瞑眩反应是疾病趋愈的一种祛邪反应,古今医家对其发生机制的观点不一。文章收集、整理瞑眩反应相关文献,总结古、今对瞑眩反应成因的看法,介绍吕文良教授从引阳入阴论瞑眩的经验,以期为临床对其进一步探索提供参考。吕文良教授认为久病之体,宿邪内伏,湿热痰瘀交阻而脏腑经络不通,致卫阳独行于外,不得入阴,机体阴阳失交,而瞑眩反应是机体借助药物作用,引阳入阴,使阴阳复交、正气渐盛,疾病自去而成的一种佳兆。  相似文献

The rapidly increasing of cancer risk nationwide and worldwide has threatened human health and caused the changes of disease and death spectrum. MicroRNA (MiRNA) as cancer biomarker on susceptibility has enjoyed a high level of concern. This article will discuss the association between miR-146 rs2910164 polymorphism and cancer susceptibility in 38 independent case-control studies from 34905 individuals. The 38 case-control studies which were searched from PubMed were used for conducting a meta-analysis. There were 14670 cases and 20235 controls. ORs and 95% CIs were used for reflecting the strength of association between miR-146a rs2910164 polymorphism and cancer susceptibility. Subgroup analysis based on the cancer type, ethnicity and study designs. All analysis were performed by using the Stata 11.0 software. MiR-146a rs2910164 polymorphism and overall cancer susceptibility were significantly uncorrelated in all genetic models. In the subgroup analysis for cancer types, miR-146a rs2910164 polymorphism was associated with the susceptibility of lung cancer (CC vs. GG: OR 1.275, 95% CI 1.117–1.455 (P?=?0.000); CC?+?CG vs. GG: OR 1.166, 95% CI 1.052–1.293 (P?=?0.003); CC vs. CG?+?GG: OR 1.239, 95% CI 1.116–1.375 (P?=?0.000); C vs. G OR 1.151, 95% CI 1.080–1.227 (P?=?0.000)) and nasopharyngeal carcinoma (CC vs. GG: OR 1.713, 95% CI 1.183–2.479 (P?=?0.004); CC vs. CG?+?GG: OR 1.672, 95% CI 1.330–2.103 (P?=?0.000); C vs. G: OR 1.400, 95% CI 1.181–1.659 (P?=?0.000)), but it was not associated with hepatocellular carcinoma and gastric cancer. However, in the other subgroup analysis by ethnicity and study designs, no significant associations were found. MiR-146a rs2910164 polymorphism might be associated with the susceptibility to lung cancer and nasopharyngeal carcinoma.  相似文献

【摘要】慢性阻塞性肺疾病(COPD)是临床常见慢性呼吸系统疾病,具有起病隐匿,病程长且不可逆转,易反复加重的特点,需长期、规范诊疗,以改善症状,延缓疾病进展,降低远期发作风险。各级医疗机构的医生承担着慢性阻塞性肺疾病诊疗和管理的主要任务,对慢性阻塞性肺疾病的认知水平将极大地影响患者的疾病管理和预后。本文总结了国内临床医生对慢性阻塞性肺疾病各方面内容的认知现况,并针对认知中存在的问题,提出了应进一步加强慢性阻塞性肺疾病相关指南的落实和执行,推动基层慢性阻塞性肺疾病防治体系建设,增强临床医生尤其是基层医生对慢性阻塞性肺疾病的诊疗能力,以建立更全面的慢性阻塞性肺疾病规范诊疗和疾病防治模式,使我国慢性阻塞性肺疾病患者能最大获益。  相似文献

目的从网络视角出发,探讨枸骨叶抗肿瘤的的可能作用机制,为其临床使用提供依据。方法通过中药系统药理学数据库与分析平台(TCMSP)、中医百科全书数据库(ETCM)及查阅相关文献,获取枸骨叶的有效成分及潜在靶点。利用人类基因组注释数据库(GeneCards)结果对比,获得枸骨叶与肿瘤重合的潜在靶点基因,并通过Cytoscape 3.6.0软件构建枸骨叶的“候选成分-作用靶点”网络。利用String数据库和Cytoscape 3.6.0软件构建药物抗肿瘤的蛋白质相互作用(PPI)网络;并通过分子对接验证和筛选潜在的核心化合物,利用DAVID生物信息资源数据库对核心靶点进行基因本体论(GO)功能和京都基因与基因组百科全书(KEGG)通路富集分析。结果共筛选出枸骨叶抗肿瘤的作用的靶点137个,对应13种有效成分。关键靶点涉及TP53、MAPK1、STAT3等。主要通过调控肿瘤坏死因子(TNF)信号通路、缺氧诱导因子-1(HIF-1)信号通路、磷脂酰肌醇3-激酶(PI3K/AKT)信号通路、肿瘤抑制因子P53信号通路、T细胞受体信号通路等发挥抗癌作用。涉及的癌种包括前列腺癌、胰腺癌、非小细胞肺癌等。分子对接结果显示,长梗冬青苷与MAPK1能稳定地结合并通过蛋白氨基酸Gly32、Glu33、Gly34、Tyr36等发生相互作用。结论枸骨叶抗肿瘤作用的主要机制涉及槲皮素、熊果酸、长梗冬青苷、芒柄花黄素为代表的多成分,以TP53、MAPK1及STAT3为代表的多靶点,以及细胞增殖、细胞凋亡、信号传导为代表的多通路。  相似文献

ABSTRACT

Ample research suggests that uncertainty is a major concern for individuals undergoing genetic testing and for genetic mutation carriers, and that their management of uncertainty is largely influenced by healthcare providers, including genetic counselors (GCs). Indeed, uncertainty is inherent in genetic testing results. To help patients grapple with uncertainty, GCs need to effectively manage the uncertainty inherent in genetic testing and communicate it appropriately to patients. The present study aimed to identify GCs’ strategies for managing uncertainty and communicating uncertainty to patients. Eighteen GCs participated in five focus group interviews, and the data were analyzed using a grounded theory approach. Findings indicated that GCs used three main strategies to manage uncertainty: becoming lifelong learners, normalizing uncertainty, and seeking social support. To communicate uncertainty to patients, GCs also used three different strategies: engaging in open and honest communication, adapting to patients’ needs, and focusing on known information. The implications of these findings for uncertainty theories and for conceptual frameworks of GCs’ clinical communication are discussed.  相似文献

Accumulating evidence suggests that atherosclerosis (AS) is the underlying cause of vascular diseases, including heart disease and stroke. Ultrasound-targeted microbubble destruction (UTMD) technology provides a tolerable, efficient and effective system for drug delivery and gene transfection, which has broad application prospects in the treatment of AS. In addition, glycogen synthase kinase (GSK)-3β has been implicated as a potentially valuable therapeutic agent for AS treatment; however, the specific molecular mechanisms remain unknown. Therefore, this study was conducted to explore the effect of downregulation of GSK-3β expression via UTMD on atherosclerotic plaque stability. We established a THP-1 macrophage-derived foam cell model in vitro and an atherosclerotic plaque model in the right common carotid artery of New Zealand rabbits. We determined levels of the relevant vulnerable plaque stability elements. The results indicate that GSK-3β was upregulated in the foam cells and in atherosclerotic rabbits. Downregulation of GSK-3β expression by UTMD suppressed vulnerable plaque factors and inflammation in vitro and in vivo, changed the cytoskeleton of the foam cells in vitro, increased Young's modulus and decreased the peak intensity of atherosclerotic plaque in vivo. Moreover, GSK-3β inhibition by UTMD did not influence the viability of the foam cells. Collectively, our results indicate that GSK-3β could be a potential target for anti-atherogenic interventions and, in particular, can improve the stability of AS plaques in combination with UTMD.  相似文献

目的:了解芜湖市建筑工人艾滋病知识、态度、行为状况,为今后开展建筑工人艾滋病健康教育活动提供科学依据。方法:方便整群抽取芜湖市五个建筑工地445名工人进行调查。结果:调查对象的艾滋病知识知晓率为39.1%,仅有24.7%的人知晓"蚊子叮咬不会传播艾滋病",多因素Logistic回归分析发现:参加工作时间长、文化程度高、通过报刊书籍和免费宣传材料获取知识是艾滋病知识知晓率的促进因素。艾滋病歧视方面仅有36.6%的人愿意和艾滋病人做朋友,艾滋病知识得分越低越容易发生歧视行为(r=0.356,P=0.000)。13.4%的建筑工人初次性行为年龄≤18岁,21.6%的人有多个性伴侣;10.0%的人最近1年有过商业性行为,56.0%的人在发生商业性行为时从不使用安全套。结论:建筑工人艾滋病知识知晓率比较低,对艾滋病病人或感染者存在歧视现象,其自身存在着艾滋病传播的高危行为。  相似文献
