目的 探讨快速康复外科 (FTS)理念在经后颅窝乙状窦后入路三叉神经微血管减压术围术期患者护理中的应用效果。方法 选择2018年1月~2019年1月我院收治并行经后颅窝乙状窦后入路三叉神经微血管减压患者100例,随机分成观察组和对照组,每组50例。对照组应用常规方法进行围术期护理,观察组应用FTS理念进行围术期护理。比较两组焦虑情况、术后并发症发生率、平均住院日及平均住院费用。结果 观察组轻度焦虑多于对照组,中、重度焦虑少于对照组(P<0.05);观察组术后并发症发生率低于对照组[恶心呕吐(4.00% vs 16.00%)、尿路感染(4.00% vs 18.00%)、颅内感染(0 vs 8.00%)、颅内血肿(2.00% vs 14.00%)和术后应激性疼痛(16.00% vs 34.00%)],差异有统计学意义(P<0.05);观察组平均住院日和平均住院费用均低于对照组[(9.13±1.14)d vs(12.44±0.89)d];[(2.15±0.66)万元 vs (3.05±0.61)万元],差异有统计学意义(P<0.05)。结论 应用FTS理念能缓解经后颅窝乙状窦后入路三叉神经微血管减压术患者术前紧张恐惧的心理,减少术后并发症,缩短患者住院日,降低住院费用,护理效果较好。  相似文献

目的:探讨改良大骨瓣联合软通道微创介入逐渐减压术治疗重型颅脑损伤的疗效。方法方便选择自2009年12月—2014年11月该院收治的重型颅脑损伤病例70例,随机分为改良组和对照组2组,每组35例,改良组施行改良大骨瓣联合软通道微创介入逐渐减压术,对照组按标准大骨瓣开颅、硬脑膜一次性全切开,对比分析两组的并发症发生率及预后。结果改良组发生迟发性颅内血肿、大面积脑梗塞及弥漫性脑肿胀等并发症9例(25.7%,9/35),预后良好者23例(65.7%,23/35),对照组发生并发症19例(54.3%,19/35),预后良好者14例(40.0%,14/35),两组并发症发生率和预后差异具有统计学意义(P<0.05)。结论改良大骨瓣联合软通道微创介入逐渐减压术治疗重型颅脑损伤,效果良好,显著降低死亡率。  相似文献

Aim: To describe differences in the deep lateral orbital wall (specifically, trigone) between Chinese, Malay, Indian and Caucasian subjects

Methods: Single-centre retrospective Computed Tomogram (CT)-based study; 20 subjects of each ethnicity were used from existing databases, matched for gender, average age and laterality. Subjects below 16 years of age were excluded. DICOM image viewing software CARESTREAM Vue PACS (Carestream Health Inc., USA) and OsiriX version 7.5 (Pixmeo., Switzerland) were used to measure deep lateral wall length, thickness and volume, as well as orbital depth and statistical analyses performed using Statistical Package for Social Sciences version 21 (IBM, USA).

Results: In each group, there were 12 males (60%) and average age was not significantly different (p = 0.682–0.987). Using Chinese subjects as a reference, in Chinese, Malay, Indian and Caucasian subjects, mean trigone thickness was 13.68, 14.02, 11.60 (p < 0.001) and 13.80 mm, curved total wall length 45.23, 42.29 (p = 0.048), 41.91 (p = 0.020) and 45.00 mm, curved trigone length 23.03, 22.61, 17.19 (p = 0.011) and 18.76 mm (p = 0.030) and trigone volume 3120.97, 3221.01, 1613.66 (p < 0.001), 2498.46 mm3 (= 0.059) respectively. Similarly, perpendicular orbital depth was 27.54, 24.97, 22.12 (p = 0.001) and 25.93 mm and diagonal orbital depth was 34.19, 33.27, 29.48 (p = 0.01) and 34.63 mm respectively.

Conclusions: Indian and, to a lesser extent, Caucasian subjects have smaller trigones compared to their Chinese and Malay counterparts. Indian subjects also have shallower orbits and due care should be taken during decompression surgery.  相似文献

Background: For management of bowel obstruction due to colorectal cancer, endoscopic trans‐anal decompression technique has been first reported by Lelcuk et al. in 1986 using balloon dilatation technique. Since then, various types of trans‐anal decompression tubes have been clinically used for patients suffering from left side obstructing colorectal cancer as an emergent decompressing device. At present, two types of trans‐anal ileus tube (trans‐anal decompression tube) have been available for clinical use, but they have two main problems that are late colon perforations caused by the tip of the tube and tube obstruction by stool. Methods: Analysis on three late colon perforations experienced with the use of conventional devices drew possible improvements to make a trans‐anal ileus tube less harmful. To overcome the pitfalls inherent to conventional tubes, the author has developed an improved trans‐anal ileus tube with a balloon installed at the very end of the tube (‘balloon‐tipped type’) made of silicone, measuring 1200 or 1700 mm in total length and 22 Fr in outer diameter. It has been used for 12 cases with obstructing colorectal cancer etc. and its outcomes were compared with those obtained by the use of conventional trans‐anal ileus tube. Results: No late perforations have been encountered, but tube obstruction did occur in one of 12 cases. Conclusion: The new trans‐anal ileus tube with a balloon installed at the tip of ileus tube is considered to be safer and especially effective in preventing late colon perforation and tube obstruction.  相似文献

经皮激光椎间盘减压术治疗椎间盘源性腰痛   总被引:1,自引:0,他引:1  
目的探讨经皮激光椎间盘减压术(percutaneous laser disc decompression,PLDD)治疗椎间盘源性腰痛的疗效。方法2002年6月~2004年12月我院对36例椎间盘源性腰痛,采用英国DIOMED公司半导体激光仪,激光功率15W,每个激光脉冲持续1s,间隔1s,照射能量800~1200J。VAS评分评价治疗效果。结果手术时间15~60min,平均30min。32例出现“疼痛复制效应”。36例随访6~36个月,平均11个月,32例有效(术后VAS评分改善≥3分18例,≥分14例),4例无效,有效率88.9%(32/36)。结论PLDD治疗椎间盘源性腰痛安全、有效、微创。  相似文献

后路治疗胸腰椎爆裂骨折   总被引:3,自引:0,他引:3  
目的:探讨后路环椎管减压,椎弓根钉系统复位固定并植骨治疗胸腰椎爆裂骨折的疗效。方法:1996-2001年,环椎环椎管减压,结合具有钉杆角的椎弓根钉系统复位固定,并横突及小关节突间植骨或椎间植骨治疗128例病人,从伤椎椎体前后缘高度恢复,Cobb角矫正度及神经功能恢复情况评价疗效。结果:128例病人脊髓神经损害无加重,神经功能有不同程度恢复;伤椎高度恢复理想,Cobb角明显减少,植骨融合成功108例(占84.4%),术后部分病例出现伤椎复位度丢失现象。结论:后路环椎管减压内固定治疗胸腰椎骨折效果好,应重视植骨融合以获得良好稳定性。  相似文献

The aim of this study was to define criteria for the selection of patients for percutaneous or open operations for the cure of drug-resistant trigeminal neuralgia (TN). Trigeminal percutaneous radiofrequency thermorhizotomy (TPRT) has an established place because of its safety in elderly patients, while microvascular decompression (MVD) has appeal in younger patients beause of its non-destructive nature and because it attacks what is believed to be the primary etiology of tic douloureux. Nevertheless, MVD is a successful operation only when true neurovascular conflict (NVC) is ascertained, rather than a simple arterial loop and neurovascular contract. Probably, many immediate failures and early relapses are the consequence of the inadequate patient selection for MVD on the presumption that this operation is in any case the ideal cure. The inadequate selection can be explained by the difficult preoperative diagnosis of NVC in the past. Indeed, angiography and computed tomography showed the neurovascular contact but not the size of compression. Fortunately, today magnetic resonance imaging is a reliable instrument to ascertain NVC. So, the diatribe between the supporters of percutaneous techniques and MVD can be concluded with the following: (1) percutaenous techniques are indicated for patients without demonstrated NVC (including patients with TN in multiple sclerosis) and in those with NVC if MVD is contraindicated by ill-health or refused by the informed patient; and (2) MVD is incated for patients with ascertained NVC who are in good health and who, informed of the surgical risk, favor this operation desiring no sensory deficit. 本文介绍香港九龙海底铁路建设中高气压作业与减压病发病情况。该工程有11条坑道,共计259 448个高压工作日,减压288 140人次,发生减压病1 534例。根据不同的工作压力,以暴露次数、高压工作日数及暴露4小时以上次数3种方法分别计算减压病发病率。结果表明:各种方法计算出的发病率都有随工作压力增高而上升的趋势,说明现用的减压表对不同的工作压力达不到同样的预防效果。本文还对3大主要工种和14个小工种的减压病作了分析。  相似文献

目的:探讨经皮激光椎间盘减压术(PLDD)治疗拒绝开放手术的脊髓型颈椎病患者的疗效。方法:2002年~2006年应用PLDD治疗16例拒绝开放手术的脊髓型颈椎病患者,手术前后采用日本骨科学会(JOA)17分法进行评价.计算改善率及优良率,随访观察治疗效果。结果:手术时间平均为15min,术中无明显出血,无血管、神经损伤及感染等并发症发生。术后1、3、6、12个月时JOA评分改善率分别为28.54%、34.82%、41.09%和47.57%.优良率分别为31.3%、37.5%、37.5%和56.0%。随访24个月者11例,改善率为46.56%,优良率为54.4%。结论:PLDD治疗脊髓型颈椎病患者有一定疗效,对拒绝开放手术的脊髓型颈椎病患者是一种可供选择的微创治疗手段。  相似文献
