基于温病学理论,探讨新型冠状病毒肺炎的中医病因病机与诊治思路。温病学说源于明清之前的历代中医理论积累,经过长期的温热病防治实践而形成,其对于外感热病(包括烈性传染病)的病因病机、证治方药有着较为系统的论述,近年来在我国的新发传染病防治中发挥出积极的作用。温瘟相通,瘟疫病因多为热毒疫邪,表现为热证阳证,可按温病学中的卫气营血或三焦辨证分期论治,或治以吴又可、庞安时等的经方验方,这些因、机、证、治理论可试用于新型冠状病毒肺炎的中医诊疗。且肺与大肠相表里,胃肠道也是新型冠状病毒侵袭或繁殖的部位,采用中医清热通下之法,使邪从肠道而出,可能是减轻体内病毒感染与肺脏炎症的一种途径。 相似文献

目的了解丽水市呼吸道病原体感染情况及流行病学特征,为呼吸道传染病诊疗与防控提供科学依据。方法回顾性分析2020年3月至2021年12月丽水市人民医院收治的4035例呼吸道感染患者13项呼吸道病原体PCR毛细电泳核酸检测结果,并分析病原体感染分布特征。结果呼吸道病原体感染患者阳性检出率为32.24%(1301/4035),检出率前5位分别是HRV(16.60%)、HRSV(7.09%)、HPIV(2.87%)、HMPV(2.21%)、HADV(1.96%)。混合感染阳性占比为7.61%(99/1301),Boca病毒(76.0%)与Ch(46.15%)易与其它病原体形成混合感染。不同性别呼吸道病原体感染率差异无统计学意义(P>0.05)。虽然不同季节呼吸道病原体整体感染率差异无统计学意义(P>0.05),但HRV好发于春秋两季,而HPIV在春秋两季感染率较低,春季HRSV感染率较低,HADV在夏秋两季感染率较低,HMPV在冬季较为流行。未成年人呼吸道病原体感染率较高,HRSV、HRV、HPIV、HADV和Boca主要发生于未成年人组,InfB和HMPV在未成年和青年人群感染率都较高。结论未成年人群是呼吸道病原体易感人群,应加强未成年人呼吸道传染病防控。 相似文献

目的探讨目前老年护理医院院内感染病原菌及其耐药特征。方法对2000年4月-2004年3月徐汇区两家老年护理院出院病人的病史进行统计分析。结果两家老年护理院共计发生医院感染939例次,医院感染率12.86%,检出病原菌284株,阳性率30.28%;病原菌以革兰阴性菌为主,占44.65%,其次为真菌、革兰阳性菌,分别为29.12%、25.92%,位于前3位的病原菌分别为其他真菌(14.36%)、白色念珠菌(12.23%)、铜绿假单胞菌(8.69%);除嗜麦芽寡养单胞菌外,革兰阴性菌以亚胺培南最敏感,敏感率87%,对氨苄西林、阿莫西林的耐药率〉51%,对头孢哌酮、哌拉西林的耐药率〉33%;MRSA和MRSE的检出率高达86.76%和76.32%;混合感染率34.51%。结论老年护理院医院感染率高.真菌所占的比例高;细菌对抗菌药物的耐药性相当严重。 相似文献

文中基于免疫系统有关免疫活性细胞或分子与外来抗原相互作用的非线性的动力学模型对中医学正邪相争理论及其扶正祛邪治则进行了新的诠释,提出了新的研究思路。认为:中医学关于外感邪气致病的六经、卫气营血和三焦辨证在本质上是识别抗原与免疫活性细胞或分子相互作用的三种不同的非线性动力学诊断模式,其证相应于这一过程中不同的非线性动力状态,可以用一个或一组非线性动力学方程进行定量描述。一个证常常对应于抗原与免疫活性的细胞或分子非线性动力学相互作用相空间中的一个稳定或不稳定的极限环和混沌,这可以作为证诊断的图形或形象标识。以中医学扶正祛邪的治疗思想为先导,有可能研制出全新靶向的和更有效的治疗传染性疾病的药物。 相似文献

Microbiologic examinations of implants have shown that certain micro‐organisms described as periodontal pathogens may have an influence on the development and the progression of peri‐implant disease. This experimental study aimed to examine the bactericidal effect of irradiation with a soft laser on bacteria associated with peri‐implantitis following exposure to a photo‐sensitizing substance. Platelets made of commercially pure titanium, either with a machined surface or with a hydroxyapatite or plasma‐flame‐sprayed surface or with a corundum‐blasted and etched surface, were incubated with a pure suspension of Actinobacillus actinomyetemcomitans or Porphyromonas gingivalis or Prevotella intermedia. The surfaces were then treated with a toluidine blue solution and irradiated with a diode soft laser with a wave length of 905 nm for 1 min. None of the smears obtained from the thus treated surfaces showed bacterial growth, whereas the smears obtained from surfaces that had been subjected to only one type of treatment showed unchanged growth of every target organism tested ( P <0.0006). Electron microscopic inspection of the thus treated platelets revealed that combined dye/laser treatment resulted in the destruction of bacterial cells. The present in vitro results indicate that lethal photosensitization may be of use for treatment of peri‐implantitis. 相似文献

Bacterial resistance to antibiotics in community-acquired respiratory tract infections is a serious problem and is increasing in prevalence world-wide at an alarming rate. Streptococcus pneumoniae , one of the main organisms implicated in respiratory tract infections, has developed multiple resistance mechanisms to combat the effects of most commonly used classes of antibiotics, particularly the β -lactams (penicillin, aminopenicillins and cephalosporins) and macrolides. Furthermore, multidrug-resistant strains of S. pneumoniae have spread to all regions of the world, often via resistant genetic clones. A similar spread of resistance has been reported for other major respiratory tract pathogens, including Haemophilus influenzae , Moraxella catarrhalis and Streptococcus pyogenes . To develop and support resistance control strategies it is imperative to obtain accurate data on the prevalence, geographic distribution and antibiotic susceptibility of respiratory tract pathogens and how this relates to antibiotic prescribing patterns. In recent years, significant progress has been made in developing longitudinal national and international surveillance programs to monitor antibiotic resistance, such that the prevalence of resistance and underlying trends over time are now well documented for most parts of Europe, and many parts of Asia and the Americas. However, resistance surveillance data from parts of the developing world (regions of Central America, Africa, Asia and Central/Eastern Europe) remain poor. The quantity and quality of surveillance data is very heterogeneous; thus there is a clear need to standardize or validate the data collection, analysis and interpretative criteria used across studies. If disseminated effectively these data can be used to guide empiric antibiotic therapy, and to support—and monitor the impact of—interventions on antibiotic resistance. 相似文献

Visceral leishmaniasis (kala-azar) affecting HIV-infected patient is being reported in increasing frequency. A 40-year-old German bisexual patient with full-blown AIDS is described who presented with Kaposi's sarcoma, epigastric pain, diarrhea, and weight loss but without fever.Leishmania amastigotes were initially found in biopsies from stomach, duodenum, and a cutaneous Kaposi's sarcoma lesion but were later also recovered from bone marrow and lymph node. The patient received three courses of a combination of pentavalent antimony and interferon-. In addition to the common side effects such as fever, thrombocytopenia, and elevated amylase and lipase, a vivid progression of the Kaposi's sarcoma was noted. Tumor progression was temporally closely associated with treatment with interferon-. Because this phenomemon has also been observed in other patients, we advise caution when using interferon- in patients with Kaposi's sarcoma.Abbreviations AIDS

An outbreak of a fatal haemolytic anaemia in a dairy herd of cattle in Switzerland was shown to be associated with infections
with five vector-borne pathogens, namely Anaplasma marginale, A. phagocytophilum, Babesia bigemina, a Theileria spp belonging to the buffeli/sergenti/orientalis complex and haemotrophic Mycoplasma spp. The latter three had not been documented before this outbreak in Switzerland. To characterise the haematological and blood
chemical changes in these unique cows, packed cell volume was determined in all 286 blood samples, blood smears, and complete
haematology were performed from 285 and 173 blood samples, respectively, and biochemical parameters were assayed in 105 serum
samples. Regenerative anaemia was the key sign of illness. Red blood cells of anaemic cattle were hypochromic and macrocytic.
Anaemic animals had reduced platelet cell counts and increased total white cell counts. In addition, increased serum bilirubin,
blood aspartate aminotransferase, gamma glutamyltransferase, glutamic dehydrogenase and blood urea nitrogen and decreased
magnesium, calcium and albumin levels were found in anaemic cattle when compared to animals with normal packed cell volume.
Most changes could not be attributed to a single infection. A. marginale seemed to be important in causing the outbreak, but co-infections may have aggravated the disease development and clinical
signs. Thus, when encountering cattle with haemolytic anaemia, all of the mentioned pathogens should be included as differential

重症监护病房中呼吸机相关肺炎的病原学和耐药性特征 总被引:2,自引:0,他引:2
目的探讨重症监护病房(ICU)中呼吸机相关肺炎(VAP)的流行病学、病原菌分布、耐药性特征。方法对广州市3所三级甲等医院ICU发生VAP的53例患者进行前瞻性研究,对病原菌进行细菌鉴定和耐药性分析。结果VAP平均发病时间为机械通气后7.8d。97株病原菌中革兰阴性细菌58株(59.8%),革兰阳性细菌23株(23.7%),真菌16株(16.5%)。最常见病原菌分别为铜绿假单胞菌16株(16.5%),金黄色葡萄球菌13株(13.4%),嗜麦芽窄食单胞菌8株(8.2%),肺炎克雷伯菌8株(8.2%),白色念珠菌8株(8.2%)。耐甲氧西林金黄色葡萄球菌(MRSA)检出率为100.0%;未发现耐万古霉素金黄色葡萄球菌;VAP病原菌存在严重的多重耐药性。结论ICU中VAP的病原菌构成以革兰氏阴性杆菌为主且呈现多重耐药现象,适当的经验性抗菌治疗应以病原学和耐药性的监测结果为依据。 相似文献
