PURPOSE: To evaluate the effectiveness, technical feasibility and incidence of complications after combining pars plana vitrectomy, phacoemulsification and intraocular lens implantation. MATERIAL AND METHODS: The results of combined vitreoretinal and cataract surgery in 100 eyes of 96 patients were retrospectively and prospectively analyzed. The mean follow-up period was 8.4 months. All patients had clinically significant lens opacities and vitreoretinal pathology requiring pars plana vitrectomy. Indications for vitreoretinal surgery included: persistent vitreous haemorrhage (28 eyes), vitreous hemorrhage combined with tractional retinal detachment (50 eyes), tractional retinal detachment without vitreous haemorrhage caused by proliferative diabetic retinopathy (7 eyes), rheumatogenous retinal detachment with proliferative vitreoretinopathy (10 eyes) and dislocated crystalline lens in the vitreous (5 eyes). RESULTS: Postoperatively, best corrected visual acuity improved in 81 eyes (81%)- by two lines or more in 31 eyes (31%) - by less than two lines in 50 eyes (50%). In 14 eyes (14%) visual acuity was unchanged and was worse in 5 cases (5%). Postoperative complications included fibrin reaction, posterior synechias of the iris, vitreous hemorrhage, neovascular glaucoma, posterior capsule opacification, redetachment of retina. CONCLUSIONS: Our cases confirm previous study, that performing phacoemulsification, IOL implantation and vitrectomy in one operation is safe and allows visual recovery with good technical results.  相似文献

Purpose : To assess the outcome and complications after combined pars plana vitrectomy and phacoemulsification with intraocular lens implantation in patients with concomitant cataract and vitreoretinal abnormalities. Methods : This retrospective study consisted of 113 eyes of 109 patients with various vitreoretinal abnormalities and visually significant cataract. Vitreoretinal surgery was combined with clear corneal phacoemulsification and intra‐ocular implantation. Results : The combined surgery resulted in minimal complications, and postoperative visual acuity was improved in 83 eyes (71.6%). Visual acuity was improved by two or more lines in 26 eyes (66.6%) with proliferative diabetic retinopathy, 22 eyes (73%) with proliferative vitreoretinopathy, eight eyes (30.7%) with macular abnormalities, four eyes (55.5%) with trauma and two eyes (40%) with retinal vein occlusion. Conclusions : The combined vitreoretinal surgery and phacoemulsification with intraocular lens implantation was safe and effective in treating vitreoretinal abnormalities coexisting with cataract. Combined surgery is recommended for all patients having simultaneous vitreoretinal pathological changes and cataract. The addition of phacoemulsification does not prolong vitreoretinal operative time notably nor increase the risk of intraoperative and post‐operative complications significantly.  相似文献

PURPOSE: To determine the rate of secondary opacification of the visual pathway following pediatric cataract surgery in children between the ages of 10 months and 7 years. METHODS: The medical records of children less than 7 years old who underwent lens aspiration, posterior chamber intraocular lens (IOL) implantation, primary pars plana posterior capsulectomy, and anterior vitrectomy were reviewed retrospectively. Twenty-six eyes in 19 children were included in the study. All procedures were performed by an anterior segment surgeon and a vitreoretinal surgeon. Main outcome measures were the prevalence of re-opacification of the visual pathway and of early postoperative complications. RESULTS: The visual pathway remained clear in 25 of 26 eyes (96%) after pediatric cataract surgery combined with primary pars plana posterior capsulectomy and anterior vitrectomy. The mean age at surgery was 46 +/- 23 months (+/-SD). Secondary opacification of the visual pathway occurred in one eye (4%), requiring another surgical procedure. At last follow-up (mean, 26 months; range, 6 to 79 months), the visual pathway was clear in all 26 eyes (100%). No cases of clinically significant IOL displacement or of retinal detachment were noted. CONCLUSIONS: For children undergoing pediatric cataract surgery between the ages of 10 months and 7 years, IOL implantation combined with primary pars plana posterior capsulectomy and anterior vitrectomy is effective in preventing re-opacification of the visual pathway.  相似文献

Purpose: This study aimed to evaluate the longterm results of phacovitrectomy and foldable intraocular lens (IOL) implantation in eyes with significant cataract and co‐existing vitreoretinal diseases. Methods: We carried out a retrospective study of 186 eyes of 149 patients with various vitreoretinal abnormalities and visually significant cataracts. Vitreoretinal surgery was combined with phacoemulsification and foldable IOL implantation. Main outcome measures were visual acuity (VA), preoperative data, and intraoperative and postoperative complications. Results: The most common indications for surgery were non‐diabetic vitreous haemorrhage and proliferative diabetic retinopathy. Preoperative vision ranged from 0.6 to light perception; postoperative vision ranged from 1.2 to no light perception. Postoperatively, in 162 eyes (87.1%) VA improved by ≥ 3 lines on the decimal chart. In 14 eyes (7.5%), vision remained within 3 lines of preoperative levels and in 10 eyes (5.3%), vision had decreased by the last follow‐up. Postoperative complications included elevated intraocular pressure and posterior capsule opacification, corneal edema, macular edema, fibrinous reaction, vitreous hemorrhage, corneal epithelial defects, anterior chamber hyphema, choroidal detachment, persistent macular hole, posterior synechiae, recurrent retinal detachment, rubeosis iridis, neovascular glaucoma. Conclusions: Combined vitreoretinal surgery and phacoemulsification with foldable IOL implantation is safe and effective in treating vitreoretinal abnormalities co‐existing with cataract. Based on extensive experience with the combined procedure, we suggest that combined surgery is recommended in selected patients with simultaneous vitreoretinal pathological changes and cataract.  相似文献

PURPOSE: To determine the short-term outcomes in eyes with posterior capsule rupture (PCR) during cataract surgery. SETTING: The Eye Institute, National Healthcare Group, Tan Tock Seng Hospital, Singapore, Singapore. METHODS: All intraoperative complications during cataract surgery were reported prospectively as part of a clinical audit program. The clinical charts of all patients who had PCR during cataract surgery from July 1995 to December 1998 were retrospectively reviewed. RESULTS: Posterior capsule rupture occurred in 155 (1.9%) of 8230 consecutive eyes that had extracapsular cataract extraction (ECCE) or phacoemulsification. Thirteen cases were excluded from analysis for missing data or insufficient follow-up. Of the remaining 142 eyes, 100 (70.4%) achieved a best corrected visual acuity (BCVA) of 6/12 or better between 6 weeks and 3 months postoperatively. Excluding 27 eyes with preexisting ocular pathology contributing to impaired vision, 87.0% achieved a BCVA of 6/12 or better. Fifteen eyes (13.0%) failed to achieve that acuity because of cystoid macular edema (CME) (2 eyes), posterior capsule opacification (2 eyes), endophthalmitis (1 eye), glaucoma (1 eye), retinal detachment (1 eye), CME, central retinal artery occlusion, and vitreous hemorrhage (1 eye) or unknown reasons (7 eyes). Risk factors for a poor visual outcome included older age, presence of coexisting ocular pathology, ECCE, implantation of an anterior chamber intraocular lens, and vitreous loss requiring anterior vitrectomy. CONCLUSIONS: Most eyes with PCR achieved a good BCVA in the early postoperative period.  相似文献

目的评价晶状体超声乳化、囊袋内人工晶状体植入联合玻璃体切除术治疗增生型糖尿病视网膜病变(PDR)的临床效果。方法回顾性分析合并不同程度白内障的增生型糖尿病视网膜病变6l例(66眼)。其中PDRlV期20眼,V期33眼,Ⅵ期13眼。实施晶状体超声乳化、囊袋内人工晶状体植人联合玻璃体切除术。观察术后视力改善程度和术中术后并发症。结果术后视力改善:PDR1V期19眼(95.0%),V期28眼(84.8%),Ⅵ期13眼(38.5%)。术后视力低下者多伴有明显的糖尿病黄斑病变。术中术后主要并发症包括:医源性裂孔7眼(10.6%);玻璃体积血,术中17眼(25.8%),术后7眼(10.6%);角膜上皮延期愈合9眼(13.6%);角膜水肿8眼(12.1%)。结论超声乳化、人工品状体植入联合玻璃体切除术治疗合并不同程度白内障的增生型糖尿病视网膜病变是安全有效的,可使大多数患者视力改善,避免玻切术后并发白内障再次手术。糖尿病黄斑病变是影响术后视力提高的主要原因。  相似文献

AIM: To describe the results of a combined procedure including phacoemulsification, insertion of posterior chamber intraocular lens (PCIOL) and pars plana vitrectomy (PPV) in eyes with vitreoretinal pathology and coexisting significant cataract. DESIGN: Retrospective, consecutive, noncomparative, interventional case series. MATERIALS AND METHODS: Medical records of patients who had undergone phacoemulsification, PPV and PCIOL implantation as a combined procedure between January 2000 and December 2004 were retrospectively reviewed. The main outcome measures were the anatomical success of retina, defined as reattached retina, intraoperative and postoperative complications and functional success in terms of final best corrected visual acuity. RESULTS: In all, 65 eyes of 64 patients were included. The mean age of the patients was 50.9 years +/- 17.1 (range, five to 82 years). Vitreous hemorrhage with or without retinal detachment (19 eyes, 29.2%) was the most common indication for the vitreoretinal procedure. Primary anatomical success of retina was achieved in 59 eyes (90.7%). Visual acuity improved in 48 eyes (73.8%), was unchanged in 12 eyes (18.5%) and deteriorated in five eyes (7.7%). Postoperative inflammation was significantly more in the subgroup of previously vitrectomized eyes (42%) (P =0.014, Fisher exact test) compared to those which underwent primary vitrectomy. CONCLUSIONS: Combined surgery is a feasible option for patients with vitreoretinal diseases and cataract.  相似文献

PURPOSE: To evaluate the safety and effectiveness of phacoemulsification with clear corneal incision in previously vitrectomized patients as well as factors affecting the development time and type of cataract occurring after pars plana vitrectomy (PPV). METHODS: The authors conducted a prospective study of 100 consecutive eyes of patients who developed a cataract after PPV. Three groups were established based on the underlying vitreoretinal pathology. The main outcome measurements were intraoperative and postoperative complications and changes in best-corrected visual acuity (BCVA). RESULTS: The median interval between PPV and phacoemulsification was 11.5 months. Patients with proliferative diabetic retinopathy required phacoemulsification earlier (p=0.018). Posterior subcapsular cataracts developed more frequently in patients <50 years (73.7%, p=0.000) and affected those who underwent vitrectomy primarily for complicated retinal detachment (48.8%, p=0.046). Intraoperative complications included posterior capsular tears (4%), luxated nucleus into vitreous (2%), and zonular dialysis (5%). Postoperative complications were vitreous hemorrhage (6%), retinal redetachment (4%), pupillary synechiae (6%), ocular hypertension (4%), and Seidel phenomenon (3%). Posterior Nd:YAG laser capsulotomy was required in 44% of eyes. BCVA was improved in 85% of cases at the end of follow-up (median, 15.5 months). Twenty-one patients with one functioning eye (61.9%) demonstrated visual improvement compared with 79 patients with bilateral vision (91.1%; p=0.003). CONCLUSIONS: The technique allows stable improvement in BCVA through long follow-ups. It is more risky than in nonvitrectomized eyes. The visual results after phacoemulsification in vitrectomized eyes seem to be limited by retinal comorbidity and surgical complications.  相似文献

PURPOSE: To evaluate the technique of silicone oil removal through a posterior capsulorhexis combined with phacoemulsification and intraocular lens (IOL) implantation. SETTING: Dr. Rajendra Prasad Center for Ophthalmic Sciences, New Delhi, India. METHODS: Fifteen eyes of 15 patients had phacoemulsification with removal of silicone oil, which had been used for intraocular tamponade after a previous pars plana vitrectomy. Eyes with a stable retina were included in the series. In all eyes, the silicone oil was removed through a planned posterior capsulorhexis after phacoemulsification. The parameters evaluated were the primary diagnosis, duration between silicone oil instillation and phacoemulsification, type of cataract, preoperative and postoperative best corrected visual acuities (BCVAs), and complications such as frequency of retinal redetachment and secondary cataract. RESULTS: Vitreoretinal surgery with silicone oil instillation was performed for rhegmatogenous-tractional detachment resulting from Eales' disease in 6 eyes and from proliferative diabetic retinopathy in 2 eyes, for primary rhegmatogenous retinal detachment in 6 eyes, and for traumatic rhegmatogenous detachment in 1 eye. The mean duration between the silicone oil instillation and phacoemulsification was 7.5 months +/- 3.8 (SD). Fourteen eyes had posterior subcapsular cataract, and 10 had nuclear sclerosis. Preoperative BCVA was worse than 6/60 in all eyes. The BCVA was 6/60 or better in 9 eyes after a minimum follow-up of 6 months. Two eyes had choroidal detachment in the early postoperative period. No eye had vitreous hemorrhage, retinal redetachment, secondary cataract, clinically significant endothelial decompensation or macular edema, or a dislocated IOL. CONCLUSION: The results indicate that silicone oil removal through a posterior capsulorhexis during phacoemulsification is a viable option and can be performed in selected cases of cataract with previous silicone oil instillation and a stable retina.  相似文献

BACKGROUND AND OBJECTIVES: To evaluate the outcomes of vitreoretinal surgery for the ocular complications of Behcet's disease. PATIENTS AND METHODS: This retrospective study included 20 eyes of 15 patients with Beh?et's disease (9 men, 6 women; mean age at surgery, 31.0 +/- 1.8 years; range, 21-44 years) who had undergone vitreoretinal surgery for Behcet's disease at Ondokuz Mayis University, Faculty of Medicine, Department of Ophthalmology, Samsun, Turkey, between January 2000 and September 2003. Postoperative visual acuity outcome, course of uveitis, and complications were evaluated in all patients. RESULTS: Indications for surgery were vitreous opacities in 10 eyes (50%), cystoid macular edema in 5 eyes (25%), epiretinal membrane in 3 eyes (15%), and vitreous hemorrhage in 2 eyes (10%). At the last postoperative follow-up, visual acuity had increased 2 Snellen lines or more in 10 (50%) of 20 eyes. Cystoid macular edema completely improved in 3 (60%) of 5 eyes after vitrectomy. Uveitis attacks significantly decreased during follow-up (P = .015). Immunosuppressive treatment was stopped in 11 (73%) of 15 patients. Retinal detachment and phthisis were not observed in any eye. CONCLUSION: Vitreoretinal surgery may be useful in the treatment of posterior segment complications of Behcet's disease.  相似文献

PURPOSE: To assess the outcome of simultaneous implantable contact lens (ICL) removal and cataract extraction with pseudophakic intraocular lens (IOL) implantation. SETTING: CODET Aris Vision Institute, Tijuana, Mexico. METHODS: This retrospective noncomparative interventional case series evaluated 14 eyes of 12 patients with ICL implantations who developed a cataract and simultaneously had ICL removal and cataract extraction with IOL implantation. The follow-up time was at least 6 months (range 6 to 24 months). Visual acuity (logMAR), manifest refraction, intraocular pressure, and adverse events were recorded. RESULTS: Of the 12 patients (14 eyes), 10 patients (12 eyes) had ICL surgery to correct high myopia and 2 patients (2 eyes), to correct hyperopia. The mean uncorrected visual acuity after ICL implantation (before cataract development), before cataract surgery, and after cataract surgery were 0.48 +/- 0.32, 0.83 +/- 0.34, and 0.40 +/- 0.27, respectively. The mean best corrected visual acuity (BCVA) before ICL implantation, after ICL implantation, and after cataract surgery were 0.31 +/- 0.21, 0.28 +/- 0.19, and 0.27 +/- 0.21, respectively. The mean final manifest spherical equivalent was 0.30 diopters (D) +/- 1.07 (SD) (range +2.38 to 2.0 D). Ten eyes (71.4%) were within +/-1.0 D of the calculated target. One eye had a tear in the posterior capsule with vitreous loss during cataract surgery. No other intraoperative, perioperative, or postoperative complications were observed. No loss of BCVA was recorded at the last postoperative visit. CONCLUSIONS: Lens opacities and cataract formation are a potential complication of ICL surgery. The removal of the ICL and the cataract with IOL implantation was found to be safe, with predictable refractive results.  相似文献

PURPOSE: To evaluate the rate of complications and visual outcome after vitreous loss due to an unintended rupture of the posterior lens capsule during routine cataract surgery. PATIENTS AND METHODS: The study included 32 consecutive patients who underwent planned cataract surgery by extra-capsular extraction or phacoemulsification during which the posterior lens capsule ruptured and vitreous prolaps occurred. In all patients, the same surgeon was called to continue with the operation. After substantial transpapillary vitrectomy, a posterior chamber lens was implanted in all patients. The pseudophakos was transsclerally fixed in 12 patients. Mean follow-up time was 20.9 months (range, 1.5 to 71.6 months). RESULTS: The list of complications included cystoid macula edema in 2 patients (2/32 or 6.2%), persisting corneal endothelial decompensation in 1 patient (1/32 or 3.1%), and dislocation of the pseudophakos in one patient (1/32 or 3.1%). Rhegmatogenous retinal detachment did not occur. Compared with the preoperative status, visual acuity increased in 26 patients (26/32 or 81%). Visual acuity of equal or better than 20/30 was achieved in 15 eyes (15/32 or 47%). In 5 eyes (5/32 or 16%), visual acuity was equal to or better than 20/25. CONCLUSIONS: In eyes with a ruptured posterior lens capsule during routine cataract surgery eventually requiring vitrectomy, frequencies of complications are relatively low, if all vitreous adherent to the edges of the lens capsule rupture is removed by substantial transpapillary vitrectomy, and if in doubt of a sufficient lens capsule support for the posterior chamber lens, the pseudophakos is fixed by transscleral sutures.  相似文献

目的评价成人后极性白内障的临床特点、手术技巧和疗效。方法回顾性分析连续病例20例30眼,总结术前术中后囊情况、手术技巧、术中并发症以及术后视力恢复情况。结果术中后囊完整透明14眼(占46.7%),病理性后囊16眼(占53.3%),其中术前后囊破裂2眼(占6.7%),中央后囊缺损1眼(占3.3%),点状混浊但保持完整8眼(占全部病例的26.7%),后极偏鼻下混浊伴原始玻璃体动脉残留但保持后囊完整1眼(占3.3%),术中后囊破裂4眼(占13.3%)。2眼在轻柔水分层时出现破裂,2眼在试图清除后囊点状混浊时破裂。26眼囊袋内植入人工晶状体,4眼睫状沟植入。术后矫正视力:0.1~0.3者7例7眼(占23.3%),0.4~0.8者2例3眼(占10%),1.0~1.2者11例20眼(占66.7%)。结论后极性白内障很可能具有后囊先天异常、后囊破裂或缺损,可能术前已经存在或术中发生,充分的术前散瞳检查、轻柔的水分层、不作后囊抛光是避免术中并发症和提高手术预后的关键。大部分患者视力均明显提高,单眼患者视力恢复较差。  相似文献

目的:研究2mm微切口白内障超声乳化联合23G微创玻璃体切割手术治疗合并白内障玻璃体视网膜疾病的临床疗效.方法:回顾性分析2013-02/2016-02在本院接受治疗的合并白内障玻璃体视网膜疾病的患者92例99眼的临床基本资料,其中男49例53眼,女43例46眼,平均年龄57.1±1.9岁;患者进行眼球麻醉后行玻璃体切割术结合白内障的超声乳化;观察患者的手术疗效和术中、术后出现的并发症情况.结果:患者联合手术均进行得比较顺利,手术中没有后囊膜发生破裂的患者出现.患者中有7眼使用BSS液进行填充,有46眼注射入硅油,有46眼注射入C3F8;有49眼Ⅰ期时放置人工晶状体,有21眼在取出硅油时Ⅱ期放置人工晶状体;患者手术以后视力得到提升的有84眼(85%),视力没有发生变化的有15眼(15%);患者手术后出现的并发症包含一过性高眼压18眼(18%)、前房反应7眼(7%)和角膜水肿8眼(8%).结论:2mm微切口白内障超声乳化联合23G微创玻璃体切割手术对患者的创伤小、并发症少,是一种安全有效的手术方式.  相似文献

BACKGROUND: Simultaneous eye muscle and lens surgery in patients with strabismus and lens abnormalities offers the advantage of avoiding staged surgery. METHODS: Thirty-three combined strabismus and lens surgeries were performed on 30 patients who ranged in age from 22 months to 91 years. Fifteen of the strabismus procedures were performed for esotropia, 12 for exotropia, 4 for vertical deviations, and 2 for combined vertical-horizontal deviations. Surgical amounts often were reduced to lessen the risk of overcorrection, to minimize anesthetic requirements (when using topical rather than general anesthesia), or to avoiding additional surgery on the contralateral or ipsilateral eye. The intraocular surgeries included cataract extraction without or with posterior chamber intraocular lens, secondary intraocular lens implantation, and YAG laser posterior capsulotomy. In 28 cases, muscle and intraocular surgery was performed on the same eye, and in 5 cases the strabismus surgery was performed on the eye opposite the intraocular surgery. RESULTS: The average length of postoperative follow-up was 23.2 months (range, 1-94 months). Surgical, anesthetic, and postoperative complications, other than unsatisfactory ocular alignment, were limited to one retinal detachment in a patient with persistent fetal vasculature. Strabismic undercorrections (>12(Delta) of horizontal deviation or >5(Delta) of vertical deviation) occurred in 11 cases (37%). There were no overcorrections. A poor visual response (<20/50) to the intraocular surgery was encountered in 6 patients, all as the result of amblyopia or preexisting vitreoretinal pathology. CONCLUSIONS: Simultaneous extraocular muscle and lens surgery is an option for patients with strabismus and lens abnormalities. Standard strabismus surgical amounts are recommended.  相似文献

PURPOSE: To report the preferred technique and the outcomes of surgery for posterior polar cataract. SETTING: Hayashi Eye Hospital, Fukuoka, Japan. METHODS: Medical records of 28 eyes of 20 consecutive patients with posterior polar cataract who had cataract surgery were reviewed. The surgical techniques used, intraoperative complications, preoperative and postoperative visual acuities, and the causes of impaired visual acuity after surgery were examined. RESULTS: Of the 28 eyes, 25 (89.3%) with a small to medium posterior polar opacity had standard phacoemulsification or aspiration surgery. Two eyes (7.1%) with a large opacity and soft lens nucleus had pars plana lensectomy, and 1 eye (3.6%) with a large opacity and hard nucleus had intracapsular cataract extraction. Among the eyes having phacoemulsification or aspiration surgery, posterior capsule rupture occurred in 2 (7.1%). The mean visual acuity improved significantly after surgery (P <.0001); however, the postoperative visual acuity was worse than 20/20 in 7 eyes (25.0%). The cause of the low acuity was amblyopia in 4 eyes (14.3%), impaired foveal function after retinal detachment in 2 eyes (7.1%), and macular degeneration in 1 eye (3.6%). CONCLUSION: Posterior polar cataracts can be safely extracted if the appropriate surgical technique is selected. Visual acuity improved significantly in most cases, although some patients with a unilateral cataract had previously developed amblyopia.  相似文献

PURPOSE: To evaluate the results and complications of combined pars plana vitrectomy (PPV), phacoemulsification and aspiration (PEA), and intraocular lens (IOL) implantation. METHODS: A total of 117 eyes from 114 patients who had undergone PPV combined with PEA and IOL implantation were retrospectively analyzed. Combined surgery was performed for a wide variety of vitreoretinal diseases. Intraoperative and postoperative complications were also reviewed. RESULTS: The postoperative BCVA improved by 2 lines or more in 85 eyes (72.6%). Intraoperative complications consisted of retinal tears in 14 eyes (12.0%) and posterior capsular rupture in 2 eyes (1.7%). Iatrogenic retinal tears occurred more frequently in eyes with a macular hole than in eyes with any other disease (p=0.005, chi-square test). Postoperative complications consisted of posterior capsule opacification (PCO) (21 eyes), transient IOP elevation (29 eyes), vitreous hemorrhage (6 eyes), anterior chamber fibrin exudation (11 eyes), posterior iris synechia (8 eyes), neovascular glaucoma (1 eye), and recurrent retinal detachment (RD) (2 eyes). Fibrin exudation occurred more frequently in eyes with proliferative diabetic retinopathy (PDR) and RD than in eyes with any other disease (p=0.03, chi-square test). PCO occurred more frequently in eyes with PDR than in eyes with any other disease (p=0.03, chi-square test). CONCLUSIONS: The present study suggests that a high success rate can be achieved when recently improved PPV techniques are combined wi th PEA and IOL implantation. The complications that were observed following this combined treatment varied with respect to the vitreoretinal disease present prior to surgery.  相似文献

AIM: To identify the long-term complications of extracapsular cataract extraction with posterior chamber intraocular lens implantation surgery, in the National Eye Centre, Kaduna, Nigeria. METHODS: The study is a retrospective review of patients that have had extracapsular cataract extraction with posterior chamber intraocular lens implantation, in the National Eye Centre, Kaduna, Nigeria from January 1996 to December 1999. The medical records of all patients that satisfy these criteria were retrieved. Information collected included age, sex, eye operated upon, intra-operative complications, pre-operative visual acuity and last follow-up, and long-term post-operative complications. RESULT: Medical records of only 66 patients (71 eyes) that maintained a minimum follow-up of at least one year were retrievable. No major intra-operative complications were recorded. Only one eye had posterior capsule rent. Ninety percent of the operated eyes had vision of 6/60 or better one year post-operative. The overall visual outcome (WHO criteria) was borderline. Of the corrected eyes, the consultants have a statistically significantly better outcome than residents. The following post-operative complications were recorded in 25 (35.2%) eyes: pseudophakic bullous keratopathy (11.3%); posterior capsular opacification (7.0%); persistent inflammation (7.0%); secondary glaucoma (2.8%); pupil distortion (5.6%); and ocular hypotony (1.4%). CONCLUSION: ECCE + PC-IOL microsurgery is associated with long-term complications like PBK (Pseudophakic bullous keratopathy) and PCO (Posterior capsular opacification). Their management creates new demands in our country. This study is limited by the small number of eyes eligible for review at least one year after surgery.  相似文献

PURPOSE: To analyze the features of retinal detachment (RD) occurring after cataract surgery performed by Kelman phacoemulsification (KPE) and to identify any potential correlations between KPE intraoperative complications and the features of RD (incidence apart) as well as the final visual outcome. DESIGN: Interventional consecutive case series. METHODS: Retrospective review of 114 eyes of 114 consecutive patients with RD occurring after KPE and followed for 6 months or more after surgery. RESULTS: Removal of posteriorly dislocated lens fragments during KPE by pars plana vitrectomy (PPV) was associated with a significantly shorter time interval between KPE and RD (3.89 vs. 15.7 months, P =.0044). Once RD occurred, no other statistically significant correlation between its features or the final visual outcome and KPE intraoperative complications (posterior capsular rupture, vitreous loss, posteriorly dislocated lens fragments) was detected. The overall anatomic reattachment rate was 94%. Only redetachment associated with the development of proliferative vitreoretinopathy (PVR) was significantly correlated with ultimate anatomic failure (P =.0036). A best-corrected visual acuity of 20/60 or better was achieved in 58 out of 114 eyes (51%). Three variables were independently correlated with visual results: more extensive RD (P =.0001), redetachment associated with the development of PVR (P =.0029), and failure to identify retinal breaks (P =.0114). CONCLUSIONS: Posterior capsular rupture and vitreous loss during KPE do not seem to affect the features (incidence apart) or the final visual outcome of RD occurring afterwards, except for a shorter time interval between KPE and RD in eyes that underwent PPV to remove posteriorly dislocated lens fragments.  相似文献

目的 探讨晶状体超声乳化、玻璃体视网膜手术与眼内光凝联合治疗糖尿病性视网膜病变合并白内障的临床效果。方法 对23例(24眼)糖尿病性视网膜病变合并白内障的患者行晶状体超声乳化、玻璃体切除、眼内视网膜手术、视网膜激光凝固与人工晶状体植入的联合手术治疗。结果 术后出院成功率87.5%(21/24),24眼术后随访5~38月,平均11.3月,随访成功率83.3%(20/24)。术后视力>0.05者占66.7%,随访视力>0.05者占70.8%。结论 对患糖尿病性视网膜病变合并白内障者,在条件允许的情况下采取联合手术治疗,可取得较理想的效果。  相似文献
