ABSTRACT

Objective: To describe satisfaction with current health status in patients with a recent history of an acute coronary syndrome (ACS) event and to determine the association between satisfaction and patient-specific variables.

Research design and methods: Patients from an ACS registry who were discharged from a university affiliated hospital over a 3-year period were mailed the study questionnaire.

Main outcomes measures: Data included demographics, cardiac-specific measures, and general health status (SF-8 PCS, MCS and the EQ-5D VAS) and health status preference weight (EQ-5D Health Index). Satisfaction with current health status was assessed by a single question derived for this study with a 5-point Likert scale from ‘not satisfied at all’ to ‘highly satisfied.’ ANOVA determined the association between levels of satisfaction and health status scores. A multivariate linear regression model determined the association of patient, disease, and treatment variables with satisfaction. Independent variables were determined to be significant if the p-value in the model was <0.05.

Results: Respondents (490, 40.3% response rate) averaged 65.2?± 11.3 years of age; 71% male; 92% Caucasian; 64% with MI history; and 17% had their most recent cardiac event within 6 months. 63% of respondents were either mostly satisfied or highly satisfied with there current health status. Based on level of satisfaction, the mean PCS ranged from 36.9?± 8.9 to 63.0?± 6.2; mean MCS from 38.3?± 13.1 to 55.7?± 5.7; mean EQ-5D VAS from 37.0?± 21.7 to 90.8?± 20.7, and mean EQ-5D Health Index from 0.38?± 0.3 to 0.93?± 0.14, all ANOVA models p?<?0.001. Statistically significant independent variables in the multivariate linear regression model included the number of symptoms, DASI cardiac function score, perceived severity of disease, and age. Satisfaction with current health state was associated with older age, fewer symptoms, better DASI cardiac function scores, and lower perceived severity of illness.

Conclusion: Health-related quality of life, or health status, and satisfaction with health status have a direct, positive association. Greater burden of illness and younger age were significantly associated with dissatisfaction with health status. Extrapolation of results is limited to determining association but not causation due to the cross sectional study design.  相似文献

BackgroundThe proposed criteria of the Diagnostic and Statistical Manual for Mental Disorders, Fifth Edition for hypersexual disorder (HD) included symptoms reported by patients seeking help for excessive and out-of-control non-paraphilic sexual behavior, including sexual behaviors in response to dysphoric mood states, impulsivity, and risk taking. Although no prior studies of cognitive-behavioral therapy (CBT) for the treatment of HD have been performed, CBT has been found effective for dysphoric mood states and impulsivity.AimTo investigate the feasibility of a CBT manual developed for HD explored through symptom decrease, treatment attendance, and clients’ treatment satisfaction.MethodsTen men with a diagnosis of HD took part in the CBT group program. Measurements were taken before, during, and at the end of treatment and 3 and 6 months after treatment.OutcomesThe primary outcome was the Hypersexual Disorder: Current Assessment Scale (HD:CAS) score that measured the severity of problematic hypersexual symptoms and secondary outcomes were the Hypersexual Disorder Screening Inventory (HDSI) score, the proportion of attended sessions, and the Client Satisfaction Questionnaire (CSQ-8) score.ResultsMain results were significant decreases of HD symptoms from before to after treatment on HD:CAS and HDSI scores and a decrease in the number of problematic sexual behaviors during the course of therapy. A high attendance rate of 93% and a high treatment satisfaction score on CSQ-8 also were found.Clinical ImplicationsThe CBT program seemed to ameliorate the symptoms of HD and therefore might be a feasible treatment option.Strengths and LimitationsThis study provides data from a CBT program for the treatment of the specific proposed criteria of HD. Because of the small sample and lack of a control group, the results can be considered only preliminary.ConclusionAlthough participants reported decreased HD symptoms after attending the CBT program, future studies should evaluate the treatment program with a larger sample and a randomized controlled procedure to ensure treatment effectiveness.Hallberg J, Kaldo V, Arver S, et al. Objectives - To describe where patients in Danish general practice get information about health and disease, particularly how patients prepare for a visit to their GP, with special reference to use of the Internet. Design - Structured interviews. Setting - Four Danish general practices. Patients - 93 consecutive patients after visiting their GP. Main outcome measures - The patient's report about use of the Internet and different mass media in preparation for the consultation. Results - Only two patients never looked for health information. Of all patients, 20% had used the Internet to get health information, 8% because of the current visit, i.e. a third of all with Internet access had used it because of the current visit. Women used the sources of information more than men did. Personal contact with family, friends or neighbours was the most commonly used source. Conclusion - The Internet is used in direct preparation for a visit to the general practitioner. The vast majority of patients use the mass media for information. In general practice, the main source of information on a health-related subject is personal contact with family and friends.  相似文献

目的探讨助产土产前门诊对初产妇焦虑抑郁情绪和初产妇对门诊助产士提供护理服务态度和方式的满意度的影响。方法应用随机数字表法将2011年12月~2012年12月在本院产科建卡并定期产检的106例正常初产妇分为对照组和观察组,对照组给予常规产前门诊,观察组在对照组基础上给予助产士产前门诊,观察并比较两组初产妇焦虑、抑郁情绪和初产妇对门诊助产士提供护理的服务态度和方式的满意度。结果观察组焦虑、抑郁标准分均明显低于对照组,且观察组初产妇对门诊助产士提供护理的服务态度和方式的满意度明显高于对照组,差异均有统计学意义(P〈0.05)。结论助产土产前门诊能够有效减少初产妇的焦虑及抑郁程度,提高初产妇对门诊助产士提供护理服务态度和方式的满意度。  相似文献

目的:探讨开展疼痛评估对住院患者的护理服务满意度的影响。方法对2935例住院患者进行护理服务满意度调查,按开展疼痛评估前后进行分组,其中2012年7~12月开展疼痛评估前所获得的1463例病人为对照组,2013年1~6月开展疼痛评估后所获得的1472例病人为实验组,对两组病人的护理服务满意度调查结果进行比较。结果住院患者对护理服务的满意度,开展疼痛评估后明显高于开展疼痛评估前(P<0.05)。结论开展疼痛评估可提高住院患者对护理服务的满意度。  相似文献

目的:回顾分析近视患者经准分子手术激光治疗后的临床客观检查结果(裸眼视力、散光及减少度数)改变;分析其对术后满意度的影响,以期提高患者术后满意度。方法:对256例(511眼)准分子激光手术患者术后随访1个月;分析其术后疗效及不同满意度之间关系。结果:术后满意度与近视度数改善程度有统计学意义;与另两项客观检查结果的改善无统计学意义,其他因素也会对术后满意度有所影响。结论:LASIK术后满意度与临床疗效密切相关,应采取综合措施改善术后满意度。  相似文献
