居民健康档案和慢性病信息系统的开发

目的提高社区卫生服务中心对居民健康档案信息的管理水平，解决慢性病患者动态管理问题。方法依据国家有关标准、规范，根据需求分析，利用计算机语言编写程序。结果开发出适用于社区卫生服务的居民健康档案和慢性病信息化管理系统，并可以与其它按国家卫生信息标准开发的区域卫生信息化管理系统实现互联互通。结论该系统业务流程统一、规范，具有自动人群分类和随访提醒功能，限制功能全面，协调性、连续性强，实现了对居民健康状况和慢性病患者的动态管理。

Design and Application of Information Management System for Resident Health Record and Chronic Non-Communicable Diseases

Objective To improve the Management level of community service centers on health record information and chronic non-communicable diseases of residents. Methods According to the national standards, technical specification and requirement analysis, the information management system were programmed using computer language. Results The information management system for resident health record and chronic non-communicable diseases were carried out. This system can also interconnect with other information management systems developed according to national health information standards. Conclusion The system have a unified business process, automatic classification for the crowd and automatic follow-up reminders, and fully functional limitations, very well coordination and continuity. This system also makes the dynamic management of resident health status and chronic non-communicable diseases available.
