CRISPR-Cas13a检测在肺结核诊断中的价值

目的:评价应用CRISPR-Cas13a检测临床样本对肺结核的诊断价值。方法:采用回顾性研究方法，收集首都医科大学附属北京胸科医院2022年9月至2022年12月收治住院的102例经GeneXpert MTB/RIF检测(简称“Xpert检测”)确诊的肺结核患者的支气管肺泡灌洗液标本，作为结核病组；收集同期收治入院的100例非结核病患者(包括肺癌，细菌性肺炎，支气管扩张，慢性阻塞性肺疾病)的支气管肺泡灌洗液标本，作为非结核病组。所有患者的支气管肺泡灌洗液标本均进行涂片抗酸杆菌染色镜检、培养、Xpert检测及CRISPR-Cas13a检测。以Xpert检测结果为参照，评价CRISPR-Cas13a检测支气管肺泡灌洗液对肺结核的诊断价值。结果:结核病组102份标本中，CRISPR-Cas13a检测阳性为99份(97.1%),涂片阳性40份(39.2%),培养阳性68份(66.7%)。CRISPR-Cas13a检测阳性率明显高于涂片和培养法，差异有统计学意义(涂片：χ²=58.141,P<0.001;培养：χ²=30.250,P<0.0...     

Research progress of Gasdermin D北大核心CSCD

Pyroptosis is a novel process of programmed inflammatory necrosis, which is closely related to microbial infections and autoimmune diseases. In recent years it has been proved that Gasdermin D is an executive protein of pyroptosis. Cleaved Gasdermin D leads to the formation of pores in cell membrane to induce pyroptosis, and the release of inflammatory factors such as IL-1β and IL-18 aggravates the occurrence of inflammatory responses. With the deepening of Gasdermin D research, it is gradually clear its protein structure and activation sites, which has other activation pathways apart from inducing inflammasome activation. Activation of Gasdermin D does not necessarily mean cell death, even if it could induce the formation of pores in cell membrane and regulation of inflammatory cytokine secretion. These findings offer insight into Gasdermin D function and change our understanding of pyroptosis and programmed necrosis. 冠脉血管介入类高值医用耗材临床使用综合评价指标体系构建

目的:在医用耗材带量采购等综合治理改革措施持续推进的背景下，构建冠脉血管介入类高值医用耗材临床使用综合评价指标体系，为及时监测和评估改革措施影响提供技术支持。方法:通过文献研究及专家访谈法初步确定评价对象及维度，运用德尔菲法经过两轮专家咨询确定评价指标维度，构建综合评价指标体系。结果:两轮专家咨询回收率均高于80%,专家权威系数≥0.75,通过两轮专家咨询，最终确定由4项一级指标、12项二级指标、50项三级指标构成的血管介入类高值医用耗材临床使用综合评估指标体系。一级指标分别为供应保障、临床使用、服务绩效、经济影响。结论:构建的冠脉血管介入类高值医用耗材临床使用综合评价指标体系科学合理，具有可操作性和实用性，对于未来持续构建国家层面高值医用耗材监测评估平台具有重要开创性价值和现实意义。     

“水虚土乘”是更年期女性代谢综合征的核心病机

目前一般将代谢综合征病机归之于“脾虚”。一方面,随着生活方式和饮食结构的改变,过食肥甘、贪逸少动等致使脾高负荷运化,脾运亢进。另一方面,气机阻滞,气化失常,日久代谢物不能及时布散,蓄积为毒,代谢紊乱,发为代谢综合征,故“脾实”是代谢综合征的核心病机。“肾虚”和“脾实”并存是更年期女性代谢综合征的病机特点,且“肾虚”是始动因素。根据五行相乘理论,水行虚弱,过亢之土行乘之,如此循环,水行愈弱,土行愈亢,终致更年期女性代谢紊乱不断加重,故提出“水虚土乘”是更年期女性代谢综合征的核心病机。
