我院住院部抗菌药物应用的调查分析

目的 :了解我院住院部抗菌药物的应用情况 ,评价各科抗菌药物应用的合理性。方法 :抽查 2 0 0 2年 1 2月～ 2 0 0 3年 5月出院病历资料 ,统计住院患者抗菌药物的应用情况 ,并按抗菌药物的应用率、费用比、DDD指数、联合用药等因素进行分析、评价。结果 :患者抗菌药物的平均应用率 83 6 5 % ;用药以单独用药为主 ,Ⅰ联用药占 6 3 6 4 % ,Ⅲ联以上联合用药率 8 6 8% ;药费 /住院费 (% )和抗菌药费 /总药费 (% )分别为 2 5 95 %和 37 0 1 % ;约 70 %药物利用指数 (DUl)值 <1 ,基本合理 ;用药前的相关标本送检率仅占 1 2 75 % ,比较低 ;选用抗生素以经验用药为主 ,按药敏结果用药者仅 2 85 % ,针对性不强 ;围手术期用药时机不当 ,预防性用药时间过长。结论 :我院住院患者的抗菌药物应用率较高 ;用药前的相关标本送检率太低 ,抗菌药的选择针对性不强 ;大多数品种的利用指数 (DDl)基本合理 ;围手术期用药时机不当 ,预防性用药时间过长 ,应引起重视

Applicational Analysis of Antibacterial Drugs in Inpatient Department

OBJECTIVE: To investigate the usage of antibacterial drugs in inpatient department in our hospital and evaluate the rationality of the usage of antibacterial drugs in each department. METHODS: To check randomly discharged medical record data during Dec,2002 to May,2003, count the usage of antibacterial drugs in inpatients during staying in the hospital and make an evaluation and analysis according to applicational rate of antibacterial drugs, cost ratio,DDD index and combined drug usage. RESULTS: The average applicational rate of antibacterial drugs in inpatients was 83 65%; the usage of one drug was 63 64%; the usage of more than three drugs was 8 68%; the ratio of drug costs to hospitalization costs(%) and antibacterial drug costs to total drug costs (%) were 25 95% and 37 01%,respectively; 70% drug use index (DUI) was less than one, it was basically rational; examing rate of the samples before drug usage was only 12 75 %; the choice of antibacterial drugs mainly depended on experiences, drug usage was only 2 85% according to the result of drug sensitivity of antibacterial drugs was not strong to the bacterials. The opportunity of perioperative drug usage was inadequate, duration of prophylactic drug usage was too long. CONCLUSION: Applicational rate of antibacterial drugs in inpatients in our hospital is high; Examing rate of correlated samples before drug usage is low; the sensitivity of the chosen antibacterial drugs is not strong; DUI of most kinds of antibacterial drugs is basically rational; the opportunity of perioperative drug usage was inadequate, duration of prophylactic drug usage was too long,the attention must be paid to.
