Recruitment and retention in the unique and immensely growing field of radiology nursing is challenging as the nursing role evolves in response to advanced imaging and sedation procedures. At Children's National Medical Center, the radiology nursing team was confronted with recruiting enough nurses to respond to the rapid expansion of its radiology department. The department was preparing to open a new magnetic resonance imaging suite with three new magnetic resonance imaging scanners and additional anesthesiologist support for sedation, which would result in increased patient volumes and a need for more nurses. In 2010, the radiology nursing department chose to develop a program for new graduate nurses to acquire the skill set needed for radiology nursing while meeting the needs of the expanded services. In this article, we report the process and outcomes of structuring a 12-week comprehensive New Graduate Nurse Orientation Program that met the professional development needs of novice nurses and the specialty's need to produce competent radiology nurses. Through the curriculum and as documented in self- and preceptor evaluations, the orientation program's success was measured by 100% retention of the new graduates, positive self-assessments and continued involvement in the nursing profession. Implementing a quality orientation program for new graduate nurses resulted in a positive transition into their first year of practice. This program not only developed four new nurses in the field of radiology nursing, but it also engaged the experienced clinical nurse in supporting and mentoring the novice nurse.  相似文献

The primary aim of this article is to present a broad overview and to describe and analyze the existing literature related to the effect of radiology nurses on health and health outcomes. Combinations of search terms related to the role of the radiology nurse and patient outcomes were used to search three electronic databases. Of the studies reviewed, 15 met inclusion criteria. This review concludes that further work must be completed to obtain an appropriate understanding of the effect of radiology nursing on patient outcomes. Given the lack of literature on this topic, there is significant opportunity to continue research on nursing in radiology.  相似文献

BackgroundModerate sedation administration is an assumed role of the nurse in interventional radiology. There remains limited documentation to define this practice and its relationship to the radiology nurse. The use of moderate sedation is common for patients receiving interventional radiology procedures, and therefore, defining the administrative parameters for this variable will allow for the future study of outcomes for this patient population. The primary aim of this article is to describe moderate sedation parameters in interventional radiology in hospitals offering adult interventional radiology services. Parameters included total case volumes, presence of provider type during interventional radiology cases using moderate sedation, existence of moderate sedation privileges, and the total percentage of moderate sedation administration by provider type. The results of this study are part of a larger nationwide survey that described administratively mediated variables in interventional radiology.MethodsA 26-question survey was developed and sent to a sample of radiologic administrators (n = 510) across the United States. The survey was developed using the Minnick and Roberts Outcomes Production Model, which identifies the relationship among essential health services concepts and their influence on patient outcomes. Recipients could answer via paper survey or by electronic link. Completion of the survey constituted consent.ResultsA total of 82 individuals responded to the survey (response rate = 16.8%). Respondents reported a median value of 2656.5 (n = 74, IQR: 1175.0, 5833.0) interventional radiology cases completed in the last year and a median value of 1500 patients receiving moderate sedation (n = 82, IQR: 600.0, 2725.0). Respondents reported that the registered nurse was responsible for a median of 90.0% of the sedation administered for adult patients (IQR: 80.0, 98.0).ConclusionsBased on the results of the survey, it is determined that moderate sedation is primarily administered by the registered nurse in interventional radiology. Future studies can include relational designs that focus on aspects of the registered nurse as a highly influential factor in moderate sedation patient outcomes.  相似文献

This article presents a broad overview of the literature available since 2004 describing the study of patient satisfaction in radiology. The primary aim for this literature review was to analyze and update the literature related to patient satisfaction from a prior systematic review. The secondary aim was to examine how nursing has been studied within radiology with regard to patient satisfaction research. Combinations of search terms related to radiology and patient satisfaction were used to search three electronic databases. Of the studies reviewed, 14 references met inclusion criteria. This review concludes that there are challenges with the present study of patient satisfaction within radiology. Lack of consistent definitions, no conceptual frameworks, and limited studies involving nursing highlight the significant opportunity to continue research in this area.  相似文献

This article aims to report the role of the quality improvement radiology nurse. The quality improvement nurse must possess the knowledge, skills, and attitudes to deliver safe and quality care. There is a lack of guidance that describes the role of the quality improvement radiology nurse. The Quality Education for Nurses Framework and Kotter's model both provide a foundation for describing the role of the quality improvement radiology nurse. More research is needed to analyze the role of the quality improvement radiology nurse. Implications to nursing practice, education, and research are discussed.  相似文献

This review article is designed as an introductory reference to the clinical management of trauma patients by interventional radiologists. The article is geared toward the radiology nurse participating in the care of these patients alongside the interventionalist to provide a framework to understand the process of working up, treating, and managing trauma patients using minimally invasive techniques. Special emphasis is placed on the preoperative and postprocedural care issues that are relevant specifically to nursing staff as they relate interventional radiology. Medical Imaging (MI) nurses require advanced education to increase their ability to positively influence patient care, leading to improved patient outcomes. The MI nursing field has been a neglected specialty with regard to postgraduate tertiary education programs, where the eminent skills, knowledge, and expertise of other specialist nursing fields have been recognized and sought after, such as critical care environments. Postgraduate tertiary education incorporates the application of evidence-based theory to clinical practice and can address the theory-practice gap and improve the critical thinking, confidence, and competence of MI nurses. Postgraduate tertiary education can contribute to improved patient outcomes and reduced mortality. A postgraduate tertiary MI nursing program was developed to support the clinical practice and critical thinking of nurses in radiology departments.  相似文献

Aim This paper outlines the current state of Australian practice nursing, describes the context of general practice and establishes the importance of promoting leadership and management in this setting. Background Australian general practice nurses have emerged as key stakeholders in primary health care. However, their role in leadership and management has been largely invisible. The reasons for this are multifactorial, including the delay to establish a strong professional organization, their negative power relationships with general medical practitioners, limited nursing leadership and poorly defined roles. To date, the impetus for practice nurse growth has been largely external to the nursing profession. Growth has been driven by the increasing burden of chronic disease and workforce shortages. This has further weakened the control of nurse leaders over the development of the specialty. Conclusions The Australian practice nurse role is at a crossroads. While the practice nurse role is a viable force to improve health outcomes, the growing strength of the practice nurse challenges traditional professional roles and practice patterns. Implications for nursing management There is an urgent need to develop practice nurse leaders and managers to not only embrace the challenges of Australian general practice from an operational perspective, but also undertake a clinical leadership role. As clinical leaders, these nurses will need to develop a culture that not only optimizes health outcomes but also advances the status of the nursing profession.  相似文献

Trauma is a leading and preventable cause of death in many age groups. Emergency care of trauma patients is complex and requires a team approach, which includes nurses and physicians. Interventional radiology (IR) plays a significant part in the management of some trauma patients with solid organ or pelvic injuries. Advances in radiology and imaging technologies and the emergent scope of practice have led to the capacity to provide services to a growing population of high-acuity patients with comorbid conditions. These procedures are often performed with the radiology nurse administering procedural sedation. Monitoring patients is challenging due to certain patient conditions and the unique environment, that is, the radiology procedure suite. The addition of capnography monitoring, along with standard monitoring, is a valuable modality that provides a continuous objective assessment of the patient's ventilatory status even when direct visualization of the patient is compromised. The purpose of this article is to provide clinical practice recommendations for the use of capnography by procedural sedation nurses outside the operating room setting.  相似文献

Simulation offers an opportunity for realistic assessment of nurse competency. Designing a simulation competency testing process for a specialty setting such as radiology requires careful thought regarding purposes of assessment and fair uses of assessment for further learning or personnel decisions. Aims and objectives. This paper aims to explore the critical elements of advanced nursing practice in relation to policy, education and role development in order to highlight an optimal structure for clinical practice. Background. The evolution of advanced nursing practice has been influenced by changes in healthcare delivery, financial constraints and consumer demand. However, there has been wide divergence and variations in the emergence of the advanced nurse practitioner role. For the successful development and implementation of the role, policy, educational and regulatory standards are required. Conclusion. The paper highlights the value of a policy to guide the development of advanced nursing practice. Educational curricula need to be flexible and visionary to prepare the advanced nurse practitioner for practice. The core concepts for the advanced nursing practice role are: autonomy in clinical practice, pioneering professional and clinical leadership, expert practitioner and researcher. To achieve these core concepts the advanced nurse practitioner must develop advanced theoretical and clinical skills, meet the needs of the client, family and the community. Relevance to clinical practice. In a rapidly changing people‐centred healthcare environment the advanced nurse practitioner can make an important contribution to healthcare delivery. The challenges ahead are many, as the advanced nurse practitioner requires policy and appropriate educational preparation to practice at advanced level. Historically, mental health services have faced challenges in their ability to attract and retain a competent nursing workforce in the context of an overall nursing shortage. The current economic downturn has provided some respite; however, this is likely to be a temporary reprieve, with significant nursing shortages predicted for the future. Mental health services need to develop strategies to become more competitive if they are to attract and retain skilled nurses and avoid future shortages. Research demonstrates that creating and maintaining a positive nursing practice environment is one such strategy and an important area to consider when addressing nurse retention. This paper examines the impact the nursing practice environment has on nurse retention within the general and mental health settings. Findings indicate, that while there is a wealth of evidence to support the importance of a positive practice environment on nurse retention in the broader health system, there is little evidence specific to mental health. Further research of the mental health practice environment is required.  相似文献

The pivotal role of nurse managers, leaders and educators in enabling excellence in nursing care Aim The aims of this paper are to present the findings from a discursive analysis of key issues associated with providing excellence in nursing care; and to provide an exemplar framework to support excellence in nursing care and describe the potential benefits when excellence in nursing care occurs. Background The challenge facing the nursing profession is in ensuring that the core principles of dignity, respect, compassion and person (people) centered care become central to all aspects of nursing practice. To regain the public and professional confidence in nursing, nurse leaders, managers and educators play a pivotal role in improving the image of nursing. Key issues Excellence in nursing care will only happen by ensuring that nurse managers, leaders and educators are able to respond to the complexity of reform and change by leading, managing, enabling, empowering, encouraging and resourcing staff to be innovative and entrepreneurial in practice. Conclusions Creating healthcare environments that enable excellence in nursing care will not occur without the development of genuine shared working partnerships and collaborations between nurse managers, leaders and educators and their associated organizations. Implications for nursing management The importance of adopting an authentic sustainable leadership approach to facilitating and supporting frontline staff to innovate and change is imperative in restoring and evidencing that nurses do care and are excellent at what they do. By focusing attention on what resources are required to create a healthcare environment that enables compassion, safety and excellence in nursing care and what this means would be a reasonable start on the journey to excellence in nursing.  相似文献

目的通过规范护理服务,提高护理质量。方法成立了总护士长-护土长-组长三级管理体系,制订了规范服务的相关文件,编写了临床常见的情景练习,对全体护士进行了培训及效果评价。结果护理服务示范基地实施后,患者对开展健康教育、护理服务、操作技术的满意度比实施前显著提高(P〈0.01或P〈0.05).结论规范护理服务可以提高患者满意度。  相似文献

The role of the nurse practitioner (NP) represents a unique blend of nursing and medical functions. Few studies exist that address the nursing component of the NP role and no literature is available to document the extent to which nurse practitioners utilize nursing diagnoses in their clinical practice. Thus, the author reports findings of a study that sought to describe the use of nursing diagnosis in current NP practice and to identify advantages and barriers to the use of nursing diagnoses by NPs. Fifteen percent of nurses surveyed used nursing diagnoses in their practice. The diagnoses most commonly used reflected attention to clients' lifestyles and daily living problems.  相似文献
