目的:探讨隔药灸神阙八阵穴对术后粘连性肠梗阻(AIO)患者疗效及血清白细胞介素-6(IL-6)、肿瘤坏死因子(TNF-α)水平的影响。方法:选取2013年1月至2017年4月唐山市人民医院收治的行腹腔手术患者280例作为研究对象,按照随机数字表法随机分为对照组和观察组,每组140例,对照组采用常规治疗;观察组在对照组基础上给予隔药灸神阙八阵穴,均治疗1周。比较2组患者临床疗效、治疗后肠功能评分、治疗前后胃动素(MOT)水平、IL-6、TNF-α水平。结果:观察组总有效率为91.43%,显著高于对照组77.14%(P<0.05);治疗后,观察组恢复排气时间、腹部症状缓解时间、肠鸣音恢复时间均短于对照组(P<0.05)、2组患者MOT水平均低于治疗前,且观察组的MOT水平低于对照组(P<0.05)、2组患者的血清IL-6、TNF-α水平均显著低于治疗前,且观察组的血清IL-6、TNF-α水平均低于对照组(P<0.05)。结论:隔药灸神阙八阵穴治疗AIO疗效显著,能改善患者胃肠功能,降低MOT水平,有效调节血清IL-6、TNF-α水平。  相似文献

目的:探讨生长抑素联合奥美拉唑治疗上消化道出血的临床效果。方法整群选取该院2013年1月_2016年5月期间收治的79例上消化道出血患者,将其随机分为研究组(40例﹚和对照组(39例﹚,研究组采取生长抑素联合奥美拉唑治疗,对照组单纯采取奥美拉唑治疗,比较两组止血时间、住院时间等。结果研究组平均止血时间(12.9±5.3﹚h、住院时间(10.4±2.2﹚d与对照组比较差异有统计学意义,P﹤0.05;研究组总有效率90.0%、再出血率12.5%与对照组比较差异有统计学意义,P﹤0.05。结论生长抑素联合奥美拉唑治疗上消化道出血效果确切,优于单纯使用奥美拉唑效果,值得临床推广。  相似文献

Pituitary adenomas (PA) represent in the majority of cases, benign tumors whose treatment currently associate surgery, medical therapies and radiotherapy in a multidisciplinary approach. While trans-sphenoidal surgery remains, except for prolactin-secreting adenomas, the first-line treatment of PA, it can considerably be hampered by the existence of an invasive and/or aggressive tumor for which medical therapies are often requested. In this review, we extensively discuss, both at molecular and clinical levels, the medical therapies currently used and in development in the different phenotypes of pituitary adenomas.  相似文献

The islet-acinar axis is of prime importance to the optimal functioning of the human pancreas. Not only is this inter-relationship important for normal physiological processes, it is also relevant in diseased states, including chronic pancreatitis and pancreatic ductal adenocarcinoma (PDAC). Early experiments, nearly 4 decades ago, explored the role of islets in the development and progression of PDAC. These led to further studies that provided compelling evidence to support the role of islets and their hormones in PDAC. This association presents oncologists with therapeutic options not only for managing, but potentially preventing PDAC, a cancer that is well known for its poor patient outcomes. This review will discuss the accumulated evidence regarding the role of islets and their hormones in PDAC and highlight areas for future research.  相似文献

Objective

To determine the efficacy of endoscopic surgery in combination with long-acting somatostatin analogues (SSAs) in treating patients with growth hormone (GH)-secreting pituitary tumor.

Methods

We performed retrospective analysis of 133 patients with GH producing pituitary adenoma who underwent pure endoscopic transsphenoidal surgery in our center from January 2007 to July 2012. Patients were followed up for a range of 3-48 months. The radiological remission, biochemical remission and complication were evaluated.

Results

A total of 110 (82.7%) patients achieved radiological complete resection, 11 (8.2%) subtotal resection, and 12 (9.0%) partial resection. Eighty-eight (66.2%) patients showed nadir GH level less than 1 ng/mL after oral glucose administration. No mortality or severe disability was observed during follow up. Preoperative long-acting SSA successfully improved left ventricle ejection fraction (LVEF) and blood glucose in three patients who subsequently underwent success operation. Long-acting SSA (20 mg every 30 days) achieved biochemical remission in 19 out 23 (82.6%) patients who showed persistent high GH level after surgery.

Conclusion

Endoscopic transsphenoidal surgery can biochemically cure the majority of GH producing pituitary adenoma. Post-operative use of SSA can improve biochemical remission.  相似文献

Background

Compounds targeting somatostatin-receptor-type-2 (SSTR2) are useful for small bowel neuroendocrine tumor (SBNET) and pancreatic neuroendocrine tumor (PNET) imaging and treatment. We recently characterized expression of 13 cell surface receptor genes in SBNETs and PNETs, identifying three drug targets (GIPR, OXTR, and OPRK1). This study set out to characterize expression of this gene panel in the less common neuroendocrine tumors of the stomach and duodenum (gastric and duodenal neuroendocrine tumors [GDNETs]).

Methods

Primary tumors and adjacent normal tissue were collected at surgery, RNA was extracted, and expression of 13 target genes was determined by quantitative polymerase chain reaction. Expression was normalized to GAPDH and POLR2A internal control genes. Expression relative to normal tissue (ddCT) and absolute expression (dCT) were calculated. Wilcoxon tests compared median expression with false discovery rate correction for multiple comparisons.

Results

Gene expression was similar in two gastric and seven duodenal tumors, and these were analyzed together. Like SBNETs (n = 63) and PNETs (n = 51), GDNETs showed significant overexpression compared with normal tissue of BRS3, GIPR, GRM1, GPR113, OPRK1, and SSTR2 (P < 0.05 for all). Of these, SSTR2 had the highest absolute expression in GDNETs (median dCT 4.0). Absolute expression of BRS3, GRM1, GPR113, and OPRK1 was significantly lower than SSTR2 in GDNETs (P < 0.05 for all), whereas expression of GIPR was similar to SSTR2 (median 4.3, P = 0.4).

Conclusions

As in SBNETs and PNETs, GIPR shows absolute expression close to SSTR2 but has greater overexpression relative to normal tissue (21.1 versus 3.5-fold overexpression). We conclude that GIPR could provide an improved signal-to-noise ratio for imaging versus SSTR2 and represents a promising novel therapeutic target in GDNETs.  相似文献

Autosomal dominant polycystic kidney disease (ADPKD) is the most frequent Mendelian inherited disorder. It covers 6.1% of incident ESRD patients in France in 2011. Long left untreated, this disease will soon benefit from targeted therapies currently under evaluation. Several molecules have already reached the stage of clinical trials: the evaluation of mTOR inhibitors yielded deceiving results and, more recently, 2 different molecules demonstrated a slight impact on the progression of total kidney volume (TKV): tolvaptan, vasopressin receptor-V2 inhibitor and somatostatin analogues; both of these molecules acting throughout the decrease of intracellular AMPc. The purpose of this review is to briefly describe the signaling pathways involved, then to present both the published and ongoing clinical trials and the promising molecules evaluated in murine models.  相似文献

应用放射免疫法对68例小于胎龄儿(其中早产36例;足月32例)生后喂奶前及第7日空腹血中生长抑素(soma-tostatin,SS)、胃动素(motilin,MOT)、胃泌素(gastrin,Gas)浓度进行测定,并以30例足月新生儿作为对照组。结果小于胎龄儿组生后喂奶前及第7日空腹血浆胃动素、胃泌素均明显低于对照组,而生长抑素浓度明显高于对照组.并且与胎龄、开始喂养时间及当日进奶等因素相关。结论小于胎龄儿消化道机能适应胃肠道营养,在严密观察下应用合理的喂养方式早日开始胃肠道营养,将能促进小于胎龄儿胃肠道的发育和成熟。  相似文献

本文采用放免测定法研究了生长抑素(somatostatin,ss)在胃及食管癌患者血浆与癌组织中的变化。结果表明:食管癌和胃癌患者血浆和癌组织中ss的含量(食管癌:17.87±5.31pg/ml,40.41±13.26pg/mg;胃癌:11.24±5.11pg/ml,130.18±37.73pg/mg)显著低于正常人(26.83±5.92,212.62±47.46pg/mg)P<0.01,提示:ss的减少可能与上消化道癌肿形成有关。  相似文献
