目的研究胰岛素样生长因子-1(IGF-1)在实验性大鼠脑出血后脑组织中的表达厦其对细胞凋亡的影响。方法应用立体定向技术,将自体未抗凝血注入大鼠基底节区以制备脑出血模型;将动物分为正常对照组、实验组厦干预组,分别在不同时间断头取脑以制作标本,连续切片分别作IGF-1阳性细胞免疫组化染色及TUNEL染色。结果脑出血后2h血肿周围脑组织表达IGF-1,24h达表达高峰,7d时恢复正常;TUNEL染色阳性细胞于脑出血后8h开始出现,3d时达高峰,7d时仍有表达;给予外源性IGF-1后,凋亡细胞显著减少,与同时点实验组相比,差别有显著性。结论脑出血后IGF-1可抑制细胞凋亡的发生,从而减轻脑出血后继发性脑损伤。  相似文献

Abstract  Gastrointestinal stromal tumor (GIST) is a recently described mesenchymal tumor that can develop in any portion of the gastrointestinal tract. The occurrence of a GIST in the urinary tract is rare, but GIST can present as tumor of the urinary tract or invade the urinary tract. This is the first reported case of GIST in the ileal neobladder, which presented as a submucosal tumor. The differences between the postprandial mixing or propulsion and the interdigestive motility of the gastrointestinal (GI) tract are already known. Earlier studies showed dose-dependent differences in the effects of erythromycin on interdigestive motility. The various GI side-effects (vomiting, diarrhoea) also suggest that there are different effects of erythromycin on the GI motility. The aim of our study was to examine postprandially the propulsive effects of different doses of erythromycin on the movement of intraluminal contents in the upper GI tract of the rat. The animals were fasted for 24 h before the experiments but water was given freely. The rats received 1.5 ml 1.5% methylcellulose painted with 0.05% phenol-red intragastrically (test solution). Erythromycin(E. lactobionate) was given intravenously at doses of 0.05, 0.1, 0.25, 0.5, 1.0 and 5.0 mg/kg 15 min before the administration of a test solution. The animals were sacrificed 20,60 and 120 min after administration of methylcellulose, when the distance between the front of the painted intraluminal contents and the pylorus was measured and expressed as a percentage of the total length of small intestine. The phenol-red content in the stomach and small intestine was measured spectrophotometrically and the gastric emptying was calculated from the ratio of the measured total and intestinal phenol-red content. Our results showed that the small doses of erythromycin (0.1 and 0.25 mg/kg) accelerated gastric emptying after 20 min but did not change significantly the propulsive motility of upper small intestine; however, large doses of erythromycin (1.0 and 5.0 mg/kg) decreased gastric emptying and upper GI motility after 20 and 60 min. In summary, the prokinetic action of small doses of erythromycin was demonstrated, but its effecttime on GI motility is short and the ratio of the stimulating and inhibitory doses is 1:10. PURPOSE: This study was undertaken to clarify the importance of bleeding vascular ectasia of the colon as the etiology of massive lower gastrointestinal hemorrhage in patients 40 years of age or younger. METHODS: An otherwise healthy 21-year-old male was admitted to a tertiary medical center with massive lower gastrointestinal hemorrhage. Technetium-labeled red blood cell scan, selective visceral angiography, and colonoscopy identified the source of bleeding as vascular abnormality of the descending colon. Segmental colonic resection was performed. RESULTS: Histologic review of the specimen demonstrated a vascular ectasia. The patient recovered uneventfully and has had no further stigmata of hemorrhage. A review of the literature was undertaken to make clear the significance of vascular ectasia as the source for massive colonic hemorrhage in the young adult. CONCLUSION: This is the first report that documents histologically a vascular ectasia as the source of massive lower gastrointestinal hemorrhage in an otherwise healthy patient less than 40 years of age. 急性脑血管病肿瘤坏死因子-α的临床研究   总被引:1,自引:0,他引:1  
目的:探讨肿瘤坏死因子-α(TNF-α)在急性脑血管病(ACVD)中的作用及其变化的临床意义。方法:选取95例ACVD患者,并设性别及年龄相匹配的血清对照组(20例)及脑脊液对照组(10例)。应用酶联免疫吸附法(ELISA)于发病后第1、3、7、14d测定血清和脑脊液中TNF-α。结果:①三种脑血管病患者血清TNF-α动态变化存在差异,脑出血和脑梗死组的峰值单间为发病后第3d,而蛛网膜下腔出血(SAH)组为第1d。②脑梗死患者血清TNF-α水平与梗死面积、神经功能缺损程度及病情恶化与否相关。③SAH组于发病后第1d、3d、7d脑脊液TNF-α值高于血清,14d时降至对照组水平并与血清无差异。结论:①脑梗死患者血清TNF-α的动态观察,可为临床预测梗死灶大小、神经功能缺损程度及病情恶化与否提供依据。②发病早期的脑梗死患者若CT未能显示病灶时,血清TNF-α的明显升高有助于临床诊断和治疗。③SAH患者脑脊液中TNF-α水平明显高于血清,进一步提示脑内神经组织可产生TNF-α。脑脊液中TNF-α的持续明显升高可能与SAH后脑血管痉挛有关。④TNF-α参与了ACVD的炎性反应过程,早期抑制TNF产生及抗炎性反应的治疗可能具有潜在的临床价值。  相似文献

Intraventricular hemorrhage (IVH) in preterm infants is well known to be associated with the high morbidity and mortality of this group. Previous studies have suggested altered cerebral blood flow (CBF) as an important pathologic factor. We measured the CBF in nearterm rabbit fetuses using the hydrogen clearance technique. The local CBF of the rabbit fetuses was significantly low compared with that of the maternal rabbits. The response of CBF to changes in PaCO2 was observed in rabbit fetuses. The CO2 reactivity index of the fetal rabbit was lower than that of the maternal rabbit. This low CO2 reactivity might reflect the immaturity of the fetal brain and its low CBF. We were unable to monitor the fetal blood pressure, but the fetal CBF remained stable when the maternal blood pressure was altered. It is well known that IVH in preterm infants originates from the subependymal germinal matrix and that this has many fragile vessels. Our observation suggests that even a small increase of CBF during hypercapnia might have a large effect towards producing hemorrhage.  相似文献

目的分析与产后出血相关的因素,提出合理的防治措施。方法以12600例产妇为研究对象,将胎儿娩出后2h内出血量达到或超过400ml,或胎儿娩出后24h出血量达到或超过500ml定义为产后出血,测量产后出血量。通过非条件Logistic回归分析,筛选出与产后出血相关的主要危险因素,对比出血发生率的变化。结果在12600例分娩产妇中,符合产后出血的有198例,单因素分析中有10个因素有统计学意义。结论加强孕前宜教和孕期管理,提高助产技术,规范缩官素在产前的应用,降低了产后出血的发生。  相似文献
