目的探讨~(18)F-FDG PET显像观察特发性快眼动睡眠期行为障碍(iRBD)患者脑葡萄糖代谢改变和iRBD脑葡萄糖代谢改变与病程间的相关性。方法纳入多导睡眠监测(PSG)确诊的iRBD患者20例(iRBD组)和年龄、性别匹配的健康对照者19例(对照组)。两组均行~(18)F-FDG PET脑显像。基于自动解剖标记模板将大脑划分为90个左右对称的脑区,计算各脑区葡萄糖代谢半定量值。对iRBD组和对照组各脑区葡萄糖代谢半定量值进行独立样本t检验;并对iRBD组脑葡萄糖代谢改变与病程行Pearson相关分析。结果 (1)与对照组比较,iRBD组的双侧背外侧额上回、双侧眶部额上回、双侧眶部额中回、双侧海马、双侧海马旁回、双侧杏仁核、左侧眶部额下回、左侧岛叶、左侧内侧与旁扣带脑回、左侧中央旁小叶、左侧苍白球的葡萄糖代谢半定量值均增高(P0.05);双侧距状裂周围皮质、双侧楔叶、双侧舌回、双侧枕上回、双侧枕中回、双侧枕下回、双侧角回、双侧颞上回、双侧颞中回、右侧颞横回的葡萄糖代谢半定量值均降低(P0.05)。Pearson相关分析结果,iRBD组双侧杏仁核、双侧颞上回、右侧楔叶、右侧枕上回、右侧颞横回、左侧海马、左侧颞中回的葡萄糖代谢半定量值与病程呈正相关(P0.05);而双侧眶部额上回、双侧眶部额中回、左侧中央旁小叶、左侧眶部额下回、左侧内侧和旁扣带回、右侧背外侧额上回、右侧海马旁回的葡萄糖代谢半定量值与病程呈负相关(P0.05)。结论 iRBD患者脑内存在疾病相关的葡萄糖代谢水平改变,有助于客观评估iRBD病情的变化。  相似文献

目的采用静息态功能磁共振成像技术探讨有先兆偏头痛患者的自发神经元活动,分析其脑功能网络的变化,以便更好地认识有先兆偏头痛的发病机制。方法对7例发作间期有先兆偏头痛患者和7例年龄、性别及受教育程度相匹配的健康对照行静息态功能磁共振成像扫描,分析原始数据,得出全脑低频振幅(ALFF),进行双样本t检验,并以ALFF差异脑区为感兴趣区(ROI)校正后行功能连接(FC)分析,比较两组之间影像学表现的差异。结果病例组双侧额上回、左侧眶额皮质低频振幅值显著高于对照组(t=2.18~5.12,P0.05)。病例组左侧颞中回、左侧颞下回、左侧尾状核、双侧丘脑、右侧运动前区低频振幅ALFF值显著低于对照组(t=-5.12~-2.18,P0.05);与对照组相比,病例组右侧眶额皮质、左侧额中回、双侧前扣带皮质、右侧缘上回与左侧额上回功能连接增强,病例组左侧小脑、右侧脑岛、脑干与左侧额上回之间的功能连接减弱。结论有先兆偏头痛患者头痛发作间期疼痛处理相关脑区功能异常,支持偏头痛并非单纯的发作性疾病。  相似文献

目的 研究戒断期海洛因依赖者(AHD)的脑认知功能.方法 采用3.0 T磁共振成像系统,对30例AHD(AHD组)和18名健康对照者(对照组)在完成Go/NoGo任务时行全脑功能磁共振成像(fMRI)扫描,记录反应时间(RT).结果 (1)RT:AHD组在完成Go/NoGo任务时的RT[(394±34)ms]长于对照组[(374±26)ms;P<0.05].(2)fMRI:对照组在完成Go/NoGo任务时,诱发激活双侧前额叶内侧回、前扣带回以及双侧额下回等脑区;AHD组全脑活动普遍低,仅有双侧额上回和左侧额中回激活.两组间比较,AHD组激活显著低于对照组的区域主要位于在双侧额内侧回、前扣带回、额下回及双侧颞叶等脑区(P<0.005).结论 戒断期海洛因依赖者反应抑制功能障碍仍然存在,促进其认知功能的恢复可能成为戒毒、抗复吸的有效策略.  相似文献

目的探讨18F-FDG PET显像在阿尔茨海默病与额颞叶痴呆鉴别诊断中的应用价值。方法对临床明确诊断的阿尔茨海默病(20例)和行为异常型额颞叶痴呆(20例)患者的18F-FDG PET显像资料进行回顾,分析两组患者皮质代谢降低脑区间的差异。结果视觉分析显示,两组患者均表现为皮质代谢降低,阿尔茨海默病患者以双侧颞顶叶和后扣带回代谢降低明显,以及部分额叶皮质代谢降低,而基底节和丘脑不受累,18/20患者双侧大脑半球皮质代谢降低范围和程度基本对称;额颞叶痴呆患者额叶和前颞叶皮质代谢均降低,其中11例同时伴部分顶叶皮质和基底节、丘脑等皮质下核团不同程度降低,16/20患者双侧大脑半球代谢降低程度和范围明显不对称,4例以右侧为主、12例以左侧为主。结论由于18F-FDG PET显像所显示的阿尔茨海默病和额颞叶痴呆患者之皮质代谢降低图型不同,故具有较好的鉴别诊断价值。  相似文献

目的探索急性脱瘾治疗后酒依赖患者脑区皮质厚度的异常特征及与酒依赖临床特征的关联。方法以2017年4月至2018年4月的住院男性酒依赖患者(患者组,n=33)与年龄、受教育程度匹配的健康对照(对照组,n=35)为研究对象,收集大脑皮质功能磁共振的影像学数据和患者一般人口学及临床资料;使用FreeSurfer软件处理影像学数据并计算双侧74个脑区的皮质厚度,采用协方差分析的方法比较2组间各脑区皮质厚度的差异,并对差异脑区皮质厚度与患者的临床特征进行相关分析。结果患者组皮质厚度在左侧枕中回、右侧枕上回、双侧顶下角回、双侧顶下缘上回、双侧顶上回、左侧枕极及右侧顶内沟均显著小于对照组(均P<0.00034,经Bonferoni校正);酒依赖患者初始饮酒年龄与左侧顶上回皮质厚度呈正相关(r=0.428,P=0.024)。结论顶叶后部皮质厚度的减少可能是酒依赖患者较为特征性的皮质损伤,初始饮酒年龄可能对左侧顶上回皮质的影响更明显。  相似文献

目的 探索难治性抑郁症的脑局部葡萄糖代谢模式.方法 对符合国际疾病分类标准第10版(ICD.10)抑郁症诊断标准的8例难治性抑郁症患者和8名正常对照进行静息态正电子发射计算机断层/18F-氟代脱氧葡萄糖(PET/FDG)扫描,利用参数统计图(SPM2)方法分析组间脑局部代谢差异.结果 患者组的双侧额中回、左侧眶额皮质、左顶下小叶、左腹侧前扣带回、右侧额下回、右颞是回和颞中回以及双侧背侧前扣带回FDG代谢水平明显低于对照组;而左侧中央前/后回、右侧额内侧回、右颞极、右岛叶以及双侧小脑等脑区代谢水平则明显高于对照组.上述差异均有统计学意义(P<0.005).结论 难治性抑郁症患者存在旁边缘系统代谢增高和皮质代谢降低的交互性改变的异常代谢模式.  相似文献

目的 研究抑郁症首次发作(以下简称首发)患者对不同性质情绪线索的差异脑激活反应,以探讨抑郁症患者"负性情绪偏向性"的脑活动特征.方法 14例抑郁症首发患者与14名配对健康对照者,接受国际情绪图片系统中正性-中性-负性三组图片刺激的脑功能磁共振成像(fMRI)扫描,任务为组块设计;以文拉法辛(75~150 mg/d,口服)治疗患者,随访12周;以汉密尔顿抑郁量表(HAMD17)减分率评估疗效;用神经功能影像分析(AFNI)软件处理影像数据.结果 (1)文拉法辛治疗8周时有效率为58%;12周时有效率为92%,HAMD17总分减分率为60%.(2)两组均激活的脑区包括双侧额中回、双侧背外侧前额叶皮质、左侧丘脑、双侧岛叶、双侧颞叶、双侧杏仁核和海马.(3)在正性/中性图片激活的脑区中,患者组双侧额中回(右侧0.11%,左侧0.09%)及左侧丘脑(0.31%)激活强度均低于对照组(分别为0.98%,1.17%和1.32%;P<0.05);左侧岛叶(1.03%)及双侧杏仁核(右侧0.47%,左侧0.11%)的激活强度高于对照组(分别为0.45%,-0.34%和-0.49%;P<0.05).对于负性图片,患者组左侧额中回(2.77%)、左背外侧前额叶皮质(0.18%)、左侧岛叶(1.36%)、左侧颞叶(0.33%)和右侧杏仁核(0.44%)的激活强度高于对照组(分别为1.91%.-0.32%.0.91%,-0.31%,-0.29%;P<0.05);患者组左侧丘脑激活强度(-0.79%)低于对照组(1.15%;P<0.05).(4)治疗后,对于正性图片,患者组左侧、右侧额中回及左侧丘脑激活增加为1.21%,1.14%及1.23%(P<0.05).对于负性图片,左侧额中回(2.05%)、左外侧前额叶皮质(-0.42%)及左侧岛叶(0.73%)的激活降低(P<0.05);左侧丘脑(1.53%)激活增加(P<0.05).结论 前额叶、左侧岛叶、左侧颞叶、杏仁核和左侧丘脑对不同性质情绪诱导线索的异常激活,与抑郁症首发患者偏向性情绪障碍相关;文拉法辛对前额叶、左侧岛叶和丘脑的异常激活有调节作用.  相似文献

目的采用静息态功能磁共振成像(rf MRI)技术描述月经性偏头痛患者发作间期的脑功能改变,探索其可能的病理生理学机制。方法对10例发作间期的月经性偏头痛患者和10例受教育程度、年龄相匹配的健康对照行rf MRI扫描,计算低频振幅(ALFF),采用双样本t检验得到差异的脑区。结果偏头痛组左侧额叶ALFF值较对照组显著增强(P<0.05);偏头痛组双侧枕叶、双侧丘脑ALFF值较对照组明显减弱(P<0.05)。结论月经性偏头痛患者在头痛发作间期仍然存在皮质及皮质下脑区功能异常。  相似文献

目的:基于磁共振探讨首发与复发老年抑郁症患者脑结构特点。方法:对首发与复发老年抑郁症患者30例(首发组20例,复发组10例)和正常人30名(正常对照组)进行脑磁共振成像检查并运用弥散张量成像(DTI)扫描,比较首发与复发抑郁症患者与正常对照组的异常脑区以及首发与复发患者脑白质、灰质容量及白质各向异性分数(FA)值的差异。结果:与正常对照组比较,患者组双侧额叶、右顶叶、左楔叶、左海马回灰质以及左颞上回白质T1信号强度减低;右扣带回灰质T1信号及白质T1信号强度升高(P0.01,像素集合30)。首发组和复发组患者脑白质容量未发现显著差异脑区,但复发组右海马区灰质容量较首发组明显减小(P0.01,像素集合30)。与首发组比较,复发组右额中回、左颞中回白质FA值减低(P均0.01,像素集合30)。结论:首发与复发老年抑郁症患者可能存在前额叶及右扣带回功能缺陷;双侧海马区灰质容量可能与临床症状有关。  相似文献

目的探讨偏头痛与脑白质变性的关系及其可能形成机制。方法偏头痛患者(n=55)行头颅MRI检查,部分行脑ECT脑血流灌注显像。同期本院非神经系统疾病并行头颅MRI检查的内科住院病人作为对照组(n=51)。结果偏头痛组中,头颅MRI显示16例脑白质内见小点片样等T1或稍长T1,长T2异常信号,以多发性为主;对照组仅3例患者可见大脑半球白质内异常信号,两者之间差异有显著性意义。偏头痛组中3例患者做头颅ECT脑血流灌注显像检查,在单侧或双侧的额、颞部的脑局部血流灌注下降。对照组中4例行头颅ECT,未见脑局部血流灌注下降。结论反复偏头痛发作可能同脑白质变性具有相关性,其形成机制可能与脑血管舒缩功能异常等有关。  相似文献

Volumetric alterations in limbic structures have been detected in adults, but not in adolescents with borderline personality disorder (BPD). We examined adolescents in the early stages of BPD to provide a unique opportunity to investigate which parts of the brain are initially affected by the disorder before confounding factors such as long-term medication or chronicity can mask them. A group of 60 right-handed female adolescents between 14 and 18 years of age (20 patients with BPD, 20 clinical controls, and 20 healthy controls) underwent magnetic resonance imaging (MRI). Focus was on the examination of hippocampal and amygdalar volume differences. Furthermore, a cortical thickness analysis was conducted. FreeSurfer software detected significant group differences in the right and left hippocampus and in the right amygdala. Additionally, significant volume reductions in frontal (right middle frontal gyrus, orbital part of the inferior frontal gyrus bilaterally), and parietal regions (superior parietal gyrus bilaterally) were found in adolescents with BPD compared with controls. No group differences in cortical thickness were revealed.  相似文献

The large majority of structural MRI studies of major depressive disorder (MDD) investigated volumetric changes in chronic medicated patients in whom course of illness and treatment effects may impact anatomic measurements. Further, in few studies, separate measurements of cortical thickness and surface area have been performed that reflect different neurobiological processes regulated by different genetic mechanisms. In the present study, we investigated both cortical thickness and surface area in first-episode, treatment-naïve, mid-life MDD to elucidate the core pathophysiology of this disease and its early impact on the brain. We observed increased cortical thickness in the right hemisphere, including medial orbitofrontal gyrus, pars opercularis, rostral middle frontal gyrus and supramarginal gyrus. Increased thickness of rostral middle frontal gyrus was negatively related with depression severity on the Hamilton Depression Rating Scale. Furthermore, MDD patients showed significantly increased associations in cortical thickness measurements among areas where increased cortical thickness was observed. Analysis of pial area revealed a trend toward increased surface area in the left parahippocampal gyrus in MDD. To permit comparison of our data with those of previous gray matter volume studies, voxel-based morphometry was performed. That analysis revealed significantly increased gray matter volume in left paracentral lobule, left superior frontal gyrus, bilateral cuneus and thalamus which form limbic-cortico–striato–pallido–thalamic loops. These changes in first-episode, treatment-naïve, mid-life MDD patients may reflect an active illness-related cortical change close to illness onset, and thus potentially provide important new insight into the early neurobiology of the disorder.  相似文献

PURPOSE: To determine the nature and extent of regional cortical thinning in patients with mesial temporal lobe epilepsy (MTLE). METHODS: High-resolution volumetric MRIs were obtained on 21 patients with MTLE and 21 controls. Mean cortical thickness was measured within regions of interest and point-by-point across the neocortex using cortical reconstruction and parcellation software. RESULTS: Bilateral thinning was observed within frontal and lateral temporal regions in MTLE patients relative to controls. The most striking finding was bilateral cortical thinning in the precentral gyrus and immediately adjacent paracentral region and pars opercularis of the inferior frontal gyrus, extending to the orbital region. Within the temporal lobe, bilateral thinning was observed in Heschl's gyrus only. Ipsilateral only thinning was observed in the superior and middle temporal gyri, as well as in the medial orbital cortex. Greater asymmetries in cortical thickness were observed in medial temporal cortex in patients relative to controls. Individual subject analyses revealed that this asymmetry reflected significant ipsilateral thinning of medial temporal cortex in 33% of patients, whereas it reflected ipsilateral thickening in 20% of MTLEs. DISCUSSION: Patients with MTLE show widespread, bilateral pathology in neocortical regions that is not appreciated on standard imaging. Future studies are needed that elucidate the clinical implications of neocortical thinning in MTLE.  相似文献

ObjectiveTo investigate structural and functional alterations in patients with idiopathic rapid eye movement (REM) sleep behavior disorder (iRBD) compared with healthy controls.MethodsTwenty-seven patients with polysomnography-confirmed iRBD and 33 healthy subjects were recruited. All subjects underwent a 3-tesla structural and resting-state functional magnetic resonance imaging (fMRI) examination. Voxel-based morphometry (VBM) analysis was performed to assess grey matter alterations between groups. The amplitude of low-frequency fluctuations (ALFF) was calculated and then compared to measure differences in spontaneous brain activity. Correlations were performed to explore associations between imaging metrics and clinical characteristics in iRBD patients.ResultsCompared with healthy controls, patients with iRBD had decreased grey matter volume in the frontal, temporal, parietal, occipital cortices as well as increased grey matter volume in cerebellum posterior lobe, putamen, and thalamus. Patients with iRBD also exhibited increased ALFF values in the right parahippocampal gyrus. Olfaction correlated with ALFF value changes in occipital cortices.ConclusionsPatients with iRBD had widespread decreases of grey matter volume. Increases of grey matter volume in cerebellum, putamen, and thalamus may suggest a compensatory effect, while the altered ALFF values in parahippocampal gyrus and occipital cortices may play a role in the underlying process of neurodegeneration in this disorder.  相似文献

To determine longitudinal rates of cortical atrophy in classical Amyotrophic lateral sclerosis (ALS) and ALS variants. Rates of cortical thinning were determined between 2 scans, 3–15 months apart, in 77 ALS patients: 51 classical, 12 upper motor neuron (UMN), and 14 lower motor neuron (LMN) ALS variants. Cortical thickness at the first assessment was compared with 60 healthy controls matched by age and gender. Atrophy rates were compared between patient sub-groups and correlated with disease duration, progression, and severity. Using a cross-sectional analysis, we found a significant difference in cortical thickness between ALS patients and controls in the motor and extra-motor areas (left medial orbito frontal gyrus, left inferior parietal gyrus, bilateral insular cortex, right fusiform gyrus, bilateral precuneus). Using a longitudinal analysis, we found a significant decline of cortical thickness in frontal, temporal, and parietal regions over the course of the study in ALS patients. Effects were independent of the clinical subtype, with exception of the precentral gyrus (p < 0.001). The LMN ALS variants demonstrated the highest rates of cortical thinning in the precentral gyrus, the UMN-dominant subjects exhibited intermediate rates of atrophy, and the classical ALS patients exhibited no such change. Atrophy of the precentral gyrus in classical ALS indicates a floor effect at the first assessment, resulting in a lack of further atrophy over time. Structural loss of the precentral gyrus appears to be an early sign of classical ALS. Over time, patterns of cortical thinning in extra-motor areas can be identified in ALS, regardless of the phenotype.  相似文献

The aim of the current study was to use whole brain voxel-based morphometry(VBM)to assess the gray matter(GM)changes in unmedicated patients with obsessive-compulsive disorder(OCD)compared with normal controls.We compared the GM volumes in28 patients with 22 matched healthy controls using a1.5T MRI.Three-dimensional T1-weighted magnetic resonance images were obtained from all participants.VBM was performed to detect GM volume differences between the two groups.We detected increased regional GM volumes in the bilateral middle temporal gyri,bilateral middle occipital gyri,bilateral globus pallidus,right inferior parietal gyrus,left superior parietal gyrus,right parahippocampus,right supramarginal gyrus,right medial superior frontal gyrus,and left inferior frontal opercular cortex in the OCD patients relative to controls(P〈0.001,uncorrected,cluster size〉100 voxels).No decreased GM volume was found in the OCD group compared with normal controls.Our findings suggest that structural changes in the GM are not limited to fronto-striato-thalamic circuits in the pathogenesis of OCD.Temporo-parietal cortex may also play an important role.  相似文献

OBJECTIVE: To investigate whether congenital and clinically quiescent arachnoid cysts (AC) in the left temporal fossa alter the functional organization of adjacent cortices. METHODS: fMRI mapping was applied in five right-handed asymptomatic patients to determine the functional organization of language. Moreover, morphometry was performed in each patient to gain the size of cortical surface areas and cortical thickness values in the neighboring brain adjacent to the AC and explicitly in the left opercular region. RESULTS: Four patients showed a clear left hemisphere language dominance regardless of the cyst size; a mixed laterality of language organization was found in the remaining patient. An interesting dissociation of morphometric data was assessed when comparing strongly language-related cortices in the inferior frontal gyrus with the entire neighboring cortices. Morphometry in the neighboring brain regions of the AC showed 1) overall reduced cortical surface areas and 2) a decrease in cortical thickness compared to the homologous right side. However, the surface area of the fronto-opercular region in the left inferior frontal gyrus-i.e., the pars triangularis and the pars opercularis-was larger on the left as compared to the right side. Both structures have earlier been identified to represent the morphologic substrate of language dominance in the left hemisphere. CONCLUSION: Arachnoid cysts do not disturb the normal asymmetry of hemisphere language organization despite delicate locations adjacent to the left inferior frontal gyrus.  相似文献

Purpose: Atypical morphology of the surface of the cerebral cortex may be related to abnormal cortical folding (gyrification) and therefore may indicate underlying malformations of cortical development (MCDs). Using magnetic resonance imaging (MRI)–based analysis, we examined cortical morphology in patients with juvenile myoclonic epilepsy (JME). Methods: MRI data was collected for 24 patients with JME and 40 demographically matched healthy controls. FreeSurfer, an automated cortical surface reconstruction method, was applied to compare cortical morphology between patients and controls. Areas of anomalous cortical morphology were defined as regions of interest (ROIs) to contrast regional cortical parameters, such as surface area, average thickness, and mean curvature between patients and controls. Key Findings: In patients with JME, changes to cortical morphology were detected in several regions. In the left hemisphere, these were in insular and cingulate cortices, occipital pole, and middle temporal and fusiform gyri. In the right hemisphere, changes were detected in insular cortex, inferior temporal gyrus, and precuneus. Further analysis of ROIs revealed that these changes are related to differences in surface area rather than average cortical thickness. In addition, mean curvature abnormalities were detected in the insula bilaterally, the left cingulate cortex, and right inferior temporal gyrus. Significance: The morphologic findings in this study suggest that structural abnormalities in JME extend beyond mesial frontal lobe regions of the brain. These may be indicative of areas of subtle cortical folding abnormality related to early disruption of cortical development.  相似文献

目的 运用MRI及FMRIB software library(FSL)和Freesurfer软件包分析技术,研究遗忘型轻度认知功能损害(amnestic mild cognitive impairment,aMCI)患者全脑皮质结构改变情况.方法 对20例aMCI患者和20名年龄、性别、文化程度相匹配的健康志愿者,应用SEMENTS trio3.0 T MRI仪,采用高分辨扫描技术获取大脑精细结构立体像,然后应用FSL软件和Freesurfer软件包进行数据分析和后处理,计算出全脑不同部位皮质密度和厚度,比较aMCI组与健康对照组皮质结构特征的区别.结果 与健康对照组相比,aMCI组的左侧额叶、顶叶、颞叶皮质密度显著降低,右侧丘脑、颞叶及左侧岛叶皮质密度轻度降低;aMCI组左侧前扣带回[(2.19±0.24)mm]、顶下小叶[(2.27±0.15)mm],双侧海马旁回[(2.03±0.15)、(2.04±0.17)mm]、额上回[(2.42±0.34)、(2.40±0.28)mm]、额中回[(2.31±0.31)、(2.33±0.29)mm]、颞极[(3.41±0.68)、(3.30±0.56)mm]、颞横回[(2.04±0.12)、(2.01±0.11)mm]、中央前回[(2.20±0.11)、(2.31±0.19)mm]、中央后回[(1.88±0.11)、(1.82±0.09)mm]、缘上回[(2.53±0.33)、(2.55±0.23)mm]的皮质厚度显著降低(t=2.13~3.75,P<0.05),其余部位无明显改变(t=0.09~1.88,P>0.05).结论 aMCI患者大脑多个部位存在皮质结构改变,皮质厚度的变薄早于密度的降低.  相似文献
