[目的]了解成都市基层医疗卫生机构公共卫生人员配置现状,分析存在的问题,为基层医疗卫生机构公共卫生人员队伍建设提供决策参考和政策建议,为促进基本公共卫生均等化提供人才保障。[方法]问卷调查法、访谈法。[结果]成都市基层医疗卫生机构卫生技术人员学历低、职称低,部分卫生技术人员无执业医师资格;公共卫生人员配置不足,每万人口公共卫生执业(助理)医师数平均为0.25人,公共卫生医师需求量与实际之间缺口较大,基本公共卫生服务团队的专业素质有待提高。[结论]应进一步加强基层医疗卫生机构公共卫生人员队伍建设,提高公共卫生人员素质,促进基层医疗卫生机构公共卫生服务的可持续发展。  相似文献

Biological weapons have the potential to inflict deliberate, potentially devastating epidemics of infectious disease on populations. The science and technology exist to create deliberate outbreaks of human disease, as well as disease among plants and animals, crops, and livestock. A new awareness among policymakers of the link between public health and national security requires the attention of public health professionals. The issues posed by biological weapons are likely to challenge the political assumptions of many progressive public health professionals and will demand new coalitions. In the new millennium the world has become a ?global village?. Urbanization, economical imbalance, changing social structures and globalization resulting in ever-increasing interpenetration and interdependence between local, national and transnational events constitute the new operational field of public health. This article discusses the challenges and constraints faced by developing countries in its efforts to improve equity in health care. It is noted that the challenges include globalization; emergence of diseases and epidemics; privatization of health care; improving the quality of care; developing Healthy Public Policy; effective decentralization; and a focus on the poor, vulnerable and marginalized groups. In terms of the constraints, most notable is the disagreement between providers of individual care and practitioners of population-based medicine, and the noncontribution of public health research to public policy. In view of such, several recommendations are cited in order to overcome these challenges and constraints and in turn, achieve the goal of improving equity in health care. In this regard, the importance of health systems development including health sector reform in shaping the system and practice of public health in the new millennium is emphasized.  相似文献

This review examines past, current, and future issues in developing and using public health performance data for improving the public health system. Issues are explored relating to public health performance data collection and analysis, and inferences made from those data, largely by examining public health performance data collected since the Year 2000. More research is needed to improve understanding of the context in which public health systems operate and how that context affects performance and its relationship to health outcomes. There are major areas of concern that must be addressed by the public health practice organizations, governmental public health entities, and the public health systems research communities, such as ensuring that data are collected on public health infrastructure, practice, and performance and that data from various sources are collected in a harmonious fashion. At issue also is the examination of the impact of new arrivals to the public health system, such as accreditation and credentialing of the public health workforce, on public health system performance.  相似文献

目的探索构建新型农村卫生服务体系的途径与方法.方法对南通市3个县(市)的所有乡镇(中心)卫生院、卫生所(防保科)和县级卫生机构负责人进行定量与定性调查相结合方法调查.结果通过乡镇卫生院产权制度改革、以政府办独立卫生所或政府向民营乡镇医院购买防保服务的形式向农村居民提供公共卫生服务、以多元化的投资建立村级社区卫生服务站等形式,重新构建了南通市农村卫生服务体系.结论农村卫生服务体系中,乡镇医院的经营状况在好转,农村社区卫生服务体系的建立与完善尚依赖于政府对民营乡镇医院采取有效的管理与协调措施,独立卫生所在公共卫生服务、农村卫生管理方面的工作优于政府购买公共卫生服务的形式.  相似文献

The issues raised in this editorial and exemplified within a number of the studies reported in this issue indicate new directions for public health, directions which take feminist scholarship, both outside and within the medical framework, into account. The changing potential of feminist public health, as derived from the articles in this issue, can be summarised within the following issues: new research areas, positioning women as actors, development of theoretical frameworks, reflexive theory of science, interplay between sex and gender, gender-sensitive methods, diversities among women/men, pro-feminist research on men's health and using the results for change. Thus, feminist public health represents a shift towards the new public health, with holistic and multidisciplinary activities, based on theoretical pluralism, multiple perspectives and collective actions with the aim of improving the health of gender-subordinated groups.  相似文献

The rapid development of public health and medicine started in the second half of the 19th century leading to a massive decline or disappearance of previously deadly communicable diseases. However, almost parallel to this process, chronic non-communicable diseases appeared and have become widespread in the populations of developed countries presenting a new challenge for public health. 'New public health' and health promotion emerged representing new ways of thinking and approaches as a societal answer to population health problems. The Ottawa Charter, the decisive document of health promotion laid the ground for health promoting activities, naming healthy public policy, supporting environments, community actions, personal skills and and reoriented health services as major fields of action towards improving population health. The article describes examples for these actions and how they are related to health, and provides ideas as to how medical practitioners can contribute to the global common goal of health promotion and medicine.  相似文献

As the nation's health system moves away from earlier models to one grounded in population health and market-based systems of care, new challenges arise for public health professionals, primary care practitioners, health plan and institutional managers, and community leaders. Among the challenges are the need to develop creative concepts of organization and accountability and to assure that dynamic, system-oriented structures support the new kind of leadership that is required. Developing tomorrow's integrated community health systems will challenge the leadership skills and integrative abilities of public health professionals, primary care practitioners, and managers.
