Near-infrared diffuse correlation spectroscopy/tomography (DCS/DCT) has recently emerged as a noninvasive measurement/imaging technology for tissue blood flow. In DCT studies, the high-dense collection of light temporal autocorrelation curves (g2(τ)) via fiber array are critical for image reconstruction of blood flow. Previously, the camera-based fiber array limits the field of view (FOV), precluding its applications on large-size human tissues. The line-shape fiber probe based on lens combination, which is predominantly used in current DCT studies, requires rotated-scanning over the surface of target tissue, substantially prolonging the measurement time and increasing the system instability. In this study, we design a noncontact optical probe for DCT based on collimating micro-lens fiber array, termed as FA-nc-DCT system. For each source/detector fiber, a single optical path was collimated by coupling with one micro-lens in the fiber array that is integrated in a square-shape base. Additionally, an 8×8 optical switch is used to share the hardware laser and detectors without spatial scanning. The FA-nc approach for the precise collection of g2(τ) curves was validated through a speed-varied phantom experiment and the human experiments of cuff occlusion, from which the expected value of the blood flow index (BFI) was obtained. Furthermore, the flow anomaly in the phantom and the ischemic muscle in human were accurately reconstructed from the FA-nc-DCT system, which is combined with the imaging framework based on the Nth-order linear algorithm that we recently created. Diffuse large B-cell lymphoma (DLBCL) is a clinically aggressive and heterogenous disease. Although most patients can be cured by immunochemotherapy, 30% to 40% patient will ultimately develop relapsed or refractory disease. Here, we investigated the molecular landscapes of patients with diverse responses to R-CHOP. We performed capture-based targeted sequencing on baseline samples of 105 DLBCL patients using a panel consisting of 112 lymphoma-related genes. Subsequently, 81 treatment-naïve patients with measurable disease and followed for over 1 year were included for survival analysis. Collectively, the most commonly seen mutations included IGH fusion (69%), PIM1(33%), MYD88 (29%), BCL2 (29%), TP53 (29%), CD79B (25%) and KMT2D (24%). Patients with TP53 mutations were more likely to have primary refractory disease (87.0% vs 50.0%, P = .009). For those with TP53 disruptive mutations, 91.7% patients were in the primary refractory group. Interestingly, BCL-2 somatic hypermutation was only seen in patients without primary refractory disease (P = .014). In multivariate analysis, BCL-2 amplification (hazard ratio [HR] = 2.94, P = .022), B2M mutation (HR = 2.99, P = .017) and TP53 mutation (HR = 3.19, P < .001) were independently associated with shorter time to progression (TTP). Furthermore, TP53 mutations was correlated with worse overall survival (P = .049). Next, we investigated mutation landscape in patients with wild-type (WT) TP53 (n = 58) and found that patients harboring MYD88 L265P had significantly inferior TTP than those with WT or non-265P (P = .046). Our study reveals the mutation spectrum of treatment-naive Chinese DLBCL patients. It also confirms the clinical significance of TP53 mutations and indicates the prognostic value of MYD88 L265P in TP53 WT patients. 相似文献

肝靶向抗病毒药Lac-PLL-ACV的制备及其趋肝性研究 总被引:1,自引:0,他引:1
目的:减少抗病毒药阿昔洛韦(Acyclovir,ACV)在治疗乙型肝炎中的肝外毒副作用,获得肝靶向ACV偶联物。方法:多聚赖氨酸(Poly-L-lysine,PLL)在氰硼酸钠的催化作用下与乳糖反应生成乳糖化多聚赖氨酸,乳糖化多聚赖氨酸与咪唑化阿昔洛韦在37℃pH9.5的条件下反应合成偶联物乳糖化多聚赖氨酸-阿昔洛韦(Lactosaminated poly-L-lysine-acyclovir,Lac-PLL-ACV)。偶联物经尾静脉注射进入小鼠体内后,收集小鼠血浆及肝脏样品,用高效液相色谱法测定药物浓度,研究遇联物的体内分布和药代动力学。结果:HPLC检测证实Lac-PLL-ACV在血浆中十分稳定,不易解离出ACV。偶联物的肝最大摄取率约为60%,是ACV组的10倍以上,偶联物肝中的T1/2,AUC,CL分别为ACV的4.2倍,56.6倍和1/57,具有明显的肝靶向性。结论:乳糖化多聚赖氨酸作为肝去唾液酸糖蛋白受体(Asialoglycoprotein receptor,ASGPR)介导的一种药物载体,能使抗病毒药ACV获得较满意的肝靶向性。 相似文献

目的 探讨妊娠合并肝内胆汁郁积症(ICP)患者致围生儿不良结局的相关指标。方法 总结本院57例ICP患者的相关临床资料,将其与围生儿结局进行分组对照分析。结果 血清胆汁酸升高与围生儿不良结局关系不明显(P>0.05);血清转氨酶水平在产生围生儿不良结局组与未产生围生儿不良结局组之间有显著性差异(P>0.01);皮肤瘙痒持续时间大于3周组围生儿不良结局发生率与持续时间小于3周组有显著性差异(P<0.05);有、无NST基线变异消失组的围生儿不良结局发生率有极显著性差异(P<0.01)。结论 血清转氨酶水平、皮肤瘙痒持续时间及NST基线变异是否消失是预示围生儿不良结局的相关指标。 相似文献
