肛瘘手术前后肛管直肠压力测定研究

目的研究肛管直肠压力测定对肛瘘手术的疗效评价。方法对38例肛瘘患者进行手术前后肛管直肠压力测定,观察直肠肛门反射、直肠静息压、肛管静息压及肛管最大收缩压等指标情况。结果38例患者18例术后直肠肛门反射增强,10例降低,10例手术前后无显著性差异。手术前后肛管最大收缩压比较无显著性差异（P〉0.05）。术后直肠静息压、肛管静息压较术前明显降低,与术前比较,有显著性差异（P〈0.05,P〈0.01）。结论可用肛管直肠压力测定来评价肛瘘手术的临床疗效,作为肛肠术后患者肛门直肠功能评价指标,为手术方式的改进提供客观依据。

Study of Anorectal Manometry Before and After Fistula Surgery

Objective To study anorectal manometry for the evaluation of efficacy on fistula surgery.Methods Anorectal manometry was measured before and after fistula surgery in 38 cases of anal fistula patients.It was to observed anorectal reflex,rectal resting pressure（RRP）,anal canal resting pressure（IARP）,anal maximal contraction pressure（AMCP）,etc.Results Of 38 cases,anorectal reflex was increased in 18 cases and decreased in 10 cases;and 10 cases had not significant difference in observed indexes before and after surgery.In comparison of AMCP before and after surgery,there was no significant difference（P〉0.05）.After surgery,RRP and IARP were decreased apparently compared with those before surgery,indicating significant difference（P〈0.05,P〈0.01）.Conclusion Anorectal manometry can be used to evaluate clinical efficacy of fistula surgery and anorectal function indexes after anorectal operation,which provides objective evidence for the innovation of operation approaches.
