粉防己碱对实验性急性胰腺炎合并肺损伤的治疗作用

摘    要:目的 观察粉防己碱(tetrandrine,Tet)对实验性急性胰腺炎(acute pancreatitis,AP)合并肺损伤的治疗作用,并对其作用机制进行探讨。方法 取176只SD大鼠,雌雄不拘,随机分为模型组,Tet组等6组,经胰胆管逆行注射3%牛磺脱氧胆酸钠0.1ml/100g体重,复制成AP模型,制模后10min,给予不同药物或生理盐水,记录造模后4h,8h腹水量,测定血浆钙离子浓度和淀粉酶活性以及肺组织匀浆中磷脂酶A2(PLA2)活性的变化;计算肺系数,光镜下观察胰腺及肺组织的病理变化;记录动物的存活时间及24h死亡率。结果 Tet能减轻AP大鼠胰腺及肺组织的病理性损伤;减少腹水的生成量;与对照组相比,Tet组血浆中钙离子浓度下降的程度减轻,淀粉酶活性下降,肺匀浆中PLA2活性显著降低,大鼠的死亡率降低,生存时间延长。结论 粉防己碱可以通过钙通道拮抗作用,抑制PLA2的活性,减轻胰腺及肺的损伤从而对急性胰腺炎发挥治疗作用。

Abstract:Objective To Investigate the therapeutic effect of tetrandrine on acute experimental pancreatitis (AP) of rats and its possible mechanism.Methods 176 male or female Sprague Dawley rats were divided into six groups randomly.AP models were made by injecting 3% sodium deoxycholate (1 ml/kg body weight) into the biliopancreatic ducts except the rats of sham operation group.10 minutes later,rats were injected intraperitoneally with saline,Tetrandrine or Verapamil,respectively.At 4 h and 8 h after treatment,the volume of ascites and pulmonary coefficient were measured.The amylase activity,and plasma calcium concentration and PLA2 in homogenate of lung were determined.The pathologic change of pancreas and lung tissue was examined under microscope.The survival time and mortality rate of rats in different groups were recorded.Results Tetrandrine reduced the mortality rate ( P <0.05), prolonged the survival time of rats with AP significantly ( P <0.01), attenuated ascites volume,amylase activity and hypocalcemia,PLA2 activity in lung homogenate,and ameliorated the inflammatory injury of pancreas and lung.Conclusion The results suggested that Tetrandrine possesses a therapeutic effect on AP in rats.The effective mechanism might be due to inhibiting PLA2 activity by blocking calcium channel to reduce the damage of pancreas and lung which were induced by the overload of Ca 2+ .
