长骨骨干骨肉瘤X线、CT及MRI表现   总被引:2,自引:0,他引:2  
目的 分析长骨骨干骨肉瘤X线、CT和MRI表现,探讨有关的临床特点和鉴别诊断.方法 28例长骨骨干骨肉瘤患者,均经手术与病理证实,其中病变位于股骨干18例、腓骨干4例、肱骨干4例、胫骨干2例.所有患者均行X线、CT和MR检查,对其影像学表现与手术病理结果进行对照,并由双盲法分析确认.结果 28例中,X线和CT显示广泛骨质破坏16例,骨膜反应22例.X线显示软组织肿块18例,肿瘤骨和瘤样钙化12例.CT平扫显示软组织肿块22例,增强扫描显示软组织肿块24例,肿瘤骨和瘤样钙化16例.MRI显示骨质破坏和骨膜反应10例,软组织肿块26例,其周围可见软组织水肿及骨髓水肿.骨膜反应在SE T1WI上呈等低信号,T2WI呈等信号.软组织肿块在T1WI为等信号,T2WI及STIR呈等高信号.软组织水肿及骨髓水肿在T2WI及STIR呈高信号.MRI增强检查显示病灶均呈不均匀强化,骨髓水肿和软组织肿块均见强化.结论 X线、CT和MRI从不同方面反映长骨骨干骨肉瘤的影像病理特点,其发病率低,骨破坏范围大,无病理性骨折.成骨型骨干骨肉瘤较易诊断,溶骨型应与Ewing瘤、恶性巨细胞瘤等鉴别.
Abstract:
Objective To explore the findings of diaphysial osteosarcoma in long bone on X-ray,CT and MRI, and discuss their clinical features and manifestations for differential diagnosis. Methods Twenty-eight cases with diaphysial osteosarcoma in long bone proved by surgery and pathology were reviewed retrospectively. Eighteen tumors were located in the femur, 4 in fibula, 4 in humerus and 2 in tibia. All of the patients were examined by X-ray, CT and MRI. The imaging manifestations on X-ray, CT and MRI were analyzed, and the relationship of the imaging features with the pathological types was also observed. The imaging signs were correlated with the pathologic findings with a double blind method. Results Of the 28 cases, there were 16 cases with large bone destruction, 22 cases with periosteal reaction on X-ray and CT. On X-ray, 18 cases showed soft tissue mass and 12 cases with neoplastic bone and tumor calcification.While on CT, 22 cases showed soft tissue mass on plain scan and 2 more cases displayed soft tissue mass after the injection of contrast mediun. Sixteen cases showed neoplastic bone and tumor calcification on CT.On MRI, there were 10 cases with bone destruction and periosteal reaction with iso- and hypo-intense on T1WI and iso- signals on T2WI. Twenty-six cases showed soft tissue edema and bone marrow on MRI. The soft mass were iso-signals on T1 WI and iso-hyperintense signals on T2 WI or STIR. The soft tissue edema was found hyperintense signals on T2WI or STIR. The lesions had heterogeneous enhancement especially in bone marrow with edema and adjcent soft tissue. Conclusion The X-ray, CT and MRI can reflect the pathological changes of diaphysial osteosarcoma in long bone from different aspects. Lower incidence, large bone destruction and no pathological fracture were the features of diaphysial osteosarcoma. The osteogenic type is diagnosed easily, but the osteolytic lesion should be differentiated from Ewing sarcoma, malignant giant cell tumor of bone and so on.  相似文献

目的 探讨儿童戈谢病骨骼病变的X线平片及MRI表现特点.方法 109例戈谢病患儿均行常规脊柱正、侧位及双股骨正位X线检查,同时行骨盆止位检查18例,左腕正位检查14例;14例患儿同时行股骨MR平扫.MR扫描序列为标准T1WI、T2WI、T2压脂像冠状面及矢状而扫描.综合分析其影像特点.结果 骨骼异常X线征象包括:长骨干骺端烧瓶样畸形89例(81.7%)、骨质稀疏91例(83.5%)、长骨十骺端密度不同程度的减低86例(78.9%)、骨皮质变薄69例(63.3%)、溶骨性破坏31例(28.4%)、骨硬化12例(11.0%)、骨囊性病变16例(14.7%),病理性骨折26例(23.9%),其中椎体压缩性骨折24处、股骨颈陈旧性骨折5处、脊柱弧形后凸3例、股骨头碎裂变形4例、髋关节脱位4例.,14例股骨MRI显示,T1WI及T2WI骨髓内均可见异常信号,分别表现为T1WI、T2WI信号减低4例;在T2WI及压脂像信号广泛减低的基础上出现限局性高信号或混杂信号10例.结论 儿童戈谢病骨骼病变的影像表现有一定特征性,对临床诊治及观察疗效具有较大的帮助.
Abstract:
Objective To discuss the imaging features of skeletal changes in children with Gaucher disease on X-ray and MRI images.Methods One hundred and nine children with Gaucher disease were enrolled in this study.They all received routine X-ray for spine with anterior-posterior(A-P)and lateral view and bilateral femurs with A-P view.Among them.18 patients received X-ray for pelvic with A-P view.14 patients received X-ray for left wrist with A-P view.and 14 patients received MRI scan for femur.The MRI scan included T1-weighted imaging,T2-weighted imaging and fat-suppressed T2-weighted imaging with short tau inversion recovery(STIR)sequence.The imaging features of the X-ray and MRI images were analyzed retrospectively.Results The most common feature is osteoporosis,which presented in 91 cases (83.5%).Besides this,decreased density of metaphysis occurred in 86 cases(78.9%).erlenmeyer flask deformity of metaphysis occurred in 89 patients(81.7%),thinner cortex occurred in 69 cases(63.3%),osteolytic destruction occurred in 31 cases(28.4%).pathological fractures occurred in 26 cases (23.9%),osteosclerosis occurred in 12 cases(11.0%).cystic degeneration of bone occurred in 16 cases (14.7%),and dislocation of the hip occurred in 4 cases.All 14 patients received MRI presented abnormal signals.Among them,4 patients presented low signal intensity both on T1-weighted and T2-weighted images in bone marrow;the other ten presented high signal intensity mixed in low signal intensity areas on T2-weighted and fat-suppressed T2-weighted images.Conclusions The imaging features of skeletal changes in children with Gaucher disease are of some characteristics,which could provide useful information for the clinical treatment.  相似文献

目的 分析原发性骨淋巴瘤(PLB)及"浮冰征"的影像表现和病理基础.方法 回顾性分析经病理证实且临床资料齐全的41例PLB患者的临床与影像特征.41例全部行X线检查,20例行CT检查,12例行MR检查(3例同时行增强扫描).结果 41例PLB中位于扁骨12例,脊柱8例,长骨17例,其他不规则骨4例.发病最多的部位是股骨(10例,24.4%),其次为髂骨(8例,19.5%).长骨病变位于骨干干骺处者11例.41例中无骨膜反应者37例(90.2%).CT显示骨质破坏轻微而出现软组织肿块者12例,伴有明显软组织肿块者9例,出现"浮冰征"者4例.12例MR检查结果中,10例显示软组织肿块,MRI显示病变范围全部大于CT所显示病变范围,其中1例MRI显示大范围骨质异常,但平片及CT显示骨质破坏不明显.结论 (1)骨质破坏轻而软组织肿块明显,或MRI显示骨质异常范围明显超过平片及CT所见,或MRI显示骨质异常而平片及CT显示骨质破坏较轻或未见明显骨质破坏者提示PLB.(2)"浮冰征"是PLB较为特殊的一种征象,其出现对淋巴瘤的诊断具有一定提示意义.
Abstract:
Objective To analyze the imaging features of primary lymphoma of the bone,and discuss the special feature of the "floating ice sign". Methods Forty-one cases of primary lymphoma of the bone in our unit from 1963.1-2009.6 were retrospectively studied. All 41 patients underwent X-ray examination, and 20 patients underwent CT examination, 12 patients underwent MR examination (3 cases simultaneously with enhancement).Results Involvement of the flat bone was seen in 12 cases. Vertebral column was affected in 8 cases, and 17 cases showed lesions in long bones and irregular bones were involved in 4 cases. The most common location was the femur(10, 24.4%), followed by the ilium(8, 19.5%). Lesions were found in the metaphyses of the long bone in 11 cases (64.7%). "Floating ice sign" was showed in the calcaneus of 2 patients and in the lumbar vertebra of 2 cases respectively, accounted for 9.8% of all cases. Slight bone destruction with soft tissue mass on CT image could be found in 12 cases, while obvious soft tissue mass was found in 9 cases. No periosteal reaction was found in 37 cases (90.2%). MRI examinations of 12 patients revealed soft tissue mass in 10 patients, and the extent of the lesion was larger in MR than CT. One case showed extensive bone destruction on MR but inconspicuous bone destruction on X-ray plain film and CT scan. Conclusion Slight bone destruction with conspicuous soft tissue mass, conspicuous bone destruction on MR but slight or inconspicuous bone destruction on X-ray film and CT,could strongly imply the diagnosis of primary lymphoma of the bone. "Floating ice sign" was a special imaging feature of primary lymphoma of the bone, which could be used as a clue for the diagnosis of lymphoma.  相似文献

目的 分析长骨血管瘤的影像特征,旨在提高其诊断水平.方法 回顾性分析经手术病理证实的18例长骨血管瘤患者的X线平片(14例)、CT(9例)及MRI(6例).结果 18例长骨血管瘤中髓型10例、骨膜型5例、皮质内型3例.X线表现:髓型8例,其中3例表现为典型的蜂窝状,3例为溶骨性病灶周围骨质硬化明显,单纯溶骨、毛玻璃样密度各1例;骨膜型3例,表现为骨皮质增厚、硬化;皮质内型3例,表现为边界清晰的溶骨性病灶.CT表现:髓型6例,表现为溶骨膨胀型病灶5例,其中3例可见薄厚不一的硬化边,蜂窝状骨嵴影2例,骨皮质呈筛孔状1例,垂直于骨皮质的放射状骨膜增生2例;毛玻璃样密度1例.2例骨膜型表现为骨皮质均匀增厚1例,不规则骨膜增厚1例,局部髓腔狭窄消失.1例皮质内病变表现为病灶呈均匀软组织密度,局部皮质膨胀、增厚.MRI表现:2例显示髓腔内斑片状T1WI稍低信号,T2WI均呈稍高信号,边界清晰;1例T1WI及T2WI均呈等或稍低信号,病灶与正常组织分界不清;1例显示病灶突破局部骨皮质,肿物向外突出,外突肿物及相连髓腔部分T1WI呈稍低信号,T2WI呈高信号;2例显示骨膜增厚,其中1例呈等信号,1例无信号;2例周围肌肉组织内可见不均匀斑片状长T2信号,T1WI与肌肉组织呈等信号.结论 长骨血管瘤的囊性蜂窝状X线表现较为特征型;CT及MRI有助于长骨血管瘤的诊断.
Abstract:
Objective To explore the imaing features of hemangioma in the long bone and improve the diagnostic level of this disease. Methods The X-ray(14 cases), CT(9 cases) and MRI(6 cases)findings of 18 patientswith histologically proven hemangioma in the long bone after surgery were retrospectively reviewed. Results Ten tumors occurred in medullary cavity or bone end(medullary type),6 on the surface of bone (periosteal type) and 3 in cortex (intracortical type). X-ray findings: among 8 cases of medullary type, 3 showed honeycomb appearance, 3 lytic areas with sclerotic borders, one purely osteolyticchanges, and 1 frosted glass; 3 cases of periosteal type showed sclerosis and thickening of the underlying cortex; 3 cases of intracortical type showed well-defined osteolytic foci. CT findings: among 6 cases of medullary type, 5 appeared as expansile lytic lesion with uneven selerotic rim (3 cases)orhoneycomb appearance (2 cases), 1 cribriform appearance in the cortical bone, 2 periostealnew bone formation in vertical radiation pattern; 1 ground-glass appearance; among 2 cases of periosteal typeone showed regular cortical thickening, and the other irregular periosteal proliferation with marrowing of medullarycavity; 1 case of intracorticaltype showed density similar to that of soft tissue, with cortical thickening and expansion . MRI findings: 2 apeared as well-defined lesions with low signal intensity on T1WI and high signal intensity on T2WI; 1 appeared as ill-defined lesion with low to intermediate signal intensity on T1 WI and T2 WI. One showed breakthrogh of cortex and formation of soft tissue mass with low signal intensity on T1 WI and high signal on T2WI. Two showed thickening of periosteumwith intermediate signal intensity in one of them and very low signal intensity in the other. Two showed abnormal signal intensity in surrounding muscles, which was high on T2 WI and intermediate on T1 WI. Conclusions The soap-bubble or honeycomb appearance is the typical radiographic finding of hemangioma in long bone. CT and MRI can provide useful information for the diagnosis of hemangioma in long bone.  相似文献

Objective To investigate the curative effect and feasibility of closed reduction with elastic intramedullary nailing and open reduction with Kirschner wire fixation in treatment of obviously displaced radial neck fractures in children. Methods From July 2006 to December 2007, 18 children with types Ⅱ and Ⅲ O' Brien radical neck fractures were admitted to Nanjing Children' s Hospital. Of all, six children received open reduction and fixation with Kirschner wire and six weeks of cast immobilization; the other 12 children received closed reduction and fixation with elastic intramedullary nail and three weeks of cast immobilization. The children were followed up for mean 12 months to investigate postoperative functional recovery of the elbow joint and presence of complications. Results All children achieved excellent clinical outcomes, without excessive radial bone growth, early closure of radial bone marrow, bone bridge formation or ectopic calcification around the joint. According to Tibone and Stoltz' s clinical evaluation methods, closed reduction with elastic intramedullary nailing had less trauma, faster recovery, fewer complications and better cosmesis than traditional open reduction with Kirschner wire fixation for children with obviously displaced radial neck fractures. Conclusion Closed reduction with elastic intramedullary nailing is a better option for obviously displaced radial neck fractures in children.  相似文献

目的 探讨成人白血病中枢神经系统并发症(CNSCL)的CT和MRI表现及其诊断价值.方法 回顾性分析18例经临床及手术病理证实的成人CNSCL的CT和MRI表现,其中急性淋巴细胞白血病7例,急性非淋巴细胞白血病10例,慢性粒单细胞白血病1例.CT平扫11例,其中增强1例;MR平扫16例,其中增强11例.结果 颅内受累14例:(1)颅内出血7例,其中脑内血肿4例,脑内血肿合并微出血1例,微出血2例.脑内血肿均为多发病灶.CT表现为团状高密度影;MRI表现为T1WI低信号或高低混杂信号,T2WI高信号或等高信号伴环状低信号环,病灶呈环形强化或无明显强化.脑微出血在磁敏感加权成像(SWI)上表现为多发斑点状及小条状低信号,其病灶检出率明显优于CT及MRI其他常规序列检查;7例中,伴脑梗死及蛛网膜下腔出血各1例.(2)颅内肿块5例:其中左额部内板下梭形病灶或跨颅板肿块2例,T1WI呈低信号,T2WI呈高低混杂信号,并有明显均匀强化,均见脑膜尾征;鞍区肿块1例,CT示鞍区高密度影,MRI示鞍区T1WI稍低信号,T2WI高信号,有不均匀强化;右侧脑室体旁肿块1例,T1WI、T2WI均呈等信号,有明显均匀强化.左额顶叶壁厚囊性肿块1例,呈环形强化.(3)梗阻性脑积水1例,表现为中脑导水管以上脑室系统扩张.(4)脑膜病变1例,MRI表现为广泛脑膜增厚伴明显均匀强化.椎管内病变4例:其中胸腰椎左侧椎旁软组织肿块2例,侵犯椎管内,伴邻近肋骨骨质破坏1例;椎管内肿块1例,表现为胸椎管后方梭形T1WI等高信号,T2WI等低信号灶,无明显强化;胸髓信号异常1例,表现为胸髓条状T2WI及液体衰减反转恢复(FLAIR)序列高信号影.结论 成人CNSCL影像表现多种多样,CT与MRI对该病的诊断价值相辅相成;白血病患者疑脑内病变者,建议常规使用SWI检查,以尽早发现脑微出血,降低脑内血肿发生的风险.
Abstract:
Objective To evaluate the CT and MRI findings and their diagnostic value of central nervous system complications of leukemia (CNSCL). Methods The CT and MRI findings of 18 adult patients with CNSCL proved by clinical features or pathology were retrospectively analyzed. Among 18 cases,7 were acute lymphocytic leukemia, 10 acute non-lymphocytic leukemia and 1 chronic myelomonocytic leukemia. Eleven cases underwent plain CT scan with one of them also receiving contrast-enhanced CT scan enhancement, 16 cases underwent plain MR scan with 11 of them receiving contrast-enhanced MR scan.Results Intracranial lesions in 14 cases: (1)intracranial hemorrhage was found in 7 cases, including intracerebral hematoma in 4 cases, micro-haemorrhage in 2 cases, and intracerebral hematoma accompanying by multiple intracerebral micro-haemorrhage foci in 1 case. All cases with intracerebral hematoma showed multiple lesions, which demonstrated high-density on CT images, and low or mixed signal on T1 WI, high- or intermediate signal with low-signal rim on T2 WI and ring enhancement or no evident enhancement. Microhaemorrhage manifested as multiple mini-mottling and strip hypointense foci on susceptibility weighted imaging, on which the detection rate of micro-haemorrhage foci was much higher than that on CT and other sequences of MRI. Among the 7 cases, one also had cerebral infarction and one subarachnoid hemorrhage.(2) Intracranial mass was found in 5 cases, among which two appeared as masses under or bestride cranium in the left frontal region with hypointensity on T1 WI, mixed signal on T2WI, strong homogeneous enhancement and dural tail sign;one showed a mass in saddle area, with high density on CT, slightly low signal on T1WI, high signal on T2WI and heterogeneous enhancement; one case displayed a mass near lateral ventricle with iso-intensity on T1 WI and T2WI and strong homogeneous enhancement; and one case manifested as cystic mass in the left fronto-apical lobe, with thick wall and ring enhancement (3)Obstructive hydrocephalus was found in 1 case, manifesting dilation of ventricles above the aquaeductus mesencephali. (4) Meningopathy was found in 1 case, manifesting diffuse thickening of meninges with strong homogeneous enhancement on MRI. Pathological changes of spinal canal was found in 4 cases among which two showed para-spinal mass involving vertebral canal and causing bone destruction of adjacent ribs; one case showed fusiform mass posterior to vertebral canal with high and intermediate signal on T1 WI and low and iso-signal on T2WI without enhancement; one showed zonale leison in thoracic cord with high signal on T2WI and fluid attenuated inversion recovery. Conclusion The radiologic manifestations of adult CNSCL are various and the role of CT and MRI for the diagnosis of CNSCL may complement each other. SWI is suggested as routine examination for patients of leukemia, in whom intracerebral lesions were suspected in order to find micro-haemorrhage as early as possible and reduce the risk of intracerebral hematoma occurrence.  相似文献

Objective To label rat bone marrow mesenchymal stem cells with superparamagnetic iron oxide (SPIO) and to explore the tropism of BMSCs for hepatocellular carcinoma cells after transplantation in vivo. Methods BMSCs from bone marrow of Sprague-Dawly (SD) rats were cultured isolated and purified, Labeled BMSCs was achieved using Feridex. Twenty-four hepatocellular carcinoma models of SD rats were induced two weeks before tmnsplantation. The models were divided into three groups in random: the labeled BMSCs and unlabeled BMSCs were transplanted respectively into the rat's livers of experimental group (n = 12) and control group A(n =6) via spleens, and no transplant was done for control group B (n =6). MR imaging was performed to monitor the transplanted cells after 1,3,7,14 d using 1.5 T MR system. Signal intensity ratio (SI/SI*) between tumor and hepatic tissue on T2 * WI were measured and compared by one-factor analysis of variance. After MR imaging, Prussian blue staining was performed. MR imaging findings were compared with histological sections. Results Prussian blue staining confirmed the labeling efficiency of BMSCs was above 90%. SI/SI* of experimental group before and 1, 3, 7, 14 d after transplantation were 3.18±0.21,1.98±0.20,2.38±0.28,2.70±0.25 and 3.16±0.24 respectively. Following transplantation of BMSCs, signal intensity decrease was found in hepatocellular carcinoma of experimental group(F =56.65,P <0.05) and low signal change decreased gradually and disappeared at two weeks after transplantation, while no remarkable low signal change was seen in the control group by T2 * WI (P > 0.05). A large number of Prussian blue staining positive cells were found in hepatocellular carcinoma in experimental group. Histological section with Prussian blue staining had a good correlation with the signal intensity changes on MR images at different time. Conclusion BMSCs display significant tropism to hepatocellular carcinoma and may be an ideal gene therapy vehicle against hepatocellular carcinoma.  相似文献

Objective To investigate the clinical application of epinephrine hydrochloride in the prevention of bone cement implantation syndrome in the cemented hip replacement. Methods The clinical data of 48 patients treated with cemented hip replacement from July 2008 to April 2009 were retrospectively analyzed. All the patients were divided into control group and intervention group. The bone marrow cavities of 24 patients in the control group were not pretreated with saline epinephrine hydrochloride before implantation of bone cement; the bone marrow cavities of 24 patients in the intervention group were pretreated with saline epinephrine hydrochloride before implantation of bone cement. Systolic blood pressure (SBP), diastolic blood pressure (DBP), the mean arterial pressure (MAP), heart rate (HR)and pulse oxygen saturation ( SPO2 ) were compared between the two groups before bone cement implantation and 1,2, 3, 4, 5, 6, 7, 8, 9, 10 minutes after bone cement implantation. The data were analyzed with variance analysis and Q test. Results (1) In the control group: the blood pressure was decreased in control group one minute after bone cement implantation and a significant decrease of the blood pressure was observed at 2-6 minutes after the implantation ( P < 0. 01 ). The blood pressure was increased seven minutes after the implantation, with the most significant increase of DBP ( P < 0.05 ).The blood pressure recovered to normal 10 minutes later. The SPO2 was decreased significantly ( P <0.05 ) but no significant change was observed in HR ( P > 0.05). (2) In the intervention group: the bone marrow cavity was pretreated with saline epinephrine hydrochloride before implantation of bone cement.ity. No significant difference was found in SBP, DBP, MAP, HR and SPO2 at different time points before and after bone cement implantation (P >0.05 ). Significant decrease of blood pressure and SPO2 was observed in control group and a significant hemodynamic change was detected at 2-6 minutes after the bone cement implantation. In the intervention group, no hemodynamic change was found in all the patients except that one patient was found with decrease of blood pressure and another one with the occasional premature ventricular contractions. Conclusion Pretreatment of bone marrow cavity with saline epinephrine hydrochloride can effectively prevent bone cement implantation syndrome.  相似文献

Objective To investigate periprosthetic femoral shaft remodeling with tapered femoral stems after total hip arthroplasty (THA) for elderly patients and evaluate the mid-term clinical outcomes.Methods The study involved 30 elderly (70-90 years) patients (34 hips) treated with femoral shaft remodeling with tapered stems after THA from January 2003 to January 2005. The postoperative X-ray images were collected and perioperative periprosthetic bone mineral density was analyzed by dual-energy X-ray absorptiometry (DEXA). The Harris score was applied in follow-up observation, and Kaplan-Meier method was used to evaluate the working life of the femoral prosthesis. Results Four patients were died of lung cancer. The remaining 26 patients ( 30 hips) were followed up for 5-7 years ( mean, six years). The postoperative X-ray measurements showed that total hip prosthesis subsidences were less than 1.5 mm within one year in 12 hips, with no prosthesis loosening observed. Bone proliferations were seen in Gruen zones 2, 3, 4, 5, 6, 11 and 12, and bone resorptions were seen in zones l and 7. DEXA showed that bone mineral density was increased in Gruen zones 2,3,4 and 5, but decreased mainly in Gruen zones 1,6 and 7. The increase of bone mineral density in zones 2 and 5 was faster compared to other sites six months after the operation ( P < 0.05 ) and the change of bone mineral density was prone to be stable in two years ( P > 0.05 ). Within one year after initial implantation, periprosthetic bone mineral density was significantly decreased ( P < 0.05 ). Two years after the operation, rare changes of periprosthetic bone mineral density were found ( P > 0.05 ), with only regional redistribution of bone mass from the proximal to the distal femur. The Harris score of hip joint function was increased from preoperative 38.56 ± 8.21 to 86.32 ± 6. 01 at the final follow-up. The 6-year survival rate of the prosthesis was 100%. Conclusion Femoral shaft remodeling with tapered stems after total hip arthroplasty for the elderly patients shows good periprosthetic bone remodeling and satisfactory mid-term clinical results.  相似文献

目的 分析获得性骨肥大(SAPHO)综合征的影像表现.方法 回顾性分析11例SAPHO综合征的影像资料,男6例、女5例,年龄28~68岁,平均51岁,X线平片检查9例、CT检查10例、MR检查3例、核索扫描检查3例.结果 9例胸前壁X线平片表现为胸骨上部、第一前肋及锁骨增生、硬化并肥大,相互之间骨桥形成或骨性融合;10例胸前壁CT检查,9例表现为胸、肋、锁骨区骨质增生硬化伴有骨质破坏,胸锁关节和胸肋之间骨性融合,1例仅表现为胸骨柄体关节骨质硬化,2例在轴面像上胸骨柄上缘似"海鸥翅"样改变.11例患者中除1例胸骨病变仪累及胸骨柄体关节外,其余10例胸前壁均多骨受累,其中8例呈对称性,2例为偏侧性(均为右侧).除胸前壁骨病变外,伴有骶髂关节炎、髂骨骨髓炎1例,胸椎骨髓炎2例,腰椎致密骨炎1例和双手掌指关节骨关节炎1例.MR检查3例,分别是胸锁关节、胸椎和骶髂关节,MRI显示锁骨头周围软组织肥厚,胸椎病变表现为椎体内T1WI呈低信号,T2WI呈混杂信号,抑脂像以高信号为主,骶髂关节见软骨下长T1WI短T2WI信号.核素扫描3例,显示胸骨上部及锁骨区呈"T"形异常高浓聚.结论 胸前壁多骨受累是SAPHO综合征主要影像特点,可并发骶髂关节炎、脊椎及髂骨骨髓炎、腰椎致密骨炎和双手骨关节炎.
Abstract:
Objective To explore the imaging characteristics of SAPHO syndrome in 11 cases.Methods Clinical features and imaging findings from 11 patients (6 male,5 female, 28 to 68 years old)with SAPHO syndrome were analyzed retrospectively Including DR in 9 cases, CT in 10 cases, MRI and radioisotope scanning in 3 cases. Results Multi-bones of anterior chest wall disorders were shown in 9cases on DR images including superior sternum , anterior first rib and clavicle hyperostosis. Bony fusion and bony bridge were also seen in these cases. Hyperostosis osteosclerosis, bone destruction and bony fusion of sternoclavicular articulation and first rib were shown on CT images in 9 cases. Osteosclerosis of the joint between manubrium and midsternum was seen in 1 case on CT image. Thc sign of flying sea gull was seen in 2 cases on axial anterior chest wall CT images. The disorders of anterior chest wall were bilateral in 8 cases and unilateral in 2 cases. Sacroiliitis and osteomyelitis of ilium were found accompanied in 1 case.Osteomyelitis of thoracic vertebrae were found in 2 cases, while sclerosing osteitis of lumbar vertebrae and ostearthritis of bilateral hands were observed respectively in 1 case. The thicken soft tissue surround clavicle head, thoracic vertebra disease with long T1 ,jumbly T2 and high fat suppression signal ,long T1 and short T2signal under sacroiliac joint were shown on MRI. Radioisotope scanning displayed higher radioactive uptake of radionuclides, with T shape in sternoclavicular area in 3 cases. Conclusions Multi-bones of anterior chest wall involvement was the common imaging characteristics in 11 patients. Sacroiliitis, osteomyelitis of vertebrae and ilium, sclerosing osteitis, ostearthritis of hand could be seen in some cases.  相似文献

目的 研究软骨母细胞瘤的影像表现,探讨其MRI与X线平片、CT征象的对应关系.方法 分析16例经手术、病理证实的良性软骨母细胞瘤的影像资料,总结其MRI表现与X线平片、CT征象的对应关系.结果 16例软骨母细胞瘤均位于骨骺,大小为0.9 cm×0.8 cm×1.0 cm~4.8 cm×4.3 cm×5.1 cm,呈不同程度的分叶状.在T1WI上以等、低信号为主,T2WI上呈混杂信号,CT上为软组织密度,内见钙化和更低密度区.边缘呈长T1、短T2信号,在CT上表现为硬化边.MRI可见病灶周围有骨髓水肿,在X线和CT上表现为骨质硬化区.病灶邻近软组织肿胀.MRI显示骨膜异常9例,8例与病灶不相邻;X线和(或)CT显示骨膜新生骨6例.MRI显示关节积液12例,CT显示6例.MRI上病灶均呈不均匀强化,骨髓水肿、骨膜反应和软组织肿胀均见强化.软骨母细胞瘤在扩散加权成像(DWI)上呈等、高信号,在MR平扫中的等T1、等T2成分和长T1、长T2成分,以及骨髓水肿、骨膜反应和软组织肿胀在DWI上均呈高信号.结论 MRI和X线平片、CT从不同方面反映软骨母细胞瘤的病理改变,联合应用不同检查手段可更全面显示软骨母细胞瘤的特点.  相似文献

目的 通过观察急性白血病治疗前后骨髓的MRI表现和T1及对比噪声比 (CNR)的测定 ,对照临床、病理资料 ,探讨急性白血病骨髓MRI定性和定量诊断价值。材料与方法 选择经临床确诊为急性白血病的患者 38例 ,缓解组 2 5例 ,对照组 35例进行MRI检查及T1值和CNR的测定。结果 急性白血病T1WI:治疗前组 38例 ,腰骶椎、髂骨及股骨近段均表现为信号降低 ;化疗缓解组 2 5例表现为不同程度的信号增高。T2 WI :38例治疗前组和 2 5例化疗后组各椎体、髂骨、股骨上段呈等信号。外周血B %、骨髓B %、腰椎T1值、腰椎 /皮下脂肪CNR、股骨骨髓 /皮下脂肪CNR于急性白血病治疗前组与正常对照组、治疗前组与缓解组间的差异均有显著性 (P <0 .0 5 ) ,缓解组与正常对照组间的差异无显著性 (P >0 .0 5 ) ,不同细胞类型间的差异无显著性 (P >0 .0 5 )。腰椎T1值于不同性别、年龄组间的差异无显著性 (P >0 .0 5 )。治疗前组骨髓B %与腰椎T1值、腰椎 /皮下脂肪CNR ,外周血B %与腰椎T1值、腰椎 /皮下脂肪CNR及腰椎T1值与腰椎 /皮下脂肪CNR之间成直线相关 (P <0 .0 5 )。结论 急性白血病的MRI检查为一种无创性检查方法 ,MRI表现具有一定的特征性 ,在估计病情的严重程度及治疗效果的评价简便实用 ,为避免多次重复骨髓穿刺检查以及全面  相似文献

目的 分析成软骨细胞瘤的MRI表现特点.方法 对经病理证实的20例成软骨细胞瘤的MRI信号特点、病灶形态和生长方式,以及周围骨髓和软组织有无水肿、有无骨膜反应、相邻关节有无积液等征象进行分析.结果 20例病灶在T1WI和T2WI上表现为不均质的混杂信号,病灶形态为分叶状;16例病灶呈膨胀性生长;18例病灶周围出现骨髓水肿;14例病灶周围软组织水肿;6例病灶周围有骨膜反廊;7例病灶突破骨皮质向周围突出;6例棚邻的关节有关节积液.结论 成软骨细胞瘤在MRI上多表现为分叶状和膨胀性生长,病变呈不均质混杂信号,病灶周围有明显的骨髓水肿和邻近软组织水肿.  相似文献

目的 分析原发性骨骼恶性纤维组织细胞瘤(MFH)影像表现与病理的关系,以提高诊断准确性.方法 回顾性分析经手术病理证实的原发性骨骼MFH 13例,术前分别经X线平片、CT平扫、MR SE T1 WI、T2 WI和增强T1 WI检查,复习影像表现并与手术病理作对照.结果 13例原发骨骼MFH中,11例位于骨端,2例位于骨干,位于骨端者与骨性关节面的距离为1~5 cm.肿瘤呈溶骨性骨质破坏,直径5.3~12.7 cm.其中,破坏区偏心10例,破坏区内见少许分隔7例,周边不连续硬化11例,伪足样突起7例,骨膜增生2例,软组织肿块9例,所有患者软组织肿块体积较小.CT扫描显示肿瘤密度与肌肉密度相仿.在MR T1 WI肿瘤为等低信号,在T2 WI为混杂信号,T2 WI常见片状等低信号,病灶周围常见不完整的低信号环,环外见伪足样突起.动态增强呈进行性延迟强化,强化显著,且较均匀.大体病理显示病灶偏心,膨胀不明显,肿瘤多突破皮质形成范围较小的软组织肿块,呈结节状或假性包裹样,肿瘤内见纺锤形成纤维细胞和胞体较小的组织细胞样细胞,伴有数量不等的胶原纤维,细胞密集呈席纹状或放射状排列.肿瘤微血管丰富.结论 原发骨骼MFH好发于中年患者长骨骨端,T2 WI常见等低信号成分,增强扫描强化显著;MRI表现与病理关系密切.  相似文献

血液病腰椎骨髓的MRI鉴别诊断价值(附94例分析)   总被引:5,自引:1,他引:4  
目的探讨血液和造血系统疾病的MRI鉴别诊断价值。方法选择经骨髓穿刺或活检证实的血液和造血系统疾病94例行MRI检查。全部病例均行腰骶椎矢状面T1WI和T2WI扫描。观察腰椎骨髓的MRI信号及形态改变,同时测量其T1值。结果38例急性白血病和8例地中海贫血T1WI均表现为均匀低信号,T2WI均匀等信号;平均T1值均延长,分别为(972.5±36.5)ms和(879.6±98.56)ms,两者间差异无显著性意义(q=1.52,P>0.05)。16例再生障碍性贫血T1WI4例呈均匀高信号,12例呈不均匀的高信号,T2WI14例呈等信号,平均T1值缩短,为(279.42±54.19)ms;3例急性造血功能停滞T1WI、T2WI均为等信号,平均T1值变化不明显,为(418.65±32.49)ms,两者平均T1值差异具有显著性意义(q=6.67,P<0.01)。20例多发性骨髓瘤15例有椎体骨质破坏及变形,呈多发斑片状病灶,T1WI呈不均匀低信号,T2WI呈不均匀高信号;3例骨髓异常增生综合征T1WI呈等或不均匀低信号,T2WI呈等信号;6例骨髓纤维化在T1WI和T2WI均表现为均匀低信号。结论(1)观察MRI信号改变可对急性白血病与再生障碍性贫血、多发性骨髓瘤、骨髓纤维化进行鉴别;(2)MRI比较容易区别临床难以鉴别诊断的再生障碍性贫血和急性造血功能停滞;(3)部分血液造血系统疾病的MRI鉴别诊断还需结合临床资料进行综合分析。  相似文献

119 children with acute lymphoblastic leukemia at onset underwent radiography of the whole body in order to determine whether initial radiological bone lesions were correlated with subsequent clinical course. Radiological findings were divided into three groups: 101 children (84.8%) had no bone involvement (group 0); 10 had bone changes compatible with acute lymphoblastic leukemia, such as slight metaphyseal transverse lucent bands with or without diffuse demineralization (group 1); 8 had bone changes, such as periosteal reaction with or without intramedullary osteolytic mottling. Bone involvement was not correlated with prognostic factors. Furthermore there was no significant correlation between bone changes and complete remission or survival.  相似文献

目的 探讨成软骨细胞瘤影像学表现特点。资料与方法 分析22例经手术病理或穿刺活检证实的成软骨细胞瘤资料,总结其X线平片、CT和MRI表现。结果 胫骨上端18例,股骨下端2例,肱骨上端2例。表现为骨骺区分叶状骨质破坏区,内见钙化和更低密度区;MR T1WI以低信号为主,T2WI上信号混杂,扩散加权成像(DWI)上呈等高信号。硬化边厚薄不一。病灶周围在X线和CT上见高密度骨质硬化区,MRI表现为骨髓水肿。骨膜反应常不与病灶相邻,MRI上为长T1、短T2信号,与皮质间见条状高信号。邻近软组织肿胀,部分患者见关节积液。MRI上病灶可呈环状或较均匀强化,骨髓水肿、骨膜反应和软组织肿胀均强化。结论 成软骨细胞瘤影像学表现较有特征性,结合X线平片、CT和MRI能做出正确诊断。  相似文献

Pediatric oncology patients are at risk for the development of numerous skeletal complications, and radiologic studies are important in the identification and evaluation of these conditions. Methotrexate osteopathy manifests as osteopenia, dense provisional zones of calcification, pathologic fractures, and sharply outlined epiphyses. Hypertrophic osteoarthropathy may occur with nasopharyngeal carcinoma or tumors of the lungs or pleura and manifests as cortical thickening, lamellar periosteal new bone formation, and soft-tissue swelling. Biomechanical abnormalities are often seen at bone scintigraphy in patients who have undergone surgery for bone tumors. Growth plate injury may manifest as marked deformity, sclerotic metaphyseal bands, metaphyseal fraying, and longitudinal striations. Radiation "osteitis" is seen as an initial decrease in bone density with subsequent development of a mixed radiolucent and sclerotic appearance. Ischemic necrosis of the femoral heads is best demonstrated at magnetic resonance (MR) imaging and has low signal intensity on T1-weighted images and a high-signal-intensity rim on T2-weighted images. Bone infarcts are seen as well-demarcated, often ring-shaped areas of decreased signal intensity on T1-weighted MR images and as areas of increased signal intensity on short-inversion-time inversion recovery images. Radiographic signs of infection include bone destruction, periosteal new bone formation, and sclerotic changes. Short-inversion-time inversion recovery MR imaging is particularly useful in evaluating posttherapy changes in bone marrow. Osteochondroma may demonstrate a cartilaginous cap at MR imaging, whereas the most important finding in radiation-induced sarcoma is a soft-tissue mass. Radiologists who work with children with cancer need to be familiar with these complications and their imaging appearances.  相似文献

成人急性白血病初诊骨髓MRI及临床应用研究   总被引:18,自引:0,他引:18  
目的:研究成人急性白血病(acute leukemia,AL)患者初诊自然病程状态下骨髓的MRI表现及其临床应用价值。方法:搜经细胞形态学及免疫表型综合分析确诊的成人AL54例,其中髓细胞白血病(AML)28例、急性淋巴细胞白血病(ALL)26例,行骨盆及股骨中上段骨髓MRI,采用SE序列及短时反转恢复(STIR)序列行冠状面扫描,选取15例正常志愿者作为对照组,搜集AL患者同期骨髓象及血常规结果与MRI对比分析。结果:患者骨髓MRI表现按浸润程度和范围分为5级,AML患者23(82%)例属Ⅰ-Ⅲ级病例,2型患者MRI分级总体的分布差异有非常显著性意义(U=4.233),P<0.01)。分别比较2型AL患者各临床诊断指标,显示2屠 骨髓象白血病细胞计数(BMT)、外周血白血病细胞计数(PBT)、外周血白细胞计数(WBC)之间差异有显著性意义(t值分别为4.336、2.008、3.420,P值均<0.05)。MRI分级与临床诊断指标比较,两者间具有非常显著的一致性(U=6.099,P<0.01)。,MRI分级为Ⅳ级的AML患者,缓解率较Ⅰ-Ⅲ级者低(P<0.05),MRI分级为Ⅴ级的ALL患者与Ⅳ级患者缓解率的差异无显著性意义(P>0.1)。结论MRI能反映2型AL不同的骨髓浸润特征,能为AL确诊提供影像学依据。初诊AML患者骨髓MIR对病情的评估,缓解的预测有重要的临床应用价值。  相似文献
