糖尿病前期状态糖代谢异常与动脉硬化的关系

目的:观察非糖尿病人群不同糖耐量状态下颈动脉内中膜厚度(IMT)的差异性,分析口服糖耐量试验(OGTT)中四时点血糖水平及脂代谢指标与IMT的相关性。方法:19例正常糖耐量(NGT)者、34例糖调节受损(IGR)者,测定血脂、血糖等生化指标,测量IMT。结果:IMT值在IGR组显著高于NGT组(P<0.01);IMT值与TC、TG、LDL-C、OGTT 60 min和120 min血糖值、HDL-C均存在显著相关(P<0.05);TC和OGTT 120 min血糖值均为IMT的独立相关因素,后者相关性更强(P<0.01)。结论:非糖尿病人群中,OGTT 120 min血糖和TC值为早期动脉硬化的独立危险因素,OGTT 60 min血糖有加重动脉粥样硬化的趋势。

Relationship between, glucose metabolism disturbance and atherosclerosis in pre-diabetic status

Objective:To observe the variation of carotid intima-media thickness (IMT) among different glucose tolerance status and investigate the relationship of IMT with different glucose levels and lipid metabolism in non-diabetic subjects with impaired glucose regulation(IGR) . Methods: Nineteen normal control subjects, 34 subjects with (IGR) newly diagnosed by OGTT were included on the basis of measuring blood pressure, fasting glucose and lipids and recording of height, weight and other body indexes as well as IMT. Results: The carotid IMT values in IGR groups were significantly higher than that in the control group(P<0.01,respectively); In all the non-diabetic subjects, the IMT value was positively correlated with TC,TG, LDL-C and plasma glucose level of OGTT 60 min and OGTT 120 min (r=0.468, 0.350, 0.396, 0.514, 0.519, respectively, P<0.05 or P<0.01), but negatively correlated with HDL-C(r=-0.313, P<0.01). TC and plasma glucose level of OGTT 120 min were correlated with carotid IMT, but the former was in significant statistical difference. Conclusion:In non-diabetic subjects with IGR, glucose level of OGTT 120 min and TC level may be the independent risk factors of atherosclerosis and there is obvious tendency of worsening of early atherosclerosis in OGTT 60 min glucose levels.
