ⅲ,and 6 cases of type Ⅳ neck fractures.All cases were diagnosed in 3days after admission;there were no misdiagnosis or missed diagnosis.Twenty patients were treated by artificialjoint replacement(replacement group),while 22 patients by fixation with cannulated nails,intramedullary nails,or plate(fixation group).Outcomes were compared between the 2 groups. Results All cases were followed up for 1 to 3 years,with an average of 2 years.Femoral neck fractures united between 12 to 20weeks,with an average of 16 weeks;femoral shaft fractures united between 14 to 24 weeks,with an average of 18 weeks.There were significant differences between the 2 groups in average length of hospital stay,ambulation time,blood loss,postoperative activities and good to excellent rate of postoperative joint function(P<0.05).There were no significant differences between the 2 groups in the Merchan scores 1,2,and 3 years after operation for complication-free patients(P>0.05). Conclusion Surgical treatment of ipsilateral fractures of femoral neck and shaft depends on position of the shaft fracture,displacement of the neck fracture,physical constitution and age of the patient to be treated.  相似文献

Objective: To compare the clinical outcome of anterolateral minimally invasive approach versus conventional posterior approach for total hip replacement against femoral neck fractures in elderly patients.
Methods: The retrospective study was carried out on 42 patients who suffered from displaced femoral neck fractures (19 cases of Garden type Ⅲ, 23 cases of Garden type Ⅳ) treated by total hip replacement via anterolateral minimally invasive approach or conventional posterior approach by the same experienced surgeon. The average age of the patients was 78.1 years (range: 65-89 years). They were divided into anterolateral mini-invasive group (22 cases) and posterior group (20 cases). The mean time of follow-up was 13 months (range: 6-36 months). The anterolateral approach described by Hardinge goes through between anterior 1/3 and posterior 2/3 of the gluteus medius muscle, reaching the femoral neck from anterior capsule. The traditional posterior approach described by Moore (Southern incision) goes through the insertions of short external rotation muscles, reaching the femoral neck from posterior capsule. The related variables under observation were length of incision, operation time, postoperative limp, length of hospital stay and bed stay and dislolcation rate.
Results: The length of the skin incision varied between 7 cm and 12 cm with the anterolateral minimally invasive technique, compared to 15-22 cm in the conventional procedure. It took less time (average 15 minutes) to complete the anterolateral minimally invasive approach (72 15 min), compared with the conventional approach (87 min ±10 min). The average Harris hip score was 91.23±10.20 in anterolateral approach, 90.03±11.05 in the posterior approach. The average length of hospital stay for patients with the anterolateral approach was (6.4±2.2) days (range: 4-9 days), while that in posterior approach was (9.2 ±3.1) days (range: 6-13 days). The average length of bed stay was (3.4±1.1) days (range: 2-5 days) in anterolateral group and (6.2±2.8) days (range: 3-10 days) in posterior group. No patients in anterolateral group experienced dislocation. One (5%) hip in posterior approach had dislocation. Conclusions: Anterolateral mini-invasive approach can decrease trauma, operation time, length of hospital stay and bed stay and rehabilitation time. The stability and minimal muscular damage permit the acceleration of postoperative rehabilitation, which can subsequently reduce the perioperative risk in the treatment of femoral neck fractures in the elderly undergoing total hip replacement.  相似文献

The time of bony union ranged from 2 to 5 months, with the mean time of 3 months. No complications of infection, ex-tremities deformity and inner fixation failure were found. Conclusion The tuberosity avulsion fracture in children is an uncommon and special type of epiphyseal injury. These fractures occur in the narrow range of 13 to 16 years of age. The majority of patients are males, and most of them are unilateral involved. Objective To investigate surgical techniques and effects of AO distal humeral plate used as a treatment alternative for distal humerus fractures. Methods From April 2008 to July 2009, 22 cases of distal humerus fracture were treated with open reduction and internal fixation with AO distal humeral plate (DHP). They were 10 males and 12 females. Their ages ranged from 14 to 65 years (average, 40 years). According to AO classification, one case was Type A, 6 were Type B and 15 were Type C. Their elbows were not immobilized postoperatively. The mean follow-up was 15 months. Functional results were evaluated according to the Mayo elbow performance score (MEPS). Results Fractures healed in all cases. There was no hardware failure or loss of reduction. No patient complained of pain. They had an average elbow flexion of 108° (range, 60° to 130°) and an average full extension to 25° (range 0 to 80°) . The average ulnohumeral motion was 79° (range, 10° to 130°). The average MEPS was 91 points (range, 60 to 100 points). Fifteen cases scored excellent, 5 good and 2 fair. Conclusions DHP has advantages of an anatomically preshaped locking plate which may enhance anchorage in fractures difficult to manage, such as metaphyseal comminuted supra-intercondylar fractures, lower fractures with relatively small distal fragments and osteoporotic fractures, and allow early postoperative rehabilitation. However, in order to achieve an optimal result,precise location and pre-bending of the plate should be ensured.  相似文献

目的:通过对深圳市某两所小学发生的流行性腮腺炎突发疫情的流行病学特点及差异性进行分析,为制定科学、高效的防控策略提供科学依据。方法2013年5~7月深圳市大鹏新区某两所小学爆发流行性腮腺炎,以学校为整体研究对象,分别标记为学校A(24个班,学生1210例)和学校B(27个班,学生1274例),对比两所小学的疫情流行病学差异性。结果分析发现,学校A流行性腮腺炎发病率为4.30%,发病班级所占比54.17%,均较学校B1.73%和29.63%高,对比差异有统计学意义(P<0.05);分析显示学校A学生出现疫病平均年龄为(11.2±1.1)岁,较学校B(9.34±1.0)岁,对比差异明显(P<0.05);且两组疫病患儿在接种疫苗率对比上差异无统计学意义(P>0.05);但疫情发生时,学校B疫苗紧急接种率明显高于学校A,对比差异有统计学意义(P<0.05)。结论小学作为流行性腮腺炎爆发的主要场所之一,疫病爆发高峰季节前,针对易感染人群给予相应的疫苗接种等预防控制措施,同时加强流行性腮腺炎的监测,对于降低感染人群数量,减轻、遏制疫情有着积极的意义,值得相关防控部门重视。  相似文献
