心理干预联合帕罗西汀治疗高血压病伴抑郁焦虑的临床疗效观察

目的观察高血压病患者合并抑郁和焦虑（共病）给予心理干预和抗焦虑、抑郁治疗的疗效。方法应用汉密尔顿抑郁量表（HAMD）和汉密尔顿焦虑量表（HAMA）评价高血压患者抑郁、焦虑程度，HAMD〉17、HAMA〉14，定为高血压合并抑郁和焦虑。67例符合研究条件入选，随机分为治疗组35例，对照组32例。对照组给予硝苯地平缓释片20-40mg／d，口服；治疗组给予硝苯地平缓释片20mg／d，口服，同时进行心理治疗并加服盐酸帕罗西汀片10-40mg／d口服。治疗8周后用HAMD、HAMA量表评分，并记录血压变化。结果8周后，治疗组和对照组血压都控制良好，两组比较差异无统计学意义（p〉0．05）。治疗组量表评分HAMA（11．4±1．9）分、HAMD（14．2±2．1）分，显著低于对照组HAMA（19．0±1．8）分、HAMD（19．2±1．6）分（P〈0．01）。结论心理干预联合盐酸帕罗西汀治疗高血压合并抑郁和焦虑有较好疗效。

Effects of psychotherapy and Paroxetine hydrochloride in preating hypertension with anxiety and depression

Objective Observe the effects of psychotherapy and paroxetine hydroehloride in treating the hypertension patients with anxiety and depression. Methods 67 hypertension patients which HAMD scale over 17 and HAMA scale over 14,were randomly divided into two groups：the studied group （35 cases） received Nifedipine sustainedrelease tablets 20 mg/d added psychotherapy and used paroxetine hydrochloride 10-40 mg/d. The control group （32 cases） received Nifedipine sustainedrelease tablets 20-40 mg/d.The course 8 weeks. Results After 8 weeks,the blood pressure of patients in two groups significantly decreased （P〉0.05）,and the reduced scale of HAMD and HAMA.Showed significant difference between studied group and control group（P〈0.01 ）. Conclusion Psychotherapy and paroxetine Hydrochloride show good assistant for the hypetension patients with anxiety and depression.
