Objective To study the effect of acute hypervolemic hemodilution on expression of plasma bac-tericidaL/permeability-increasing protein (BPI) in patients undergoing total hip replacement. Methods Twenty ASA Ⅰ-Ⅱ patients undergoing elective total hip replacement were randomly divided into two groups (n=10 for thesia. The blood loss,blood transfusion and the time of operation were recorded. Venous blood samples were taken before anesthesia (T0) ,at the begining of operation (T1) ,30 min after operation (T2) ,and at the end of operation (T3) for determination of plasma bactericidal/permeability-increasing protein. Results The blood loss and the blood transfusion in HES group were significantly lower than that of LR group[blood loss: (560±90)ml vs (810±110) ml and blood transfusion: (200±100) ml vs (600±200) ml,t=5.562 and 5.657,P<0.001]. The plasma BPI concentrations in HES group were significantly increased at T2~T3 as compared to baseline value at T0 [(8.9±1.6)μg/L,(13.4±1.2)μg/L and (4.9±1.2)μg/L,P<0.05]. The plasma BPI concentrations in LR group were significantly increased at T2~T3 as compared to baseline value at T0 [(7.3±1.2)μg/L,(9.9±0.8) μg/L and (5.0±1.1)μg/L,P<0.05],but were lower than those in HES group (t=2.530 and 7.674,P=0.021 and 0.001 ). Conclusion Acute hypervolemic hemodilution with 200/0.5 hydroxyethyl starch can reduce blood transfusion during total hip replacement operation and also can increase the BPI level which would beneficial for the immunological function. 相似文献

Objective To compare the short-term effects of myocapsular flap repair through minimal posterior approach in Hip arthroplasty with traditional posterior approach in hip arthroplasty in elderly femoral neck fractures. Methods From August 2007 to may 2009, a total of 126 femoral neck fracture patients were randomly divided into two groups, with 63 patients treated with myocapsular flap repair through minimal posterior approach (the modified group), and 63 patients treated with traditional posterior approach (the traditional group). Fifty-eight patients underwent the first-ever total hip replacement and 68 patients underwent artificial double-acting femoral head replacement. Eighty-four cases were inserted with cemented implants and 42 cases with uncemented. Data of incision length, operation time, blood loss volume, drainage amount, blood transfusion volumes were recorded separately. Postoperative complications related to the surgery and hip joint function were also documented. Function of hip joint was scored with Harris scale. All data were statistically analyzed. Results Five patients died within 1 year of surgery, and 4 patients dropped out. All the remained 117 patients were followed up for 12 - 24 months. There were no significant difference in age, type of fracture and artificial the incision length in the modified group was significantly shorter than that in the traditional group([ 10. 5 ± 2. 4 ]cm vs [ 17. 2 ± 3.6 ] cm, t = 3. 012, P = 0. 004). The average operating time was significantly shorter in the modified group compared to the traditional group (t = 2. 455, P = 0. 038). Blood loss, drainage amount, blood transfusion volumes in the modified group were less than those in the traditional group( t = 3.211,2. 986 and 3. 352 ,Ps < 0. 01, respectively). Conclusion Myocapsular flap repair through minimal posterior approach in Hip arthroplasty is a reasonable, mini invasive technique for hip replacement in older with a less blood loss and shorter operating time.fractures 相似文献

The adequacy of implementation of present proteinuria diagnostic thresholds under examination of pregnant women was examined. The analysis was applied to all urine samples of pregnant women from December 2009 to March 20010. The amount of protein in urine was concurrently evaluated by turbidimetric analysis with sulfosalicylic acid, colorimetric analysis with pyrogallol red, "dry chemistry" technology (the diagnostic strips). It is established that the mentioned techniques of analysis of protein in urine provide independent results. The results of colorimetric analysis are characterized by better precision and adequacy. However, in case of pregnant women the diagnostic threshold of protein concentration should be shifted from 0.120 to 0.150 g/l. 相似文献

目的 探讨超声在评价放疗对颈动脉溃疡斑块形成的影响的价值。方法 回顾性收集经病理学证实为头颈部肿瘤、放疗前后的颈动脉超声资料以及其他基线资料完整的患者93例,比较放疗前后放疗侧颈动脉和非放疗侧颈动脉粥样硬化斑块和溃疡斑块的总数量、平均内膜-中膜厚度、最大斑块面积、最大溃疡斑块的面积、最大溃疡口的面积。结果 放疗前后颈动脉超声检查的平均间隔时间为(6.1±1.9)年;放疗前放疗侧斑块总数量、平均内膜-中膜厚度、最大斑块面积、溃疡斑块的总数量、最大溃疡斑块的面积、最大溃疡口的面积与非放疗侧比较差异均无统计学意义(P均>0.05);放疗后放疗侧斑块总数量、平均内膜-中膜厚度、最大斑块面积、溃疡斑块的总数量、最大溃疡斑块的面积、最大溃疡口的面积均较非放疗侧加重,差异有统计学意义(P均<0.05)。结论 放疗可导致头颈部肿瘤患者颈动脉粥样硬化斑块的形成和进展,且斑块具有易损性特点。 相似文献
