高血压患者血浆同型半胱氨酸水平与颈动脉斑块的关系

摘    要:目的研究高血压患者血浆同型半胱氨酸水平与颈动脉内中膜厚度(IMT)及稳定性的关系。方法选择200例原发性高血压患者,依据高血压分级、血浆Hcy水平、脉压及斑块稳定性分组。结果不同高血压分级组血浆Hcy、IMT及颈动脉斑块检出率显著高于对照组(P〈0.05)。高Hcy组血浆Hcy、IMT及颈动脉斑块检出率均显著高于正常Hcy组(P〈0.05),高脉压组血浆Hcy、IMT及颈动脉斑块检出率均显著高于正常脉压组(P〈0.05)。血浆Hcy与高血压分级呈正相关(P〈0.05)。血浆Hcy与IMT及24hPP均呈直线正相关(P〈0.05),IMT与24hPP呈直线正相关(P〈0.05)。血浆Hcy是高血压患者颈动脉斑块形成独立危险因素(P〈0.05)。不稳定斑块组高Hcy率及血浆Hcy水平显著高于稳定斑块组(P〈0.05)。结论高血压患者血浆Hcy升高与颈动脉IMT增厚、斑块形成及稳定性密切相关,高24hPP的高血压患者血浆Hcy升高及颈动脉斑块形成风险大。

Relationship between plasma homocysteine levels and carotid plaque in hypertensive patients

Abstract:Objective To study the relationship between plasma homocysteine and carotid intima-media thickness (IMT) and the stability in hypertension patients. Methods Two hundred cases of patients with essential hypertension were grouped, according to the grade of hypertension, plasma Hcy levels, pulse pressure and plaque stability. Resuits The plasma hypertension grading Hcy, IMT and carotid plaque detection rate was significantly higher (P〈0.05). High Hcy plasma Hcy,IMT and carotid plaque detection rate was significantly higher than normal Hcy group (P〈 0.05), high pulse pressure plasma Hcy, IMT and carotid plaque detection rate was significantly higher than normal pulse pressure group(P〈O.05). Plasma Hcy was positively correlated with the grade of hypertension (P〈O.05). Plasma Hcy and IMT and 24hPP showed linear correlation (P〈0.05), IMT and 24hPP positive linear correlation (P〈0.05). Plasma Hcy is carotid artery plaque formation in patients with hypertension were independent risk factors (P〈0.05). Unstable plaque group rates and high Hcy plasma Hcy levels were significantly higher stable plaque group (P〈 0.05 ). Conclusion Patients with hypertension and elevated plasma Hcy carotid IMT thickening, plaque formation and stability are closely related, high 24hPP elevated plasma Hcy in patients with hypertension and carotid artery plaque formation risks.
