Medial patellofemoral ligament (MPFL) reconstruction is the “workhorse” for surgical stabilization of recurrent patella instability. Complications of patella fixation (i.e., patella fracture) are rare but potentially catastrophic. Modifications to traditional MPFL reconstruction that avoid patella fixation are promising. These alternatives may be favored in high-risk scenarios such as revision, smaller pediatric cases, and patellofemoral arthroplasty with concomitant instability. Large-scale prospective and/or randomized studies are needed to differentiate between MPFL reconstruction techniques.  相似文献

BackgroundHip Osteoarthitis (OA) risk is sport-specific and depends on frequency, intensity, and type of mechanic stress the hip is subjected to. This retrospective observational study aims to investigate the safety and performance of Hymovis (HYADD-4) injection, a hexadecyl (C-16) HA-derivative, when used to manage symptomatic hip OA in active middle-aged sportsmen over a 24-month observation period.MethodsThe retrospective analysis included clinical records of active sportsmen, aged between 40 and 65 years, and suffering from symptomatic Kellgren-Lawrence grade II to III hip OA, treated with two (24 mg/3 ml) Hymovis injections, two weeks apart, every 3–4 months, for two years. When available, data on MRI examination were included in the analysis as well as Heidelberg Sports Activity Score (HAS) and Copenhagen Hip and Groin Outcome Score (HAGOS) questionnaires.ResultsThirty patients (56.4 ± 7.3 years) were included in the study, sixteen cyclists and 14 tennis players. For all patients, HAS and most HAGOS scores improved significantly (p < 0.05) at the first control visit (4 months) and further improved over time. For all other scores an important clinical benefit was experienced by more than 50% of participants. No adverse events were recorded.ConclusionTreatment of hip OA in active sportsmen with Hymovis seems a safe and effective approach for the management of OA symptoms, by potentially protecting cartilage and subchondral bone from further damage.  相似文献

AimTo compare the accuracy of the inverted greyscale CT versus the conventional CT in the assessment of post-operative spinal orthopaedic implants and osseous fusion.Methods50 patients who had CT as part of their routine spinal implant follow up were evaluated for the presence of fusion, fracture and loosening with conventional CT and with greyscale inverted CT images. 3 independent observers assessed the images 2 months apart. Diagnostic performance (sensitivity and specificity) of the conventional and greyscale inversion images relative to the reference standard were calculated. Agreement with the reference standard was assessed using Cohen's kappa for conventional and greyscale inversion images.ResultsCorrect classifications increased when using the greyscale inverted CT images for each reader compared to conventional CT images (40–46, 39 to 42 and 41 to 44 (out of 50)). Inverted images demonstrated better agreement with the reference standard than conventional grayscale images for assessment of fusion (kappa of 0.588 for inverted CT versus 0.484 for conventional CT) and loosening (kappa 0.386 for inverted versus 0.293 for conventional). Sensitivity was increased for assessment of fusion and loosening. McNemar's test performed for assessment of sensitivity differences showed statistical significance (p = 0.038 for fusion and p = 0.0313 for loosening).ConclusionGreyscale inversion CT is a useful adjunct which has advantages (improved sensitivity and better agreement) over conventional CT imaging in cases of fusion and loosening of metallic implants following spinal instrumentation. We recommend the use of both the greyscale inversion CT images and conventional CT imaging when assessing post-operative spinal orthopaedic implants.  相似文献

目的 观察微波消融(microwave ablation,MWA)联合经皮穿刺椎体成形术(percutaneous vertebroplasty,PVP)治疗腰椎转移瘤的临床疗效。方法 回顾性分析2016年6月至2019年6月广西医科大学附属肿瘤医院收治的50例腰椎转移瘤患者的临床资料,其中行经皮椎体成形术26例(PVP 组),经皮微波消融联合经皮椎体成形术24例(PVP+MWA组)。观察两组患者疼痛、功能状态以及骨水泥外渗情况和术后肿瘤复发率。结果 随访6~36个月,PVP组VAS评分从术前的(7.58±1.06) 分降至术后1个月的(3.27±1.40) 分和术后6个月的(3.08±1.60) 分;PVP+MWA组从术前的(7.67±1.05) 分降至术后1个月的(3.04±1.20) 分和术后6个月的(2.96±1.46) 分,两组术后 1个月及6个月的VAS评分低于术前(均P<0.05),但重复测量方差显示,组间效应差异无统计学意义(F=0.223,P=0.801)。术后1个月,PVP组的KPS评分较术前提升(21.50±11.32) 分,PVP+MWA组较术前提升(19.92±13.19) 分,两者差异无统计学意义(t=0.457,P=0.650)。PVP+MWA 组的骨水泥外渗率(12.5% vs 38.5%,P=0.037)和肿瘤复发率(8.3% vs 30.8%,P=0.048)均低于PVP 组。结论 经皮穿刺椎体成形术单独或联合微波消融治疗腰椎转移瘤均可取得较好临床疗效,两者联合在减少骨水泥外渗及局部肿瘤控制中更有优势。   相似文献

目的探讨胸腔镜辅助下侧前路减压融合术治疗胸腰段或高位腰椎间盘突出症(lumbar disc herniation,LDH)伴椎体后缘骨软骨病(vertebral osteochondrosis,VO)的可行性及其临床效果。方法回顾性分析2017年12月至2019年12月采用胸腔镜辅助下侧前路减压融合术治疗胸腰段或高位LDH伴VO 10例患者的病历资料,男6例,女4例;年龄37~65岁,平均49.2岁。手术节段均为单节段,累及T12L1节段5例、L1,2节段2例、L2,3节段3例。4例患者为单纯胸腰段或高位LDH伴VO;6例患者合并黄韧带增生、骨化致椎管狭窄或后凸畸形,联合后路减压内固定术或矫形手术。主要观察指标为疼痛视觉模拟评分(visual analogue scale,VAS)、Oswestry功能障碍指数(Oswestry disability index,ODI)及椎间隙前、后缘高度,根据改良MacNab标准评估临床疗效。结果10例患者均顺利完成手术,术中在胸腔镜辅助下能清晰显露并彻底去除突出的椎间盘和骨化物,脊髓、神经根和硬膜囊得到充分减压。手术时间(115.4±23.8)min(范围70~180 min);术中失血量(122.6±21.3)ml(范围40~310 ml)。术后随访时间为12~36个月,平均21.6个月。末次随访时10例患者VAS评分由术前(7.2±1.9)分降至(1.8±1.1)分,ODI由术前64.3%±13.9%降至16.3%±5.1%,椎间隙前缘高度由术前(7.8±1.5)mm改善至(11.9±2.3)mm,椎间隙后缘高度由术前(4.5±1.1)mm改善至(7.4±1.6)mm,差异均有统计学意义(P<0.05)。末次随访时改良MacNab疗效评定为优9例、良1例。结论胸腔镜辅助下侧前路减压融合术治疗胸腰段或高位LDH伴VO,可提供清晰的手术视野,能够充分显露、彻底去除突出的椎间盘及骨化物,术后近期疗效满意。  相似文献

The choice of the most suitable surgical approach to the elbow forms the foundation of any successful elbow surgery. The surgical approach is based on the injury or pathology to be addressed and therefore specific anatomical details need to be considered. The surgeon must be comfortable with the bony, ligamentous and neurovascular anatomy of the elbow to consider and execute the best approach for each problem. This is an imperative to avoid iatrogenic injury.This article provides a detailed analysis, valuable technical tips, advantages and disadvantages of the most common approaches to the elbow. The lateral approaches include the Kocher, Kaplan and Extensor Digitorum Communis (EDC) Split approaches, the medial approaches include the Hotchkiss, Flexor carpi ulnaris (FCU) splitting approach, the Taylor and Scham approach. The anterior approach includes the anterior neurovascular interval approach and the posterior approaches include the Olecranon osteotomy, triceps sparing, triceps reflecting approach and finally the Boyd interval approach. Computed tomography (CT) scans are often used for postoperative imaging in orthopedics. In the presence of metallic hardware, artifacts are generated, which can hamper visualization of the CT images, and also render the study ineffective for 3-D printing. Various solutions are available to minimize metal artifacts, and radiologists can employ these before or after processing the CT study. However, the orthopedic surgeon may be faced with situations where the metal artifacts were not addressed. To counter such problems, we present three do-it-yourself (DIY) techniques that can be used to manage metal artifacts.  相似文献

Most malignant bone tumors are treated with surgical excision, adhering to oncologic principles, followed by reconstruction to preserve form and function whenever feasible. Primary bone tumors around the elbow are rare accounting for <1% of all skeletal tumors. They pose a reconstructive challenge, due to the complex interplay between the osseous & capsulo-ligamentous structures which is essential for elbow stability and function. Tumors affecting the proximal ulna are rare and reconstruction of the defects following these tumors is extremely challenging. Various reconstruction options like arthrodesis, autogenous bone grafts, allografts, re-implantation of sterilized tumor bone, pseudoarthrosis, and endoprosthesis have been tried with variable success. However, due to lack of standardization and the rarity of the site, surgeons are often in a dilemma to choose the correct option. This can lead to suboptimal functional outcomes and long-term failures. In this article, we reviewed the published literature on proximal ulnar tumors and noted the pros and cons of various reconstructive procedures. We have also attempted to formulate reconstruction recommendations based on the level of resection of proximal ulna.  相似文献

Elbow arthrodesis is uncommon and is usually performed as a salvage procedure to provide a stable elbow. There is a significant gap in the literature about the indications, contraindications, fusion angle, technical tips, and reversibility of the procedure.
