大黄降浊冲剂对慢性肾衰竭脾胃证候作用特点研究

目的观察大黄降浊冲剂对慢性肾衰竭脾胃证候的治疗作用。方法在一般治疗的基础上,治疗组60例给予大黄降浊冲剂,每次9 g,每日2次;对照组60例予尿毒清颗粒,每次9 g,每日3次,两组疗程均为8周。结果治疗组的临床控制率、显效率及总有效率虽高于对照组,但差异无统计学意义（P〉0.05）;治疗组血肌酐（Scr）、尿素氮（BUN）水平在治疗后显著低于治疗前（P〈0.05）,且治疗后治疗组的BUN水平明显低于对照组（P〈0.05）,治疗后两组的内生肌酐清除率（Ccr）均比治疗前显著增加（P〈0.05）;治疗后治疗组的血浆总蛋白（TP）、血浆白蛋白（Alb）、血红蛋白（Hb）均较治疗前显著升高（P〈0.05）,且Alb较对照组提升更明显（P〈0.05）,而对照组只有Alb、Hb较治疗前明显升高（P〈0.05）。结论大黄降浊冲剂能较好地改善中医脾胃症状与证候,调整脾胃升降功能,改善胃肠功能紊乱,降低血清肌酐、尿素氮,改善CRF患者的营养状态,纠正肾性贫血。

Effect Characteristics of Dahuang Jiangzhuo Granules on Spleen and Stomach Pattern in Chronic Renal Failure

Objective To observe the effect of dahuang jiangzhuo granules on spleen and stomach pattern in chronic renal failure.Methods On the basis of general treatment,60 cases in a treatment group were treated with dahuang jiangzhuo granules,9 g each time,twice per day.In a control group,niaoduqing granules were given,9 g each time,three times per day.The course of treatment was 8 weeks in either group.Results The clinical control rate,remarkably effective rate and total effective rate in treatment group were higher compared with control group,but without statistical difference（P0.05）.In treatment group,the levels of serum creatinine（Scr）and blood urea nitrogen（BUN）were lower significantly after treatment as compared with those before treatment（P0.05）.After treatment,BUN level in treatment group was lower remarkably than that in control group（P0.05）.After treatment,the endogenous creatinine clearance rate（Ccr）increased in either group as compared with that before treatment（P0.05）.After treatment,in treatment group,TP（total plasma protein）,Alb（albumin）and Hb（hemoglobin）increased significantly as compared with those before treatment,of them,Alb was up-regulated much more obviously as compared with control group（P0.05）.But in control group,only Alb and Hb were up-regulated remarkably as compared with those before treatment（P0.05）.Conclusion Dahuang jiangzhuo granules can well relieved the symptoms and pattern of the spleen and stomach in traditional Chinese medicine,adjust the ascending and descending function of the spleen and stomach,improve in gastrointestinal function disturbance,decrease Scr and BUN,benefit the nutrient state of the patients and rectify renal anemia in chronic renal failure.
