丙戊酸钠治疗对血清瘦素、体重指数的影响   总被引:1,自引:0,他引:1  
目的服用丙戊酸钠(VPA)抗癫痫治疗的患者,15%~70%的成人可发生体重增加[1-2],而瘦素在体重和能量平衡的调节中起重要影响,近年来许多研究表明VPA引起的体重改变可能与瘦素有关。探讨丙戊酸钠抗癫痫治疗对血清瘦素的影响,该影响与患者体重改变的相关性,以及瘦素的可能机制。方法以丙戊酸钠治疗超过3个月的33例癫痫患者为治疗组,20例未经丙戊酸钠治疗癫痫患者作为对照组来比较,检测患者体重、身高、血清瘦素水平,比较两组的体重指数(BMI)、血清瘦素水平。结果丙戊酸钠治疗组的血清瘦素(8.8503±4.7924)ug/ml与对照组(2.8490±1.3962)ug/ml比较有增高,治疗组BMI(23.7202±2.5182)高于对照组(20.8468±1.6815)。结论丙戊酸钠治疗可能引起血清瘦素、体重指数增高,很可能通过诱导导致瘦素抵抗的机制之一,从而导致患者体重增加。  相似文献

SUMMARY: In patients with chronic renal failure (CRF), hyperleptinaemia has been widely reported, but the exact mechanisms leading to elevated leptin levels are unclear. Impaired renal clearance of leptin and the influence of other hormones may be important. In this study, we measured serum leptin levels in 150 patients on haemodialysis, peritoneal dialysis or in the predialysis phase of CRF. Furthermore, we measured plasma levels of insulin, growth hormone (GH) and insulin-like growth factor 1 (IGF-1), as well as plasma levels of triacylglycerols and total low density lipoprotein (LDL)- and high density lipoprotein (HDL)-cholesterol. We observed significantly elevated levels of leptin, particularly in female patients, and leptin was shown to correlate significantly with insulin, total and LDL-cholesterol and log triacylglycerols. Leptin was inversely correlated with GH concentrations, but was not correlated with IGF-1 levels. Despite the multiple correlations established between leptin levels and other variables, only hyperinsulinaemia in CRF seems to be important as a determinator of leptin levels.  相似文献

Breast cancer is the female malignant neoplasia with the highest incidence in the industrialized world. Despite many undeniable therapeutic successes obtained, breast cancer still remains, however, a major health issue. In the last few years, thanks to aromatase inhibitors, the hormone therapy for oestrogen-dependent breast cancer has evolved in terms of efficacy and tolerability; at the same time, it has enabled us to better define the role of oestrogens in the etiopathogenesis of this tumour. Weight increase and obesity have been identified as the most important risk and prognostic factors for breast cancer in postmenopausal women. Several hypotheses have been proposed to explain the association of obesity with postmenopausal breast cancer. A more recent hypothesis suggests that adipocytes and their autocrine (paracrine and endocrine actions) are at the centre of such an etiopathogenetic mechanism. A better understanding of the main mechanisms that link together menopause, body-weight increase and hormone-dependent breast cancer is paramount to enable the identification of key molecules involved in the development of breast carcinoma and suggest new therapeutic options. The present review will discuss important findings on the therapeutic aspects of adipose tissue and adipokines as a target for treatment of hormone-dependent breast cancer.  相似文献

Thylakoids are membranes isolated from plant chloroplasts which have previously been shown to inhibit pancreatic lipase/colipase catalysed hydrolysis of fat in vitro and induce short‐term satiety in vivo. The purpose of the present study was to examine if dietary supplementation of thylakoids could affect food intake and body weight during long‐term feeding in mice. Female apolipoprotein E‐deficient mice were fed a high‐fat diet containing 41% of fat by energy with and without thylakoids for 100 days. Mice fed the thylakoid‐enriched diet had suppressed food intake, body weight gain and body fat compared with the high‐fat fed control mice. Reduced serum glucose, serum triglyceride and serum free fatty acid levels were found in the thylakoid‐treated animals. The satiety hormone cholecystokinin was elevated, suggesting this hormone mediates satiety. Leptin levels were reduced, reflecting a decreased fat mass. There was no sign of desensitization in the animals treated with thylakoids. The results suggest that thylakoids are useful to suppress appetite and body weight gain when supplemented to a high‐fat food during long‐term feeding. 目的 :观察瘦素受体 (LR)在条件性厌味 (CTA)大鼠脑的表达 .方法 :采用高度特异的抗瘦素受体血清 ,用免疫组化ABC法观察LR在CTA和对照大鼠脑的分布与表达 .结果 :与对照组大鼠相比 ,CTA大鼠皮层Ⅲ层、视交叉上核、弓状核、杏仁基底外侧核、海马CA3、下丘脑外侧区等核团瘦素受体表达增加 (14 6vs 12 0 ,138vs 92 ,118vs 78,10 7vs70 ,85vs 38cells·mm-2 ,P <0 .0 5 ) ,而下丘脑前区、脑桥臂旁核、杏仁中央核瘦素受体表达减弱 (13vs2 6 ,70vs5 5 ,12 2vs 16 2cells·mm-2 ,P <0 .0 5 ) ,孤束核瘦素受体的表达与对照大鼠无显著性差异 (5 0vs 5 7cells·mm-2 ,P >0 .0 5 ) .结论 :瘦素可能通过杏仁核、臂旁核、海马、皮层的瘦素受体参与CTA的形成与维持  相似文献

AIM: To determine the influence of Type 1 diabetes mellitus on circulating adipocytokines in children. METHODS: The circulating concentrations of leptin, adiponectin, resistin and tumour necrosis factor (TNF)-alpha were measured in 91 children, aged 11.1 +/- 2.7 years, with Type 1 diabetes mellitus (T1DM). Ninety-one healthy children were selected as control subjects. RESULTS: Body mass index-adjusted leptin concentrations were higher in the pubertal diabetic children compared with the control children. There was a significant positive correlation between leptin and daily insulin dose in the diabetic group. Circulating adiponectin concentrations were higher in the prepubertal diabetic children and were positively associated with HbA(1c). Resistin concentrations were lower in the prepubertal non-diabetic subjects compared with the pubertal non-diabetic children, whose values were higher than those of the diabetic children. TNF-alpha concentrations were similar in non-diabetic and diabetic children. CONCLUSIONS: Circulating concentrations of adipocytokines are abnormal in Type 1 diabetic children, although the direction of change differs by cytokine. Pubertal development, in addition to insulin treatment and glycaemic control, also influences the concentrations.  相似文献

目的 :探索健康妇女正常月经周期中血清瘦素与雌激素的关系 .方法 :①对 18例月经规则妇女 (2 8± 1)d ,用放射免疫法分别在其月经第 1,7,14和 2 1d测定血清瘦素及性激素 (FSH ,E2 ,PRO ,T) .②对 19例不孕妇女 ,在受精前用FSH 2 2 5IU·d-1治疗后第 3d、第 5d用放免法测定其血清瘦素及雌激素浓度 .结果 :①血清瘦素在月经周期中的生理性变化 ,月经第 1d ,血清瘦素为 (5 .4 9± 1.0 8) μg·L-1,随着卵泡的发育成熟 ,瘦素水平逐渐升高 ,14d达高峰 (9.2 8± 1.87) μg·L-1,其差异有显著性 (P <0 .0 1) .②用FSH治疗 3,5d ,血清雌激素浓度和瘦素水平 [(89.0 8± 18.0 7)ng·L-1,(10 .99± 1.94 ) μg·L-13rdday],[(12 6 .2 6± 19.79)ng·L-1,(14 .78± 2 .0 0 ) μg·L-15thday]均较治疗前 [(5 8.5 9± 6 .4 0 )ng·L-1,(5 .4 6± 1.16 ) μg·L-1]明显升高 (P <0 .0 1) .结论 :月经周期中血清瘦素和雌激素的变化一致 ,雌激素参与瘦素分泌的调节或瘦素的分泌调节与雌激素有关  相似文献

BACKGROUND: Adipokines are involved in the regulation of many inflammatory processes and are present at very high concentrations in cord blood of term infants. OBJECTIVE: We analysed data of a large prospective birth cohort study to examine whether adiponectin and leptin concentration in cord blood are determinants of wheezing disorders in children within the first 2 years of life. METHODS: Seven hundred and forty mothers and their newborns were included in this analysis. Adiponectin and leptin concentrations were measured in cord blood. The cumulative incidence of physician-reported asthma or obstructive bronchitis was recorded during a 2-year follow-up. RESULTS: During the first 2 years of life, asthma or obstructive bronchitis was reported by the caring paediatricians for 157 (19.6%) of the children. We found a strong interaction of cord blood adiponectin and history of atopic disease in the mother with respect to the risk of physician-reported asthma or obstructive bronchitis (P=0.006). Compared with children with cord blood levels in the middle quintile (reference category), the odds ratios for physician-reported asthma or obstructive bronchitis in the bottom quintile and top quintile were 0.14 [95% confidence interval (CI) 0.02-0.90] and 2.12 (95% CI 0.67-6.66), respectively (P for trend=0.0003), among children of mothers with a history of atopy. This association was independent of other established risk factors. Leptin levels in cord blood were not associated with risk of asthma or obstructive bronchitis. CONCLUSIONS: In children of mothers with a history of atopy, concentrations of adiponectin in cord blood could play an important role in determining risk of wheezing disorders in early childhood.  相似文献

目的探讨重组人生长激素对COPD患者瘦素水平和营养状况的影响。方法40例COPD急性加重期患者随机分成生长激素组和对照组。均给予抗感染、氧疗、营养支持等。生长激素组在上述治疗基础上加用RhGH,每晚1次皮下注射,连续应用7~10d。观察治疗前后,患者体重指数(BMI)、肱三头肌皮褶(TSF)、上肢臂肌围(MAMC)总蛋白,白蛋白值,瘦素水平,分析相关关系。结果治疗后rhGH组BMI、TSF、AMC.总蛋白,白蛋白值,瘦素水平与治疗前比较差异均有统计学意义。瘦素与BMI、TSF、AMC总蛋白,白蛋白值显著正相关。结论生长激素联合营养支持治疗能提高老年慢性阻塞性肺疾病患者营养状态和瘦素水平。  相似文献

目的 探讨血清瘦素在男生青春期发育中的作用。方法 健康男生170名,调查首次遗精时间,测定血清瘦素、睾酮、促卵泡成熟激素、黄体生成激素水平,采用多元Logistic回归模型分析男生青春期发育的影响因素;同时,连续两年观察,分析血清瘦素在青春期的动态变化。结果 末次研究已遗精组血清瘦素、睾酮、促卵泡成熟激素、黄体生成激素水平均明显高于未遗精组。多因素分析显示血清瘦素、睾酮、促卵泡成熟激素、体重为男生青春期发育的独立影响因素。初次资料分析,遗精前组血清瘦素、睾酮明显高于未遗精组。结论 血清瘦素、睾酮、促卵泡成熟激素、体重为男生青春期发育的独立影响因素,男生遗精前期血清瘦素、睾酮已有明显改变。  相似文献
