APN对冠状动脉粥样硬化病变严重程度的评估价值   总被引:1,自引:1,他引:0  
目的:探讨脂联素(APN)对冠状动脉粥样硬化病变严重程度的评估价值。方法:通过测定不同冠状动脉病变情况患者的血浆APN、hs-CRP含量。结果:APN含量冠心病(CHD)组低于粥样硬化组(P〈0.05),粥样硬化组低于正常组(P〈0.05);hs-CRP含量和Gensini积分CHD组均高于粥样硬化组(均P〈0.05),粥样硬化组均高于正常组(均P〈0.05)。APN含量急性冠脉综合征(ACS)组低于稳定型心绞痛(SAP)组(P〈0.05);hs-CRP含量和Gensini积分ACS组均高于SAP组(均P〈0.05)。APN含量与Gensini积分呈负相关(r=-0.36,P〈0.05),hs-CRP含量与Gensini积分呈正相关(r=0.40,P〈0.05),APN含量与hs-CRP含量呈负相关(r=-0.52,P〈0.05)。结论:APN含量降低可作为冠状动脉粥样硬化病变的标志,APN含量的高低与冠脉狭窄的严重程度有关,为临床无创性评价冠脉病变严重程度提供一种新方法。  相似文献

目的 探讨糖、脂类、C反应蛋白与冠心病患者Gensini积分的相关性.方法 冠心病组选择我院2013年1月至2015年1月收治的冠状动脉硬化性心脏病患者60例,健康组选择我院体检中心同期体检的健康体检者60例,测定总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL)、低密度脂蛋白胆固醇(LDL)、超敏C反应蛋白(CRP)、糖化血红蛋白(HbA1c),并且分析糖脂代谢指标和冠脉病变严重程度、CRP之间的关系.结果 ①与健康组对比,冠心病组HbA1c、TC、TG、LDL-C和HbA1c×LDL-C/HDL-C明显升高,HDL-C明显降低,数据比较差异具有统计学意义,P <0.05.②冠心病患者糖脂代谢指标与Gensini积分、CRP的相关性分析显示,HbA1c、LDL-C水平、HbA1c×LDL-C/HDL-C和Gensini积分、CRP呈正相关关系,HDL-C水平和Gensini积分、CRP呈负相关.结论 糖脂代谢与冠状动脉硬化性心脏病患者病情、血清C反应蛋白水平正相关,通过糖脂代谢指标能够对冠状动脉硬化性心脏病患者的病情和血清C反应蛋白进行一定的预测,具有临床检测价值.  相似文献

目的 探究多发性硬化(MS)患者血清中谷草转氨酶(AST)、谷丙转氨酶(ALT)水平和临床特点之间的关系。方法 收集中山大学附属第三医院神经内科2010年1月~2016年12月99例首次确诊为MS患者作为MS组,选择同期103名健康体检者作为对照组。检测两组血清AST、ALT水平,同时将MS组患者的扩展残疾状况评分(EDSS)及头颅磁共振结果(MRI)病灶活动性纳入统计,比较两组血清AST、ALT水平,分析AST、ALT水平与疾病活动性以及EDSS评分的相关性。结果 MS组患者血清ALT水平高于正常组[(28.55±2.68)U/L vs (19.86±1.67)U/L],差异有统计学意义(P<0.05);MS组患者AST/ALT水平较正常组降低[(1.06±0.09)vs (1.37±0.07)],差异有统计学意义(P<0.05);MS组男性患者血清ALT水平高于对照组男性,男性及女性患者的AST/ALT值均低于对照组的男性和女性,差异均有统计学意义(P<0.05);MS组患者男性血清ALT水平高于女性患者,AST/ALT低于女性患者,差异均有统计学意义(P<0.05);未发现ALT、AST水平和EDSS评分及MRI病灶活动性之间有相关性。结论 MS患者血清转氨酶水平高于正常人群,其可能是MS患者血脑屏障破坏的一个生物学标记物。  相似文献

目的 研究冠状动脉粥样硬化性心脏病(CHD)患者血浆中氧化型高密度脂蛋白(ox-HDL)水平与冠状动脉病变程度(Gensini评分)之间的相关性,ox-HDL与高密度脂蛋白胆固醇(HDL-C)的比值(ox-HDL/HDL-C)相较HDL-C对血浆HDL真实水平的代表性。方法 选取2018年7月~2019年2月于新乡医学院第一附属医院心内科二病区住院行冠状动脉造影被确诊为冠心病的患者84例为实验组,另选取冠状动脉造影正常者44例为对照组,通过酶联免疫吸附剂(ELISA)检测所有研究对象血浆中ox-HDL水平,应用Pearson相关性分析实验组内ox-HDL、ox-HDL/HDL-C分别与Hcy、冠状动脉粥样硬化病变程度(采用Gensini评分评价)的相关性。结果 实验组ox-HDL水平高于对照组,差异具有统计学意义(P<0.05);ox-HDL与Gensini评分呈现明显的正相关(r=0.335,P<0.01);oxHDL/HDL-C与Gensini评分之间呈显著正相关(r=0.819,P<0.01);多因素Logistic回归分析结果表明ox-HDL水平与冠心病的病变程度相关。结论 ox-HDL是冠心病的危险因素,应用ox-HDL及ox-HDL/HDL-C可以更好的评价冠状动脉病变程度以及冠心病患者血浆HDL-C的真实水平,ox-HDL可能会成为动脉粥样硬化的新的危险因素和治疗的新靶点。  相似文献

背景:相关研究已经显示,冠心病和骨质疏松存在共同的病理生理机制,动脉粥样硬化和骨质疏松有明显的相关性。 目的:分析骨质疏松症和冠心病患者冠脉病变严重程度的相关性。 方法:回顾性分析首次行冠脉造影并明确诊断为冠心病的322例患者资料,将其分为骨质疏松组和非骨质疏松组患者,采用多元线性回归模型分析骨质疏松和反映冠脉病变严重程度的Gensini积分间的相关性。 结果与结论:骨质疏松组比非骨质疏松组患者具有较少的冠脉单支病变(P < 0.001),更多的冠脉多支病变(P < 0.001)和更高的Gensini积分(P < 0.001)。在校正可能的影响因素后,多元线性回归显示,骨质疏松是Gensini积分的危险因素(P < 0.001)。相关性分析显示,骨密度和Gensini积分呈负相关关系(r=-0.36, P=0.004)。结果表明,骨质疏松和冠心病冠脉病变严重程度密切相关。  中国组织工程研究杂志出版内容重点:组织构建;骨细胞;软骨细胞;细胞培养;成纤维细胞;血管内皮细胞;骨质疏松;组织工程  相似文献

目的 探讨血脂异常与老年冠心病患者冠脉病变程度的相关性。方法 选取2015年5月~2017年1月本院心内科住院的明确诊断为冠状动脉粥样硬化性心脏病(CHD)的150例老年患者设为CHD组。选取同期收治的150例非CHD患者为对照组。比较两组血清中总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平,并计算N-HDL-C、LDL-C/N-HDL-C、LDL-C/HDL-C、TC/HDL-C、TG/HDL-C值。比较不同冠脉病变支数、不同病情严重程度的CHD患者的血脂及其比值。对血脂与冠脉病变支数、病情严重程度积分进行相关性分析。结果 CHD组患者LDL-C、TG、TC、N-HDL-C水平、LDL-C/HDL-C、TC/HDL-C、TG/HDL-C值均高于对照组,HDL-C水平低于对照组,差异均有统计学意义(P<0.05);不同冠脉病变支数各亚组间的TC、TG、N-HDL-C水平、TC/HDL-C比值差异存在统计学意义(P<0.05);TC、TG、N-HDL-C水平、TC/HDL-C比值均与冠脉病变支数均呈正相关(P<0.05);不同病情严重程度亚组间的LDL-C、TC、TG、N-HDL-C水平、LDL-C/HDL-C、TC/HDL-C、TG/HDL-C比值差异存在统计学意义(P<0.05);CHD患者LDL-C、TC、TG、N-HDL-C水平、LDL-C/HDL-C、TC/HDL-C、TG/HDL-C比值与冠状动脉病变积分均呈正相关(P<0.05)。结论 可以通过对血脂指标的监测,并计算N-HDL-C、TC/HDL-C、LDL-C/HDL-C、TC/HDL-C、TG/HDL-C值,来评估CHD病情的临床进展。  相似文献

目的:探讨冠状动脉粥样硬化心脏病患者血清脂蛋白(a)[Lp(a)]及血浆D-二聚体(D-D)水平与冠状动脉病变程度关系。方法:经冠状动脉造影确诊的冠状动脉粥样硬化心脏病患者66例和正常对照组50例,检测两组血清Lp(a)及血浆D-D水平。根据冠状动脉粥样硬化心脏病患者的Gensini积分,分为(1~30)分组、(31~90)分组及>90分组,并分析各组与血清Lp(a)及血浆D-D的关系。结果:冠状动脉粥样硬化心脏病组血清Lp(a)及血浆D-D与正常对照组相比差异有统计学意义(P<0.05),相关分析显示,冠状动脉病变程度与血浆D-D、血清Lp(a)呈正相关。结论:冠状动脉粥样硬化心脏病患者冠状动脉病变程度与血清Lp(a)及血浆D-D显著相关,血清Lp(a)及血浆D-D应用于冠状动脉粥样硬化心脏病的早期诊断和治疗具有重要意义。  相似文献

目的 对冠脉CTA中关于射血分数进行分析和探讨.方法 运用随机抽样的方法选择2012年3月~2014年3月在本院进行检查的可疑冠心病患者120例,其中男性患者69例,平均年龄(54.01±10.53)岁,女性患者51例,平均年龄(58.04±11.02)岁,对他们行64排动态容积CT心脏冠脉CTA检查,分析冠脉狭窄程度、冠脉病变范围以及冠脉病变Gensini积分均与射血分数之间的相关性.结果 冠脉狭窄程度、冠脉病变范围以及冠脉病变Gensini积分均与射血分数呈负相关(P<0.05).结论 冠脉CTA应用于冠心病的临床检查中具有可行性,且冠状动脉狭窄程度越重、狭窄的支数越多、Gensini积分越高,射血分数越低,表明左心室功能损害越大.  相似文献

Increasing evidence suggests an association between elevated serum aminotransferase level and the metabolic syndrome. However, the significance of relatively low levels of aminotransferase in relation to the metabolic syndrome has not been fully investigated in the general population. We investigated the association between serum amiontransferase level and the metabolic syndrome using data from a nationwide survey in Korea. We measured serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) levels and metabolic conditions among 9771 participants aged 20 or more in the 1998 and 2001 Korean National Health and Nutrition Examination Surveys. Metabolic syndrome was defined according to NCEP-ATP III criteria with a modified waist circumference cutoff (men > 90 cm; women > 80 cm). Serum aminotransferase level, even within normal range, was associated with the metabolic syndrome independent of age, body mass index, waist circumference, smoking, and alcohol intake. Compared with the lowest level (< 20 IU/L), the adjusted odds ratios (95% CI) for an AST level of 20-29, 30-39, 40-49 and > or = 50 IU/L were 1.10 (0.85-1.42), 1.37 (1.02-1.83), 1.62 (1.08-2.43), and 2.25 (1.47-3.44) in men, and 1.18 (0.99-1.41), 1.43 (1.29-1.83), 1.71 (1.09-2.68), and 2.14 (1.20-3.80) in women, respectively. Corresponding odds ratios for ALT levels were 1.27 (0.99-1.63), 1.69 (1.28-2.23), 2.17 (1.58-2.99), and 2.65 (1.96-3.58) in men, and 1.44 (1.22-1.70), 1.65 (1.26-2.15), 2.94 (1.93-4.47), and 2.25 (1.54-3.30) in women, respectively. In conclusion, elevated serum aminotransferase levels, even in the normal to near normal range, are associated with features of the metabolic syndrome.  相似文献

目的:探讨三七总皂苷(PNS)对金黄地鼠实验性高脂血症的调节机制。方法:30只金黄地鼠随机分为正常组、模型组和PNS组3组,每组10只。正常组地鼠饲喂普通饲料,其余2组地鼠饲喂高脂饲料;4周后高血脂症模型已经建成,PNS组腹腔注射血塞通注射液(50 mg/kg),正常组和模型组给予同体积生理盐水。给药12周后,处死地鼠,收集血液,全自动生化分析仪检测血清中总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、丙氨酸氨基转移酶(ALT)和天门冬氨酸氨基转移酶(AST)水平;取肝组织,采用real-time PCR、免疫组织化学和Western blot检测肝脏前蛋白转化酶枯草杆菌蛋白酶/kexin 9型(PCSK9)和低密度脂蛋白受体(LDLR)在mRNA和蛋白水平的表达。结果:成功建立高脂血症地鼠模型。与正常对照组相比,高脂模型组地鼠血清TC、TG、LDL-C和ALT水平显著增高(P<0.05),PNS干预组地鼠TC、TG、LDL-C和ALT水平显著下降(P<0.05),各组地鼠血清中HDL-C和AST水平无显著变化(P>0.05)。与对照组相比,高脂模型组地鼠肝脏中PCSK9的mRNA及蛋白水平显著升高(P<0.05),LDLR的mRNA及蛋白表达水平显著降低(P<0.05);与高脂模型组相比,PNS组地鼠肝脏组织PCSK9的蛋白表达水平显著降低,LDLR的蛋白表达水平显著升高(P<0.05),PCSK9和LDLR mRNA变化不明显。结论:PNS对高脂血症金黄地鼠具有显著降脂作用,其机制可能与调节PCSK9-LDLR信号通路有关。  相似文献

STUDY OBJECTIVES: Nonalcoholic fatty liver disease (NAFLD) is a disorder that often presents with elevated serum aminotransferase levels. Although it has classically been linked with the metabolic syndrome, recent studies suggest NAFLD may also be associated with obstructive sleep apnea (OSA). This study evaluates the association between serum aminotransferase levels and factors connected with: either the metabolic syndrome (elevated body mass index [BMI], lipid profile, blood pressure, fasting glucose), or with OSA severity (apnea hypopnea index, lowest oxygen saturation level, oxygen desaturation index, percent of time below 90% saturation [%T<90]). DESIGN: Retrospective case series. PATIENTS AND SETTING: 109 adult patients with OSA at a university hospital general clinical research center. MEASUREMENTS AND RESULTS: Markers of hypoxia (lowest oxygen saturation level and %T<90), correlated significantly with aspartate aminotransferase (AST) and alanine aminotransferase (ALT) levels (Pearson's r = -0.31 to -0.38, P <0.003), while apnea hypopnea index, body mass index, blood pressure, fasting glucose, triglyceride, and cholesterol levels did not. Hierarchical linear regression was then done to determine the best predictors of aminotransferase levels. Markers of metabolic syndrome were entered as one block and markers of sleep apnea as another. Regression analyses explained 16.3% of the variance in AST and 18.9% of the variance in ALT, with %T<90 playing the largest role. CONCLUSIONS: In patients with obstructive sleep apnea, serum aminotransferase levels are better predicted by markers of oxygen desaturation than by factors traditionally associated with the metabolic syndrome.  相似文献

Background: Considering transaminase more than the upper limit of normal value as liver injury might overestimate the prevalence of liver involvement in COVID-19 patients. No meta-analysis has explored the impact of varied definitions of liver injury on the reported prevalence of liver injury. Moreover, few studies reported the extent of hypertransaminasemia stratified by COVID-19 disease severity.Methods: A literature search was conducted using PubMed and Embase. The pooled prevalence of liver injury and hypertransaminasemia was estimated.Results: In total, 60 studies were included. The overall prevalence of liver injury was 25%. Compared to subgroups with the non-strict definition of liver injury (33%) and subgroups without giving detailed definition (26%), the subgroup with a strict definition had a much lower prevalence of liver injury (9%). The overall prevalence of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) elevation was 19% and 22%. The prevalence of elevated ALT and AST were significantly higher in severe COVID-19 cases compare to non-severe cases (31% vs 16% and 44% vs 11%). In critically ill and fatal cases, no difference was found in the prevalence of elevated ALT (24% vs 30%) or AST (54% vs 49%). Sensitivity analyses indicated that the adjusted prevalence of ALT elevation, AST elevation, and liver injury decreased to 14%, 7%, and 12%.Conclusion: The overall prevalence of liver injury and hypertransaminasemia in COVID-19 patients might be overestimated. Only a small fraction of COVID-19 patients have clinically significant liver injury. The prevalence of hypertransaminasemia was significantly higher in severe COVID-19 cases compare to non-severe cases. Hence, in severe COVID-19 patients, more attention should be paid to liver function tests.  相似文献

This study grew out of observations of certain lecithin:cholesterol acyltransferase (LCAT) abnormalities in patients with atherosclerosis. We studied the interrelationships among LCAT, and total cholesterol, free and esterified cholesterol, cholesterol in individual lipoprotein fractions, triglycerides, phospholipids, free fatty acids, L-lactates in 90 angiographically examined patients with coronary artery disease and 30 control subjects without clinical manifestations of coronary artery disease. Results of the study showed LCAT activity to be significantly decreased (P<0.05) in patients with single-, double-, or triple-vessel disease than in disease-free subjects. LCAT was also found to follow the stage of coronary artery disease in angiographically examined patients. Decreased LCAT activity was accompanied by lower high-density lipoprotein cholesterol, elevated ratio of unesterified to esterified cholesterol, and increased levels of L-lactates, free fatty acids, and low-density lipoprotein cholesterol. Total cholesterol and triglycerides were within or slightly above the normal limits. Summary Android obesity is associated with metabolic disorders, but the causality of this relationship remains unclear. We investigated the association of body mass index (BMI) and waist-to-hip ratio (WHR) with hormones, glucose tolerance, insulin sensitivity, serum lipoproteins, and the serum activity of hepatic enzymes in 40 healthy premenopausal women (BMI 19.2–46.1, mean 32.6±1.3 kg/M2; WHR 0.68-1.01, mean 0.82±0.02). BMI correlated with WHR (r=0.52, P<0.01). After correction for WHR, BMI was negatively correlated with high-density lipoprotein cholesterol and positively with total and very low density lipoprotein triglycerides, insulin sensitivity, blood glucose, serum insulin and glucagon. After adjustment for BMI, WHR was significantly associated with high-density lipoprotein cholesterol, total and very low density lipoprotein triglycerides, and the serum activities of hepatic enzymes but not with insulin sensitivity, blood glucose, serum insulin, or glucagon. According to these results, body fat distribution assessed by WHR is related to hypertriglyceridemia and alterations in hepatic function such as a fatty liver. WHR is not primarily related to glucose metabolism in healthy premenopausal women without preexisting metabolic disorders such as glucose intolerance. Therefore the observable association between android obesity and manifest impairment in glucose metabolism may develop secondarily during persisting hyperinsulinemia, which itself is primarily related to obesity. 目的 探讨中老年人血脂水平与脑白质病变(WML)发生之间的关系。方法 纳入 2012年6月—2013年6月第三军医大学大坪医院神经内科1 282例年龄≥50 岁的住院患者的临床资料进行回顾性分析,其中男606例、女676例,平均年龄(65.9±7.9)岁。所有患者行头颅 MRI检查和血脂相关指标检测。在1 282例患者中,WML者781例,按照WML发生的部位分为脑室旁白质病变(PVWML)678例和深部白质病变(DWML)724例,两者均存在621例。按照WML严重程度分类:轻度 PVWML 469例(36.6%)、中度110例(8.6%)、重度99例(7.7%),轻度DWML 486例(37.9%)、中度147例(11.5%)、重度91例(7.1%)。应用logistic 回归方法分析各血脂指标变化与WML的关系。结果 1 282例分析显示,PVWML组比无PVWML组比年龄更大(P<0.01),男性更多(P<0.01),卒中史、冠心病、高血压和糖尿病的患病率更高(P值均<0.01),血浆空腹血糖(FBG)浓度更高(P<0.05)。DWML组与无DWML组比较与PVWML类似,不同的是,两组间血浆 FBG 浓度差异无统计学意义(P>0.05)。血浆高密度脂蛋白胆固醇(HDL-C)、载脂蛋白(Apo) A-Ⅰ和TC水平在PVWML和DWML各级间的分布差异均有统计学意义(P值均<0.05);血浆低密度脂蛋白胆固醇和Apo B水平在PVWML的各级间的分布差异均有统计学意义(P值均<0.05)。在调整年龄、性别、卒中史、冠心病、高血压和糖尿病后,血浆HDL-C和Apo A-Ⅰ水平降低与PVWML和DWML的患病风险密切相关。结论 中老年人血浆HDL-C 和Apo A-Ⅰ水平降低是 PVWML和 DWML的独立危险因素,可增大WML的患病风险。  相似文献

Background: Serum biochemical liver tests (LTs) (alanine aminotransferase, aspartate aminotransferase, and gamma glutamyltransferase) and platelet counts are often used to screen for chronic liver disease. We determined the prevalence and etiologies of abnormal LTs in an adult population in Jilin, China.Methods: A total of 3791 individuals between the ages of 18 and 79 years were interviewed and then underwent ultrasonography and blood tests.Results: The prevalence of abnormal LTs was 14.77% (560 out of 3791 subjects). The risk factors for abnormal LTs were non-alcoholic fatty liver disease (NAFLD) alone, which accounted for 11.61%, metabolic syndrome alone for 25%, or both for 22.14%. Abnormal LTs were more common in male than in female subjects. The development of abnormal LTs was correlated with older age males, increased daily alcohol intake, poor quality of sleep, smoking, fasting plasma glucose, body mass index, triglyceridemia, and low-density lipoprotein. Abnormal LTs in patients with metabolic syndrome and NAFLD were associated with high fasting plasma glucose, triglycerides, body mass index, low density lipoprotein, male, young age, poor sleep quality, smoking, and alcohol intake. However, abnormal LTs in patients with hepatitis B virus were associated with gender and increased age.Conclusions: The results from the current study demonstrated that the prevalence of abnormal LTs is high in the population (14.77%). Metabolic syndrome, NAFLD, and alcohol intake appear to be potentially important causes of the observed abnormal LTs.  相似文献
