奥氮平联合帕罗西汀治疗伴焦虑症状的抑郁症的对照研究

目的观察奥氮平联合帕罗西汀对伴有焦虑症状的抑郁症的临床疗效和安全性。方法选取同时符合《国际疾病分类(第10版)》(ICD-10)及《精神疾病诊断与统计手册(第5版)》(DSM-V)抑郁症诊断标准且伴有焦虑症状的患者80例,采用随机数字表分为研究组和对照组。研究组42例,给予帕罗西汀合并奥氮平治疗,对照组38例,给予帕罗西汀治疗,疗程均为8周。在治疗前和治疗后1、2、4、6、8周采用汉密尔顿抑郁量表(HAMD)及汉密尔顿焦虑量表(HAMA)评定临床疗效,采用副反应量表(TESS)评定不良反应。结果治疗8周末两组HAMD和HAMA评分均较治疗前低(P0.01)。研究组第1周和第2周睡眠障碍因子分、焦虑/躯体化因子分,HAMA总评分,以及第6~8周HAMD总评分低于对照组(P均0.05)。结论帕罗西汀联合奥氮平治疗伴有焦虑症状的抑郁症的效果优于单用帕罗西汀,起效快,不良反应少。

A comparative study of olanzapine combined with paroxetine in treatment of depression with anxiety symptoms

Objective To assess the clinical efficacy and safety of olanzapine combined with in treatment of depression with anx- iety symptoms. Methods 80 depressive patients with anxiety symptoms were randomly divided into research group which received olanzapine combined with treatment and control group which received paroxetine treatment for 8 weeks. Curative effects were assessed using Hamilton depression scale（HAMD） and Hamilton anxiety scale （HAMA）. Adverse reactions were assessed with treatment emer- gent symptoms scale （TESS）. Results The two groups showed significant score decreases on HAMD and HAMA, but the research group showed faster score drop on HAMD and HAMA and had higher effective rate than control group. There were no significant differ- ences on TESS score between the two groups . Conclusion Olanzapine combined with paroxetine is effective and safe and has better compliance in treatment of depression with anxiety symptoms.
