PurposeVedolizumab (VDZ) was reported to be effective as a first-line biological in adult ulcerative colitis (UC). The aim of this study was to investigate the safety and effectiveness of VDZ as the first-line biological in pediatric refractory UC.MethodsWe retrospectively extracted data from pediatric patients with UC who received first-line VDZ. The following were recorded: baseline characteristics; clinical activity of intestinal disease, levels of fecal calprotectin (FC), C-reactive protein, and serum amyloid A; and erythrocyte sedimentation rate. Clinical effectiveness, biochemical remission, and safety of VDZ were also investigated.FindingsEight patients were identified (median age, 12 years). FC levels were abnormal in all cases and were remarkably elevated in 3 patients. C-reactive protein, serum amyloid A, and erythrocyte sedimentation rate values were abnormal in 2, 4, and 5 patients, respectively. According to the pediatric UC activity index score, 1, 5, and 2 patients had mild, moderate, and severe disease. Fourteen weeks after VDZ administration, 5 patients achieved remission, and 3 remained in remission until week 52. Of the 3 patients who did not reach remission in week 14, two achieved remission by week 52. In week 52, five of the eight patients continued receiving VDZ and maintained remission. All patients achieved clinical remission without corticosteroid use. No adverse events were observed in any patient.ImplicationsVDZ may serve as a safe and effective first-line biological option for treating pediatric patients with UC. Plant protection products may affect the behavior of organisms which are not a target of control. The effect of Karate Zeon 050 CS (λ-Cyhalothrin -based insecticide; λ-CBI) and Amistar 250 SC (Azoxystrobin-based fungicide; ABF) was determined on Apis mellifera worker attraction towards their own colony odour, along with temperature preferences. Bees exposed to pesticides prefer the environment with the odour of their nest less often than the control group, and that insecticide-treated bees chose warmer environments than the control insects. The observed differences in the bees, especially with attraction towards their own colony, were dependent on the time of day. Chromatographic analyses indicated that λ-Cyhalothrin elimination was half that of Azoxystrobin in bee organisms, and both agents retarded each other’s clearance. Mathematical modeling estimated that despite a relatively high disappearance rate, both compounds might have been bio-accumulated at relatively high level. 相似文献

Mandatory accurate and specific diagnosis demands have brought about increased challenges for radiologists in pediatric posterior fossa tumor prediction and prognosis. With the development of high-performance computing and machine learning technologies, radiomics provides increasing opportunities for clinical decision-making. Several studies have applied radiomics as a decision support tool in intracranial tumors differentiation. Here we seek to achieve preoperative differentiation between ependymoma (EP) and pilocytic astrocytoma (PA) using radiomics analysis method based on machine learning. A total of 135 Magnetic Resonance Imaging (MRI) slices are divided into training sets and validation sets. Three kinds of radiomics features, including Gabor transform, texture and wavelet transform based ones are used to obtain 300 multimodal features. Kruskal–Wallis test score (KWT) and support vector machines (SVM) are applied for feature selection and tumor differentiation. The performance is investigated via accuracy, sensitivity, specificity, and area under the receiver operating characteristic curve (AUC) analysis. Results show that the accuracy, sensitivity, specificity, and AUC of the selected feature set are 0.8775, 0.9292, 0.8000, and 0.8646 respectively, having no significant differences compared with the overall feature set. For different types of features, texture features yield the best differentiation performance and the significance analysis results are consistent with this. Our study demonstrates texture features perform better than the other features. The radiomics approach based on machine learning is efficient for pediatric posterior fossa tumors differentiation and could enhance the application of radiomics methods for assisted clinical diagnosis. 相似文献

目的探讨急性缺血性脑卒中合并心房颤动患者不同时间窗内静脉溶栓的疗效差异。方法选取急性缺血性脑卒中行静脉溶栓治疗患者172例,根据发病-溶栓时间窗差异分为3组,时间窗分别为≤3.0 h(观察A组)、>3.0~4.5 h(观察B组)、>4.5 h(观察C组),对其中合并心房颤动者溶栓疗效进行评估分析。结果3组患者溶栓24 h后出血转化结果、溶栓3个月时神经功能结局良好率、病死率均无明显差异(P>0.05);溶栓时间窗>3 h者,心房颤动可显著增加患者发生PH型、HI型出血转化发生率,差异有统计学意义(P<0.05);单因素分析显示,合并心房颤动可造成溶栓时间窗≤4.5 h患者神经功能结局不良发生率增加,差异有统计学意义(P<0.05)。多因素分析显示,合并心房颤动与不同时间窗急性缺血性脑卒中患者静脉溶栓治疗后神经功能结局不良发生情况无明显相关性(P>0.05)。结论溶栓时间窗仍是影响急性缺血性脑卒中患者静脉溶栓疗效的重要因素,对于溶栓时间窗≤3.0 h者,合并心房颤动不会对溶栓疗效造成影响;对于发病-溶栓时间>3 h者,心房颤动可能造成患者溶栓后出血风险增加。 相似文献

BackgroundRearranged during transfection (RET) proto-oncogene gene fusions are rare in non–small-cell lung cancer (NSCLC). We compared the efficacy of pemetrexed-based chemotherapy with other chemotherapy regimens in patients with NSCLC with different RET fusion subtypes.Patients and MethodsA retrospective, multicenter study of patients with pathologically confirmed stage IIIB/IV lung adenocarcinomas was conducted. RET rearrangements were detected using next generation sequencing. We analyzed the clinical characteristics of patients with RET-rearranged NSCLC and the efficacy of chemotherapy regimens. We also evaluated the efficacy between groups of patients with and without KIF5B-RET–rearranged lung cancer.ResultsWe evaluated 62 patients with NSCLC and RET rearrangements, including 41 with KIF5B-RET, 15 with CCDC6-RET, and 6 with other rare fusion subtypes. Of these 62 patients, 50 had stage IIIB/IV. We also evaluated 40 patients with first-line chemotherapy information available. The median progression-free survival was significantly different between those receiving pemetrexed-based chemotherapy and those receiving other chemotherapy regimens (9.2 vs. 5.2 months; P = .007). The median progression-free survival for patients with KIF5B-RET fusion and non–KIF5B-RET fusion was not significantly different statistically (7.8 vs. 11.2 months; P = .847). For second-line chemotherapy, a statistically significant difference was found between the chemotherapy regimens (4.9 vs. 2.8 months; P = .049). Survival follow-up data were available for 38 patients with advanced NSCLC. The median overall survival was 26.4 months. The overall survival of the patients with RET-rearranged NSCLC who had received pemetrexed-based chemotherapy versus no pemetrexed-based chemotherapy was 35.2 versus 22.6 months (P = .052). No difference in survival was observed between the patients with KIF5B-RET and non–KIF5B-RET rearrangements.ConclusionsPemetrexed-based treatment should be considered first when selecting the chemotherapy regimen for patients with NSCLC and RET rearrangements. 相似文献
