胃癌胃镜活检标本端粒酶活性检测的价值

摘    要:目的探讨胃镜活检标本端粒酶活性检测对胃癌诊断的价值.方法应用TRAP-PCR-ELISA方法及TRAP-银染法分别检测58例胃镜活检标本的端粒酶活性并分析其对胃癌诊断的价值.结果36例胃癌活检标本端粒酶活性水平与癌疑组相近但明显高于正常对照组.端粒酶活性的检测敏感性为88.9%,特异性为77.2%,准确性为84.5%.不同病理类型胃癌的端粒酶活性阳性检出率差异无显著性;病理分级组间亦无显著差异.用TRAP-银染法测定端粒酶活性发现端粒酶活性水平较低的标本定性分析为弱阳性,条带不清晰或条带较少.结论端粒酶活性是胃癌细胞的标志物之一.用定量法检测端粒酶活性与组织检查相结合可提高肺癌的早期诊断率.

Abstract:Objective:To investigate the diagnostic significance of the detection of telomerase activity in gastroscopic biopsy tissues from patients with gastric cancer. Methods: The technique of telomeric repeat amplification protocol(TRAP) based telomerase polymerase chain reaction(PCR) enzyme linked immunosorbent assay (ELISA) and TRAP silver staining were employed to detect telomerase activity and telomerase activity levels in 58 samples of biopsy from patients with gastric cancer and in normal control group. Results:The positive rate and levels of telomerase activity in 36 biopsy samples of gastric cancer group were the same as that in 7 biopsy samples from suspecting gastric cancer patients ( P >0.05) , but significantly higher than that in normal contral group ( P <0.005). The sensitivity , specificity and accuracy of detection of telomerase activity were 88.9% ,77.2% ,84.5% respectively .There was no significant difference in the positive rate or levels of telomerase activity between different age,sex,pathologic types and differentiation degree ( P >0.05). Conclusion:The telomerase activity may be as a good marker for diagnosis of gastric cancer. Combined with pathologic analysis, it is possible to raise the early dignostic rates of gastric cancer.
