经阴道彩色多普勒超声及血清CA125检测在早期卵巢癌诊断中的应用

目的探讨经阴彩色多普勒超声及血清CA125检测对早期卵巢癌的诊断价值。方法对69例卵巢小肿块分别行经阴彩色多普勒超声检查及血清CA125检测,按血清CA12535 U/ml、阻力指数(RI)0.6为阳性,将检测结果与术后病理对照。结果术后病理证实早期卵巢癌35例,良性肿瘤34例,两组RI值分别为0.46±0.07和0.67±0.03,PI值分别为0.85±0.06和1.27±0.08,差异显著(P0.05)。RI检测早期卵巢癌的敏感性为74.3%,特异性为94.1%;血清CA125检测的灵敏度为71.6%,特异度为70.2%。结论彩色多普勒血流显像探查卵巢肿瘤的血流分布及动脉血流阻力指数、搏动指数联合血清CA125检测,能提高早期卵巢癌的诊断率。

Transvaginal Color Doppler Ultrasound and Detection of Serum CA125 in Diagnosis of Early Ovarian Cancer

Objective To explore the value of transvaginal color Doppler ultrasound（TVCDS） in early diagnosis of ovarian cancer.Methods 69 patients with ovarian tumor were diagnosed by TVCDS and serum CA125 detection.The positive standard was as follows： the level of serum CA125 was more than 35 U/ml,and resistance index（RI） was less than 0.6.Results 35 cases of early ovarian cancer and 34 cases of benign tumor were confirmed by postoperative pathological results.The RI and pulsatility index（PI） was 0.46±0.07 and 0.85±0.06 respectively in early ovarian cancer patients,and 0.67±0.03 and 1.27±0.08 respectively in benign tumor patients;there was significant difference between ovarian cancer patients and benign tumor patients（P〈0.05）.The sensibility and specificity of early ovarian cancer diagnosis was 74.3% and 94.1% respectively by TVCDS,and 71.4% and 70.6% by serum CA125 detection.Conclusion Hemodynamics detection by TVCDS combined with serum CA125 detection can improve the diagnostic rate of early ovarian carcinoma.
