左旋肉碱联合重组人红细胞生成素治疗肾性贫血的临床研究

目的探讨联合左旋肉碱与重组人红细胞生成素（rHuEPO）治疗肾性贫血的临床效果。方法选取我科接受维持性血液透析治疗合并肾性贫血的终末期肾病患者50例,随机分为两组,对照组常规补充铁剂、叶酸及复合维生素B,予血透后皮下注射rHuEPO治疗,观察组在此基础上联合左旋肉碱,即每次血透后静脉注射1.0 g,疗程3个月。比较两组治疗前后血红蛋白（Hb）、血细胞比容（HCT）水平、rHuEPO的用量以及两组血压升高的发生率。结果两组患者治疗3个月后Hb、HCT水平均较治疗前明显上升,且治疗组上升幅度显著高于对照组,差异均有高度统计学意义（P〈0.01）;观察组rHuEPO用量较治疗前显著减少（P〈0.01）,血压升高发生率显著低于对照组（P〈0.05）。结论左旋肉碱可以提高rHuEPO对肾性贫血的治疗效果,减少rHuEPO用量,降低医疗成本。

Effects of L-carnitine combined with recombinant human erythropoietin hormone in the treatment of renal anemia

Objective To investigate the effects of L-carnitine combined with human erythropoietin hormone in the treatment of renal anemia.Methods 50 cases of ESRD patients with renal anemia treated with hemodialysis were randomly divided into two groups,the control group was given conventional iron,folic acid and vitamin B and rHuEPO,on basis of which,the observation group was added with L-carnitine 1.0 g ivgtt after hemodialysis.The treatment last for 3 months.The Hb,HCT level and rHuEPO dosage were compared,as well as the rate of high blood pressure.Results The levels of Hb,HCT in both groups were significantly increased after treatment,and that in the observation group were higher(P < 0.01);the dosage of rHuEPO compared with before were significant decreased(P < 0.01),and the rate of high blood pressure were significantly lower than that in the control group(P < 0.05).Conclusion L-carnitine can improve the therapeutic effect of rHuEPO on renal anemia and reduce the amount of rHuEPO,reduce the healthcare costs.
