Delayed Reduction of Irreducible Chronic Posterolateral Dislocation of the Knee with Buttonholing of the Medial Femoral Condyle

Abstract Traumatic knee dislocations are relatively rare and almost always respond to closed reduction; however, a small percentage of knee dislocations are irreducible and in these cases open reduction is frequently required. A 65-year-old man with an unreduced posterolateral knee dislocation with laterally dislocated patella was seen 3 weeks after a motor vehicle accident. Medial femoral condyle was found buttonholed through the medial capsule together with the medial collateral ligament and lying in the medial joint space that allowed posterior rotary dislocation of the joint. Both cruciate ligaments and medial meniscus were torn. There was no evidence of any vascular or nerve injury. Reduction was accomplished by removal of the capsuloligamentous structures which were incarcerated in the trochlea and intercondylar notch and by excision of meniscal tear. Following posterior cruciate ligament reconstruction with patellar tendon autograft, lateral patellar release, vastus medialis advancement, and gracilis transfer were done.

Changing prevalence and knowledge of urinary incontinence among Hong Kong Chinese women

A territory-wide telephone survey was conducted in Hong Kong to assess the prevalence, knowledge, and treatment-seeking behaviour of Chinese women with urinary incontinence, using validated Chinese version of Urogenital Distress Inventory (UDI-6) and Incontinence Impact Questionnaire (IIQ-7). Women, 540, aged between 17 to 77 years were interviewed. Of the respondents, 40.8% reported stress urinary incontinence, 20.4% had urge incontinence and 15.9% had mixed incontinence. Among these, 16.0% reported quality of life impairment; 9.3% felt frustrated with low morale, and 15.2% had nervous and anxiety problems. However, as many as 78.3% of the respondents did not know that stress urinary incontinence is a disease entity, and 60.6% thought that leakage of urine was a normal aging process. For those respondents having stress urinary incontinence, the first treatment of choice was physiotherapy. The second choice was medication, and surgical treatment was the last option. Respondents with stress urinary incontinence showed higher education level.

目的：研究蜂胶乙醇提取物黄酮（EEP）对小白鼠衰老过程中胸腺肽合成量的影响。方法：垂直板电泳法。结果：EEP可增加小鼠胸腺肽的合成量。结论：EEP可增强小鼠的免疫力。具有抗衰老作用。     

非糖皮质激素的免疫抑制方案对同种异体移植胰岛的保护作用

目的　评价非糖皮质激素的免疫抑制方案对防止大鼠同种异体胰岛移植排斥反应的效果。方法　大鼠同种异体胰岛移植后 ,分别应用他克莫司 (FK5 0 6 ) 霉酚酸酯 (MMF)和FK5 0 6 MMF 泼尼松 (Pred)行免疫抑制治疗两周 ,并设对照组 ,动态观察受者血糖、胰岛素及C肽变化 ,并作移植部位的形态学检测。结果　FK5 0 6 MMF组和FK5 0 6 MMF Pred组与对照组相比 ,移植胰岛存活时间明显延长 ,移植后 2个月在受者肝汇管区可见较多形态完整的胰岛细胞。FK5 0 6 MMF组维持术后正常血糖、胰岛素及C肽的时间超过 6 0d ,而FK5 0 6 MMF Pred组与前者比较 ,分泌C肽较少 (P <0 .0 5 ) ,血糖维持在较高水平 (P <0 .0 1) ,胰岛素水平两组差异无显著性。FK5 0 6 MMF Pred组停药两周以后的血糖水平较用药期间、停药两周内有明显降低 (P <0 .0 5 ) ,胰岛素和C肽分泌有所增多 ,但差异无显著性。结论　应用FK5 0 6 MMF和FK5 0 6 MMF Pred均有很强的免疫抑制效应 ,但糖皮质激素对胰岛细胞有毒性作用。小剂量FK5 0 6与MMF联用对移植胰岛细胞有较强的保护作用。     

80 例人工全髋关节置换术的中期疗效分析

目的 探讨人工全髋关节置换术的疗效。方法 回顾性研究了1997年3月至2002年12月期间连续全髋关节置换术80例(92髋)患者，平均随访54个月(14～84个月)，分析其中期疗效。结果 翻修率为5．4％(5／92)，Harris髋评分为优良以上84．8％(78／92)，中等8．7％(8／92)，差6．5％(6／92)。结论 外科技术的提高和假体生物工程的发展，提高了全髋关节置换术重建关节功能的疗效。     

Pedicle screw fixation with translaminar facet joint screws for the treatment of thoracolumbar fracture

Objective:To evaluate the efficacy of Cotrel-Dubeusset (CD) instrumentation combined with translaminar facet joint screw ( TLS ) in the treatment of thoracolumbar fracture. Methods: A total of six L2-L4 spines were used to establish unstable fracture model with three-dimensional range of motion ( ROM ) of the spines measured. Fixation with CD and fixation with CD combined with translaminar facet joint screw were achieved to compare their stability. Thirty cases of thoracolumbar fracture, in whom the anterior edge of vertebral body was compressed to 59% and the posterior edge compressed to 88%, were treated by pedicle screw fixation combined with TLS. Among them, 19 received posterolateral or anterior-posterior bone grafting Results- There was significant difference in ROM between the two techniques except that in extension. In Group CD TLS, ROM was 5.38% lower, lateral bending 4.91% lower and axial rotation 11.85 % lower than those in Group CD respectively. In the clinical group, the average anterior edge restored to 97 % and posterior edge to 98%. The duration of follow-up was 5-24 months (mean, 10 months). The rate of correction loss on the anterior edge was 4.5%. Among the 19 cases of bone grafting, all of them achieved bony fusion (mean fusion time, 4.3 month) with a correction loss rate of 3.4%. Conclusions:In the treatment of thoracolumbar fracture, pedicle screw fixation combined with TLS can strengthen the stability of pedicle screws, especially antirotation stability and enhance fusion rate and reduce correction loss.     

Cervifix在陈旧性寰枢椎脱位并高位颈髓压迫症中的应用

目的:探讨Cervifix内固定系统治疗陈旧性寰枢椎脱位并脊髓不全损伤的价值。方法:36例陈旧性寰枢椎脱位并脊髓不全损伤行后路融合、Cervifix内固定。男23例、女13例,年龄15～62岁,平均38.3岁。陈旧性外伤21例,先天性畸形9例,类风湿性关节炎伴寰椎前脱位6例。结果:随访8～42个月,平均19个月。术后脊髓功能按JOA17分法评定,改善率为71.4％。全部患者均获骨性融合,内置物无松动、断裂及脊髓损伤加重等并发症。结论:Cervifix内固定可提供坚强有效的节段性固定,提高融合率。同时,可预防单纯减压融合术后早期失稳导致的高位脊髓损伤加重。     

骶髂螺钉固定在骶髂关节脱位中的应用解剖学研究

目的 :为应用骶髂螺钉固定骶髂关节脱位提供形态学依据。方法 :对 3 0具成人干燥骨盆标本 ,模拟骶髂螺钉内固定手术并作CT扫描 ,对进针点与不同解剖结构之间的距离、进针的方向、进针的深度以及S1骶孔上方的骶骨翼的前后径和上下径进行测量。结果 :进针点距臀肌线的距离为 ( 2 0 70± 3 2 7)mm ,距坐骨大切迹的距离为( 3 5 0 0± 1 91)mm ;轴位及冠状位CT测量进针与髂骨外板的交角分别为 ( 90 18± 2 69)°和 ( 90 40± 2 87)° ;进针深度为 ( 67 77± 3 63 )mm ;S1骶孔上方骶骨翼的前后径和上下径分别为 ( 18 2 6± 2 0 6)mm和 ( 18 74± 1 5 1)mm。结论 :临床应用骶髂螺钉固定需选择恰当的进针点和进针方向 ,术中需行骨盆正位、入口位和出口位的透视以确定螺钉植入的准确性
