Abstract

Purpose: We examined underlying psychosocial processes of a behavioral treatment for urinary incontinence (UI) of prostate cancer survivors.

Design: Secondary analysis of data collected from a clinical trial.

Sample: Two hundred forty-four prostate cancer survivors who participated in a clinical trial of behavioral intervention to UI as intervention or control subjects.

Methods: The participants had a 3-month behavioral intervention or usual care and were followed up for an additional 3?months. They were assessed at baseline, 3, and 6?months. Latent growth curve models were performed to examine trajectories of each study variable and relationships among the variables.

Findings: Increasing self-efficacy and social support were significantly and independently associated with more reduction of urinary leakage frequency over time.

Implications for psychosocial oncology: Providing problem-solving skills and social support, including peer support, are essential for empowering patients to reduce UI.  相似文献

Adaptation of osteology and myology lead to the formation of hydrofoil foreflippers in Cheloniidae (all recent sea turtles except Dermochelys coriacea) which are used mainly for underwater flight. Recent research shows the biomechanical advantages of a complex system of agonistic and antagonistic tension chords that reduce bending stress in bones. Finite element structure analysis (FESA) of a cheloniid humerus is used to provide a better understanding of morphology and microanatomy and to link these with the main flipper function, underwater flight. Dissection of a Caretta caretta gave insights into lines of action, that is, the course that a muscle takes between its origin and insertion, of foreflipper musculature. Lines of action were determined by spanning physical threads on a skeleton of Chelonia mydas. The right humerus of this skeleton was micro-CT scanned. Based on the scans, a finite element (FE) model was built and muscle force vectors were entered. Muscle forces were iteratively approximated until a uniform compressive stress distribution was attained. Two load cases, downstroke and upstroke, were computed. We found that muscle wrappings (m. coracobrachialis magnus and brevis, several extensors, humeral head of m. triceps) are crucial in addition to axial loading to obtain homogenous compressive loading in all bone cross-sections. Detailed knowledge on muscle disposition leads to compressive stress distribution in the FE model which corresponds with the bone microstructure. The FE analysis of the cheloniid humerus shows that bone may be loaded mainly by compression if the bending moments are minimized.  相似文献

目的探讨骶四针疗法配合盆底肌锻炼对产后压力性尿失禁患者盆底功能及性生活质量的影响。方法选取2017年5月至2020年5月新疆医科大学第一附属医院收治的98例产后压力性尿失禁患者作为研究对象。采用随机数字表法分为研究组和对照组,各49例。两组均接受常规护理,对照组在常规护理的基础上给予盆底肌锻炼,研究组在对照组的基础上给予骶四针疗法,比较两组的临床疗效?1 h漏尿量?盆底肌力恢复情况及性生活情况。结果治疗后,研究组总有效率为91.84%,明显高于对照组的69.39%,差异具有统计学意义(P<0.05);与治疗前比较,治疗后两组1 h漏尿量减少,且研究组1 h漏尿量少于对照组,差异具有统计学意义(P<0.05);治疗后,两组盆底肌力分级均升高,且研究组盆底肌力分级高于对照组,差异具有统计学意义(P<0.05);治疗后,研究组性生活质量高水平比率为38.78%,高于对照组的20.41%,差异具有统计学意义(P<0.05)。结论骶四针疗法配合盆底肌锻炼用于产后压力性尿失禁患者,能改善患者的漏尿情况,提高其盆底肌力及性生活质量,临床疗效较好。  相似文献

Clinical and research interest in sarcopenia has burgeoned internationally, Asia included. The Asian Working Group for Sarcopenia (AWGS) 2014 consensus defined sarcopenia as “age-related loss of muscle mass, plus low muscle strength, and/or low physical performance” and specified cutoffs for each diagnostic component; research in Asia consequently flourished, prompting this update. AWGS 2019 retains the previous definition of sarcopenia but revises the diagnostic algorithm, protocols, and some criteria: low muscle strength is defined as handgrip strength <28 kg for men and <18 kg for women; criteria for low physical performance are 6-m walk <1.0 m/s, Short Physical Performance Battery score ≤9, or 5-time chair stand test ≥12 seconds. AWGS 2019 retains the original cutoffs for height-adjusted muscle mass: dual-energy X-ray absorptiometry, <7.0 kg/m2 in men and <5.4 kg/m2 in women; and bioimpedance, <7.0 kg/m2 in men and <5.7 kg/m2 in women. In addition, the AWGS 2019 update proposes separate algorithms for community vs hospital settings, which both begin by screening either calf circumference (<34 cm in men, <33 cm in women), SARC-F (≥4), or SARC-CalF (≥11), to facilitate earlier identification of people at risk for sarcopenia. Although skeletal muscle strength and mass are both still considered fundamental to a definitive clinical diagnosis, AWGS 2019 also introduces “possible sarcopenia,” defined by either low muscle strength or low physical performance only, specifically for use in primary health care or community-based health promotion, to enable earlier lifestyle interventions. Although defining sarcopenia by body mass index–adjusted muscle mass instead of height-adjusted muscle mass may predict adverse outcomes better, more evidence is needed before changing current recommendations. Lifestyle interventions, especially exercise and nutritional supplementation, prevail as mainstays of treatment. Further research is needed to investigate potential long-term benefits of lifestyle interventions, nutritional supplements, or pharmacotherapy for sarcopenia in Asians.  相似文献

There is thought to be a relationship between the vastus medialis oblique muscle (VMO) and patellofemoral pain syndrome (PFPS), a condition that has a high prevalence in young athletic individuals. Following a suggestion that there may be a link between the architecture of the VMO and an individual's activity level, the aim of this study was to determine any differences in two measurable parameters of the VMO between two populations with widely differing activity levels. The parameters measured were VMO fiber angle and insertion ratio, which is the proportion of the medial patellar border with VMO fibers inserting onto it (%). Eighty‐two knees from 26 athletic volunteers and 15 sedentary volunteers (aged 20–28 years) were assessed using ultrasound. Activity level was defined using the Tegner scoring system. The mean VMO angle (°) for the athletic group was significantly higher than for the sedentary group at 67.8° and 53.6°, respectively. There was no significant difference in insertion ratio between the athletic group, 43.0%, and the sedentary group, 39.5%. This study found that greater VMO fiber angles were seen in individuals with higher activity levels, exerting a stronger medial stabilizing force on the patella. This has important implications for the treatment of PFPS, particularly in athletic patients, which frequently focus on VMO strengthening exercises. Clin. Objectives

Short successive periods of skeletal muscle disuse have been suggested to substantially contribute to the observed loss of skeletal muscle mass over the life span. Hospitalization of older individuals due to acute illness, injury, or major surgery generally results in a mean hospital stay of 5 to 7 days, during which the level of physical activity is strongly reduced. We hypothesized that hospitalization following elective total hip arthroplasty is accompanied by substantial leg muscle atrophy in older men and women.

Design and participants

Twenty-six older patients (75 ± 1 years) undergoing elective total hip arthroplasty participated in this observational study.

Measurements

On hospital admission and on the day of discharge, computed tomographic (CT) scans were performed to assess muscle cross-sectional area (CSA) of both legs. During surgery and on the day of hospital discharge, a skeletal muscle biopsy was taken from the m. vastus lateralis of the operated leg to assess muscle fiber type–specific CSA.

Results

An average of 5.6 ± 0.3 days of hospitalization resulted in a significant decline in quadriceps (?3.4% ± 1.0%) and thigh muscle CSA (?4.2% ± 1.1%) in the nonoperated leg (P < .05). Edema resulted in a 10.3% ± 1.7% increase in leg CSA in the operated leg (P < .05). At hospital admission, muscle fiber CSA was smaller in the type II vs type I fibers (3326 ± 253 μm2 vs 4075 ± 279 μm2, respectively; P < .05). During hospitalization, type I and II muscle fiber CSA tended to increase, likely due to edema in the operated leg (P = .10).

Conclusions

Six days of hospitalization following elective total hip arthroplasty leads to substantial leg muscle atrophy in older patients. Effective intervention strategies are warranted to prevent the loss of muscle mass induced by short periods of muscle disuse during hospitalization.  相似文献

背景:进行性肌营养不良(PMD)中Duchenne型肌营养不良(DMD)和Becker型肌营养不良(BMD)的临床表现、治疗和预后差异明显,目前国内外对症状前期PMD患儿的诊断尚无共识。 目的:探讨婴幼儿时期症状前期PMD患儿的临床特征和实验室检查特点,并探讨血清肌酶水平在DMD和BMD分型诊断中的价值。 设计:病例系列报告。 方法:收集2016年1月至2020年7月江西省儿童医院确诊的症状前期PMD患儿,分析临床特征和实验室检查特点,并以基因检测结果为诊断标准分为DMD组和BMD组,血清肌酶通过ROC曲线分析以约登指数最大值时的取值作为诊断界值,比较不同肌酶的诊断准确性。 主要结局指标:PMD患儿的临床表现、基因结果、血清酶水平(AST、ALT、LTH、CK、CK-MB)。 结果:24例PMD患儿纳入分析,其中DMD组18例,BMD组6例。22例(91.7%)因发现转氨酶升高就诊,2例(8.3%)因亲戚确诊前来就医。PMD患儿CK-MB、CK、LDH、AST和ALT水平均明显升高,DMD组均高于BMD组(除AST外,P均<0.05)。当ALT>224.5 U·L-1、CK>11 069 IU·L-1、CK-Mb>204 IU·L-1、LDH>1 349.5 IU·L-1时为DMD的可能性更高,尤其是LDH>1 349.5 IU·L-1具有高度特异性。 结论:血清肌酶水平增高为 PMD 患儿症状前期的主要表现。LDH>1 349.5 IU·L-1对诊断DMD具有高度特异性。  相似文献

PurposeTo investigated the effect of vibratory stimulus on masseter muscles during oral fine motor biting tasks.MethodsSixteen healthy individuals (age: 24.5 ± 2.4 years) participated in experiment I during which the participants were asked to “hold and split” half a roasted peanut placed on a force transducer with their front teeth. The participant performed ten series with ten trials of the “hold and split” behavioral task while vibratory stimulus was applied on the masseter muscle every alternate series. Further, fourteen participants participated (age: 25.2 ± 4.8 years) in experiment II during which they performed a series each of the behavioral task at baseline, an adjusted baseline without and with vibration as well as with and without visual feedback. Hold and split forces along with the variability of hold force and duration and force rate during the split were measured.ResultsThe results of the study showed an increase in the magnitude of the hold force (P = 0.002), force rate during the split (P < 0.001) and a significant decrease in the duration of split (P < 0.001) due to the vibratory stimulus. However, there was no significant effect of the vibratory stimulus on the variability of hold forces (P = 0.879) or mean split force (P = 0.683) during the “hold and split” behavioral task. The results of experiment II also showed an increase in hold force due to the vibratory stimulus (P < 0.001).ConclusionsVibratory stimulus to the masseter muscles impairs the oral force control during a standardized biting task and provide further insight into the sensorimotor regulation of the masticatory system.  相似文献
