颞浅动脉额支皮瓣的应用解剖学   总被引:12,自引:1,他引:12  
目的:为带蒂颞浅动脉额支皮瓣转移修复眼部软组织缺损提供解剖学资料。方法:对头面部乳胶灌注的标本进行解剖,并对颞浅动脉的走行、分支、分布进行观测。结果:颞浅动脉起自颈外动脉,穿腮腺实质上行,在颧弓上方分为额、顶两终末支。额支又于外毗上方分为额顶支和额眶支。额支、额顶支、额眶支血管外径均在1mm左右,可游离血管长度符合要求。结论:以颞浅动脉额支为蒂的皮瓣可用来转位修复眶周软组织缺损。  相似文献

The temporoparietal, parieto-occipital flaps or the forehead flaps that are used in reconstructive surgery are prepared on the superficial temporal artery (STA) and its branches. For a successful surgery and a suitable flap design, adequate anatomical knowledge is needed. In our study, the red colored latex solution was injected into the external carotid artery; the STA and its branches were dissected in 27 specimens. The mean diameter of the STA at the zygomatic arch was determined as 2.73±0.51 mm. The diameters of the frontal branch were bigger than those of the parietal branch in 15 samples out of 27. The diameters of both the frontal and parietal branches were equal in four samples. The diameter of the parietal branch was bigger than that of the frontal branch in eight samples. In 20 samples out of 27 (74.07%), the bifurcation point of the STA was above the arch. In six samples (22.22%), the STA bifurcated directly over the arch. In only one sample (3.70%), bifurcation was not observed and the STA continued only as a frontal branch (absence of the parietal branch). The absence of the frontal branch was not encountered. In one sample (3.70%), double parietal branches were observed. In six samples out of 27 (22.22%), zygomatico-orbital artery was not encountered. In 21 samples (77.77%), zygomatico-orbital arteries ran towards the face, parallel to zygomatic arch and distributed in the orbicularis oculi muscle. The transverse facial artery existed in all samples. The auricular branches running to the helix and tragus were observed in all samples. The STA was 16.68±0.35 mm at the front of the tragus. Some landmarks were chosen on the head and then the STA was observed where it crossed all of these landmarks. This paper confirms the well-known variability of the superficial temporal arterial branches and their relation to the pericranial region. Knowledge concerning the arterial features of the lateral forehead region is important for the aesthetic surgeon. STA and its branches have been found to be suitable for use in microvascular anastomoses. A better understanding of the midline forehead vascularity should allow modification of reconstructive techniques and reduce postoperative complications.  相似文献

Additional localizing superficial landmarks for intracranial structures can be of use to the neurosurgeon. This study was performed to evaluate the usefulness of the superficial temporal artery (STA) as an external landmark for deeper brain structures. Thirteen adult cadavers (26 sides) underwent latex injection of their STA bilaterally. Dissections were next carried out to identify this vessel. Once the STA and its frontal and parietal branches were skeletonized, craniectomies were performed and the underlying dura mater excised. Measurements were made between the frontal and parietal branches of the STA and deeper brain structures. The STA was found to branch on average 3 cm superior to the tragus. The bifurcation of the STA was found to commonly bifurcate at the level of the floor of the middle cranial fossa or superior temporal gyrus. The Sylvian fissure was found at a mean of 2 cm superior to the STA bifurcation. The angle between the frontal branch of the STA and the zygomatic arch had a mean of 37 degrees. The angle between the frontal and parietal branches of the STA had a mean of 87 degrees. At the level of the glabella, the frontal branch of the STA was on average 3 cm posterior to the frontal pole. The temporal tip was located a mean of 3.2 cm anterior to the frontal branch of the STA. The plane of the foramen of Monro was found to lie at a mean distance of 2.3 cm posterior to the frontal branch of the STA. The parietal branch of the STA was noted to travel more or less parallel with the central sulcus in all specimens and to travel an average of 2 cm posterior to this sulcus. At the level of the lateral attachment of the tentorium cerebelli, the parietal branch of the STA was found to travel a mean of 4.8 cm anterior to the entrance of the vein of Labbé into the transverse sinus. The parietal branch of the STA was also found to travel a mean of 4.2 cm anterior to the angular gyrus and 3.9 cm anterior to the supramarginal gyrus. Palpation or Doppler identification of the STA and its branches with subsequent mapping on the lateral cranium may prove useful as an additional superficial landmark for the neurosurgeon.  相似文献

目的:为带颞浅血管蒂颅骨外板骨瓣转位修复颌面部缺损提供解剖学基础。方法:在30侧经动脉灌注乳胶的成人尸体标本和8头颈铸型标本(5例保留骨骼)上,对颞浅动脉的走行、分支及分布进行观测,并在标本上进行模拟术式设计。结果:颞浅动脉起自颈外动脉,于外耳门前方越过颧弓根部浅出至皮下,在皮下组织和颞浅筋膜之间向前上走行,至颧弓上方分为额、顶二终支,其分支营养颅顶部大至3/4区域的软组织、骨膜和颅骨外板,颞浅动脉主干及额支、顶支动脉外径均大于1mm,浅出部位至颞线处可游离长度为8cm以上。结论:设计以颞浅动脉为蒂颅骨外板骨瓣转位修复颌面部缺损具有可行性。  相似文献

目的为颞动脉活检提供颞浅动脉额支相关的解剖学资料。方法对16具防腐成人尸体标本进行解剖,对颞浅动脉额支进行相关的解剖学观测、测量。结果①颞浅动脉额支在起点和末梢分叉点之间的长度为(45.93±7.29)mm;②颞浅动脉额支在起点和末梢分叉点的坐标值分别为(22.95±3.81,17.32±4.54)mm和(59.93±8.94,37.48±5.37)mm;③颞浅动脉额支在起点和末梢分叉点之间直线相关,直线回归方程为y赞=7.06+0.50X,相关系数r=0.68,P<0.001。结论深入了解颞浅动脉额支的解剖学特点及其与周围结构的解剖学关系,有利于颞动脉活检中颞浅动脉额支的定位及防止周围结构的损伤。  相似文献

目的:提供带腓浅神经及其营养血管筋膜皮瓣的形态学依据。方法:在32侧成人下肢标本上,观测了腓浅神经及其营养血管、周围皮肤的血供情况。结果:腓浅神经(皮下段)近侧的血供为腓浅动脉深支的皮动脉、第1支肌间隔动脉,起始处外径分别为0.8mm、1.1mm;第1支肌间隔动脉穿出深筋膜前长为1.8cm。远侧则为腓动脉穿支之升支、降支的皮支和足背动脉皮支,起始处外径分别为0.9mm、0.7mm和0.8mm,穿出深筋膜前长分别为1.2cm、0.7cm和0.8cm。其神经支在神经干内或旁彼此吻合,构成纵向链式血管网,并借分支与皮肤、皮下及筋膜血管网沟通。结论:可设计带皮神经及其营养血管筋膜皮瓣,顺行或逆行转位修复邻近部位软组织缺损  相似文献

目的:为前臂桡侧头静脉-皮神经营养血管远端蒂复合瓣设计提供解剖学基础。方法:动脉灌注红色乳胶成人上肢标本,解剖观测头静脉-前臂外侧皮神经下1/3段营养血管的来源、分支及其与桡骨膜血管的关系。结果:头静脉-前臂外侧皮神经下1/3段的营养血管来自:桡动脉皮支,掌浅支皮支,桡骨茎突返支皮支和桡动脉肌间隙骨皮支。上述诸支血管发皮支、筋膜支、骨膜支、神经-浅静脉营养血管,形成皮神经-头静脉血管链以及深、浅筋膜和骨膜血管网。结论:前臂桡侧缘头静脉-皮神经营养血管与肌、骨、皮营养血管同源,以桡骨茎突返支为蒂的远端蒂复合瓣,旋转轴点在腕关节平面,可用于手部远处组织缺损修复。  相似文献

目的:为带桡神经浅支及其营养血管筋膜皮瓣的临床应用提供形态学基础.方法:在32例成人上肢标本上,观测了桡神经浅支浅段及其营养血管、以及该血管与筋膜皮肤的供血关系.结果:桡神经浅交浅段的血供主要来自桡动脉的肌皮支或皮支,其中以桡动脉显露段的粗大皮支,鼻咽壶段的茎突返支及虎口区的皮动脉较为恒定,起点外径分别为0.8mm、0.7mm及0.6mm:穿出深筋膜前长分别为0.8cm、1.1cm及0.5cm.皮动脉的神经支在神经旁或神经干内相互沟通形成纵向链状血管网,并借吻合支与皮动脉筋膜皮支构成的皮下血管网、筋膜血管网等连接.结论:可设计成带桡神经浅支及其营养血管的顺行或逆行筋膜皮瓣,转位修复邻近部位的软组织缺损.  相似文献

Purpose

This study was performed to investigate any bilateral differences in the vascular pattern and microsurgical relevant parameters of the superficial temporal artery (STA) in vivo.

Methods

Digital subtraction angiographies of the STA of 38 individuals were retrospectively analyzed. A bilateral comparison of the branching pattern as well as of surgically relevant diameters and lengths of the main branches of the STA was performed. Moreover, gender-specific differences were assessed.

Results

Only 10 cases (26 %) demonstrated an identical type and subtype of the STA pattern bilaterally. The diameters of the STA at its origin and bifurcation level as well as of its parietal branch were statistically significant wider on the right than on the left side (p o  = 0.0009, p b  = 0.006, p p  = 0.030). Moreover, the diameters of the STA at its origin level on the right side, at its bifurcation level on both sides and of the frontal branch on both sides were statistically significant wider in males than females. No statistically significant differences of the lengths of the STA and its main branches between the right and the left side were identified.

Conclusion

The occurrence of the vascular pattern of the STA in vivo is random for each side of the same person and cannot be predicted by the vascular pattern of the opposite side. The calibers of the main branches of the STA are gender specific and commonly larger on the right than on the left side.

目的探讨小儿面神经的动脉来源及分布。方法10具小儿尸体头颈部标本经双侧颈总动脉插管,加压注入红色乳胶,观察面神经的动脉来源及分布。结果面神经颅外段营养动脉来源于茎乳动脉、颞浅动脉面神经支、面横动脉、颈外动脉上面神经支、颈外动脉下面神经支、面动脉后面神经支和面动脉前面神经支。面神经颅内段营养动脉来源于茎乳动脉、脑膜中动脉的岩支、迷路动脉。结论为面神经血供来源及分布提供形态学基础,避免手术时损伤。  相似文献

目的:为侧颌颈部真皮下血管网皮瓣提供形态学基础。方法:①2例新鲜成人尸体头颈部标本,分别灌注红色乳胶及明胶墨汁,切取皮瓣,后者制成透明标本;②在10侧头面部铸型标本上,观测侧颌颈部皮肤和皮瓣蒂部的动脉血管分布及静脉回流。结果:侧颌颈部皮肤血供主要来源于面动脉、颞浅动脉、枕动脉的分支,亦有甲状腺上动脉的终末支分支营养,并具有一定的方向性。其蒂部血供来源于颞浅动脉小穿支形成的真皮下血管网。颞浅动脉和面动脉的小穿支在侧颌颈区相互吻合。结论:侧颌颈部真皮下血管网皮瓣可以依赖于耳前蒂部小穿支真皮下血管网供血成活,但临床应用时应考虑其方向性。  相似文献

目的对人体颌面部血管进行巨微解剖及血管铸型,为临床皮瓣设计提供解剖学依据。方法①3例正常成人头颈部标本,经颈总动脉灌注红色乳胶,由浅入深逐层解剖,观测并记录颌面部血管的起始处外径、走行、分支、分布、吻合及伴行情况;②3例正常成人新鲜头颈部标本,加压注入红色过氯乙烯填充剂,经腐蚀后观察颌面部血管的来源、走行、分支、分布、吻合及伴行情况。结果颌面部的血供主要来源于面动脉、眼动脉、上颌动脉和颞浅动脉的分支。面动脉的分支主要供血面颊部、唇部、鼻部和颏区;眼动脉的分支主要供血眼球、眼球外肌、泪腺、鼻背和眼睑;上颌动脉的分支主要供血牙及牙龈、颊部、下颌关节、鼻腔、腭、咀嚼肌;颞浅动脉的分支主要供血腮腺、眼轮匝肌、上下睑外侧皮肤、颞部的软组织及面神经颧支、上颊支和颞支。结论颌面部动脉血管系统的解剖学资料,可为临床颌面部整形美容术提供了应用参考依据。  相似文献

Palsy of the external branch of the superior laryngeal nerve (EBSLN) during thyroid surgery is an important complication reported with varying frequency. This study was carried out to investigate the relationship between the EBSLN, the upper part of the thyroid gland and the inferior constrictor muscle of the pharynx (IC), and also to define consistent landmarks for identifying and preserving the EBSLN. Forty neck halves of 20 cadavers were dissected. Measurements were obtained between the crossing point of the EBSLN with the superior thyroid artery (STA) and the upper pole of the thyroid gland, the point where EBSLN penetrates the IC and the inferior thyroid tubercle, and the middle point of the oblique line of the thyroid cartilage, and the EBSLN. In 22.5%, the EBSLN crossed the STA more than 1 cm above the upper pole of the thyroid gland (Type I of Cernea et al. [1992a] Head Neck 14:380-383). In 60%, the EBSLN crossed the STA less than 1 cm above the upper pole of the thyroid gland (Type IIa of Cernea et al. [1992a] Head Neck 14:380-383). In 17.5%, the EBSLN crossed the STA under the upper pole of the thyroid gland (Type IIb of Cernea et al. [1992a], Head Neck 14:380-383). In 22.5%, the full course of the nerve was superficial to the IC (Type 1 of Friedman et al. [2002] Arch Otolaryngol Head Neck Surg 128:296-303). In 67.5%, the nerve penetrated the IC (Type 2 of Friedman et al. [2002] Arch Otolaryngol Head Neck Surg 128:296-303). In 10%, the nerve could not be identified at the lateral side of the IC (Type 3 of Friedman et al. [2002] Arch Otolaryngol Head Neck Surg 128:296-303). In conclusion, it is possible to identify the nerve superficial to the IC in 90% of specimens on average. Knowledge of the relationship between the EBSLN, IC, inferior thyroid tubercle, oblique line of the thyroid cartilage and the sternothyroid muscle will be useful for the surgeon in avoiding unexpected complications.  相似文献

目的 研究上颌动脉翼腭段的走行及分支规律,为翼腭窝内动脉结扎、肿瘤切除和颅面外科手术提供解剖学依据。方法 采用3种手术入路解剖21具成人尸头,观测上颌动脉翼腭段及分支的行程、管径、长度和毗邻关系。结果 上颌动脉翼腭段行于上颌骨颞下面后上区内,分为5型:Y型26.19%、中间型33.33%、T型21.43%、M型11.90%和其他型7.14%。上颌动脉翼腭段外径为(2.61±0.39)mm,总长为(19.44±3.62)mm;其分支有上牙槽后动脉、眶下动脉、圆孔动脉、翼管动脉、腭降动脉、蝶腭动脉、腭鞘动脉,分支走行变异常见;颞深前动脉可作为确定上颌动脉翼腭段的参考标志。结论 熟悉上颌动脉的分支、分型及走行对指导翼腭窝区手术及降低术后并发症具有重要意义。  相似文献

目的 探讨阴阜区穿支血管的解剖学特征,为修复头面部带毛区皮肤软组织缺损提供解剖学基础。 方法 选用体积分数10%甲醛常规防腐固定的成人尸体标本15具(30例,男18例、女12例),采用乳胶灌注、显微解剖、摹拟手术等方法,重点观测:①阴阜区穿支血管的起始、走行、分支与分布;②阴阜区穿支间的吻合情况。 结果 阴阜区皮肤血供主要由阴部外浅动脉供养,该动脉自股动脉发出后行向内上,在大隐静脉末段内、外侧分出上、下两支。上支向内上跨过腹股沟韧带进入阴阜部,并在前正中线附近与对侧同名动脉相吻合,走行过程中发皮支供养阴阜部阴毛覆盖区大部皮肤。下支近水平行向内侧进入耻骨前区,沿途分支营养股内侧部上份、耻骨前区及阴囊(唇)。上、下支起始外径分别为(1.2±0.2)mm和(1.1±0.3)mm。 结论 阴阜区皮肤血供丰富,可以阴部外浅动脉穿支为蒂设计带阴毛阴阜区皮瓣修复头面部带毛区皮肤软组织缺损术式。  相似文献

Detailed observations were made of the a. malaris in 25 adult goats by means of the acryl plastic injection method and the findings obtained were evaluated in comparison with those for other mammals. The malar artery arose from the superior wall of the infraorbital artery, lateral to the infraorbital nerve and superomedial to the maxillary tuber, independently or rarely in common with the superior alveolar artery. It first passed anterolaterally in the sulcus malaris on the superior surface of the lacrimal bulla and gave rise to the third palpebral branch independently or rarely in common with the inferior oblique muscular branch beneath the obliquus inferior muscle, and also the main and accessory inferior oblique and the maxillary sinus branches. The third palpebral branch gave off the periosteal, the conjunctive, the supero- and inferolateral branches. After the malar artery gave off the zygomatic branch on the orbital surface of the zygomatic bone, it passed anterosuperiorly up to the incisura malaris at the medial end of the infraorbital margin of the lacrimal bone and gave off the medial superior and inferior palpebral arteries or a common trunk between them. It continued to pass forwards as the nasal radical branch after giving off the infraorbital marginal branch and anastomosed with the nasal dorsal branch of the superficial temporal artery. The medial inferior palpebral artery formed the inferior palpebral arterial arch by anastomosing with the lateral inferior palpebral artery of the superficial temporal at the lateral canthus. The inferior palpebral marginal, the ocular orbicular muscular and the conjunctive branches diverged from the above arterial arch. The medial superior palpebral artery gave off the lacrimal canalicular and the nasolacrimal canal branches and anastomosed with the lateral superior palpebral artery or the frontal branch of the superficial temporal at the medial canthus. The characteristic features of the malar artery in the goat were thus the third palpebral branch occasionally diverging from the external ophthalmic artery of the maxillary artery, a main and several accessory inferior oblique muscular, the maxillary sinus branches and the zygomatic branches.  相似文献

Knowledge of the anatomy of the median nerve is important in surgery of the palmar aspect of the hand. The purpose of our study was to investigate the ramification pattern of the thenar branch before entering the thenar fascia and the distribution of the terminal branches in the thenar musculature. The observations were carried out on 144 hands of 74 dissecting room cadavers. According to the number of the thenar branches entering the thenar fascia we classified our specimens into four types. In 121 hands (84%, Group I) the thenar branch piercing the thenar fascia was a single trunk. In 19 hands (13.2%, Group II) two branches; in three (2.1%; Group III), three branches; and in one hand (0.7%; Group IV), four branches were identified entering the thenar fascia. Accessory thenar nerve was found in 8.3% of hands. The further division of each branch to its terminal branches was investigated in detail. Our results show that the more the number of thenar branches entering the thenar fascia, the less the terminal branching. Because more than one branch was seen in 16% of the specimens, meticulous dissection is required for preventing injury of the thenar branches before entering the thenar fascia.  相似文献
