Perineal hidradenitis suppurativa is a complex problem to treat. Various methods of treatment have been tried in the past, but recurrence was common. In this paper, we describe the use of the lotus petal flap in hidradenitis suppurativa of the perineum in three patients. Many local flaps have been used for covering the defect after excision of hidradenitis suppurativa; in our cases, we have used the lotus petal flap. Until now, this flap has been used for vaginal reconstruction after cancer surgery [Yii NW, Niranjan NS (1996) Lotus petal flaps in vulvo vaginal reconstruction. Three patients had five lotus petal flaps after the excision of perineal hidradenitis suppurativa, and there were no recurrences of the lesion in the excised area after follow-up at 2 years.  相似文献

Purpose. To investigate in vivo the fate and the behavior of lectin-particle conjugates after oral administration. Methods. Two plant lectins were selected, namely Lycopersicon esculentum L. and Lotus tetragonolobus lectins, which have been reported to be specific for oligomers of N-acetyl-D-glucosamine and L-fucose, respectively, and conjugated to small poly(lactide) microspheres. Their intestinal transit was investigated in detail using radiolabeled particles. The transport and the distribution of the particles along the intestine, as well as their interactions with the intestinal mucosa, were determined after oral administration in rat. Results. The overall transit of the particles was shown to be strongly delayed when the microspheres were conjugated to the lectins, mainly due to the gastric retention of the particles. A significant fraction of the conjugates adhered to the gastric and intestinal mucosae. No significant differences were observed after a preliminary incubation of lectin-microsphere conjugates with specific sugars. Conclusion. Although specific interactions could not be excluded, especially in the stomach, it was likely that adhesion was predominantly due to nonspecific interactions. These results could be attributed both to unfavorable physicochemical characteristics of the conjugates and to premature adsorption of soluble mucin glycoproteins, preventing any further specific adhesion.  相似文献

目的研究荷叶碱对大肠杆菌代谢作用的影响。方法从荷叶中提取其中的生物碱,用热活性检测仪测定了37℃下不同浓度的生物碱对大肠杆菌代谢作用的热功率-时间曲线,计算出细菌的生长速率常数。结果荷叶碱对大肠杆菌的代谢具有抑制作用,随浓度的增加,大肠杆菌生长速率常数线性降低。结论荷叶生物碱液对大肠杆菌代谢作用的最佳用药浓度为0.01146mg·mL^-1。  相似文献

目的:评价莲龙消积方改善中晚期原发性肝癌患者生活质量的临床疗效,探讨中医药治疗肝癌的效应机制,并探索肝癌治疗的最佳方式方法。方法:采用开放性随机对照研究方法,将中晚期原发性肝癌患者分为中药组(莲龙消积方组)(40例)、综合治疗组(莲龙消积方联合榄香烯西药介入组)(40例)及单纯西药介入组(45例)。疗程30 d,为1个观察周期,观察比较3组中医证候、体能状况(Karnofsky Performance Score,KPS评分)、肿瘤患者生活质量(Quality of Life,QOL评分)及AFP值变化情况等指标。结果:3组比较,在改善中医症候、KPS及QOL方面,中药组明显优于单纯化疗介入组,差异有统计学意义(P0.05);综合治疗组降低甲胎蛋白(Alpha Fetal Protein,AFP)值明显优于中药组、化疗介入组,差异有统计学意义(P0.05)。结论:莲龙消积方能够缓解中晚期原发性肝癌患者临床症状、改善AST及ALT、提高体能、改善生活质量。  相似文献

荷叶胶囊对人体血脂异常的调脂作用研究   总被引:17,自引:0,他引:17  
目的:探讨荷叶水提物对人体血脂异常的调脂作用。方法:从郴州市各类人员体检检出的高脂血症者中,选择符合高血脂诊断标准的自愿者,共90人。随机分成3组,每组30人,即荷叶胶囊组(A组),洛伐他汀组(B组)和对照组(C组)。A组服用荷叶水提物制作的胶囊,4粒/次,3次/d;B组服用洛伐他订,20mg/次,1次/d,C组给予安慰剂;各组均进行健康教育和膳食指导,调脂40天。采用日本产全自动生化分析仪(OLYMPUS-640)测定调脂前、后的血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)水平。结果:A组调脂后血清TC、TG、LDL-C水平明显下降,HDI-C水平明显升高,与调脂前比较有显著差异(P<0.001.<0.001,<0.002,<0.01);B组调脂后TC、TG、LDL-C水平亦明显下降(P<0.001,<0.05,<0,001),与A组无显著差异(P>0.05),但升高HDL-C效果不明显(P>0.05);C组调脂前、后血清TC、TG、LDL-C和HDL-C水平无显著差异(P>0.05),且调脂后的血脂水平明显高于A、B组(P<0.01)。结论;荷叶水提物对人体具较好的调脂作用,且无毒,未发现副作用。  相似文献

Aim

To assess the efficacy of the lotus petal flap in difficult pilonidal sinus management.

Background

Pilonidal sinuses may be difficult to treat. Five year recurrence rates range from 18% to 50% Recurrence rates fall with primary closure rather than healing by secondary intention. Techniques such as z-plasty however, distort natal architecture. The lotus petal flap taken from the superior buttock fills dead space whilst conforming to the natural concave appearance of the natal cleft. The contralateral buttock is spared. It is straightforward and reproducible. Multiple perforators make it robust and promote wound healing in this difficult group.

Method

We present a series of 14 patients who had a lotus petal flap reconstruction following pilonidal sinus excision between 2007 and 2015. The majority of these patients had previous multiple failed attempts at eradication. Recurrence rates, re-operation rates, time for complete healing, wound breakdown, discharge, infection, cosmesis and patient comfort were assessed.

Results

Many patients had coarse hair; felt to predispose to recurrence of symptoms, poor hygiene and prolonged wound healing. 50% of patients underwent Alexandrite laser (755?nm wavelength) for a minimum of 6 treatments. There was excellent compliance and good patient outcome. Overall in this complex patient group only 14% had prolonged wound healing and recurrence rates were lower than those in the literature.

Conclusion

The lotus petal flap is an excellent choice for problematic pilonidal sinus reconstruction. When combined with tumour like excision, meticulous surgical technique and complimentary laser hair removal results can be improved further.  相似文献
