目的:探讨原发性高血压合并糖尿病老年患者血压与靶器官损害的关联性。方法:将本院收治的70例高血压合并糖尿病患者纳入A组,68例单纯高血压患者纳入B组,52例单纯糖尿病患者纳入C组,并将同时期的46例健康体检志愿者纳入D组。对四组患者的临床指标进行统计学分析。结果:A组患者的TC、LDL-C以及IMT水平均显著高于B、C两组;与B组比较,A组患者的24 h SBP、n SBP水平显著增高,同时24 h DBP、d DBP显著降低;A组患者的靶器官损害比例也显著高于其他组,比较差异均有统计学意义(P0.05)。结论:原发性高血压合并糖尿病能够显著加重患者的血压变异性以及靶器官的损害程度。  相似文献

Hypertension mediated organ damage (HMOD) refers to structural or functional changes in arteries or target organs that can be present in long-standing hypertension, but it can be also found in naïve never treated patients. Traditionally, cardiovascular risk is stratified with charts or calculators that tend to underestimate the real cardiovascular risk. The diagnosis of HMOD automatically reclassifies patients to the highest level of cardiovascular risk. Subclinical HMOD can be present already at the diagnosis of hypertension and more than 25% of hypertensives are misclassified with the routine tests recommended by hypertension guidelines. Whether HMOD regression improves cardiovascular outcomes has never been investigated in randomized clinical trials and remains controversial. However, different drugs have been probed with promising results in high cardiovascular risk patients, such as the new antidiabetic or the novel non-steroid mineralocorticoid antagonists. Accordingly, trials have shown that lowering blood pressure reduces cardiovascular events. In this narrative review, we will discuss the role of HMOD in cardiovascular risk stratification, the different types of organ damage, and the evidence available to define whether HMOD can be used as a therapeutic target.  相似文献

目的探讨围术期自控镇静技术中靶控输注不同镇静药物的临床镇静效果。方法对在该院接受手术的80例患者入院资料进行分析,随机分为丙泊酚组和咪达唑仑组。本研究中二组患者在围术期靶控输注不同镇静药物,咪达唑仑组靶控输注咪达唑仑镇静剂,丙泊酚组靶控输注丙泊酚镇静,注射完后记录患者平卧10min、注射镇静药后2min、5min、10min以及手术结束前10min、手术结束时等6个时间点的OAA/S镇静评分值,并密切观察患者的镇静效果和呼吸情况。结果丙泊酚组T1时刻镇静评分为(5.0±0.0),镇静评分值在注射药物2min到5min后都明显下降,T2时刻镇静评分为(3.6±0.2)、T3时刻镇静评分为(2.5±0.4)与咪达唑仑组差异有统计学意义(P<0.05);T4、T5、T6时刻镇静评分二组患者差异无统计学意义(P>0.05);镇静后二组患者的MAP,HR均明显下降,丙泊酚组患者在镇静后2min,5min时的MAP、HR均低于咪达唑仑组(P<0.05),其他时间二组差异无统计学意义;丙泊酚组中有5例患者呼吸抑制,咪达唑仑组有3例(P>0.05)。结论围术期自控镇静技术中靶控输注丙泊酚镇静效果较好,患者麻醉后对其生命体征影响比较小,值得推广使用。  相似文献

Purpose

The aim of our study is to research the role and efficacy of cerebral oximetry in predicting neurologic prognosis when applied during TTM to patients experiencing coma after CA.

Methods

This study was performed on surviving adult comatose patients after CA treated with TTM. The average scores of rSO2 was measured at 6 h intervals for the first 2 days and once a day for the following 3 days with a NIRS device during TTM. The CPC scale was used to define the neurologic outcomes of patients. We compared the correlations of rSO2 values between good (CPC 1–2) and poor (CPC 3–5) neurologic outcomes in CA patients.

Results

There was no statistically significant difference identified between the prognosis groups in terms of rSO2, CPR durations, hemoglobin values and admission body temperature (p > 0.05). When the variation in rSO2 values over time is investigated, though there was no significant difference between the good and poor prognosis groups, it appeared to fall in the first 6 h in both prognosis groups. The median NT-proBNP and lactate values were observed to be higher in the poor prognosis group.

Conclusion

There is no significant correlation between rSO2 values and neurologic outcomes. Multimodal monitoring methods may be useful and further studies with a larger patient population are necessary in this area.  相似文献

目的探讨DNA甲基化在肺腺癌发生中的作用机制。方法收集2019年1月至12月新疆肿瘤医院确诊的肺腺癌和正常对照者外周血各4例,850K芯片甲基化检测平台检测肺腺癌组与正常对照组甲基化区域,Bump hunter寻找两组差异区域。Gene Ontology数据库和KOBAS软件对差异区域对应目的基因进行GO分析和KEEG分析。结果1.共筛选出DMR-1、DMR-2、DMR-3、DMR-4、DMR-6(以上位于chr6),DMR-5(位于chr11)和DMR-7(位于chr20)(P<0.05)七组差异甲基化区域。DMR-1目的基因为HLA-DPB1、HLA-DPA1,DMR-2的为POU5F1,DMR-3的为RP5-1186N24.3、SCAND3,DMR-4的为ZFP57,DMR-5的为LDHC,DMR-6的为LTA,DMR-7的为OXT。其中HLA-DPB1、HLA-DPA1等为高甲基化,SCANDS3和LTA为低甲基化。2.GO分析表明目的基因主要在干扰素-γ、MHC-Ⅱ类复合物等功能中发挥重要作用。KEGG分析显示目的基因甲基化主要在Ⅰ型糖尿病、NF-κB信号通路中高度富集。结论1.肺腺癌患者DNA甲基化的状态可能是引起肺腺癌发生的关键因素,尤其是HLA-DP,POU5F1及LDHC的甲基化状态,在肺腺癌的发生中起着重要的作用。2.在GO功能和KEGG通路中,阐明了DNA甲基化异常导致疾病发展的作用机制。  相似文献

A highly effective medicine is urgently required to cure coronavirus disease 2019 (COVID-19). For the purpose, we developed a molecular docking based webserver, namely D3Targets-2019-nCoV, with two functions, one is for predicting drug targets for drugs or active compounds observed from clinic or in vitro/in vivo studies, the other is for identifying lead compounds against potential drug targets via docking. This server has its unique features, (1) the potential target proteins and their different conformations involving in the whole process from virus infection to replication and release were included as many as possible; (2) all the potential ligand-binding sites with volume larger than 200 Å3 on a protein structure were identified for docking; (3) correlation information among some conformations or binding sites was annotated; (4) it is easy to be updated, and is accessible freely to public (https://www.d3pharma.com/D3Targets-2019-nCoV/index.php). Currently, the webserver contains 42 proteins [20 severe acute respiratory syndrome-related coronavirus 2 (SARS-CoV-2) encoded proteins and 22 human proteins involved in virus infection, replication and release] with 69 different conformations/structures and 557 potential ligand-binding pockets in total. With 6 examples, we demonstrated that the webserver should be useful to medicinal chemists, pharmacologists and clinicians for efficiently discovering or developing effective drugs against the SARS-CoV-2 to cure COVID-19.  相似文献

Mental representation of numbers is believed to be spatial in nature, with small numbers occupying the left and large numbers the right side of a putative mental number line. Consistent with this, presentation of numbers from the low and high ends of the mental number line induces covert shifts of spatial attention to the left and right side of visual space, respectively. However, the effect of the presentation of the middle range (containing numbers below and above the midpoint) of the number line on visual perception has so far not been studied. Here we show in two experiments, using a line bisection task and a simple target detection task, that processing of middle-range numbers affects allocation of visuospatial attention in a similar way as processing of small numbers, with attention shifted to the left side of space. We suggest that this pattern of results arises due to “anchoring” heuristics that participants use in number processing.  相似文献
