人工心脏瓣膜心内膜炎的外科治疗   总被引:9,自引:0,他引:9  
目的 总结人工心脏瓣膜心内膜炎(PVE)的外科治疗。方法 1990年至2003年8月,手术治疗PVE病人21例,其中亚急性16例,急性5例。血细菌培养阳性13例。心脏超声检查主动脉瓣瓣周漏6例,二尖瓣瓣周漏3例,主动脉瓣区赘生物3例,二尖瓣区5例。应用机械瓣再次手术行二尖瓣置换11例,主动脉瓣置换10例,同期行升主动脉假性瘤切除和主动脉修补成形1例。术中均见有赘生物;二尖瓣环脓肿7例,瓣周脓肿3例;主动脉瓣环脓肿8例,瓣周脓肿4例。结果术后早期死亡5例,其中3例死于感染复发,2例死于多脏器功能衰竭;晚期死亡1例。随访4个月至13年,1例PVE再发,内科治疗无效,死亡。结论及时诊断PVE,正确掌握手术时机,彻底清除感染组织和围术期应用大剂量敏感抗生素,是提高PVE手术效果的关键。  相似文献

目的总结感染性心内膜炎(IE)外科治疗的临床经验。方法回顾性分析1998年6月至2005年12月收治的23例IE患者外科手术治疗的临床资料,其中先天性心脏病13例,风湿性心瓣膜病7例,原发性细菌性心内膜炎3例。术前16例行血液细菌培养,阳性7例。术前心功能分级(NYHA)级2例、级12例、级9例。行主动脉瓣置换术4例、二尖瓣置换术4例、主动脉瓣和二尖瓣双瓣膜置换术3例、主动脉瓣、二尖瓣、三尖瓣置换术2例;单纯心内分流修补术8例,主动脉瓣置换加心内分流修补术2例。术后应用足量的敏感抗生素4~6周。结果术后第5d和9d分别死亡2例,其中1例因全身肢端和多器官栓塞、脑出血死亡,1例因心脏骤停死亡。术后20例患者心功能恢复至~级,治愈出院;1例心功能级患者好转出院。术后随访19例,随访时间6个月至7.5年,18例心功能~级,1例心功能级;发生主动脉瓣瓣周漏2例,其中1例于术后3年自愈,1例随访2.6年,无任何自觉症状,心功能级。结论IE经外科手术治疗能取得较好的治疗效果。  相似文献

目的总结既往有二尖瓣闭式扩张术、瓣膜成形术、瓣周漏及生物瓣失功能等的患者再次瓣膜手术的经验。方法自1998年1月至2005年8月,实施心脏瓣膜病再次手术221例,其中急症手术8例。其中二尖瓣闭式扩张后再狭窄105例,二尖瓣或主动脉瓣成形术后复发性瓣膜病变37例,瓣周漏29例,生物瓣衰败18例,其他瓣膜再发病变11例,人工瓣膜机械功能障碍9例,Ebstein畸形矫治术后三尖瓣关闭不全7例,人工瓣膜心内膜炎5例。再次手术方式包括二尖瓣置换、二尖瓣和主动脉瓣双瓣置换、主动脉瓣置换、三尖瓣置换。两次手术间隔时间1~21年。结果全组术后死亡19例,占8.6%。早期死亡主要原因为术后低心排综合征、恶性心律失常、多脏器功能衰竭与肾功能衰竭,其中急症手术8例中死亡3例,术前心功能Ⅳ级者手术死亡9例,病死率为14.5%(9/62例)。结论瓣膜病再次手术危险因素包括急症手术、术前心功能差、合并其他重要脏器功能不全、体外循环时间和主动脉阻断时间长等。针对这些因素积极防治,可以进一步降低这类患者手术病死率和并发症发生率。  相似文献

报告1985年4月至1993年12月460例二尖瓣、主动脉瓣双瓣替换术远期疗效,并对术后并发症发生因素进行分析。手术死亡30例(6.5%)。420例随访6个月~8.4年(平均2.97年),随访率97.7%。术后1.5年实际生存率90.3±1.21%、81.1±6.76%。远期死亡33例(2.64%/病人-年),主要死亡原因为心衰、人工瓣感染、出血或栓塞。远期生存387例中心功能Ⅰ级172例,Ⅱ级190例,Ⅲ级19例,Ⅳ级6例。远期出现人工瓣相关并发症48例(3.84%/病人-年),其中栓塞19例,出血16例,人工瓣感染10例,瓣周漏2例,生物瓣坏损1例。本组资料显示术前左心室明显扩大、瓣膜明显关闭不全、心功能Ⅳ级是术后心衰的易发生因素。  相似文献

心脏瓣膜置换术后并发感染性心内膜炎的外科治疗   总被引:4,自引:1,他引:3  
目的总结心脏瓣膜置换术后并发感染性心内膜炎的外科治疗经验。方法对21例心瓣膜置换术后并发感染性心内膜炎的患者行再次二尖瓣置换术和主动脉瓣置换术,植入适当大小的机械瓣;术前、术后均进行内科治疗。结果本组近期死亡6例,其中3例患者死于金黄色葡萄球菌感染毒血症及中毒性休克,3例死于术后心力衰竭、多器官功能衰竭。随访15例,随访时间5个月至13年,远期再发感染性心内膜炎4例,其中2例死于感染性心内膜炎复发、败血症及中毒性休克;2例经内科治疗后痊愈。其余患者心功能明显改善(射血分数>0.45),9例复查心脏超声心动图未发现瓣周漏。结论心瓣膜置换术后发生感染性心内膜炎应早期诊断、适时手术和内外科联合治疗是治疗成功的关键。  相似文献

目的总结成人小主动脉瓣环行心瓣膜置换术的临床经验,以提高手术效果。方法对26例成人小主动脉瓣环患者行人工机械瓣膜置换术,单纯主动脉瓣置换17例,二尖瓣、主动脉瓣双瓣膜置换9例。采用Manougnian法主动脉瓣环加宽7例,瓣膜侧倾缝合置换主动脉瓣膜6例,瓣环上主动脉瓣置换13例,在双瓣膜置换中均先置换主动脉瓣后再置换二尖瓣。结果26例患者中无手术死亡,术后随访时间6~48个月(12±3个月),心功能均明显改善(Ⅰ级10例、Ⅱ级16例),无远期死亡。术后主动脉瓣有效瓣口面积指数(EOAI)1.02~1.44cm^2/m^3(〉0.85cm^2/m^3),无瓣膜-患者不匹配现象(PPM)。结论主动脉瓣病变伴小主动脉瓣环的成人患者行心瓣膜置换,选择新型人工瓣膜行瓣环上主动脉瓣置换是理想的选择,瓣膜侧倾缝合是可选择的方法,二尖瓣、主动脉瓣双瓣膜置换时先置换主动脉瓣可降低手术操作难度,大部分患者无需行瓣环扩大术。  相似文献

二尖瓣置换术后远期功能性三尖瓣关闭不全的外科治疗   总被引:34,自引:5,他引:29  
目的:报告二尖瓣置换(MVR)术后远期三尖瓣关闭不全(TR)外科治疗的结果及作用,方法:37例MVR术后中重度IR病人,其中人工二尖瓣为生物瓣者13,机械瓣24例,有11例行内科保守治疗,26例行外科手术治疗,手术类型,MVR加三尖瓣置换2例,MVR加三法瓣成形11例,三尖瓣置换3例,三尖瓣成型10例,三尖瓣成形术包括改良Kay形成形12例,改良DeVega成形术7例,加成形环的三尖瓣成形术2例,结果:11例内科治疗者,7个月-7.5年后76例死亡,病死率为54.5%,26例手术治疗者,术后早期病死2例,病死率为7.7%,随访个月-10.5年,晚期死亡例,仍中度TR2例,结论:MVR术后远期TR的产生与不可逆的左心损害或(和)严重肺动脉高压有关,对重度TR伴有临床症状、左心功能基本正常者,行三尖瓣成形或三尖瓣置换术可取得良好的效果。  相似文献

目的评价瓣周脓肿、心肌脓肿以及瓣膜严重毁损等复杂性感染性心内膜炎手术治疗的近、远期疗效.方法回顾性分析1988年12月至2002年6月手术治疗的复杂性心内膜炎患者57例临床资料,均为原发性心内膜炎,其中感染侵犯主动脉瓣25例、二尖瓣16例、二尖瓣和主动脉瓣16例.术中发现瓣叶严重毁损32例、主动脉瓣周脓肿19例、主动脉根部环形脓肿导致左心室-主动脉连接破坏4例、二尖瓣后瓣环脓肿11例、心肌脓肿6例、瓣膜赘生物形成55例.脓肿清除后遗留残腔采用间断褥式缝合6例、自体心包片修补19例、牛心包片修补6例、聚四氟乙烯膨体补片修补4例;施行以带瓣管道作升主动脉根部替换和左、右冠状动脉移植术4例,主动脉瓣替换术21例,二尖瓣替换术16例,主动脉瓣及二尖瓣双瓣替换术16例.结果早期死亡6例(11%),死亡主要原因为低心输出量综合征、人造心脏瓣膜性心内膜炎和多脏器功能衰竭.随访4个月至14年,平均(5.93±0.20)年.晚期死亡5例,晚期主要并发症为人造瓣膜性心内膜炎.术后1年心功能恢复NYHA分组Ⅰ~Ⅱ级占96%(44/46);5年再手术免除率为(84±3)%,5年实际生存率为(61±9)%.结论复杂性心内膜炎局部组织破坏较多,应限期手术或急症手术,清创后残腔的处理是影响手术本身能否成功以及术后近、远期效果的关键.  相似文献

目的 探讨感染性心内膜炎的临床特点、手术时机选择及围术期处理。 方法 回顾分析 2 8例感染性心内膜炎患者手术治疗的临床资料。病因为原发性心内膜炎 2 4例 ,人工心脏瓣膜感染性心内膜炎 4例。施行主动脉瓣置换术 2 0例 ,同期施行右冠状窦破裂自体心包修补和经主肺动脉缝闭未闭动脉导管各 2例 ;二尖瓣置换术 7例 ,其中4例行再次二尖瓣置换术 ;肺动脉瓣置换术 1例。 结果 术后早期死亡 2例 ,随访 2 6例 ,随访时间 3个月至 12年 ,1例术前合并肺部感染 ,术后 6个月因心内膜炎复发死亡 ,1例再次二尖瓣置换术后 2年出现瓣周漏。其余患者疗效良好。 结论 感染性心内膜炎早期诊断、正确选择手术时机、术中彻底清除病灶、合理矫正病变及良好的围术期处理是提高疗效的关键。  相似文献

目的总结严重主动脉瓣感染性心内膜炎患者行主动脉根部置换术治疗的临床经验,探讨其手术适应证和手术方法,以期提高外科治疗效果。方法1995年9月~2008年6月间手术治疗11例严重主动脉瓣或人工瓣膜感染性心内膜炎患者,其中活动期6例,静止期5例;术前动脉血细菌培养阳性6例。术前心脏超声心动图提示均有不同程度的主动脉瓣反流或瓣周漏,左心室收缩期末内径(LVESD)6.0±0.7cm,其中≥5.5cm 7例;左心室射血分数(LVEF)47.8%±11.2%,其中≤509/8例。手术均在彻底清创后应用人工带瓣管道(9例)或同种带瓣管道(2例)行主动脉根部置换术,同期行冠状动脉旁路移植术4例,二尖瓣环缩术3例,室间隔缺损修补术1例。结果术后心脏骤停死亡1例。发生Ⅲ°房室传导阻滞1例,后期植入永久性起搏器。术后随访10例,随访时间3个月~13.2年,术后32d因感染性心内膜炎复发死亡1例;其余患者均无感染复发和晚期死亡。结论当感染性心内膜炎合并主动脉根部或窦部瘤、感染累及主动脉窦壁或冠状动脉开口处、瓣环严重毁损或彻底清创后瓣环缺损广泛时,宜置换主动脉根部。手术的关键是彻底清创和防止根部出血。尽管手术较复杂,但局部清创彻底,有利于提高手术效果。  相似文献

From July 1985 to 1988 Feb. 16 patients with large heart (C-T ratio 65-85%) underwent valve replacements with mechanical valve. Isolated mitral valve replacement was performed in 15 cases and combined aortic and mitral valve replacement in 1. The valvular lesions was rheumatic in origin. The heart function (NYHA) was grade III in 5 cases (32.5%), and IV in 11 (67.5%), 60% of the cases were suffered from moderate or severe pulmonary hypertension. 1 patient died postoperatively. In the survivals the cardiac function was improved and the heart-size reduced distinctly. Some points about the operative results, selection of the time for operation and the postoperative management were discussed.  相似文献

二尖瓣主动脉瓣三尖瓣同时置换治疗重症风湿性瓣膜病   总被引:5,自引:0,他引:5  
目的 总结二尖瓣主动脉瓣三尖瓣同期置换治疗重症风湿性心脏瓣膜病的手术疗效。方法  1999年 6月至 2 0 0 1年 6月 94 1例病人进行瓣膜置换术 ,其中 2 4例同期进行二尖瓣、三尖瓣和主动脉瓣置换 ,占瓣膜置换病人的 2 5 5 %。 2 4例病人中女 17例 ,男 7例 ;年龄 18~ 5 9岁 ,平均 36岁 ;体重 37~ 5 6kg。其中 8例曾行二尖瓣闭式扩张术、11例合并左房血栓、16例病人合并有肝肿大 (肋下 2~ 8cm)和下肢水肿、8例合并有腹水。X线胸片示心胸比率为 0 6 6~ 0 91。超声检查示三尖瓣均有严重反流 ,反流面积为 4 2~ 34 0cm2 ,平均 (16 8± 9 3)cm2 。术前心功能III级 9例 ,VI级 15例。 6例病人因药物不能控制心衰而行急诊换瓣手术。结果 死亡 1例 ,死亡率为 4 2 %。术后 1周、3、6个月复查超声心动图示各心腔内径较术前明显缩小。出院者均得到随访 ,随访时间 2 0~ 36个月 ,平均 2 6 4个月。术后心功能I~II级2 0例 ,III级 4例。术后 3~ 12个月复查超声心动图未见机械瓣功能障碍及血栓形成。结论 对于联合瓣膜病变 ,三尖瓣有严重器质性病变的病人 ,在进行二尖瓣主动脉瓣置换的同时进行三尖瓣置换 ,有利于术后右心功能的恢复 ,能更好地改善心脏的血流动力学特性 ,改善心功能 ,并有利于术后病人的康  相似文献

A 50-year-old woman was admitted to our hospital because of heart failure (NYHA III) due to mitral valve regurgitation (MR) with pulmonary hypertension (PH) and tricuspid valve regurgitation (TR). She had a history of chronic renal failure undergoing dialysis (peritoneal dialysis, homodialysis) since 1996. Cardiac catheterization and ultrasonic cardiography showed severe MR (Sellers III), severe TR and PH (mean pressure 33 mmHg). So we performed mitral valve replacement and tricuspid annuloplasty (DeVega). Frequent blood transfusion was needed because severe hemolytic anemia appeared after operation. Ultrasonic cardiography demonstrated moderate aortic valve regurgitation (AR) with no paravalvular prosthetic leakage. We diagnosed hemolytic anemia due to AR. We performed aortic valve replacement. Hemolytic anemia improved soon after second operation. We investigated the mechanical process of the AR. She had a very short subaortic curtain (5.9 mm) compared with the average (8.7 +/- 2.1 mm: mean +/- SD) of cardiac patients. We think that we must be very careful with suture to short subaortic curtain. In addition measurement of subaortic curtain before operation is very useful.  相似文献

BACKGROUND: The results of mitral valve replacement (MVR) with Sorin mechanical valves in patients who had tight mitral stenosis with high pulmonary artery pressure were reviewed. METHODS: During a period of two years, from August 1998 to May 2000, a mitral valve replacement with a Sorin Bicarbon mechanical valve was performed in 51 patients with a diagnosis of tight mitral stenosis associated with severe pulmonary hypertension (preoperative mean systolic pulmonary artery pressure was 72+/-12 mmHg, range from 60 to 105 mmHg). There were 37 women and 14 men; mean age was 47.2+/-12 years. Forty-eight patients (94.12%) were in NYHA functional class III or IV. All the patients discharged from the hospital were submitted to a clinical follow-up program. A 100% follow-up was obtained with a mean of 12.6+/-6.4 months (range 2 to25 months). RESULTS: Operative mortality was 3.9%, 2 patients who had concomitant CABG died due to low cardiac output. Twelve patients (23.5%) needed an inotropic pharmacological support during the postoperative time. In one patient a re-exploration for bleeding was necessary, and in another one a cerebrovascular accident occurred 3 days after the operation. After 6 months, one patient was reoperated on because of mechanical valve dysfunction due to pannus formation. All survivors underwent a postoperative echocardiographic assessment. The systolic PAP decreased from a mean preoperative value of 72+/-12 mmHg to 39.9+/-12 mmHg. NYHA functional status significantly improved and 86% of the patients were in NYHA functional class I or II. CONCLUSIONS: The mitral valve replacement with Bicarbon mechanical valve prosthesis shows excellent results in patients with mitral valve stenosis associated with a severe pulmonary hypertension.  相似文献

目的总结重症心脏瓣膜病的外科治疗结果,探讨提高早期生存率的措施。方法自2000年6月至2005年7月,对78例重症心脏瓣膜病患者施行瓣膜替换术。其中单纯二尖瓣置换12例,二尖瓣置换 三尖瓣成形22例,单纯主动脉瓣置换8例,二尖瓣 主动脉瓣置换 三尖瓣成形35例,二尖瓣置换 冠状动脉旁路移植术1例。结果死亡6例,其中术后并发低心排血量5例,心室颤动治疗无效死亡1例,死亡率7.69%。随访53例,平均随访2.5年,死亡5例。结论对重症心脏瓣膜病患者,注重改善术前心功能,掌握手术时机,尽量保留瓣下组织,选择合适瓣膜,重视围手术期处理,可提高手术成功率。  相似文献

Repair of congenitally malformed mitral valve in children   总被引:1,自引:0,他引:1  
We report our experience with 30 children under 15 years of age treated surgically for congenital mitral valve disease from March 1972 to July 1986. Valve reconstruction was possible in 26 patients (87%), whereas in four the valve was replaced with a mechanical prosthesis. Five patients died in the hospital (16.6%), four after conservative operations and one after mitral valve replacement. There was only one late death in a child in chronic congestive heart failure. Three patients, treated conservatively, required valve replacement 2, 22, and 24 months, respectively, after the operation. Follow-up data reveal that 22 of 24 patients are asymptomatic 5 months to 15 years after operation. Two-dimensional echocardiographic studies were performed in 19 patients treated conservatively, 17 of whom are asymptomatic. Eleven of them have no signs of mitral regurgitation or stenosis, six show only mild mitral incompetence, and two have moderate mitral regurgitation or stenosis. Peak pulmonary artery pressure is within normal limits in all. Our results indicate that mitral valve reconstructive procedures for congenital valve dysplasia may be effective and reliable in children despite the frequent severity of valve malformation. Although no major prosthesis-related complications were observed in the present series, we believe that mitral valve repair should always be attempted in the pediatric population to avoid the drawbacks of the currently available prostheses.  相似文献

In 14 patients aged 5–329 (mean 131) months a CarboMedics® valve was implanted because of congenital heart disease. The preoperative NYHA function class was III-IV in ten cases. Seven aortic and seven atrioventricular valves were replaced without early mortality. All patients were followed up, with mean observation time 27 months (total 384 months). One of the 14 patients died of heart failure 10 months postoperatively. Thrombosis occurred in four valves, three in tricuspid and one in mitral position. In all patients who received only warfarin, anticoagulation was demonstrably inadequate. Consequently we now recommend antiplatelet medication in addition to warfarin for children with atrioventricular mechanical valve replacement. In our experience the complication rate with CarboMedics prosthesis is acceptable, provided that anticoagulant therapy is adequate.  相似文献

目的 总结先天性心脏病术后因瓣膜功能不全再手术的经验。方法 回顾分析先天性心脏病术后再行瓣膜手术13例患者的临床资料,其中室间隔缺损修补术后8例,部分心内膜垫缺损修补术后3例,法洛四联症和房间隔缺损修补术后各1例。第1次手术时即存在二尖瓣轻~中度关闭不全6例,主动脉瓣关闭不全1例;新出现瓣膜功能异常6例,其中2例因补片漏致三尖瓣关闭不全,2例因前叶腱索断裂致三尖瓣关闭不全,1例因残留右心室流出道狭窄继发三尖瓣关闭不全,1例因伤及主动脉瓣并发二尖瓣和三尖瓣关闭不全。13例中,行二尖瓣置换6例,三尖瓣置换2例,主动脉瓣置换1例,行主动脉瓣置换并二尖瓣、三尖瓣成形1例,三尖瓣成形3例。同时修补残余漏,疏通右心室流出道。结果 术后发生低心排综合征3例。2例术后早期分别死于脑气栓和呼吸循环衰竭。11例术后痊愈出院,随访1~8年,心功能良好。结论 先天性心脏病矫治术中应注意心脏瓣膜的保护,合并的瓣膜功能异常应积极修补,及时地再手术可取得良好效果。  相似文献

A high speed electric rasp was used to remove fibrous thickening from the aortic valve in conjunction with aortic valve commissurotomy in ten patients. All patients had moderate rheumatic aortic valve disease combined with severe mitral valve disease, and were treated by mitral valve replacement and aortic valve repair. All patients survived the operative procedure. There were no deaths or complications during hospitalization related to the valve repair process. The transaortic valve gradient was relieved from an average of 21.0 ± 8.6 mmHg to 5.6 ± 4.0 mmHg (catheterization), and from moderate to less-than-mild stenosis (echocardiography). Aortic valve regurgitation was reduced from an average of 2.2+ to 0.7+ on a scale of 0 to 4+ (aortography), and from an average of 2.5+ to 1.1+ on a scale of 0 to 4+ (echocardiography). During the follow-up period, no patients were reoperated on because of aortic valve dysfunction. Follow-up echocardiographic study demonstrated that the transaortic pressure gradient and valvular regurgitation had not progressed, and immediate postoperative conditions were maintained. There were two late deaths not related to the aortic valve. One patient died of prosthetic valve endocarditis in the mitral prosthesis 14 months postoperatively, and the other of a cerebrovascular accident 21 months postoperatively. Based on these data, we believe that aortic valve repair with a high speed electric rasp can effectively relieve aortic stenosis, reduce valvular regurgitation, and provide an excellent hemodynamlc result at early and mid-term follow-up. 目的探讨左心瓣膜置换术后远期三尖瓣关闭不全(TR)发生的可能机制以及外科治疗方法的选择和结果.方法 56例左心瓣膜置换术后远期发生TR行再次瓣膜手术的病人,10例人工瓣膜功能正常(A组)者中行二尖瓣置换(MVR)4例,主动脉瓣、二尖瓣双瓣置换(DVR)6例;46例人工瓣膜功能障碍(B组)者中MVR 36例,主动脉瓣置换(AVR)4例, DVR 6例.在A、B两组中,46例第1次手时三尖瓣未见明显异常,10例第1次手术时已行DeVega三尖瓣成形(TVP),第2次手术时发现缝线断裂3例,缝线撕脱7例.56例TR病人再次手术时9例行三尖瓣替换(TVR),其中6例三尖瓣呈风湿性改变;47例行TVP.结果 TVP和TVR各死亡1例,病死率3.6%.54例获随访,随访时间6~132个月,平均(79.4±34.8)个月.8例TVR病人术后心功能恢复良好,46例TVP者40例为轻度TR,5例出现中度TR,仍需强心、利尿药维持,1例再次出现重度TR.结论左心瓣膜置换术后远期TR可能与持续肺动脉高压、右心室不可逆损害、三尖瓣风湿性病变、左心功能的恢复情况以及持续心房纤颤有关.重度功能性TR和三尖瓣风湿性病变者行TVR的疗效可靠.随访发现部分TVP病人功能性TR仍有逐渐加重趋势.  相似文献
