运动对骨骼－肌腱界面愈合组织的影响

利用纽西兰兔作体内实验，来分析手术后石膏固定，笼内活动，连续被动性活动三种不同的复健方法对肌腱－骨骼间界面愈合组织的影响。结果显示在术后第１５天愈合组织所能承受的最大张力，笼内活动组为０．８７５ｋｇ，石膏固定组为２．０１４ｋｇ，是笼内活动组的２３１．５２％，而连续被动性活动组为２．６３８ｋｇ，是笼内活动组的２６１．３５％。石膏固定组虽有很高的张力，但关节内粘连也较多。     

手部肌腱吻合术后康复

目的作者自2000年初至2004年7月进行手部肌腱吻合120例,随访120例,手部肌腱吻合术后进行程序化的康复训练,方法肌腱吻合法均采用3-0或4-0聚丙烯单针缝线、改良的Bunnell、Kessler和肌腱编制缝合法,患者肌腱均为I期手术吻合,术后伤口I/乙愈合105例、II/乙15例,手部肌腱吻合术后均采用程序化的康复训练, 结果对手部肌腱吻合术后的患者进行程序化的康复训练,取得良好的效果. 结论本康复训练程序目前已逐渐形成我院的治疗常规,并逐步推广和普及.     

应用跖肌腱转移治疗腓骨肌腱滑脱症

[摘要]目的总结应用跖肌腱转移治疗腓骨肌腱滑脱症的临床应用结果。方法 2008年9月~2014年1月，对9例腓骨肌腱滑脱症患者采用跖肌腱转移治疗。其中，男7例，女2例。年龄22~46岁，平均32.6岁。左足5例，右足4例。结果 术后随访13个月~5年，平均2.5年。术后恢复率按根据Sefton踝关节手术疗效评定标准进行评定。优7例，良2例，没有发生与该项技术操作相关的并发症，取得了满意的治疗效果。结论 应用跖肌腱转移术很适宜治疗腓骨肌腱滑脱症，修复后的牢固性较好。但是，对该肌腱缺如者不适宜。     

Tensile properties of fresh human calcaneal (Achilles) tendons

The purpose of this study was to measure the tensile properties of fresh human calcaneal (Achilles) tendons. Twenty fresh cadaveric (age range = 57-93 years) bone-Achilles tendon complexes were harvested within 24 hr postmortem. The calcaneus together with 15 cm of the Achilles tendon extending proximally from the insertion on the calcaneus was clamped and biomechanically tested. Each tendon was firmly fixed in clamps in an MTS Systems Corporation MTS testing machine and tension was applied at a displacement rate of 8 cm per minute until the tendon failed. The tensile force and tensile strain (as measured using an extensometer) were recorded and plotted using onboard software. The narrow age range of our donors prevented any meaningful correlation between age and tensile properties; however, the results showed that: 1) the average ultimate tensile strength (UTS) of the human Achilles tendon was 1189 N (range = 360-1,965), 2) there was a correlation between left and right legs for UTS, 3) there was a correlation between left and right legs in regard to cross sectional area, and 4) there was no correlation between UTS and cross-sectional area.     

Zur Topographie des oberen Bicepssehnenabschnittes

Zusammenfassung Die polarisationsoptische and histologische Untersuchung des oberen Bicepssehnenabschnittes an 26 Schultern bestätigt, daß die Sehne ihren Ursprung nicht nur, wie allgemein angenommen, am Tuberculum supraglenoidale hat, sondern außerdem -förmig im Labrum glenoidale. Dieser Befund kann phylogenetisch erklärt werden. Im weiteren Verlauf der Sehne können neue Befunde im Bereich des Sulcus bicipitalis erhoben werden. Hier ist regelmäßig ein Mesotenonium nachweisbar, das durch einen bindegewebigen Ausläufer an Sulcusdach oder -grund befestigt ist. Darüberhinaus wird gezeigt, daß das Sulcusdach nicht als Ligament (Ligamentum transversum humeri) anzusprechen ist, es handelt sich vielmehr um Ausläufer der Kollagenfasern von Subscapularis- bzw. Kapselansatz am Tuberculum minus.

Topography of the upper biceps tendon

Summary 26 shoulders are studied by polariscopic and histologic examination. It can be confirmed that the origin of the long biceps tendon is not only at the supraglenoid tubercle, but also -shaped in the glenoid labrum. This finding can be explained by phylogenetic means. Further details are found during the course of the tendon through the bicipital groove. A mesotenonium is seen regularly. This is fixed either to the roof or the ground of the sulcus by a band of connective tissue. It is shown also, that the roof of the groove is not a ligament (ligamentum transversum. humeri) but consists of collagen fibers continuing from the subscapularis and capsular attachment at the lesser tubercle.

     

Electrophysiological effects of Chinese medicine Shen song Yang xin (SSYX) on Chinese miniature swine heart and isolated guinea pig ventricular myocytes

Background Shen song Yang xin (SSYX) is a compound of Chinese medicine with the effect of increasing heart rate (HR). This study aimed to evaluate its electrophysiological properties at heart and cellular levels. Methods The Chinese miniature swines were randomly assigned to two groups, administered with SSYX or placebo for 4 weeks (n=8 per group). Cardiac electrophysiological study (EPS) was performed before and after drug administration. The guinea pig ventricular myocytes were enzymatically isolated and whole cell voltage-clamp technique was used to evaluate the effect of SSYX on cardiac action potential (AP). Results SSYX treatment accelerated the HR from (141.8±36.0) beats per minute to (163.0±38.0) beats per minute (P=0.013) without changing the other parameters in surface electrocardiogram. After blockage of the autonomic nervous system with metoprolol and atropin, SSYX had no effect on intrinsic HR (IHR), but decreased corrected sinus node recovery time (CSNRT) and sinus atrium conducting time (SACT). Intra cardiac EPS showed that SSYX significantly decreased the A-H and A-V intervals as well as shortened the atrial (A), atrioventricular node (AVN) and ventricular (V) effective refractory period (ERP). In isolated guinea pig ventricular myocytes, the most obvious effect of SSYX on action potential was a shortening of the action potential duration (APD) without change in shape of action potential. The shortening rates of APD30, APD50 and APDgo were 19.5%, 17.8% and 15.3%, respectively. The resting potential (Em) and the interval between the end of APD3o and APD9o did not significantly change.Conclusions The present study demonstrates that SSYX increases the HR and enhances the conducting capacity of the heart in the condition of the intact autonomic nervous system. SSYX homogenously decreases the ERP of the heart and shortens the APD of the myocytes, suggesting its antiarrhythmic effect without proarrhythmia.     

几丁糖预防屈肌腱粘连的临床效果

目的：观察几丁糖预防肌腱粘连的临床效果。方法：对Ⅱ区肌腱损伤４８例７２条肌腱断裂行一期修复,术中采用改良Ｋｅｓｓｌｅｒ法缝合肌腱,在肌腱鞘管内注入２％医用几丁糖０．５￣１．０ｍｌ,缝合皮肤。     

血管瘤不同治疗方法的疗效评价

目的　评价四种不同方法治疗血管瘤远期疗效。方法　取 1992～ 1998年共七年间我院门诊及住院分别患有毛细血管瘤、海绵状血管瘤、蔓状血管瘤病人 2 33例 ,分别采取血管瘤营养血管结扎 平阳霉素注射、5 %鱼肝油酸钠注射、平阳霉素 强的松龙注射、平阳霉素 5 %鱼肝油酸钠注射 4种治疗方法 ,并对上述 4种方法血管瘤注射后进行随访及疗效评定。结果　血管瘤营养血管结扎 平阳霉素注射组总有效率及显效率均较其余 3组高 ,而单纯应用鱼肝油酸钠血管瘤注射组较其余 3组无论是总有效率或显效率均低。结论　联合用药较单一用药治疗血管瘤效果好 ,而血管瘤远期疗效与药物在瘤体内存留时间长短有关 ,停留时间愈长 ,其有效率愈高 ,而缝扎血管瘤营养血管后无疑为药物在血管瘤内长时间停留提供条件。     

正常冈上肌腱乏血管区胶原分布的图像分析研究

目的 运用计算机图像分析方法研究正常冈上肌腱胶原分布的规律,以探讨胶原分布与冈上肌腱撕裂之间的联系。方法 动用组织化学及免疫组织化学方法染色获得的正常冈上肌腱切片标本,分为２组,即：距冈上肌腱止点１ｃｍ处为一组（Ａ组）,２ｃｍ处为另一组（Ｂ组）;并用计算机图像分析系统检测肌腱横切面上胶原所占面积百分比,所得数据用Ｓｐｓｓ８．０作统计学处理。结果 Ｍａｓｓｏｎ三重染色的切片中,Ａ组胶原所占横截面积百     

黄芪灯盏花注射液对阳气虚型冠心病心率变异的影响

观察黄芪灯盏花注射液对 2 5例阳气虚型冠心病患者的心率变异性(HRV)的影响 ,并设对照组及正常人组 ,采用动态心电图含有 HRV时域分析软件进行分析。结果 :黄芪灯盏花注射液组 HRV数值明显提高 ,差异显著 (P<0 .0 5 ) ,或非常显著 (P<0 .0 1)。提示 :黄芪灯盏花注射液对冠心病患者 HRV心脏自主神经功能具有益影响。
