盐酸戊乙奎醚降低体外循环患者心肌损伤的实验研究

目的 探讨盐酸戊乙奎醚预处理对体外循环(CPB)下心内直视手术后患者心肌保护作用的机制.方法 随机抽样选取60例无心梗的心脏瓣膜置换手术患者,随机分为对照组和试验组,每组各30例.各组病例在CPB前,CPB后即刻,术后1、2、3d 5个时点定量测定血清中的cTn-I、CK-MB浓度,比较各组同一时点cTn-I、CK-MB浓度的差异.结果 两组患者术前的cTn-I和CK-MB均在正常参考值内,差异无统计学意义(P＞0.05);术后即刻,术后1、2、3d各时点的cTn-I和CK-MB试验组明显低于对照组,差异有统计学意义(P＜0.05).结论 盐酸戊乙奎醚可以降低心肌细胞损害的特异性指标,进而推断盐酸戊乙奎醚对心肌缺血再灌注损伤具有保护作用.

Research of protecting cardiac muscle pretreated with penehyclidine hydrochloride during open heart surgery with cardiopulmonary CPB

Objective To Explore the mechanism of cardiac muscle protecting during open heart surgery with cardiopulmonary CPB using penehyclidine hydrochloride.Methods Sixty patients undergoing valve replacement with CPB were randomly divided into control group(n=30) and test group(n=30).Blood samples were collected and cTn-I,CK-MB concentration were measured in the time of pre-CPB,the moment of CPB finished,one-day,two days and three days post-CPB respectively.The results of control group and test group at the same time point were compared with statistical method.Results cTn-I and CK-MB of both groups were no significantly different before CPB. cTn-I and CK-MB of test group were significantly lower than control group in the time of after CPB,one day,two days and three days post operation respectively.Conclusion Penehyclidine hydrochloride decreased the value of some specific markers of cardiac muscle impairing during heart ischemical reperfusion.It is possible that penehyclidine hydrochloride has protective effect in the course of heart ischemical reperfusion.
