舌癌组织中MMP-2、9的表达及意义

目的：研究MMP-2、9在人舌鳞癌组织中的表达及意义。方法：应用免疫组化法检测98例舌鳞癌和18例舌正常组织标本中MMP-2,9蛋白的表达，统计分析MMP-2,9的阳性表达与患者临床病理因素的关系。结果：①98例舌鳞癌标本中MMP-2、9的阳性表达率分别是57．1％（56／98）和61．2％（60／98）。正常舌组织中MMP-2,9则均为阴性表达，和舌癌组织相比，差异有显著性（P=0．0000）；有颈淋巴结转移者MMP-2,9的阳性率显著高于无转移者（P=0．015，0．048）;Logistlc回归分析结果表明，MMP-2、9的阳性表达均为影响患者肿瘤复发的独立危险因素（P=0．026，0．002）；MMP-9表达还与隐匿性淋巴结转移相关（P=0．044）。结论：Ⅳ塑胶原酶在舌鳞癌中有较高的阳性表达率。Ⅳ型胶原酶的蛋白表达与舌鳞癌患者的颈淋巴结转移及肿瘤复发密切相关，MMP-9表达还与隐匿性淋巴结转移相关。

Significance of MMP- 2 and 9 expression in. tongue cancer

Objectives: To study the expression of MMP-2 and 9 in tongue squamous cell carcinoma(TSCC) and its significance.Methods: MMP-2 and 9 expression levels were examined immunohistochemically in samples from 98 TSCC patients and 18 normal tongue tissues(NTT).The patterns of MMP-2 and 9 expression was compared with clinicopathological characteristics,lymph node metastasis and tumor recurrence.Results: Positive immunostaining for MMP-2 and 9 was observed in 57.1% and 61.2% of the TSCC samples respectively.Both MMP-2 and 9 were negatively immunostained in 18 samples of the NTT.The difference of MMP-2 and 9 immunoexpression between TSCC and NTT was statistically significant(p=0.0000).The percentage of positive cases for MMP-2 and 9 expression in node-positive patients was significantly higher than in node-negative patients(p=0.015,0.048). Logistic regression analysis showed that positive expression of both MMP-2 and 9 were independent risk factors for tumor recurrence(P=0.026,0.002).MMP-9 positivity also correlated to occult lymph node metastasis(P=0.044).Conclusions MMP-2 and 9 were highly expressed in TSCC.The expression of MMP-2 and 9 significantly correlated with lymph node metastasis and tumor recurrence.MMP9 positivity also correlated with occult lymph node metastasis(OLNM).
