The authors compared postoperative anterior chamber inflammation of triple procedure; diabetic pars plana vitrectomy, lensectomy with anterior capsule left intact and posterior chamber intraocular lens implantation anterior to anterior capsule, to those of various cataract surgeries with posterior chamber intraocular lens implantation and vitrectomies in diabetic retinopathy eyes. The inflammation was evaluated in terms of the incidence of inflammatory complications (fibrin reaction and posterior synechia of iris), and by periodical measurement of flare counts for postoperative 6 months using a laser flare-cell meter. The inflammation was more intense than those after the following 3 surgeries; phacoemulsification and in the bag intraocular lens implantation after continuous curvilinear capsulorhexis, extracapsular extraction and in the bag intraocular lens implantation after can opener capsulotomy, and vitrectomy alone. The inflammation, however, was less intense compared with that of another method of triple procedure; pars plana vitrectomy, phacoemulsification and in the bag intraocular lens implantation after continuous curvilinear capsulorhexis or can opener capsulotomy, and development of posterior synechia was rarely observed.  相似文献

We reviewed the records of 2,100 consecutive eyes that had undergone extracapsular cataract extraction with intraocular lens implantation between January 1981 and December 1989. Of these eyes, 21 had inactive and four had active proliferative diabetic retinopathy at the time of cataract extraction. Twenty-one eyes with inactive proliferative diabetic retinopathy received extracapsular cataract extraction with posterior chamber intraocular lens implantation, and four eyes with active proliferative diabetic retinopathy had both extracapsular cataract extraction with posterior chamber intraocular lens implantation and pars plana vitrectomy with endophotocoagulation. The mean follow-up period was 27 months. Final visual acuity was 20/40 or better in 12 of 25 eyes (48%). Of 25 eyes, five (20%) showed progression of the retinopathy after the operation, and two (8%) developed macular edema. Extracapsular cataract extraction and posterior chamber intraocular lens implantation was well tolerated in most eyes.  相似文献

目的评价白内障囊外摘出术中后囊破裂行前段玻璃体切割联合Ⅰ期后房型人工晶状体植入的手术效果。方法对1480例白内障囊外摘出联合后房型人工晶状体植入术中49例后囊破裂,行前段玻璃体切割联合Ⅰ期后房型人工晶状体植入的手术技巧、术后并发症和术后视力等进行分析。结果手术后囊破裂率为3.3%,70%患者人工晶状体植入囊袋内,30%患者为睫状沟,89.9%患者出院时裸眼视力≥0.5。结论白内障囊外摘出术中后囊破裂,行前段玻璃体切割联合Ⅰ期后房型人工晶状体植入是安全有效的。  相似文献

Eight eyes exhibited massive proliferation of lens epithelial remnants following Nd-YAG posterior capsulotomy. All eyes had pre-existing retinal pathology. Six had undergone vitrectomy (four for proliferative diabetic retinopathy) before extracapsular cataract extraction with posterior chamber intraocular lens implantation. The other two eyes had familial exudative vitreoretinopathy or retinopathy of prematurity, respectively. Five eyes required removal of the lens proliferations via a pars plana approach. High levels of growth factors in the posterior segment associated with proliferative disorders of the retina may play a role in lens cell proliferation.  相似文献

Combined cataract removal, posterior chamber intraocular lens (PC IOL) implantation, and pars plana vitrectomy were performed in 15 diabetic patients who presented with coexisting cataract and vitreoretinal complications from proliferative diabetic retinopathy (PDR). Posterior chamber IOLs were placed anterior to the anterior lens capsule after pars plana lensectomy and vitrectomy in nine eyes, whereas extracapsular cataract extraction (ECCE) with PC IOL placement was performed before vitrectomy in six eyes. Panretinal laser endophotocoagulation was applied in 13 of the 15 eyes as an important part of the operative procedure. Because of inactive diabetic retinopathy or satisfactory preoperative panretinal photocoagulation, 2 of the 15 eyes did not receive laser endophotocoagulation. Visual acuity was improved in 12 eyes and was similar to preoperative vision in 3 eyes. After a minimum of 6 months and a maximum of 28 months of follow-up, the visual results are the following: 20/40 or better (4 eyes), 20/50 to 20/200 (5 eyes), 20/400 to 5/200 (5 eyes), and hand motions (1 eye). Although 5 of the 15 eyes required secondary vitreoretinal procedures, neovascular glaucoma and complications attributable to the IOL did not occur. A recurrent postoperative retinal detachment (RD) developed with subsequent hypotony and rubeosis iridis in 1 of the 15 eyes.  相似文献

目的:探讨小切口白内障囊外摘出和后房型人工晶状体囊袋内植入联合预置小梁切口小梁切除术治疗青光眼合并白内障的临床疗效。 方法:对21例青光眼合并白内障患者实施白内障囊外摘出和后房型人工晶状体囊袋内植入联合预置小梁切口小梁切除术,观察术后患者视力、眼压及并发症情况。 结果:所有患者术后眼压均降至正常范围内。术后视力均有不同程度的提高。术后1眼发生早期人工晶状体前膜,2眼暂时性角膜水肿。 结论:预置小梁切口和小切口白内障摘除及后房型人工晶状体植入联合小梁切除是安全、有效、经济的治疗方法,值得推广。  相似文献

Removal of a cataract during a vitrectomy can be performed to improve visualization for the surgeon or to facilitate visual rehabilitation for the patient. Because aphakic eyeglasses or contact lenses are often poor options to correct aphakia, placement of an intraocular lens during or after a vitrectomy is becoming increasingly common. A cataractous lens can be removed by pars plana lensectomy, phacoemulsification, or extracapsular cataract extraction. Options for intraocular lens placement include no intraocular lens, anterior chamber lens or sutured posterior capsular lens in the absence of capsular support, "in-the-bag" posterior capsular lens, or sulcus-fixated posterior capsular lens. This article reviews the current indications and techniques for cataract removal with or without intraocular lens placement in patients undergoing vitrectomy.  相似文献

目的探讨增生性糖尿病视网膜病变(proliferative diabetic retinopathy,PDR)合并白内障行玻璃体手术硅油填充联合超声乳化吸出术或晶状体切除术的临床疗效。方法对合并有不同程度的晶状体混浊和伴有累及后极部的牵拉性视网膜脱离或牵拉-孔源性视网膜脱离以及术中出现医源性裂孔的PDR患者共76例(86眼),在行玻璃体切除手术中,根据晶状体混浊程度,采用经睫状体平坦部切除晶状体保留前囊膜(2~3级核;46眼)作为玻璃体切除+晶状体切除术组或超声乳化手术摘出晶状体保留后囊膜(3级核以上;40眼)作为玻璃体切除+超声乳化吸出术组,一期不植入人工晶状体;完成玻璃体手术后予硅油填充。术后观察的指标包括:视力、眼压、前囊膜或后囊膜混浊情况、虹膜新生血管、虹膜粘连情况、视网膜复位情况以及二期人工晶状体植入情况。结果 2组患者的术后最佳矫正视力为0.05的为32眼、25眼,分别占69.6%和62.5%,2组间差异无统计学意义(P>0.05);术后2组出现高眼压眼分别为7眼和8眼,组间差异无统计学意义(P>0.05);2组间在各级囊膜混浊程度上比较,差异均无统计学意义(均为P>0.05);2组中虹膜不同程度粘连于囊膜分别为5眼和6眼,差异无统计学意义(P>0.05);超声乳化术组中发生虹膜新生血管2眼,在晶状体切除术组中为5眼,差异无统计学意义(P>0.05);视网膜复位情况以及二期人工晶状体植入情况2组比较,差异也无统计学意义(P>0.05)。结论玻璃体手术硅油填充联合超声乳化吸出术或晶状体切除术,术后酌情行二期人工晶状体植入,都是处理严重PDR合并白内障眼的较好方法。  相似文献

目的分析晶状体超声乳化联合玻璃体手术治疗合并白内障的增生性糖尿病视网膜病变(PDR)的疗效。设计回顾性临床病例系列。研究对象123只合并不同程度白内障的PDR患眼。方法对123只合并不同程度白内障的PDR患眼实施晶状体超声乳化联合玻璃体手术治疗,同时I期植入人工晶状体(IOL),观察术后视力改善程度及术中术后并发症。主要指标术后视力改善程度、术后并发症发生率。结果123眼均实施晶状体超声乳化联合玻璃体手术,并同时一期植入IOL于囊袋内。随访时间3 ̄21月(平均10个月)。99眼(81%)术后均有不同程度的视力改善。其中93眼(76%)术后视力提高2行或以上。术后无明显角膜水肿和角膜内皮失代偿发生。1例I型糖尿病患者术后6个月发生新生血管性青光眼;1眼术后发生视网膜脱离,再次手术后复位;4眼因玻璃体腔出血再次手术。术后视力无明显改善或视力提高不足2行的病例均合并不同程度的糖尿病黄斑病变。结论晶状体超声乳化联合玻璃体手术是提高合并白内障的PDR患者视力的有效手段。糖尿病黄斑病变是影响术后视力提高的主要因素。(眼科,2006,15:198-201)  相似文献

Combined operation of cataract removal with posterior chamber intraocular lens implantation and pars plana vitrectomy were performed on 25 eyes in 22 patients with cataract concurrent with diabetic retinopathy. In 21 eyes, extracapsular cataract extraction followed by intraocular lens insertion, aiming at in-the-bag fixation, was performed, and in 4 eyes pars plana lensectomy with anterior capsule left and intraocular lens insertion between the iris and anterior capsule was carried out. Mean postoperative follow-up period was 14 months, ranging from 3 to 32 months. Visual acuity on the last examination was 0.5 or better in 2 eyes (8%), 0.1 or better in 12 eyes (48%), and worse than 0.05 in 9 eyes (36%). Major postoperative complications were fibrin reaction (20 eyes, 80%), pupillary deformation (19 eyes, 76%), pupil capture by intraocular lens (3 eyes, 12%), rhegmatogenous retinal detachment (1 eye, 4%), neovascular glaucoma (2 eyes, 8%), and recurrent vitreous hemorrhage (13 eyes, 52%). Intraocular pressure was well controlled in neovascular glaucoma cases. At the last examination ocular fundus was invisible due to vitreous hemorrhage in two eyes.  相似文献

The anterior capsule was preserved intact in pars plana lensectomy in 14 cases of pars plana vitrectomy carried out for proliferative diabetic retinopathy in 9 cases, sarcoid uveitis in one case, proliferative vitreo-retinopathy in 3 cases and retinal detachment with a giant break in one case. Follow-up periods ranged from 6 to 17 months, with a mean of 11 months. Simultaneous posterior chamber intraocular lens implantation anterior to the anterior capsule was performed in 7 cases, and in one case secondary implantation was performed 4 months later. In 11 cases anterior capsules remained clear and in 9 of them lens epithelial cells had been thoroughly removed up to the equator using a Terry squeegee. Posterior synechia occurred in 4 cases in which air or gas tamponade had been done. This method allows complete anterior vitrectomy and is adaptable not only to simultaneous but also secondary posterior chamber intraocular lens implantation.  相似文献

BACKGROUND AND OBJECTIVE: To compare the results of combined pars plana vitrectomy and cataract extraction with intraocular lens implantation by phacoemulsification versus manual extracapsular cataract extraction through a sclerocorneal tunnel. PATIENTS AND METHODS: Retrospective chart review of all patients who underwent combined pars plana vitrectomy with intraocular lens implantation and phacoemulsification or extracapsular cataract extraction through a sclerocorneal tunnel between March 1992 and July 2002. RESULTS: Fifty-four eyes of 51 patients were evaluated. Mean age was 69.68 +/- 9.11 years, and mean follow-up was 9 months. Phacoemulsification was performed in 35 cases (64%) and manual extraction in 19 cases (36%). Visual acuity improved from mean logarithm of the minimal angle of resolution 1.69 +/- 0.54 before surgery to 1.22 +/- 0.76 after surgery. There was a greater improvement of visual acuity in the phacoemulsification group (0.59 +/- 0.67) than in the extracapsular cataract extraction group (0.22 +/- 0.68) (P = .06). Final visual acuity was correlated to the vitreoretinal pathology. Ultrasound biomicroscopy showed well-positioned foldable or rigid intraocular lens in the bag or in the sulcus. CONCLUSION: Combined cataract extraction by phacoemulsification or extracapsular cataract extraction through sclerocorneal tunnel and pars plana vitrectomy are safe and effective procedures.  相似文献

PURPOSE: To evaluate the technical feasibility, outcome, and incidence of complications after combined clear corneal phacoemulsification with intraocular lens (IOL) implantation and vitreoretinal surgery. SETTING: Department of Ophthalmology, Giessen, Germany. METHODS: The results of combined cataract and vitreoretinal surgery in 38 eyes (36 patients) were retrospectively analyzed. All patients had clinically significant lens opacities and vitreoretinal pathology requiring pars plana vitrectomy. Thirty-seven IOLs were implanted in the capsular bag, and 1 was sulcus fixated. RESULTS: Postoperatively, visual acuity improved in 20 eyes (52.6%), was unchanged in 16 (42.1%), and was worse in 2 (5.3%). Postoperative complications consisted of anterior chamber fibrin exudation (3 eyes), hyphema (2 eyes), vitreous hemorrhage (1 eye), posterior capsule opacification (16 eyes), neovascular glaucoma (2 eyes), proliferative vitreoretinopathy and redetachment (1 eye), and retinal redetachment after silicone oil removal (1 eye). CONCLUSION: Compared with 2 separate operations in patients with significant lens opacities and vitreoretinal pathology, combined cataract and vitreoretinal surgery provided more rapid visual rehabilitation. The visual outcome and complications depended primarily on underlying posterior segment pathology and were not related to the combined procedure technique.  相似文献

We report 2 cases of spontaneous postoperative haptic disinsertion with the Alcon AcrySof MA60BM posterior chamber intraocular lens (PC IOL). A 14-year-old girl with a history of acute retinal necrosis and pars plana vitrectomy with lensectomy for retinal detachment repair had secondary implantation of a PC IOL in the ciliary sulcus. Three months later, the superior haptic disinserted from the optic and dislocated into the anterior chamber. In the second case, a 22-year-old man had lens aspiration and PC IOL implantation in the capsular bag. One year after surgery, the PC IOL had a similar problem, with the disinserted superior haptic remaining in the capsular bag while the optic and inferior haptic dislocated into the anterior chamber. In both patients, the PC IOLs were exchanged for rigid single-piece PC IOLs. The patients have had no further problems.  相似文献

目的探讨玻璃体手术联合晶状体超声乳化治疗增生性糖尿病性视网膜病变(PDR)合并白内障的临床疗效。方法分析19例(20只限)增生性糖尿病性视网膜病变合并不同程度白内障患者行白内障超声乳化联合玻璃体手术同时进行人工晶状体囊袋内植入的临床资料,观察术后视力改善程度及术中术后并发症。结果随访2—16个月,所有术眼人工晶状体位置良好,手术后20只限中有16只限(80%)视力有不同程度的提高,其中视力提高二行以上13只限(65%);视力较术前无改善4只限(20%)。术后视力恢复不佳的原因主要与不同程度的糖尿病黄斑病变、视网膜广泛缺血有关。术后并发症包括高眼压、虹膜后粘连、玻璃体腔再出血、视网膜再脱离、晶状体后囊混浊及新生血管性青光眼等。结论玻璃体手术联合白内障超声乳化人工晶状体植入术治疗增生性糖尿病性视网膜病变合并白内障是安全和有效的,可使大多数患者视力改善,且无明显并发症。  相似文献

目的评价晶状体超声乳化、囊袋内人工晶状体植入联合玻璃体切除术治疗增生型糖尿病视网膜病变(PDR)的临床效果。方法回顾性分析合并不同程度白内障的增生型糖尿病视网膜病变6l例(66眼)。其中PDRlV期20眼,V期33眼,Ⅵ期13眼。实施晶状体超声乳化、囊袋内人工晶状体植人联合玻璃体切除术。观察术后视力改善程度和术中术后并发症。结果术后视力改善:PDR1V期19眼(95.0%),V期28眼(84.8%),Ⅵ期13眼(38.5%)。术后视力低下者多伴有明显的糖尿病黄斑病变。术中术后主要并发症包括:医源性裂孔7眼(10.6%);玻璃体积血,术中17眼(25.8%),术后7眼(10.6%);角膜上皮延期愈合9眼(13.6%);角膜水肿8眼(12.1%)。结论超声乳化、人工品状体植入联合玻璃体切除术治疗合并不同程度白内障的增生型糖尿病视网膜病变是安全有效的,可使大多数患者视力改善,避免玻切术后并发白内障再次手术。糖尿病黄斑病变是影响术后视力提高的主要原因。  相似文献

PURPOSE: We describe a technique for secondary intraocular lens implantation in the ciliary sulcus in patients with congenital cataract or with primary aphakia. MATERIAL AND METHODS: Secondary intraocular lens implantation was performed in 119 eyes of 74 children. The average age at this procedure was 6.8 (range 22 months to 10 years), whereas the average age at primary cataract surgery was 18 weeks (range 7 weeks to 19 months). The average follow-up was 5.7 years. All eyes received a PMMA IOL. The sites of IOL fixation was ciliary sulcus. All patients had cataract extraction (lensectomy) via the pars plana, leaving in situ peripheral collarette of capsular bag to enable secondary lens implantation. RESULTS: Complications included IOL dislocation in five eyes, visual axis opacification in four, pupillary capture in one eye. CONCLUSIONS: Secondary IOL implantation in the ciliary sulcus is a safe and effective method to correct aphakia in pediatric patients with adequate capsular support. This surgical procedure is associated with a low rate of complications.  相似文献

BACKGROUND: Congenital cataract surgery can be performed using a pars plicata/plana or a limbal approach, if placement of an intraocular lens at the time of cataract removal is not a consideration. Because of the high incidence of secondary cataract formation in children the operation should be combined with capsulotomy and anterior vitrectomy. METHODS: The series consisted of 30 eyes from 20 consecutive children who suffered from congenital cataract and underwent cataract surgery between May 1995 and June 2000. The inclusion criterion was congenital cataract affecting the visual axis. We performed the operations as lens aspiration with anterior and posterior capsulotomy and anterior vitrectomy via the pars plana or plicata. We used contact lenses to rehabilitate vision. All patients received intensive orthoptic and pleoptic treatment. RESULTS: This surgical technique provided in all eyes a clear visual axis. During follow-up of 3 months to 4.5 years, secondary cataract developed in five eyes. Retinal detachment, glaucoma and endophthalmitis did not occur. One patient developed contact lens intolerance and a secondary intraocular lens was placed in the ciliary sulcus. DISCUSSION: Lentectomy via a pars plana or pars plicata approach is a suitable and safe method for treating cataract in children. Our chosen method of lentectomy is an alternative to early implantation of an intraocular lens. It is possible to perform uncomplicated secondary implantation of an intraocular lens in the ciliary sulcus.  相似文献

目的:比较睫状沟缝线固定人工晶状体术和前房人工晶状体植入术治疗白内障后囊破裂的疗效。方法:回顾性分析采用睫状沟缝线固定人工晶状体术、前房人工晶状体植入术治疗白内障后囊破裂45例45眼,比较两组手术的疗效。结果:睫状沟缝线固定人工晶状体术组和前房人工晶状体植入术组均能有效治疗白内障后囊破裂。但后者术后有2例6mo后出现虹膜睫状体炎及眼压升高,其余视力均改善。前者视力均改善,未见有明显的并发症。结论:睫状沟缝线固定人工晶状体术和前房人工晶状体植入术均能有效治疗白内障后囊破裂,改善视力,但前者更接近于生理位置,术后并发症更少。  相似文献

Unilateral posterior chamber intraocular lens (IOL) implantation was performed after phacoemulsification or extracapsular cataract extraction in ten of 11 patients with phakodonesis or lens subluxation, which was bilateral in two cases. To facilitate placement of the IOL, the anterior capsule was opened with a YAG laser in nine patients. Intraoperative vitrectomy was performed in two patients, and posterior chamber IOL implantation was accomplished with fixation in the ciliary sulcus. Postoperative follow-up studies failed to demonstrate significant decentration or dislocation of the IOLs. Phakodonesis and lens subluxation are no longer considered absolute contraindications to implantation of posterior chamber IOLs.  相似文献
