In this work, the dosimetric characteristics of a new commercial carbon fiber treatment table are investigated. The photon beam attenuation properties of the Siemens image-guided radiation therapy (IGRT) tabletop were studied in detail. Two sets of dosimetric measurements were performed. In the first experiment a polystyrene slab phantom was used: the central axis attenuation and the skin-sparing detriment were investigated. In the second experiment, the off-axis treatment table transmission was investigated using a polystyrene cylindrical phantom. Measurements were taken at the isocenter for a 360° rotation of the radiation beam. Our results show that the photon beam attenuation of the Siemens IGRT carbon fiber tabletop varies from a minimum of 2.1% (central axis) to a maximum of 4.6% (120° and 240° beam incidence). The beam entrance dose increases from 82% to 97% of the dose at the depth of maximum for a clinical 6-MV radiation field. The depth of maximum also decreases by 0.4 cm. Despite the wedge cross section of the table the beam attenuation properties of the IGRT tabletop remain constant along the longitudinal direction. 目的建立个体患者适形调强放疗(IMRT)计划的模体内剂量实测验证技术。方法选择1例鼻咽癌患者,设计IMRT计划。将患者计划转移到模体上设计杂交计划。执行杂交计划时,用针点电离室测量感兴趣点的剂量,并与该点的计算剂量比较。用胶片剂量测量系统测量杂交计划中感兴趣平面的剂量,胶片与计划剂量矩阵登记后,依次进行计划,胶片分析、计划,胶片剖面分析和计划/胶片等剂量线分析。采用复合判断标准评价验证结果。结果针点电离室测量得到杂交计划单次照射的总剂量为121.5cGy,比计算值低约4%。计划,胶片分析得到高剂量、高梯度区域的距离差别均在4mm以内;计划/胶片剖面分析显示,计划与胶片在通过靶区的剖面具有较好的一致性;计划,胶片等剂量线分析显示,计划与胶片对应值的等剂量线重合良好。按照复合判断标准,该计划验证通过。结论初步建立了个体患者IMRT计划的模体内剂量实测验证技术,建立并优化了剂量登记技术、剂量归一方法和评价方法。  相似文献

The aim of this study was to survey the present status and patterns of reirradiation (Re-RT) practice using external beam radiotherapy in Japan. We distributed an e-mail questionnaire to the Japanese Society for Radiation Oncology partner institutions, which consisted of part 1 (number of Re-RT cases in 2008–2012 and 2013–2018) and part 2 (indications and treatment planning for Re-RT and eight case scenarios). Of the 85 institutions that replied to part 1, 75 (88%) performed Re-RTs. However, 59 of these 75 institutions (79%) reported difficulty in obtaining Re-RT case information from their databases. The responses from 37 institutions included the number of Re-RT cases, which totaled 508 in the period from 2009 to 2013 (institution median 3; 0–235), and an increase to 762 cases in the period from 2014 to 2018 (12.5; 0–295). A total of 47 physicians responded to part 2 of the survey. Important indications for Re-RT that were considered were age, performance status, life expectancy, absence of distant metastases and time interval since previous radiotherapy. In addition to clinical decision-making factors, previous total radiation dose, volume of irradiated tissue and the biologically equivalent dose were considered during Re-RT planning. From the eight site-specific scenarios presented to the respondents, >60% of radiation oncologists agreed to perform Re-RT. Re-RT cases have increased in number, and interest in Re-RT among radiation oncologists has increased recently due to advances in technology. However, several problems exist that emphasize the need for consensus building and the establishment of guidelines for practice and prospective evaluation.  相似文献

There are many available options for prostate cancer treatment, including active surveillance, surgery, brachytherapy and external beam radiotherapy. Based on a radiobiological rationale, which considers the prostate tumor as a low α/β tumor, the use of higher and fewer fractions to prostate cancer external beam radiotherapy treatment has been proposed. Instead of the traditional fractions of 1.8–2.0 Gy per day, fractions higher than 2 Gy per day were the subject of a number of studies. In addition, new technologies such as intensity-modulated radiation therapy, image-guided radiation therapy, volumetric-modulated arch therapy and others have emerged as background for changing paradigms. Meanwhile, moderate and ultra-hypofractionation have been the subject of studies in recent years. Some moderate hypofractionation data from randomized controlled trials are ready to use, though other non-inferiority data are still lacking. The data on ultra-hypofractionation are still very new and require further evaluation to determine its long-term safety and efficacy.  相似文献

To describe a dosimetric method using an anterior dose avoidance structure (ADAS) during the treatment planning process for intensity-modulated radiation therapy (IMRT) for patients with anal canal and rectal carcinomas. A total of 20 patients were planned on the Elekta/CMS XiO treatment planning system, version 4.5.1 (Maryland Heights MO) with a superposition algorithm. For each patient, 2 plans were created: one employing an ADAS (ADAS plan) and the other replanned without an ADAS (non-ADAS plan). The ADAS was defined to occupy the volume between the inguinal nodes and primary target providing a single organ at risk that is completely outside of the target volume. Each plan used the same beam parameters and was analyzed by comparing target coverage, overall plan dose conformity using a conformity number (CN) equation, bowel dose-volume histograms, and the number of segments, daily treatment duration, and global maximum dose. The ADAS and non-ADAS plans were equivalent in target coverage, mean global maximum dose, and sparing of small bowel in low-dose regions (5, 10, 15, and 20 Gy). The mean difference between the CN value for the non-ADAS plans and ADAS plans was 0.04 ± 0.03 (p < 0.001). The mean difference in the number of segments was 15.7 ± 12.7 (p < 0.001) in favor of ADAS plans. The ADAS plan delivery time was shorter by 2.0 ± 1.5 minutes (p < 0.001) than the non-ADAS one. The ADAS has proven to be a powerful tool when planning rectal and anal canal IMRT cases with critical structures partially contained inside the target volume.  相似文献

目的:探讨64层螺旋CT在肿瘤调强放射治疗定位中的临床应用及CT扫描床的误差分析。方法:收集我院2010年6月-2011年6月在TOSHIBA Aquiiion 64层螺旋CT机上进行肿瘤调强放疗定位CT扫描患者25例。其中,男13例,女10例,年龄35—73岁,平均年龄55岁。头颈部肿瘤10例,胸部肿瘤5例,腹部肿瘤6例,盆腔肿瘤4例。CT定位图像经DICOM3.0接口传输到XVI软件中,在Workstation上进行治疗床与定位床的误差分析。结果:组内数据符合统计学意义(P〈0.05),误差值X轴为(0.968±0.910)°,Y轴为(0.508±2.09)°,Z轴为(0.3±2.62)°。除X轴外,旋转误差均不符合国标GB/T17589—1998。结论:64层螺旋CT碳纤维平板床在肿瘤调强放疗定位中误差已超过阈值,不适合做精确调强放射治疗定位。  相似文献

Craniospinal irradiation (CSI) requires abutting fields at the cervical spine. Junction shifts are conventionally used to prevent setup error–induced overdosage/underdosage from occurring at the same location. This study compared the dosimetric differences at the cranial-spinal junction between a single-gradient junction technique and conventional multiple-junction shifts and evaluated the effect of setup errors on the dose distributions between both techniques for a treatment course and single fraction. Conventionally, 2 lateral brain fields and a posterior spine field(s) are used for CSI with weekly 1-cm junction shifts. We retrospectively replanned 4 CSI patients using a single-gradient junction between the lateral brain fields and the posterior spine field. The fields were extended to allow a minimum 3-cm field overlap. The dose gradient at the junction was achieved using dose painting and intensity-modulated radiation therapy planning. The effect of positioning setup errors on the dose distributions for both techniques was simulated by applying shifts of ± 3 and 5 mm. The resulting cervical spine doses across the field junction for both techniques were calculated and compared. Dose profiles were obtained for both a single fraction and entire treatment course to include the effects of the conventional weekly junction shifts. Compared with the conventional technique, the gradient-dose technique resulted in higher dose uniformity and conformity to the target volumes, lower organ at risk (OAR) mean and maximum doses, and diminished hot spots from systematic positioning errors over the course of treatment. Single-fraction hot and cold spots were improved for the gradient-dose technique. The single-gradient junction technique provides improved conformity, dose uniformity, diminished hot spots, lower OAR mean and maximum dose, and one plan for the entire treatment course, which reduces the potential human error associated with conventional 4-shifted plans.  相似文献

Historically treated with surgery, current practice recommends anal carcinoma to be treated with a combination of chemotherapy and radiation. This review will examine the anatomy, modes of disease spread and recurrence, and evaluate the existing evidence for treatment options for these tumors. An in-depth examination of specific radiation therapy (RT) techniques—such as conventional 3D-conformal RT and intensity-modulated RT—will be discussed along with modern dose constraints. RT field arrangement, patient setup, and recommended gross and clinical target volume (CTV) contours will be considered. Areas in need of further investigation, such as the role in treatment for positron emission tomography (PET) will be explored.  相似文献

Background and purposeCurative radiotherapy/chemo-radiotherapy for head and neck cancer (HNC) may result in severe acute and late side effects, including tube feeding dependence. The purpose of this prospective cohort study was to develop a multivariable normal tissue complication probability (NTCP) model for tube feeding dependence 6 months (TUBEM6) after definitive radiotherapy, radiotherapy plus cetuximab or concurrent chemoradiation based on pre-treatment and treatment characteristics.Materials and methodsThe study included 355 patients with HNC. TUBEM6 was scored prospectively in a standard follow-up program. To design the prediction model, the penalized learning method LASSO was used, with TUBEM6 as the endpoint.ResultsThe prevalence of TUBEM6 was 10.7%. The multivariable model with the best performance consisted of the variables: advanced T-stage, moderate to severe weight loss at baseline, accelerated radiotherapy, chemoradiation, radiotherapy plus cetuximab, the mean dose to the superior and inferior pharyngeal constrictor muscle, to the contralateral parotid gland and to the cricopharyngeal muscle.ConclusionsWe developed a multivariable NTCP model for TUBEM6 to identify patients at risk for tube feeding dependence. The dosimetric variables can be used to optimize radiotherapy treatment planning aiming at prevention of tube feeding dependence and to estimate the benefit of new radiation technologies.  相似文献
