社区糖尿病治疗达标现状分析

目的 通过对社区糖尿病专科门诊患者的临床情况进行调查分析，探讨在社区中如何作好糖尿病的综合治疗，以实现糖尿病控制的全面达标。方法 选择曹杨社区医院门诊就诊的2型糖尿病患者101例，调查其血糖、血压、血脂控制情况。结果 空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)控制良好以上者分别占23．76％、40．22％、37．62％，其中控制理想者分别占7．92％、9．78％、8．91％；血压控制良好以上者占86．74％，其中控制理想者占43．88％；胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)控制良好以上者分别占73．26％、72．10％、98．84％、67．44％，其中控制理想者分别占29．07％、40．70％、95．35％、25．58％。结论 曹杨社区卫生服务中心的糖尿病控制不理想。糖尿病、高血压、血脂紊乱并存的个体中，血糖的控制率明显低于血压、血脂的控制率。社区医院应规范化管理糖尿病患者，以进一步提高社区医院糖尿病治疗和管理水平。

Analysis of current status of management of type 2 diabetes mellitus by community diabetic clinic

Objective Through the investigation of clinical conditions of the diabetic patients in the community diabetes clinic to appraise the way of achieving 9 000 diabetic control by comprehensive treatment.Methods Type 2 diabetics seeking medical advice at the OPD of Caoyang community diabetes clinic were selected as the studied subjects.Results The well control rates of FPG, 2hPG, HbA 1c were 23.76%, 40.22%, 37.62%, respectively; among them, the optimum controlled rates were 7.92%, 9.78%, 8.91%, respectively. The well controlled rate of high blood pressure was 86.74% and the optimum controlled rate was only 43.88%. The fairly well controlled rates were 73.26%, 72.10%, 98.84%, 67.44%, among them, the optimum control rates were 29.07%, 40.70%, 95.35%, 25.58%, repectively.Conclusion Control of blood sugar is the major basis of administration work. The controlled rate of blood sugar is much lower than that of the blood pressure and blood lipid. Hence, the treatment of diabetics in this community clinic should be raised to a higher level under the administration monitoring as to avoid the occurrence of chronic diabetic complications.
